1.A clinical study on indomethacin for prevention of heterotopic ossification following surgical treatment of acetabular fractures

Objective To investigate the effect of indomethacin on prevention of heterotopic ossification (HO)after operative treatment of acetabular fractures.Methods Fifty patients with acetabular fractures received in our department operative treatment through Kocher-langenbeck(K-L)approach and oral administration of in- domethacin afer operation from February 2001 to August 2003.Forty-eight of them were successfully followed up for incidence of HO and their clinical functions were assessed.The results were compared with those of 40 patients who had been treated with the same operative procedures but without oral administration of indomethacin in our depart- ment from March 1993 to May 1998.The patients who could not tolerate the drug were not included.Results The follow-ups averaged 22.8 months(range,6 to 39 months).HO occurred in eight cases.The incidence of HO was 16.7%(8/48).According to Brooker evaluation of HO,five cases were rated as degreeⅠ,three as degreeⅡ, and none as degreeⅢorⅣ.The incidence of severe HO was 0.In the control group,the incidence of HO was 35.0%(14/40)and the incidence of severe HO was 10.0%(4/40).The differences were statistically significant (P<0.05).Conclusion Oral administration of indomethacin after operative treatment of acetabular fractures can inhibit HO.
2.Inhibiting effects of oxymatrine on hemorrhagic fever with renal syndrome virus infection in vivo and in vitro

Objective To study the effects of oxymatrine as inhibitor of hemorrhagic fever with renal syndrome virus (HFRSV) infection in vitro and in vivo.Methods In vitro studies,a dose of oxymatrine without cytotoxicity and 76-118 strain of HFRSV was taken to treat Vero cells in three ways:①After treated with oxymatrine for 48 h,Vero cells were attacked by HFRSV at dilution of 10-1 ~ 10-6,respectively for 24 h before changing to maintenance medium; ②Vero cells were first attacked by HFRSV of 10-1 ~ 10-6 dilution respectively,then oxymatrine was used for 48 h before changing to maintenance medium; ③Vero cells were attacked by HFRSV at dilution of 10-1 ~ 10-6 respectively,and meanwhile treated with oxymatrine for 48 h before changing to maintenance mcdium.Each dilution handled four porocytes,and four positive controls were set up at the same time.Indirect immunofluorescence assay (IFA) was performed to determine the inhibitory effect of oxymatrine in experimental group and positive control.In vivo studies,thirty 2-week-old hamsters,weighing about 30-40 g,were divided into experimental and control groups according to body weight,n =15.These aninals were inoculated intraperitoneally with HFRSV in 100TCID50(0.1 ml each); on days 4-13,0.1 ml of oxymatrine 1:100 were given to each hamster in experimental group daily by intraperitoneal injection,while the same amount of saline was given to the control ones.Lung tissue of hamsters was then dissected out to slice to be identified by immunofluorcscence stain.Results It was demonstrated that oxymatrine with the diluted fractions of 1:8 was safe in vitro.When the virus dilution of HFRSV was l0-4,compared with control groups,the differences were statistically significant in method 2 and 3 (z =-2.53,-2.53,all P < 0.05),while no statistical significance in method 1 (z=5.36,P> 0.05).When the virus dilution of HFRSV was 10-1 ~ 10-3,10-5,10-6,the differences were not statistically significant (z--0.00,-0.32,-0.19,4.21,4.21,all P > 0.05).In vivo studies,compared with control group,the differences were statistically significant in experimental group (z =-3.85,P < 0.05).Conclusion Oxymatrine significantly inhibites HFRSV.
4.Complications of operative treatment of acetabular fractures.

OBJECTIVETo improve the effect of operative management of acetabular fractures.
METHODSOne hundred and seventy eight acetabular fractures were treated operatively from August 1993 to December 2000. Their functional results and complications were analyzed.
RESULTSOne hundred and twelve hips were followed up for an average of 45.7 months. Heterotopic ossification was noted in 26 hips, post-operative osteoarthritis in 22 hips, avascular necrosis of the femoral head in 8 hips, and sciatic nerve injury in 7 hips after operations. No death and infection were found in this series.
CONCLUSIONSEctopic bone formation develops at extended ilio-femoral and Kocher-Langeneck approaches. Cartilaginous injury of the femoral head is contributable to post-operative osteoarthritis, and imperfect reduction is an important factor in the genesis of osteoarthritis.

5.Marginal impaction in acetabular fractures.

OBJECTIVETo understand the diagnosis and treatment of marginal impaction of acetabular fractures.
METHODEighteen of 26 patients with marginal impaction of acetabular fractures were retrospectively reviewed. Marginal impactions was confirmed in 15 patients by CT scan before operation, and in 3 during operation. All patients were treated by ORIF, and the impacted bones were elevated with bone grafts (Bone graft was not used in one patient). All fractures were fixed with a reconstruction plate.
RESULTSThe mean follow up was 36.7 months (5 - 71 months). The modified D'Aubingne and Postel score system showed excellent results in 6 patients, good 7, fair 2, and poor 3.
CONCLUSIONPosterior fractures and dislocations of acetabular fractures are easily combined with marginal impaction, and can diagnosed by CT scan. Cortical impaction should be recognized as a special type. The impacted bone should be elevated for bone grafting.

6.The application of computed tomography examination in acetabular fractures.

OBJECTIVETo study the clinical significance of computed tomography (CT) in diagnosis and treatment of acetabular fractures.
METHODSThe plain and CT films of the 66 cases of acetabular fracture from January 2001 to June 2002 were analyzed retrospectively, and the radiological characteristics and results were compared to each other.
RESULTSThe diagnosis of 6 cases were changed after CT examination, and the occurrence ratio of marginal impaction, free body in joint, femoral head fracture, sacro-iliac injury and ischial tubersoity fracture involvement were 3:17, 7:29, 6:15, 0:5 and 1:5 in plain and CT examination respectively.
CONCLUSIONSCT examination has the determinative role in detecting weight-bearing zone, marginal impaction, free body, femoral head fracture, sacro-iliac injury, etc., and for completing the diagnosis and guiding the treatment. It should be the routine examination just as the three standard plain examinations in acetabular fractures, and should be carefully read.

7.Integration of different T-DNA structures of ACC oxidase gene into carnation genome extended cut flower vase-life differently.

The cultivar 'Master' of carnation (Dianthus caryophyllus L.) was transformed with four T-DNA structures containing sense, antisense, sense direct repeat and antisense direct repeat gene of ACC oxidase mediated by Agrobacterium tumefaciens. Southern blotting detection showed that foreign gene was integrated into the carnation genome and 14 transgenic lines were obtained. The transgenic plants were transplanted to soil and grew normally in greenhouse. Of the 12 transgenic lines screened, the cut flower vase life of 8 transgenic lines is up to 11 days and the longest one is 12.8 days while the vase life of the control is 5.8 days under 25 degrees C. The vase life of 2 lines out of 3 with single sense ACO gene is same as that of the control, while the vase life of 3 lines out of 4 with single antisense ACO gene is prolonged. The vase life of cut flowers of 5 lines with direct repeat ACO genes is all prolonged by about 6 days, while the vase life of 3 out of 7 lines with single ACO gene is same as that of the control. During the senescence of cut flowers, the ethylene production of the most of the transgenic lines decreased significantly, and the production of ethylene is not detectable in lines T456, T556 and T575. The results of the research demonstrate that antisense foreign gene inhibits expression of endogenesis gene more significantly than sense one. Both sense direct repeat and antisense direct repeat foreign genes can suppress endogenous gene expression more significantly comparing to single foreign genes. The transgenic lines obtained from this research are useful to minimize carnation cut flower transportation and storage expenses.

8.Operative treatment of delayed acetabular fractures.

OBJECTIVETo assess the operative methods of delayed acetabular fractures and the operative results.
METHODSThe operative approaches, procedures, results and complications of the delayed acetabular fractures between August 1993 and August 2001 in Jishuitan Hospital were evaluated retrospectively.
RESULTSThirty-two patients were followed up 49.6 months on average. Sciatic nerve palsy was found in 1 patient. Excellent functional results of hip joints were found in 3 patients, good in 16, fair in 10, and poor in 3. Ectopic bone formation was observed in 6 patients and necrosis of the femoral head in 3.
CONCLUSIONCareful selection of operative indications of delayed acetabular fractures in combination with proper operative approach and appropriate reduction and fixation, good results can be obtained.

9.Compliance on hand-hygiene among healthcare providers working at secondary and tertiary general hospitals in Chengdu

Objective To evaluate the compliance on hand-hygiene and related factors among healthcare providers working at secondary and tertiary hospitals in Chengdu.Methods On-site observations regarding hand-hygiene compliance and facilities were conducted in 6 hospitals in Chengdu.Doctors and nurses were asked and recorded about their knowledge regarding hand hygiene.Results Of 1535 activities where hand-hygiene was deemed necessary,under observating healthcare providers would perform hand-hygiene procedures 17.8% of the time (12.8% of the time before touching a patient,21.0% of the time before touching objects around a patient,27.3% of the time after touching a patient,and 31.5% of the time after removing gloves).Only 2.2% of the treating rooms were equipped with foot-operated or automatic faucets; of these only 24.5% had soap or alcohol-based hand-sanitizer,and 6.3% had paper towel or other hand-drying equipments.92.8% of the healthcare providers knew of the six-step method on hand-washing.More than 90.0% of the healthcare providers knew that both palm and back of the hands as well as the front and back of the fingers should be washed.However,only 22.8% knew that the hand-washing procedure should last ≥15 seconds.Rates on hand hygiene among chief or more senior physicians (14.6%),attending physicians (9.2%) and junior doctors (15.6%),nurses in chief (25.0%),senior nurses (26.3%) and junior nurses (20.5%) showed no significant differences (P>0.05).Similarly,scores on related knowledge between chief or senior physicians (12.4 ± 3.2),attending physicians (13.6 ± 3.3) and junior doctors ( 13.4 ± 2.9),nurses in charge ( 15.2 ± 2.0),senior nurses ( 14.8 ± 2.1 ) and junior nurses (14.3 ± 2.6) also showed no significant differences (P>0.05).Rate on hand hygiene among nurses (22.7%) was significantly higher than that of the doctors ( 13.6% ).Rate of hand hygiene among 50-59 years old healthcare providers (7.4%) was significantly lower than those of all the other age groups ( 17.1%-25.0% ) ; rate of female health care providers ( 19.5% ) was significantly higher than that of males ( 13.8% ).Similarly,the nurse' s knowledge score ( 14.7 ± 2.3) was significantly higher than that of the doctors ( 13.2 ± 3.1 ).Among 50-59 years old healthcare providers,the rate was ( 12.2 ± 3.8) significantly lower than that of 20-29 ( 14.0 ± 2.6),30-39 ( 14.3 ± 2.9) and 40-49 year olds ( 13.8 ± 2.7).Again,the knowledge score of females ( 14.5 ± 2.5 ) was significantly higher than that of males (12.7 ± 3.2) (P<0.05).Conclusion The high-level knowledge on hand-hygiene among healthcare providers in this area did not translate into good practices.Also,most of the hospitals had poor hand-hygiene equipments.We recommend that training and periodic monitoring be conducted,and hand-hygiene equipment be improved to facilitate hand-hygiene practices among healthcare providers.
10.Characterization of diterpenoids in the bark of Pseudolarix kaempferi by HPLC-ESI/MSn.

Fragmentation behavior of diterpenoids was investigated by ESI/MSn and the qualitative analysis of diterpenoids in the bark of Pseudolarix kaempferi was performed using high-performance liquid chromatography/ multi-stage mass spectrometry (HPLC-ESI/MSn). The characteristic fragmentation behaviors of the diterpenoids are the cleavages of the lactone ring and C4-O bond. Furthermore, the eliminations of substituent groups at C-18, C-7 and C-8 can also be observed in the MS" (n = 3-4) spectra. For C-4 acetoxy subsititued diterpenoids, [M+Na-60]+ and [M-H-104] are the base peaks of MS2 spectra in the positive and negative ionization modes, respectively. For C-4 hydroxyl subsititued diterpenoids, [M+Na-44]+ and [M-H-62] are the base peaks of MS2 in the positive and negative ionization modes, respectively. For C-18 glucosylated or esterized diterpenoids, [M+Na-44]+ is the base peak of MS2 spectra in positive ionization mode. These fragmentation rules were successfully exploited in the identification of diterpenoids in methanol/water (6:4) extract of P. kaempferi by LC-MS in positive ionization mode. A total of 9 diterpenoids were identified or tentatively characterized, and one of them is reported here for the first time. The described method could be utilized for the sensitive and rapid qualitative analysis of P. kaempferi.
