Objective To observe the effects of passive movement on the functional outcome after occlusion of the middle artery in the brain and reperfusion, and to explore the molecular mechanisms involved. Methods Cerebral infarction models were established in rats using left middle cerebral artery occlusion ( MCAO). The survivors were randomly divided into a passive movement group and a natural recovery group. There was also a sham-operated group and a normal group. Passive movement treatment (twice a day, twenty min per time) was started at different times after reperfusion. The expression of brain-derived neurotrophic factor (BDNF) and B cell lymphoma/leukemia-2 gene (Bcl-2) were determined using real-time PCRs. Results Expression of BDNF and Bcl-2 was detected a-round the infarction area in both groups. The expression of BDNF and Bcl-2 was highest in the sub-groups where passive movement was begun 24 or 48 h after the operation. Conclusions The expression of BDNF and Bcl-2 in the brain peaks when daily, moderate intensity passive movement is administered beginning 24 to 48 h after reperfusion. Passive movement might have a protective and rehabilitative effect after cerebral infarction.

Objective To investigate the risk factors for postoperative nausea and vomiting (PONV) in spinal anesthesia patients.Methods A total of 841 patients received spinal anesthesia were visited after operation.Data were analyzed using univariate analysis and multivariate Logistic regression to identify risk factors related to PONV.Results PONV occurred in 94 patients (11.2％,94/841).Univariate analysis showed that PONV was unrelated with gender,age,ASA classification,anesthesia mode (P ＞ 0.05),related with operation department (P =0.026),body mass index (P =0.020),education level (P =0.000),history of previous surgery anesthesia (P =0.005),history of PONV (P =0.000),history of kinesia (P =0.002),smoke (P =0.019),intraoperative using of tramadol (P =0.018).Multivariate analysis showed that operation department (OR =4.039,95％ CI 1.331-12.259,P=0.048),education level (OR =3.504,95％ CI 1486-8.260,P=0.015),history of PONV (OR =5.113,95％ CI 1.790-14.606,P =0.002),intraoperative using of tramadol (OR =5.316,95％ CI 1.091-25.908,P =0.039) were identified as independent risk factors for PONV.Conclusions The independent factors associated with PONV following spinal anesthesia include operation department,education level,history of PONV,intraoperative using of tramadol.Identifying patients who are at high risk for PONV will enable the formation of more timely management project.

Objective To isolate and purify crude lily polysaccharide and observe anti-tumor activity of the isolated and purified polysaccharide.Methods Crude lily polysaccharide was extracted from lily by water extraction and ethanol precipitation.After dialyzing,de-proteining and freeze-drying,preliminary purification of crude lily polysaccharide was obtained.After being separated by anion-exchange chromatography,preliminary purification was further isolated and purified.Purified lily polysaccharide was given to the H22-bearing mice.The effects of purified lily polysaccharide on tumor weight and immune function were evaluated.Results Crude lily polysaccharide was isolated and purified successfully.After a Sephadex chromatography,the purified polysaccharide showed as a single peak,demonstrating its homogenicity.The purified lily polysaccharide showed an inhibitory effect on H22-bearing mice tumor growth.It significantly increased the weight of immune organs and improved the immune function of the mice.Conclusion The purified lily polysaccharide was homogeneous.It could inhibit H22 tumor growth and enhance non-specific immune functions in H22-bearing mice.This research has laid the foundation for further pharmacological study on lily polysaccharide.

Objective:To systematically evaluate the effectiveness of early enteral nutrition after hepatectomy of hepatic carcinoma in China.Methods:The database such as CNKI,VIP,WANFANG DATA and CBM were searched electronically to collect the randomized controlled trials about enteral nutrition (EN) versus parenteral nutrition (PN) after hepatectomy.Meta-analyses were performed using the RvmanS.3 software.Results:Seven randomized controlled trials involving 460 patients were included.The analysis showed that the EN had significantly higher levels of albumin (WMD =1.17,95％ CI:0.45 ～ 1.89,P =0.001) and lower levels of aminotransferase (WMD =-3.95,95 ％ CI:-7.08 ～-0.81,P =0.O1) than those in the control group.The time of gastrointestinal function recovery in EN group was shorter than that in PN group.There was no significant difference in body weight and adverse reactions of gastrointestinal tract.Conclusion:Early enteral nutrition support for patients after hepatectomy of hepatic carcinoma shows certain advantages,which can improve the nutrition status and be in favor of the recovery of liver and gastrointestinal function.

Objective To study the effect of transcranial magnetic stimulation (TMS) on the rehabilitation of rats with cerebral infarction. Methods One hundred Sprague-Dawley rats were randomly divided into a normal group, a sham-operated control group, a model group and a TMS group with 25 rats in each group. A cerebral infarction model was established in the latter two groups by left middle cerebral artery occlusion (MCAO). TMS was started at either 12 or 24 hours after reperfusion, and sham-TMS was given to the first two groups at the same time points. The expression of Bcl-2 mRNA and BDNF mRNA were measured by RT-PCR after 14 days. Results Bcl-2 mRNA and BDNF mRNA were detected in all groups. The expression of Bcl-2 mRNA in the TMS-12 h group, and that of BDNF mRNA in the TMS-24 h group were significantly higher than in the other groups. Conclusions The expression of Bcl-2 mRNA and BDNF mRNA in the brains of rats after cerebral infarction peak when TMS is administered 12 h and 24 h after reperfusion, respectively. TMS might have protective and rehabilitative effects on rats after cere-bral infarct.

Objective To study the effect of electroacupuncture (EA) combined with transcranial magnetic stimulation (TMS) on the expression of the B-cell lymphoma/leukemia-2 gene (Bcl-2) and brain derived neurotrophic factor (BDNF) after cerebral infarction. Methods One hundred Sprague-Dawley rats were randomly and equally divided into a normal group, a sham-operated control group, a model group and an EA plus TMS group. A cerebral infarction model was established in the latter two groups using left middle cerebral artery occlusion (MCAO). Five-member subgroups of the EA plus TMS group were then treated at 6, 12, 24,48 and 72 hours after reperfusion. Sham EA plus TMS was given to similar sub-groups from the other groups at the same time points. The expression of Bcl-2 mRNA and BDNF mRNA were measured using a RT-PCR at the 14th day. Results Positive expression of Bcl-2 mRNA and BDNF mRNA was detected around the infarction in all groups. The average expression of both was significantly higher in the EA plus TMS group than in the model group. Bcl-2 mRNA peaked when the therapy was administered at 24 hours and BDNF mRNA at 48 hours.Conclusions The expression of Bcl-2 mRNA and BDNF mRNA is maximized when EA plus TMS is administered 24-48 hours after cerebral infarction. EA plus TMS does have protective and rehabilitative effects on rats after cerebral infarction.

Objective To analyze the ultrasonographic appearance of thyroglossal duct cysts and thyroglossal duct ifstula, and disicuss the value of ultrasonography in the diagnosis of thyroglossal duct cysts and thyroglossal duct ifstula. Methods Totally 142 cases of surgically and pathologically conifrmed thyroglossal duct cysts and thyroglossal duct ifstula cases in Beijing Children′s Hospital Afifliated to Capital Medical University from 2005 to 2012 were retrospectively reviewed and the ultrasonographic characteristics were analyzed. Results Among 142 cases, 128 cases of thyroglossal duct cyst were diagnosed by ultrasound. The ultrasonographic manifestation generally presented as anechoic and uneven medium echo cystic masses, with posteria wall echo enhancement. Most of the cyst walls were thin, and close to the thyoid, with or without deep ifstula. If the cysts were associated with infection, the wall would be thickening and echo enhancement of the surrounding soft tissue could be seen. Cysts ranged from the root of tongue to the suprasternal fossa, mostly in midline level of the neck. Of all the 128 cases, 95 cases were single cyst, round in shape; 33 cases had irregular shape with a fistula extended deeply, 13 cysts extended to the hyoid trailing edge, 20 cases extended to the mouth lfoor. Ultrasound examination revealed ifstulas in 10 cases, which presented as a hypoechoic thin strip-like echo inside the neck soft tissue, 6 cases extended to the hyoid bone direction, 2 cases extended to the mouthlfoor, 2 cases extended to the front thyroid. The other 4 cases showed anterior hypoechoic area, without cyst or ifstula structure. Conclusions Ultrasound is valuable in detcting the size, position, internal echo of the cyst and sinus, as well as showing their relationship with the hyoid. It is an optimal choice for the diagnosis of thyroglossal cyst and thyroglossal ifstula.

AIM: To compare intraocular pressure (IOP) fluctuations measured at home and in the clinic over a 24-hour period.METHODS: A prospective investigational study.A total of 120 Chinese participants were selected from five communities in the Chengdu area.Patients underwent a clinical interview and IOP was measured both at home and in the clinic.IOP were measured at 8 a.m., 10 a.m., 12 a.m., 2 p.m., 4 p.m., 6 p.m., 8 p.m., 10 p.m., 2 a.m., 6 a.m.using the same pneumatonometer.Measurements were taken in the sitting position.RESULTS: The average 24-hour IOP measured in the clinic was slightly lower than that at home.The mean difference in 24-hour IOP measurements between home and clinic was 0.27 mmHg.The IOP fluctuation in the clinic was higher than at home (the mean difference was 0.01 mmHg).There was no statistically significant difference in the average 24-hour IOP measured at home vs in the clinic.The average IOP measured at 2 p.m.at home (16.04±5.95 mmHg) was significantly higher compared with the measurement in the clinic (15.43±5.16 mmHg) (P<0.05).The overall agreement between 24-hour IOP measurements made in the clinic and at home in diagnosis of primary open angle glaucoma was 85.0% (K coefficient: 0.68).CONCLUSION: The 24-hour IOP measured in the clinic was similar to that measured at home, and the method of measuring IOP in the clinic is acceptable in diagnosing primary open angle glaucoma.

This study examined the effect of electro-acupuncture (EA) combined with transcranial magnetic stimulation (TMS) therapy at different time windows on learning and memory ability of rats with cerebral infarction and the underlying mechanism. Two hundred SD rats were randomly divided into four groups: normal group, sham-operated group, model group and EA+TMS group, and each group was then divided into five sub-groups in terms of the different time to start treatment post operation: 6, 12, 24, 48 and 72 h. Cerebral infarction models were established in the model and the EA+TMS groups by left middle cerebral artery occlusion/reperfusion (MCAO/R). After treatment for 14 d, the Morris water maze test was applied to examine the spatial learning and memory abilities of rats. In infarcted area, the expression of caspase-3 was immunohistochemically detected, and real-time fluorescent quantitative PCR was used to measure the expression of Bcl-2 mRNA. The results showed that in EA+TMS group compared with model group at the same treatment time windows, the escape latency was substantially shortened, the expression of caspase-3 was considerably decreased and the expression level of Bcl-2 mRNA significantly increased (P<0.05). In the EA+TMS sub-groups, the escape latency was shortest, the expression level of caspase-3 lowest, and the expression level of Bcl-2 mRNA highest at the treatment time window of 24 h. It was concluded that EA combined with TMS can promote neurological function of rats with cerebral infarction by increasing the expression level of Bcl-2 mRNA and decreasing the expression of caspase-3. The best time window is 24 h after perfusion treatment to ischemia.
