Aim To study the effects of totle flavone of allium cepa L.var agrogatum Don(TFAD)on the blood-liquid metabolism in experimental hyperlipoproteinemia rats. Methods The model of experimental hyperlipoproteinemia was established by giving hyperlipid diet to Wistar rats. After the model was established, the rats were treated by TFAD.Then TC、TG、LDL-C、HDL-C of serum,MDA and SOD activties of serum and liver,TXA2 and PGI2 of plasma, blood viscosity were mesured, and pathology of liver was observed. Results After treated by TFAD(25~100 ?g?g-1 po for 21 days),TC、TG、LDL-C、AI、TC/HDL-C and MDA were reduced remarkably,HDL-C and the activity of SOD increased,PGI2 in plasma rose while TXA2 decreased, the blood viscosity of whole blood was depressed significantly, meanwhile the fatty degeneration of hepatic cells was relieved. Conclusions The results suggest that TFAD can adjust the blood-liquid metabolism of experimental hyperlipoproteinemia rats and restrain fatty deposition in liver which may be related to TFAD's function of preventing lipoprotein peroxidization, maitaining physiologic equilibrium of PGI2/TXA2,and ameliorating the abnormity of blood rheology.
2.Current research on adherence to antiviral therapy in patients with chronic hepatitis B
Journal of Clinical Hepatology 2015;31(10):1735-1739
In order to achieve the sustained suppression of hepatitis B virus replication, the long-term and even life-long antiviral therapy is needed for patients with chronic hepatitis B (CHB). So the adherence to antiviral therapy is one of the critical factors for achieving effective therapy. In this article, the current studies on the adherence to antiviral therapy in patients with CHB are reviewed. The rate of patients′ adherence to antiviral therapy and its impact on treatment outcome are analyzed, and the influencing factors for adherence are summarized. The existing problems in current studies on adherence are also analyzed. It is proposed that the attention should be focused on how to improve medication adherence so as to improve the prognosis in the future.

Stent-assisted coil embolization is a common endovascular treatment for ruptured/unruptured intracranial aneurysms. Stent implantation process can damage vascular endothelium, activate platelet and coagulation cascade, and then increase the risk of thrombosis. In order to reduce the risk of postoperative embolism, antiplatelet therapy is required. Among them, aspirin combined with clopidogrel dual antiplatelet therapy is a commonly used strategy. For patients with low response to clopidogrel, tigrelol or cilostazol can be used as an alternative drug. Although the scheme has been considered to be effective and safe, it is still controversial.

Objective To assess the diet quality of pregnant women during the second trimester using the dietary inflammatory index ( DII) and to explore the correlation between the DII in second trimester of preg-nancy and preterm delivery. Methods A total of 253 women with singleton pregnancy in 16-20 gestational weeks who had received routine prenatal care between August 2014 and April 2015 at the First Affiliated Hospi-tal of Anhui Medical University were enrolled with cluster random sampling. The included women were asked to recall daily dietary intake in the 3 days prior to the survey. All dietary data were analyzed for energy and nutri-ents intake with a nutritional analysis software, followed by calculation of DII according to previous reports. Based on quartiles of the DII scores, the participants were divided into three groups, namely the anti-inflam-matory group (DII<-5.10), intermediate group (DII -5.10--2.55), and the pro-inflammatory group (DII>-2.55).The participants were followed up until delivery and the pregnancy outcomes were recorded. The relationship between the DII in second trimester of pregnancy and preterm delivery were analyzed. Results DII scores of the 253 pregnant women during the second trimester of pregnancy ranged from -7.913 to 3.872.The risks of preterm birth in the anti-inflammatory diet group, the intermediate group, and the pro-inflammatory diet group were 0, 1.6%, and 6.3%, respectively, with statistically significant differences among the groups (P=0.034).The higher DII scores (pro-inflammatory) were associated with higher inci-dence of preterm birth ( P<0.05 ) .Conclusion DII may be used to assess diet quality of pregnant women during the second trimester and to predict the risk of preterm birth.

OBJECTIVEThe effects of different nitrogen forms on the growth and photosynthetic characteristics of Prunella vulgaris were observed.
METHODWater culture experiments were carried out to observe the effects of NH4+: NO3- ratios(100:0, 75: 25, 50:50, 25:75, 0:100 in mmol x L(-1)) on the growth and photosynthetic characteristics of P. vulgaris.
RESULTThe leaf area, fresh biomass and P(n) of these cultivars increased with the increasing of NH4(+) -N: NO3(-) -N ratios, and they were found to be the highest in 25: 75 NH4(+) -N: NO3)-) -N. However, they decreased with the increasing NH4(+) -N: NO3(-) -N ratio further. P. vulgaris had a minimum leaf area, biomass, chlorophyll content and P(n) in pure ammonium group. The biggest chlorophyll and carotenoid contents were found in the ratios of NH4(+) -N to NO3(-) -N of 50: 50.
CONCLUSIONThe results indicated that properly increasing nitrate proportion could promote the growth and photosynthesis of P. vulgaris.

Objective:To evaluate the efficacy of ultrasound-guided superior laryngeal nerve block(SLNB) combined with intravenous anesthesia for improving pediatric fiberoptic bronchoscopy. Methods:Forty pediatric patients of either sex, aged 3-6 yr, of American Society of Anesthesiologists Physical Status classificationⅠor Ⅱ, with body mass index of 18-24 kg/m 2, undergoing fiberoptic bronchoscopy in Cangzhou Central Hospital in 2022, were divided into 2 groups ( n=20 each) by a random number table method: ultrasound-guided SLNB plus intravenous anesthesia group (group A) and topical anesthesia plus intravenous anesthesia group (group B). After sedation with dexmedetomidine and esketamine, ultrasound-guided bilateral SLNB was performed with 1% lidocaine 0.5 ml (for each side)in group A, and topical anesthesia was performed with 1% lidocaine in nasal and pharyngeal cavities in group B. After completion of the surgery procedure, propofol was continuously infused at 5 mg·kg -1·h -1 until completion of diagnosis and treatment. An increment of propofol 1 mg/kg was intravenously given if severe bucking or body movement occurred during operation. Mean arterial pressure (MAP), heart rate (HR) and SpO 2 were recorded on admission to the operating room (T 0), immediately after sedation (T 1), immediately after bronchoscopy entering the glottis (T 2), 5 min after start of treatment (T 3) and at the end of examination (T 4). The occurrence of intraoperative hypoxemia, HR <60 bpm, and MAP <50 mmHg were recorded, and the additional dose of propofol was recorded. The venous blood samples were collected at T 0 and T 4 to determine plasma cortisol concentrations by chemiluminescence. The surgeon′s satisfaction score was recorded. The complications of SLNB were also recorded within 2 h after operation in group A. Results:Compared with group B, HR was significantly decreased at T 2 and T 3, SpO 2 was increased, the intraoperative additional dosage of propofol and incidence of hypoxemia were decreased, and the surgeon′s satisfaction score was increased, and the concentrations of cortisol were decreased at T 4 in group A ( P<0.05). No HR<60 bpm and MAP<50 mmHg were found in two groups. No SLNB-related complications were observed after operation in group A. Conclusions:Ultrasound-guided SLNB combined with intravenous anesthesia is safer for pediatric fiberoptic bronchoscopy and can improve the analgesic effect and is more helpful in inhibiting intraoperative stress responses when compared with conventional anesthesia.
7.Unified consensus and evolution of the definition of acute-on-chronic liver failure:Seeking common ground while reserving differences
Journal of Clinical Hepatology 2024;40(11):2173-2176
The concept of acute-on-chronic liver failure(ACLF)has been introduced for nearly 30 years, and with extensive research on its pathogenesis, diagnostic criteria, and treatment strategies, related consensus statements and guidelines have been constantly updated in China and globally;however, there is still a lack of a unified definition of ACLF,and such differences in definition may inevitably hinder the application and implementation of various treatment methods, prognostic scoring systems, and clinical recommendations. In recent years, hepatology experts have continuously proposed methods to unify the definition of ACLF,seeking common ground while reserving differences and drawing on the strengths of various definitions,in order to achieve a more unified definition of ACLF.

Objective:To translate Lee's Visual Analogue Scale for Fatigue (VAS-F) into Chinese, and test its reliability and validity in patients with acute pancreatitis. Methods:We performed translation, back translation and cultural debugging of Lee's VAS-F to form the Chinese version of VAS-F. The expert consultation was used to evaluate the content validity of VAS-F. Convenient sampling was used to select 322 inpatients with acute pancreatitis in the Department of Hepatobiliary Surgery in Affiliated Hospital of Zunyi Medical University from November 2019 to December 2020. The General Information Questionnaire, the Chinese version of VAS-F and the Chinese version of the Functional Assessment of Chronic Illness Therapy-Fatigue (FACIT-F) were used for investigation. The discrimination test was used for item analysis, and the reliability and validity of the Chinese version of VAS-F were evaluated through Cronbach's α coefficient, content validity index, exploratory factor analysis, and Pearson correlation analysis. Results:The Chinese version of the VAS-F contained two subscales, namely the Fatigue Severity (13 items) and the Energy Level (5 items) . The Cronbach's α coefficient of the total scale was 0.843, and the Cronbach's α coefficients of the two subscales were 0.965 and 0.959, respectively. The scale-level content validity index of the total scale and the two subscales were 0.89, 0.85 and 1.00, respectively, and the item-level content validity index of each item was from 0.83 to 1.00. After exploratory factor analysis, only one common factor was extracted from the two subscales, and the variance contribution rates were 71.196% and 86.135%, respectively. The factor loads of each item of the two subscales ranged from 0.698 to 0.859 and from 0.852 to 0.906, respectively. The scores of two subscales were negatively correlated ( r=-0.604, P<0.01) , and the total score of Fatigue Severity subscale was negatively correlated with the FACIT-F score ( r=-0.897, P<0.01) . Among 322 patients with acute pancreatitis, there were 3 cases (0.93%) without fatigue, 41 cases (12.73%) with mild fatigue, 181 cases (56.21%) with moderate fatigue and 97 cases (30.12%) with severe fatigue. Conclusions:The Chinese version of VAS-F has good reliability and validity and can be used to measure the fatigue and energy level of patients with acute pancreatitis in China.

To investigate the effects of Mycobacterium tuberculosis heat shock protein 65(HSP65)on Treg/Th17 immune balance in ApoE-knockout(ApoE-/-)mice, ApoE-/- mice with a high-cholesterol diet were immunized with M. tuberculosis HSP65. Sera were obtained for measurement of anti-HSP65 antibodies by ELISA; the effect of administration of different antigens was investigated, respectively, using flow cytometry analysis on the number of CD4+CD25+Foxp3+Tregs and CD4+IL-17+ Th17; the production of cytokines(IL-10, TGF-β1, IL-17 and IL-21)by these cells were determined by ELISA; total plasma cholesterol(TC), triglyceride(TG), high-density lipoprotein cholesterol(HDL-C)and low-density lipoprotein cholesterol(LDL-C)levels were detected by biochemical autoanalyzer. Atherosclerotic lesions were measured by lipid deposition stained with oil red O. The results demonstrated that the levels of anti-HSP65 IgG antibodies were increased significantly in Mycobacterium tuberculosis HSP65-treated ApoE-/- mice, revealed obvious decrease in Treg number, Treg related cytokines(IL-10, TGF-β1)levels and significant increase in Th17 number, Th17 related cytokines(IL-17 and IL-21)levels, the levels of TC, TG, HDL-C and LDL-C did not change between groups, while the atherosclerotic lesions significantly increased. Results indicate that M. tuberculosis HSP65 could interrupt the Th17/Treg immune balance in ApoE-/- mice, suggesting a potential role in the formation and progression of atherosclerosis.
