1.Investigation of Water Quality of Centralized Water Supply in Shanghai

Objective To understand the water quality of centralized water supply in Shanghai and provide the scientific data for further improvement of the health inspection and management of centralized water supply. Methods The management situation of centralized water supply, the water quality of water source, product water and tap water were investigated by using the methods in Analytical Methods for Water and Sanitary Standard for Drinking Water Quality (2001) in Shanghai in 2006. Results The current situation of management of 165 units of centralized water supply including 19 self-supply water plants in Shanghai was satisfactory. The analysis of drinking water showed that COD and total coliform in source water exceeded the standard limit remarkably(the eligible rates were 63.93% and 22.95% respectively), the eligible rates of COD, turbidity and manganese in product water and tap water were 25.45%, 18.18%, 27.88% and 18.06%, 13.89%, 11.81% respectively. Conclusion The results of the present investigation show that in Shanghai the main problems about the water quality are water source contamination by organic pollutants and the undeveloped water treatment.
2.Effects of sevoflurane and anoxia induced preconditioning on survival and apoptosis of neonatal rat orprdiac myocytes

Objective To determine the difference between sevoflurane- and anoxic preconditioning in protecting newborn rat heart muscle cells from anoxia-reoxygenation injury. Methods The second generation of primary cultured cardiac myocytes from 2-3d newborn SD rats were randomly divided into 4 groups: control group (C), anoxia/reoxygenation group (A/R) in which cultured cardiac myocytes were exposed to 2h anoxia followed by 48h reoxygenation; anoxic preconditioning group (IP) in which before A/ R the cardiac myocytes were pretreated with 20 min anoxia; sevoflurane preconditioning group (S) in which cardiac myocytes were pretreated with 20 min 2.5% sevoflurane (1.5 MAC) before A/R. Ultrastructure of heart muscles cells was observed 1 h after reoxygenation, Cell survival was determined by MTT rapid colorimetric assay and apoptosis was measured by flow cytometry at 0, 1, 12, 24, 36 and 48 h after reoxygenation. Results (1) In S and IP group there was no significant change in ultrastructure and no apoptosis cell was found, whereas in A/R group the change in ultrastructure was significant , and apoptosis cells were found. (2) The cell survival in group S and group IP was significantly lower than that in group C but significantly higher than that in group A/R. (P 0.05) . The survival of cardiac myocytes increased with prolongation of reoxygenation time in group S and group IP. Objective To assess the prognostic value of progesterone receptor (PR) in Luminal B HER2(-) lymph-node-negative breast cancer.Methods The clinical data of 196 Luminal B HER2 (-) lymph-node-negative breast cancer patients were collected in this study.The correlation between PR and clinicopathological factors and its influence on prognosis was analyzed.Results Among 196 patients with Luminal B HER2 (-) lymph-node-negative breast cancer,14.8%(29/196)were PR-negative and 85.2%(167/196)were PR-positive (HR=3.25,95.0% CI:0.73-1.45,P=0.123).PR negativity indicated postmenopausal status (P=0.029) and high tumor grade (P=0.001).Mter a median follow-up of 60 months,9.7%(19/196) of the patients had relapses and 4.1%(8/196) died of breast cancer.The 5-year disease-free survival(DFS) was 75.9% in PR-negative group and 92.8% in PR-positive group (Log-rank P=0.001).The 5-year overall survival (OS) was 89.7% in PR-negative group and 97.0% in PR-positive group (Log-rank P=0.023).In multivariate analysis,PR negativity was significantly associated with a poor 5-year DFS (HR=2.85,95.0% CI:1.05-7.72,P=0.039)but not with 5-year OS (HR=1.78,95.0% CI:0.34-9.35,P=0.495).Conclusion In this study,PR is a prognostic factor for relapse in Luminal B HER (-) lymph-node-negative breast cancer.

Objective To find a more suitable approach to pediatric anesthesia during laparoscopic operation to alleviate stress response.Methods Forty patients,1 to 3 years of age,ASA grade Ⅰ-Ⅱ,underwent general anesthesia(group GA)and combined general epidural anesthesia(group GEA)during their laparoscopic operation.Heart rate(HR),mean arterial pressure(MAP),ECG,SPO2,PETCO2,blood gas,plasma epinephrine,plasma norepinephrine,plasma IL-6 and IL-10 were monitered and recorded five minutes before anesthesia(T0),pre-pneumoperitoneum(T1),5 min after pneumoperitoneum(T2),30 min after pneumoperitoneum(T3),30 min after operation(T4)and 24 h after operation(T5).Results(1)HR,MAP of group GA were increased obviously at T2,T3,T4 compared to T0(P0.05).Significant difference was found at T1,T2,T3 and T4 between group GA and group GEA(t=4.749,10.349,13.632,10.051,P0.05).Plasma IL-10 at T1,T2,T3,T4 were higher than that at T0 in both groups(P

Objective To study the effect of glucocorticoid receptor(GCR) on the treatment of patients with nephrotic syndrome(NS) and to observe the accommodation effect of herbal gynostemmae,acanthopanax senticosus harns on GCR levels.Methods The patients were divided into 3 groups: the healthy control group,treatment group Ⅰ(GC only) and treatment group Ⅱ(combination with herbal gynostemmae,acanthopanax senticosus harns and GC),radioligand binding assay was used to measure the GCR levels of each group.At the same time,the urine protein ratio within 24 hours was observed in each group.Results Compared with healthy control group,the GCR levels of treatment group Ⅰ and Ⅱ were both reduced,but after 4 weeks of treatment in the treatment group Ⅱ,not only GCR level recovered but also the urine protein were decreased faster than treatment group Ⅰ.Conclusions Herbal gynostemmae,acanthopanax senticosus harns can accommodate the GCR levels of children with NS.They can also eliminate urine protein and cure NS ideally.
10.Dynamic analysis of pediatricians′ resource allocation in China from 2010 to 2015

Objective To analyze the dynamic flow of pediatricians from 2010 to 2015, their present deployment and workload as compared with physicians across the country, in order to provide reference for their effective allocation.Methods Selecting data indexes from Chinese Statistical Yearbook and China Health and Family Planning Yearbook, and using Excel for statistics of relevant index data.By means of horizontal and vertical comparisons, a dynamic analysis was made on such indexes as the number of pediatricians, that of pediatricians per thousand children, and the number of hospital admissions and the number of hospitalizations per pediatrician.Results The total of pediatricians only increased by 2.32% from 2010 to 2015.On the other hand, the number of pediatricians per thousand population only accounted for 23.42% of physicians per thousand population in 2015.These five years witnessed a 40.58% workload increase of outpatient/emergency visits per pediatrician, and 36.70% increase of discharged inpatients.Conclusions China is faced with the challenges of shortage and insufficient supply of pediatricians, worsened by excessive workloads.It is advisable to improve pediatrician development pattern, raise their remuneration, and ease their workload, for the sake of better healthcare for children.
