1.Clinical observation on 36 patients with diabetic peripheral angiopathy after autologous hemopoietic stem cell transplantation

Objective To observe the clinical effectiveness and prognostic factors of autologous hemopoietie stem cell transplantation on diabetic peripheral angiopathy patients under different degrees of vascular lesions. Methods Thirty-six patients with diabetic were divided into mild vascular stenosis group (group A: 20 lower extremities, vascular stenosis less than 50% ), moderate vascular stenosis group(group B :26 lower extremities, vascular stenosis of 50%-70% ) and severe vascular stenosis group (group C: 25 lower extremities, vascular stenosis greater than 70% ) according to the degree of vascular lesions. All patients were given the stem cell mobilization for 5 days, then peripheral hemopoietic stem cells were gathered on the last day and transplanted to ischemic limb by using monoeyte stem cell suspend liquid with 3 cm × 3 cm intervals.Each index of three groups was regularly observed ( 1th,12th and 18th month respectively after the operation)and comprehensive evaluated. Meanwhile,the transplantation prognostic factors (age, disease duration, post-prandial 2 hours blood glucose, glycated hemoglobin, blood pressure, triglycerides) were analyzed by multiple regression. Results Twelve months after transplantation, the ankle brachial index (ABI) and percutaneous oxygen pressure (TcPO2) of all three groups increased compared with that before the transplantation (P<0.05). In the 18th month after transplantation, the ABI still increased in group A and B (P< 0.05), and no significant changes were found in group C compared with that in the 12th month. Eighteenth month after transplantation, lower extremities of 8 patients (40%) in group A, 8 patients (31%) in group B and 6 patients (24%) in group C were examined to have neovascularizatian. Age, disease duration, blood pressure, post-prandial 2 hours blood glucose, glycated hemoglobin and triglycerides were all the important factors that affected diabetic peripheral angiopathy recurrence after transplantation. Conclusions The long-term effica-cy of stem cell transplantation on diabetic peripheral angiopathy patients at a mild to moderate degree is superior to that with severe vascular lesions. Close monitoring of blood glucose, blood pressure and triglycerides in the operation perinatal stage is required.They are the important prognostic indicators.

3.Research progress on relationship between microRNA and gastric cancer

Gastric cancer has a high incidence and accounts for a notable proportion of global cancer mortality.The pathogenesis of gastric cancer is complex involving multiple genetic and epige-netic alterations factors.In recent years,studies have also dem-onstrated the aberrant regulation of miRNAs in a variety of tumor pathological processes including the gastric cancer through af-fecting cell proliferation,differentiation and apoptosis.miRNA also plays important roles in gastric cancer growing,invasion, metastasis,and drug resistance of gastric cancers,which might become new biomarkers and potential therapeutic targets in the future.This paper summarizes several important miRNAs in-volved in gastric cancer and also discusses their function.
4.Fabrication of tissue engineered bone by seeding osteoblasts into titanium coral scaffolds

砄bjective:To fabricate bone tissue that has similar structural and mechanical characters with normal bone.Methods: Titanium meshes were molded into the shape of column in the length of 12 mm and in the diameter of 8 mm. The column was filled with natural coral granduls.4?10 7 marrow derived osteoblasts in 200 ?l cell culture medium were seeded into each of five scaffolds and incubated in vitro for 2 d to ensure that cells adhere well on the scaffolds. Then the scaffolds were implanted subcutaneously into the back of nude mice. Two months after implantation, the animals were sacrificed and the implanted materials were investigated by gross specimen inspection, X ray examination and histological observation. Results:2 months after in vivo incubation, the newly formed tissue was red and had the gross appearance of bone, and kept the original shape of column. Titanium mesh situated in the surface area. X ray examination showed that large amount of new bone formed in the scaffolds, there was no space between new bone and titanium mesh. Most of coral granduls had been absorbed. Histological observation demonstrated that in the surface area, new bone integrated well with titanium mesh and was enforced by titanium mesh(like cortical bone), and in the middle area large amount of lamellar bone formed.Conclusion: Newly formed bone in this experiment has similar structural with normal cortical bone.
5.Construction expression and activity evaluation of a novel fusion toxin hIL15M/TNF?M

Objective To construct and express fusion toxin human IL15 mutant/TNF? mutant (hIL15M/TNF?M) in order to develop a targeted toxin capable of eliminating abnormal cells expressing high levels of hIL15 receptors. Methods DNA fragments of truncated hIL15M and TNF?M coding gene were joined in frame and cloned into T7 expression vector pET16b. The expressed fusion protein was purified from E. coli . Cytotoxicity of hIL15M/TNF?M was measured by MTT assay. Results E. coli transformed with the recombinant plasmid expressed an extra band with the expected size of 30?10 3. MTT assay indicated that PHA activated T cells were 13 times more sensitive to the cytotoxic effect of hIL15M/TNF?M than to the resting T cells. Conclusion hIL15M/TNF?M has high toxic effect against cells expressing high levels of IL15 receptors, suggesting that hIL15M/TNF?M has potential therapeutic effect on T cell leukemia and other diseases related with improperly activated T cells.
6.Investigation on Weight Variations and Microbial Contaminations of5Tablets Broken off with Hands

OBJECTIVE:To improve medication safety and efficacy.METHODS:5different tablets broken off with hands were investigated with respect to weight variations and microbial limits by reference to Chinese Pharmacopeia.RESULTS:The5kinds of tablets broken off with hands were not in conformity with Chinese Pharmacopeia with respect to weight variation and which involved different degrees of microbial contaminations.CONCLUSION:It is recommended that pharmaceutical enter?prises should add more dosage forms of standard drugs that suitable for the use of children,women,the aged and particular patients.If the separately packaged drugs must be broken off with hands,aseptic method should be applied in which the drugs were broken off by the pharmacists on the sterile air bench before being separately packed with sterilized plastic pouches(which were sterilized with anprolene),and which should be administered to the patients for taking during the same day.
7.Clinical characteristics and gene mutation analysis of cerebrotendinous xanthomatosis

Objective To investigate the clinical presentation,laboratory features,imaging findings and CYP27A1 gene mutations of cerebrotendinous xanthomatosis (CTX) for improving the recognition and the early diagnosis and treatment of the disease.Methods Medical records and 8 months follow-up data of one patient who had been clinical diagnosed as CTX were collected and the pedigree and gene mutation analysis of the patient were carried out.Meanwhile,the clinical characters of CTX were analyzed according to the data from our patient and the review of the literature. Results Patient was a 36 years old male manifested with mental retardation, bilateral corticospinal tract and corticonuclear tract impairment,cerebellar lesions and peripheral neuropathy; head MRI indicated symmetric abnormal signals of bilateral basal ganglia,cerebellar dentate nucleus softening and calcification lesions; Achilles tendon MRI indicated markedly thickened Achilles tendon; gene mutation analysis showed sterol-27-hydroxylase gene( CPY27A1 )C→T homozygous mutation in 1016 nucleotide of exon 5.Ursodesoxycholic acid was given as treatment.In 8 months of follow up,for the first 6 months,the patient took medicine regularly and the illness condition was stable.But for the nearly 2 months,the patient voluntarily stopped medicine and the illness condition was worse.Conclusions CPY27A1 gene C→T homozygous mutation in 1016 nucleotide of exon 5 leads to CTX in the patient, which conforms to the characteristic of autosomal recessive disorder. CTX has some characteristic clinical manifestations,such as Achilles tendon thickening,intelligent declining and so on.But lack of specificity of early radiographic examination makes CTX easy to be delayed diagnosis and treatment.CYP27A1 gene mutation analysis has an important significance for early diagnosis of CTX,which should be paid more attention,while the early application of chenodeoxycholicacid treatment can delay the progression of the disease.
8.Analysis of nerve conduction velocity and quantitative sensory test in patients with early diabetic peripheral neuropathy

Objective To observe the characteristics of electro-neurophysiology in diabetic patients with early peripheral neuropathy. Method The nerve conduction velocity ( NCV ) and quantitative sensory test (QST) were examined in 175 diabetic patients without clinical peripheral neuropathy and in 50 normal subjects, and their results were compared. Results In 175 diabetic patients, the abnormal ratio of NCV was 7% (13/175).While using QST,the abnormal ratio was 45% (79/175). There was significant difference between two methods (P< 0.01). Compared diabetic patients with normal subjects,there was no significant difference between motor conduction velocity and sensory conduction velocity (P > 0.05 ).While using QST, the threshold values of cold sensation, warm sensation, and thermalgesia showed significant difference between diabetic patients and normal subjects (P< 0.01). Conclusion QST is more sensitive than NCV in diagnosing diabetic peripheral neuropathy, which suggests that small nerve fibers are more likely to be damaged than large fibers in early stage.
9.Treatment of cystic craniopharyngiomas by CT-guided stereotactic neuroendoscopic resection and intratumoral chemotherapy.

Objective To investigate the treatment of cystic craniopharyngiomas by CT-guided stereotactic neuroendoscopic resection and intratumoral chemotherapy. Methods 16 cases of cystic craniopharyngiomas were partial resected by CT-guided stereotactic neuroendoscopy. Intratumoral chemotherapy with bleomycin were given postoperatively. Results The clinical symptoms improved promptly after evacuations of cyst in all patients. No death or severe complications occurred. Follow-up (ranged from 2 to 3 years) CT or MRI indicated that the tumor cysts gradually regressed or disappeared. Conclusions The treatment of CT-guided stereotactic endoscopic resection and intratumoral chemotherapy for cystic craniopharyngioma is safe and effective, which should be a very useful procedure in clinical practice.
