1.Expression of interleukin-6 in ischemic cerebrovascular disease

OBJECTIVE: To comprehensively analyze the relationship between interleukin-6 (IL-6) and ischemic cerebrovascular disease. DATA SOURCES: A computer-based search was conducted in Pubmed for literatures related to the expression, effect, and significance of IL-6 in ischemic cerebrovascular disease pblished between January 1994 and March 2006 with the of "interleukin-6, Ischemia Cerebrovascular Disease; Interleukin-6, Ichemia Stroke", and the language was limited to English. STUDY SELECTION: Retrieved data were arranged, and articles with strong aim were selected. Literatures published recently and those published in authorized journals of the same region were superior. DATA EXTRACTION: A total of 122 relevant articles were retrieved, and 23 of them were in accordance with the inclusion criteria. DATA SYNTHESIS: IL-6 is a pleiotropic cytokine with both pro-inflammatory and anti-inflammatory effects. It highly expresses in the cerebral tissue, cerebrospinal fluid and peripheral blood serum after cerebral ischemia. IL-6 plays an important role in the pathophysicoloy and inflammatory response to the ischemia cerebrovascular disease of acute stage. CONCLUSION: IL-6 plays an important role in the pathophysicological mechanism and prognosis of ischemic cerebrovascular disease, which can be taken as a prediction index for evaluating the focus of infection and neurofutional defects.
3.The correlation analysis between high-sensitivity C-reactive protein and neutrophil alkaline phosphatase

Objective To evaluate the correlation between high-sensitivity C-reactive protein(hsCRP)and neutrophil alkaline phosphataee(NAP).Methods 74 patients with different hsCRP levels and 20 healthy subjects were selected.hsCRP and NAP were detected in every person,then the t test and correlation analysis were conducted.Results The NAP integral value of high-value hsCRP group and mid-value were significantly higher than the low-value hsCRP group(P < 0.01),and the NAP group average positive rate of the high,median hsCRP values were 81.8%and 82.3%,which were higher than low-value of hsCRP group(53.6%).Compared the correlation between each group of hsCRP value and integral value of NAP,the hsCRP of high-value hsCRP group has a good correlation with NAP integral value(P < 0.01),while the hsCRP of mid-value of hsCRP group and NAP integral is less relevant(P >0.05).Conclusion hsCRP as an acute reactive protein exists a good correlation with NAP,when hsCRP > 50mg/L,it can partially replace NAP.
4.Role of N-acetylcysteine in protecting rats against contrast-induced acute kidney injury

Objective To study the role of N‐acetylcysteine (NAC) in protecting rats against con‐trast‐induced acute kidney injury .Methods Forty‐five adult Wistar rats were randomly divided into control group ,model group and NAC group (15 in each group) .Their serum levels of creati‐nine ,CRP ,TNF‐a were measured before ,and 48 and 72 h after contrast medium injection .The an‐imals were killed 72 h after contrast medum injection .Their serum levels of malondialdehyde , SOD ,NO were measured .Eexpressions of Bax and Bcl‐2 were detected .Results The serum levels of creatinine ,CRP ,TNF‐a ,malondialdehyde and the expression level of Bax were significantly higher in model group and NAC group than in control group ,and in model group than in NAC group after contrast injection (P< 0 .05) .The serum levels of SOD and NO and the expression level of Bcl‐2 were significantly lower in model group and NAC group than in control group ,and in model group than in NAC group after constrast injection (P< 0 .05) .Immunohistochemistry demonstrated that the TGFβ1 expression level was significantly higher in model group and NAC group than in control group and was significantly lower in NAC group than in model group .Con‐clusion NAC can protect rats against kidney injury and reduce contrast‐induced acute kidney in‐jury by inhibiting inflammation ,oxidation ,apoptosis and cytokines .

6.Hypercortisolism and psychosis

Objective To studytherelationshipbetweenhypercortisolismand psychosis. Methods According to pathologic diagnosis, 133 cases of hypercortisolism were dividedinto adrenal lesion group, pituitary adenoma group and ectopic ACTH tumor group, and the relationship between hypercortisolism and psychosis was analyzed. Results (1) There were significant differences between groups regarding cortisol levels and the amplitude of fluctuation. (2) Depression and mania were the main manifestations of psychosis in the patients, and 79.7% of the patients showed symptoms of depression. (3) In ectopic ACTH tumor group, the level and the increased amplitude of 24 h urinary free cortisol were significantly higher than those in the other two groups, all the patients had severe depression, and the rate of suicide tendence (58.3%) was significantly higher than the other two groups. Conclusions (1) Psychosis has a close relationship with hypercortisolism,especially with the marked fluctuation in cortisol level. (2) Sufficient attention should be paid to this relationship, which will improve the diagnosisand treatment of hypercortisolism.

8.Analysis on the clinical effect of sirolimine combined with surgical operation on the treatment of colonic cancer complicated with intestinal obstruction

Objective To analyze the clinical effect of combination therapy of somatidine on colorectal cancer with intestinal obstruction. Methods 120 patients with colorectal cancer complicated with intestinal obstruction who were treated with Yujiang County Hospital of Zhejiang Province from November 2016 to May 2017 were selected as the subjects of this study and divided into observation group by dynamic random color ball extraction method And control group. The rats in the control group were given stage I colonic resection and anastomosis. The observation group was treated with somatanine combined with stage I colonic resection and anastomosis. The differences of the indexes and complication were compared between the two groups. Results There was no significant difference in the operation time between the two groups (3.01 ± 0.41) days and the hospitalization time (10.11 ± 1.21) days in the observation group, and the time of operation was significantly higher than that of the control group (5.41 ± 0.84) days, the hospitalization time was (16.33 ± 2.05) days, the number of complications occurred in 10 cases, the difference was more than that in the control group Significantly(P<0.05). Conclusion Si Tanning combined with surgical treatment of colorectal cancer with intestinal obstruction has a certain value.
9.Clinicopathological features of six cases of calcifying aponeurotic fibroma

Purpose To investigate the clinicopathologic features of calcifying aponeurotic fibroma (CAF). Methods The clinical and pathologic profiles of 6 cases of CAF were retrospectively analyzed. Results CAF appeared a firm, painless and slowly growing mass with a predilection for the distal portions of the extremities in young children and adolescents. Microscopically, the tumor consis-ted mainly of benign appearing spindle cells, chondroid cells and calcific debris. Immunohistochemically, the tumor cells were positive for vimentin, HHF-35, S-100, Calponin and CD68. Conclusion CAF is a rare, locally aggressive fibroblastic lesion. It can make an accurate diagnosis by integrating the clinical location with histopathological features. Repeated minor trauma to the extremities and some embryologic cell nests maybe its pathogenesis.
10.Role of preoperative morphologic classification in solitary small hepatocellular carcinoma treated by RFA

Objective To invcstigate the value of preoperative imaging morphologic classification in solitary small hepatocellular carcinoma treated by RFA.Methods The clinical data of 73 patients with small hepatocellular carcinoma who rcccivcd radiofrequency ablation at the Affiliated Drum Tower Hospital of Medical School of Nanjing University from January 2008 to December 2012 were analyzed retrospectively.The lesions were classified into three types according to gross appearance in preoperative contrast-enhanced computed tomography (CT):single nodular type (type 1);single nodular with extranodular growth type or confluent multinodular type (type 2);infiltrating type (type 3).Thc clinicopathological features of patients with 3 types of tumors were compared by statistical methods.Patients were followed up via outpatient examination and telephone interview up to August 2014 or death.Results There were 24 cases of type 1,35 cases of type 2,14 cases of type 3,and the proportion were 20.2%,32.9%,47.9%,19.2%,respectively.The preoperative alpha-fetoprotein of type 3 was significantly higher than othcr types.The disease-free survival time of the three types were (37.2 ± 3.3) months,(20.9 ± 4.0) months,(14.4 ±4.9) months.The prognosis of the irregular types (type 2,type 3) is poor than the regular types (type 1).The overall survival time were (72.5 ± 4.7) months,(66.2 ± 7.3) months,(31.2 ± 5.3) months.The type 3 was the worst.The results of univariate analysis showed that morphologic classification,preoperative AFP,tumor size were related factors affecting the prognosis of patients (P < 0.05).The results of multivariate analysis showed that morphologic classification was the independent risk factors affecting the prognosis of patients (P < 0.05).Conclusion The preoperative imaging morphologic classification is the independent risk factors affecting the prognoses of patients with small HCC,which is help to choose the best treatment for patients with small hepatocellular.
