1.Clinical feature and treatment of the penetrating renal trauma

Objective To study the clinical features of penetrating renal trauma. Methods From March 1996 to November 2002,28 patients (male,mean age 31) with penetrating renal trauma were treated,the clinical data of these were studied and reviewed. Results Nonoperative treatment was carried out in 6 patients of grade Ⅱ injuries were reconstructed and the other 2 patients were managed by embolization with an excellent renal salvage rate.In 12 patients with grade Ⅳ injuries,6 underwent embolization successfully,2 were successfully managed reconstruction,4 required nephrectomy.3 patients were of grade Ⅳ injuries,2 of whom underwent nephrectomy and 1 was died. Conclusions The condition of vascular injury should be considered in treatment of penetrating renal trauma.Emblization is a ideal treatment for the grade Ⅲ-Ⅳ penetrating renal trauma.
2.A study of the histopathological features of injured urethra managed by urethral realignment or suprapublic cystotomy

Objective To study the histopathological features of injured urethra managed by urethral realignment or suprapublic cystotomy and to sum up the experience on its treatment. Methods Urethral realignment or suprapublic cystotomy were carried out for 20 dogs as the instant management;tissue specimens from the two groups were studied using hematoxylin and eosin stain,immunohistochemistry and electron microscopy and the result has been studied. Results Regeneration of urethra was demonstrated in the urethral realignment group at 8 weeks after urethral injured whereas regeneration has been in the suprapublic cystotomy group.The average length of urethral emphraxis was 3.8 cm in the suprapublic cystotomy group whereas that of urethral stricture only 2.5cm in the urethral realignment group at 32 week. Conclusions Urethral realignment might be the treatment of choice for the instant managerment of urethral disruption as compared with suprapublic cystotomy.
3.Application of urine diversion in prostatectomy

Objective To evaluate the application of urine diversion in prostatectomy for hemostasis. Methods In 86 cases underwent suprapubic prostatectomy urine diversion was carried out.The prostatic bed was compressed by the inflated baloon of the urethral catheter.The operation time was recorded and the amount of blood loss both during and after the operation were measured.Postoperative blood hemoglobin and urodynamic parameters were compared with those before operation. Results The average operation time for the 86 patients was 45 min.The average blood loss during the operation was 140 ml and the average postoperative drainage 190 ml.Blood transfusion is not needed in all.The ureteral catheters for urine diversion were maintained for an average of 7.2 days and the urethral catheter for 8.6 days.Normal micturation was noted right after the removal of urethral catheter in all the 86 patients.Postoperative blood routine and biochemical assays were not significantly different from those before operation.The average maximum urine flow rate was 21.5ml/s.2 of the patients have experienced difficulty in micturation.Normal micturation was resumed on endoscopic urethrotostomy.No secondary bleeding or upper urinary tract infection has been noted. Conclusions The application of urine diversion is an effective means of hemostasis in prostatectomy.
4.Nasal primitive neuroectodermal tumor: a case report.

Primitive neuroectodermal tumor (PNET) is a small round cell malignancies, normally confined to the chest wall and soft tissues of children and young adults, and reports of this tumor existing in nasal cavity are scarce. We herein present a case of nasal PNET. A 35-year-old male patient presented with a chief complaint of stuffy nose was diagnosed with a primary nasal pPNET by the clinical, microscopic, immunohistochemical features. Since these tumors are highly aggressive and patient prognosis is typically very poor, early diagnosis, immediate surgical resection and re-resection if possible, early radiotherapy and chemotherapy and close follow-up are required. We also review literatures on pPNET.

5.Functions of human periodontal myofibroblast in vitro.

OBJECTIVETo investigate the functions of human periodontal myofibroblast (MFB) in vitro.
METHODSHuman periodontal fibroblast (hPDLFs) was cultured and induced to MFB by transforming growth factor-β1 (TGF-β1). MFB was denoted as the experimental group, whereas the hPDLFs was the control group. The groups were continuously cultured and harvested at 0, 12, 24, 48, and 72 h. The MFB marker α-smooth muscle actin (α-SMA) was examined by immunocytochemistry. The expression of fibronectin (FN) between MFB was examined by immunocytochemistry to detect the MFB contact relationship. The mRNA expression levels of α-SMA, collagen (Col) I, and Col III were measured by reverse transcription-polymerase chain reaction (RT-PCT) to analyze extracellular matrix secretion. The protein expression levels of α-SMA and Col I were also assessed by Western blot.
RESULTSThe experimental group had significantly higher α-SMA expression than the control group at 0 h (P < 0.001). A positive expression of FN was found between MFB. The experimental group had significantly higher expression levels of Col I and Col III than the control group at 24 h (P < 0.001).
CONCLUSIONHuman periodontal MFB presents a continuous, high expression of α-SMA. MFB could interact through FN. MFB is significantly capable of extracellular matrix secretion.

7.Biological tag effect of white blood cell count on left ventricular remodeling in patients after emergency percutaneous coronary intervention

ObjectiveTo study the relation between white blood cell(WBC) count and left ventricular (LV) remodeling after emergency percutaneous coronary intervention (PCI) in patients with acute myocardial infarction (AMI).MethodsA total of 117 ST segment elevation AMI patients having underwent emergency PCI were enrolled.WBC count,cardiac troponin Ⅰ (cTnI),high-sensitivity C-reactive protein (hs-CRP) and N-terminal pro-B-type natriuretic peptide(NT-proBNP) were obtained at admission before PCI.According to the WBC count level,patients were divided into normal WBC group(WBC count ≤ 10 ×109/L,60 cases) and elevated WBC group(WBC count> 10 × 109/L,57 cases).Two-dimensional echocardiography was applied after PCI.The relation between WBC count and LV remodeling prognosis including LV ejection fraction(LVEF),LV end diastolic diameter(LVEDD) and LV aneurysm were compared after AMI.ResultsAdmission NT-proBNP,hs-CRP and cTnI peak in elevated WBC group were higher than those in normal WBC group [ (2408.83 ± 3173.39) pg/L vs.(713.11 ± 636.82) pg/L,(39.64 ± 59.51) mg/L vs.(11.23 ± 14.14) mg/L,(107.76 ± 107.71) pg/L vs.(62.23 ± 87.79) pg/L,P <0.05].Admission WBC count was positively correlated with LVEDD and negatively correlated with LVEF(P <0.01 ).Patients with LV aneurysm had higher WBC count than those without LV aneurysm[ ( 12.59 ± 5.22) × 109/L vs. (9.27 ± 2.60) × 109/L,P =0.001 ].Multivariate analyses showed that admission WBC count ≥ 10.5 × 109/L was an independent determinant of LV aneurysm(OR =22.5,95% CI:2.69-187.83,P < 0.01 ),and this cut-off value yielded sensitivity of 76.9% and specificity of 69.7% respectively.Conclusion Admission WBC count may be considered as a prognostic biological tag in the prediction of the development of LV remodeling after emergency PCI in patients with AMI.
8.Effects of borderline personality disorder tendency and impulsivity on suicide ideation in counseling patients

Objective To explore the effects of borderline personality disorder (BPD) tendency and impulsivity on suicide ideation in counseling patients.Methods A total of 120 counseling patients were evaluated by the Personality Diagnostic Questionnaire (PDQ-4+),Barratt Impulsiveness Scale (BIS-11).All participants were also interviewed with self-made structured clinical questionnaire for suicide ideation.Re sults ①Patients with suicide ideation had significantly higher score of BPD tendency ((6.02 ± 2.15) vs (3.91±2.31),t=5.164,P<0.01),impulsivity ((72.05±8.58) vs (68.63±9.01),t=2.129,P=0.035) than patients without suicide ideation.There was still significant difference of BPD tendency between the two groups after controlling for gender(F=23.135,P>0.01) but there was no significant difference of impulsivity (F=3.536,P>0.05).②Logistic hierarchical regression analysis revealed that only BPD tendency (B=0.319;P=0.009;OR=1.375)had a significant role in the prediction of suicide ideation after controlling for gender and depressive diagnoses.Conclusion Compared with impulsivity,BPD tendency can better predict suicide ideation of counseling patients.
9.Effects of elevating cervical flap on tongue lymphatic mapping with blue dye tracking

Objective:To explore the significance of elevating cervical flap before vital-dye injection in identifying blue-stained nodes receiving lymphatic drainage in rabbit's tongue, so as to provide raw data for normalizing vital-dye injection in sentinel lymph node biopsy (SLNB) during tongue-carcinoma treatment. Methods: 64 adult rabbits were randomly divided into four groups according to the injecting sites: tongue-tip, tongue-lateral, tongue-center and tongue-root. Four doses of methylene blue-0.1, 0.2, 0.3 and 0.4 ml were chosen and injected into 4 tongues in every group, 2 of them were injected before and after the elevation of cervical flap respectively. Time was recorded according to the time points of colored and discolored lymphatic duct and nodes. Results:①The overall success rate in localizing blue-stained node was 93.75%(60/64). ②In the success cases in group elevating cervical flap just before injecting methylene blue, all experiments had the time records of lymphatics and nodes' turning blue; while the counterpart group did not show any blue staining. ③All the blue stained nodes were found in lymph nodeⅠto Ⅲ local area. Conclusion:t may be dependable to utilize methylene blue as tracer in the tongue carcinoma SLNB. Elevation of cervical flap before injecting methylene blue is necessary and practicable in tongue carcinoma SLNB.
10.Regulation of metabotropic glutamate subtype 7 and 8 receptors in nucleus tractus solitarius in cardiac nociception in rats

Objective To explore the role of metabotropic glutamate receptors (mGluRs)group Ⅲ and its subtypes mGluR7 and mGluR8 in nucleus tractus solitarius(NTS)in cardiac-somatic motor reflex (CMR),and to clarify the modulation role of mGluR Ⅲ and its subtypes in NTS in cardiac nociceptoion.Methods 40 SD rats were randomly divided into L-AP4 group,microinjection of mGluRs Ⅲ agonist L-AP4 0.1,1.0,10.0 or 20.0 nmol in NTS;AMN082 group,microinjection of mGluR7 agonist AMN082 1,2 or 4 nmol;DCPG group,microinjection of mGluR8 agonist DCPG 4, 6 or 8 nmol;MSOP group, microinjection of mGluR Ⅲ antagonist MSOP 20 or 100 nmol,20 nmol MSOP+410 nmol L-AP,20 nmol MSOP+2 nmol AMN082,20 nmol MSOP+6 nmol DCPG. The changes of CMR of the rats in various groups were observed.Results Compared with control,the CMR in L-AP4 and AMN082 groups was decreased (P<0.05);the CMR in DCPG group was increased (P<0.05);the CMR in MSOP group after injection of 20 nmol MSOP had no change (P>0.05);the CMR in MSOP group after injection of 100 nmol MSOP was increased (P<0.05);the CMR in MSOP group after injection of 20 nmol MSOP followed L-AP4 or AMNO82 had no change (P>0.05).Conclusion The group Ⅲ mGluRs in the NTS play an inhibitory role in cardiac nociception, and mGluR7 has anti-nociceptive effects while mGluR8 has pro-nociceptive effects.
