1.Clinical application of butorphanol and propofol in painless gastroscopy

Objective To study the clinical anesthetic effect of butorphanol combined with propofol in painless gastroscopy. Methods 90 patientswith stomachache check were randomly divided into observation group and control group,45 cases each. The observation group was received butorphanol combined with propofol and the control group were treated with fentanyl combined with propofol. The heart rate of pa-tients before anesthesia,during checking and conscious,the change of blood pressure and oxygen saturation,respiratory depression and anes-thetic effect were analyzed. Results The heart rate,blood pressure and oxygen saturation of two groups after treatment were decreased,the difference was not statistically significant(P>0. 05). The total volume of propofol and respiratory depression between the two groups were compared,the differences were statistically significant(P<0. 05). Conclusion Butorphanol combind with propofol in painless gastroscopy with long anesthesia time and low respiratory depression,which was worthy of promotion.
2.Nursing of polycystic ovary syndrome in transvaginal ultrasound small follicular puncture

Objective To explore the nursing of polycystic ovary syndrome treated by transvaginal ultrasound small follicular puncture.Methods The treatment and nursing of 58 cases with polycystic ovary syndrome treated by transvaginal ultrasound small follic~ar puncture were summarized.Results Among 58 cases of polycystic ovary syndrome treated by transvaginal ultrasound small follicular puncture,21 cases obtained postoperative pregnancy,the pregnancy rate was 36.20%.Conclusions According to the psychological characteristics of patients with polycystic ovary syndrome treated by transvaginal ultrasound small follicular puncture,good psychological nursing and sufficient preoperative preparation are helpful to reduce the patients' psychological pressure,cooperate with the operation smoothly,and obtain satisfactory clinical effect.
3.Reduction of hemoglobin induced by linezolid therapy: a retrospective study

The medical records of 197 patients on linezolid therapy were collected retrospectively.The prevalence and risk factors for linezolid-induced reduction of hemoglobin were identified.The incidence of linezolid-induced reduction of hemoglobin was 31.98%.The patients with reduction of hemoglobin had older age [(68 ± 14) vs.(59 ± 18) years,P =0.000] and lower creatinine clearance (CCr) [(72 ±42)vs.(98 ± 52) ml/min,P =0.000].Multivariate logistic regression analysis showed that age ≥65 years,treatment duration ≥ 15 days,CCr < 65 ml/min and pre-treatment hemoglobin value < 95 g/L were significant risk factors for linezolid-induced reduction of hemoglobin.

5.Evaluation of medical efficiency before and after Sanming healthcare reform

Objective To evaluate the medical efficiency of sample hospitals in Sanming city before and after Sanming health care reform.Methods One tertiary hospital,two general county hospitals and two TCM county hospitals in Sanming were sampled for the study.Two dimensions(the intensity index of medical work and work efficiency index)were selected.In addition,six indexes were identified,namely the number of discharged patients,number of outpatients and emergency diagnostics,doctor′s daily workload per capita for inpatients,bed turnover times,bed utilization rate and average days of stay.Descriptive trend analysis and comprehensive index method were applied in turn to evaluate the longitudinal changes in medical efficiency at the sample hospitals before and after the medical reform.Results The five sample hospitals in Sanming were found with less days of stay in average,and three were found with year-by-year rise of medical efficiency index,with one of them rising from 1.57 to 2.42.Conclusions Sanming′s health care reform has effectively improved internal operational efficiency at the sample hospitals,and has well addressed the relationship of medical service efficiency and scale expansion.On the other hand,the relationship between work intensity and medical efficiency needs to be further balanced,resource allocation to be further optimized,and more priority to be put in the development of TCM hospitals.
6.Cloning of telomere-associated protein T-STAR and the interaction between T-STAR and hTERT

Objective To clone the telomere-associated protein T-STAR and study the relationship between T-STAR and the telomerase catalyzed subunit hTERT in mammalian cells. Method The expression vector pGBKT7-hTERT was constructed and acted as a decoy in cDNA library screened by yeast two-hybrid technology. Recombinant vectors pVP16-T-STAR and pM-hTERT were constructed and co-transfected with report gene CAT into SGC-7901 cells with liposome. pM-53+pVP16-T and pM3-VP16 were introduced as positive controls, pM-53+pVP16-CP as negative control, and pM-hTERT+pVP16, pM+pVP16-T-STAR and pM+pVP16 as crossing negative controls. The expression of CAT was assayed by ELISA. Results The eukaryotic expression vector pGBKT7-hTERT was successfully constructed. One positive clone achieved by cDNA library screening was sequenced and compared with the isogenous sequences in GenBank by Blast software via Internet. As a result, T-STAR, a recorded cDNA sequence was obtained. The recombinant vectors of pVP16-T-STAR and pM-hTERT were constructed successfully and co-transformed into gastric cancer cells SGC-7901. The OD value of reported gene CAT was 0.258, which was significantly higher than that of the negative and crossing negative controls (0.002-0.015). It revealed that T-STAR interacted with hTERT in the mammalian cells. Conclusions T-STAR interacts with hTERT in the gastric cancer cells. T-STAR may be a new member of telomere-associated protein, and it participates in the regulation of telomerase through hTERT.
7. Research progress in miRNA, oligodendrocytes and Alzheimer's disease

miRNA, regarded as an important negative gene regulator at the post-transcriptional level, plays a critical role in oligodendrocyte differentiation and pathogenesis of Alzheimer's disease. Myelinating oligodendrocytes are important for the conduction of action potentials, synaptic plasticity and also for cognitive deficits of Alzheimer's disease. Application of miRNA, especially oligodendrocyte-specific miRNAs in studying the pathogenesis of Alzheimer's disease, will provide a new insight for the drug development and clinical diagnosis of the disease. In this paper we review the research progress in miRNA, oligodendrocytes and Alzheimer's disease.
8.Construction expression and activity evaluation of a novel fusion toxin hIL15M/TNF?M

Objective To construct and express fusion toxin human IL15 mutant/TNF? mutant (hIL15M/TNF?M) in order to develop a targeted toxin capable of eliminating abnormal cells expressing high levels of hIL15 receptors. Methods DNA fragments of truncated hIL15M and TNF?M coding gene were joined in frame and cloned into T7 expression vector pET16b. The expressed fusion protein was purified from E. coli . Cytotoxicity of hIL15M/TNF?M was measured by MTT assay. Results E. coli transformed with the recombinant plasmid expressed an extra band with the expected size of 30?10 3. MTT assay indicated that PHA activated T cells were 13 times more sensitive to the cytotoxic effect of hIL15M/TNF?M than to the resting T cells. Conclusion hIL15M/TNF?M has high toxic effect against cells expressing high levels of IL15 receptors, suggesting that hIL15M/TNF?M has potential therapeutic effect on T cell leukemia and other diseases related with improperly activated T cells.
9.Psychosocial factors and the development of breast cancer

Objective To investigate the impacts of psychosocial factors on the development of breast cancer.Methods Thirty-seven patients with confirmed breast cancer and 37 healthy women were enrolled in this study to complete psychological assessment,including ENRICH marital inventory,Life Event Scale(LES),Social Support Rate Scale(SSRS),and Trait Copying Styles Questionnaire(TCSQ).Results In comparison with the healthy controls,the patients experienced more stress events and negative response,and lowered quality of marriage over the last 5 years.The logistic regressive analyses indicated that the important contributors to breast cancer were stress events,objective social support and negative coping styles.Conclusion Stress events,insufficient access to available psychosocial support and negative responses are important risk factors of breast cancer in women.
10.Pharmaceutical Care for Anti-infective Treatment of A Case of Septic Shock

OBJECTIVE:To investigate the role of clinical pharmacists in anti-infective treatment for the patient with septic shock. METHODS:Clinical pharmacists participated in anti-infective treatment for a patient with septic shock,assisted physicians to formulate individual regimen based on empirical anti-infective treatment,ESBLs producing Escherichia coli and anti-fungal treat-ment. Clinical pharmacists suggested using Itraconazole injection 200 mg,ivgtt,qd,for anti-fungal treatment;Itraconazole cap-sules 200 mg,po,bid,instead of fusidinic for anti-fungal treatment;meropenem 0.5 g,ivgtt,q6 h,for anti-infective treatment. RESULTS:Physicians adopted the suggestion of clinical pharmacists;the infection had been controlled after 16 d treatment and then the patient discharged from the hospital. CONCLUSIONS:Clinical pharmacists participate in clinical treatment,and assist phy-sicians to make a decision and formulate individual regimen,so as to promote rational drug use.
