1.Clinical Significance of Changes in NT-proBNP, Hcy and Partial Coagulation-Fibrinolytic Indexes with Acute Cerebral Infarction

Objective To Analyze the examination meaning of b-type brain natriuretic peptide precursor (NT-proBNP),homocysteine and coagulation-fibrinolysis indexes for patients with acute cerebral infarction.Methods Selected 40 patients with acute cerebral infarction (experimental group) to hospital from March 2014 to May 2015 and 40 healthy check-up cases (control group).Then,compared the indicators in blood between the two groups of patients,namely homocysteine (Hcy),NT-proBNP,activated clotting time live enzymes enzyme (APTT),original activator inhibitory factor (PAI-1) and tissuetype fibrinolytic enzyme original activator (tPA).Meanwhile,also compared these indicators for the experimental group before and after treatment.Results Before treatment,the levels of Hcy,t-PA,NT-proBNP,PAI-1 and ATPP for experiment group were 17.7±3.6 μmol/L,29.4±7.9 μmol/L,3 212.8±2 511.4 ng/L,130.1±17.8 μmol/L and 37.8±4.5 s,respectively.The levels of Hey,t-PA,NT-proBNP,PAI-1 and ATPP for control group were 7.2± 2.1 μmol/L,15.1 ± 3.7 μmol/ L,198.7 ± 1 14.8 ng/L,67.8 ± 7.9 μmol/L and 37.8 ± 4.5 s,respectively.After treatment,the levels of Hcy,t-PA,NT-proBNP,PAI-1 and ATPP for experiment group were 12.2±1.5 μmol/L,18.2±2.3 μmol/L,348.7±194.8 ng/L,78.6±9.8 μmol/L and 32.2±4.5 s,respectively.Before treatment,the indicator of APTT for experiment patients was significantly shorter than it after treatment and that of the control group.The other four indicators were significantly higher than them after treatment and those of the control group.The differences were statistically significant (P <0.05).Conclusion Hcy,NT-proBNP,APTT,PAI-1,and t-PA had closely relation with the occurrence of acute cerebral infarction development,and they can be helpful to evaluate disease progression and predict prognosis for patients with acute cerebral infarction.
2.Killing efficacy of a medical disinfection ultrasonic coupling agent on five kinds of multidrug-resistant organisms

Objective To observe the efficacy of a medical disinfectant ultrasonic coupling agent on the killing of five clinically isolated multidrug-resistant organisms(MDROs).Methods From March 2016 to May 2017,a disin-fection ultrasonic coupling agent containing active ingredient,including triclosan and propylene glycol,was used to conduct carrier quantitative germicidal test on five clinically isolated MDROs,the killing efficacy to MDROs was ob-served.Results After 1.5,3.0,and 4.5 minute disinfection time,the killing logarithms values of disinfection ultra-sonic coupling agent to five MDROs(multidrug-resistant Acinetobacter bau m annii[MDR-AB],methicillin-resistant Staphylococcus aureus[MRSA],multidrug-resistant Pseudomonas aeruginosa[MDR-PA],carbapenem-resistant Klebsiella pneumoniae[CRKP],and extended-spectrum β-lactamase Escherichia coli[ESBLs-EC])were all>3.0. Conclusion Medical disinfection ultrasonic coupling agent can effectively kill five common MDROs,and can take the place of disinfectant during ultrasonic examination.
3.Gene mutation analysis of X-linked hypophosphatemic rickets.

OBJECTIVETo investigate the frequency and type of PHEX gene mutations in children with X-linked hypophosphatemic rickets (XLH), the possible presence of mutational hot spots, and the relationship between genotype and clinical phenotype.
METHODSClinical data of 10 children with XLH was retrospectively reviewed. The relationship between gene mutation type and severity of XLH was evaluated.
RESULTSPHEX gene mutations were detected in all 10 children with XLH, including 6 cases of missense mutation, 2 cases of splice site mutation, 1 case of frameshift mutation, and 1 case of nonsense mutation. Two new mutations, c.2048T>C and IVS14+1delAG, were found. The type of PHEX gene mutation was not associated with the degree of short stature and leg deformity (P=0.571 and 0.467), and the mutation site was also not associated with the degree of short stature and leg deformity (P=0.400 and 1.000).
CONCLUSIONSMissense mutation is the most common type of PHEX gene mutation in children with XLH, and c.2048T>C and IVS14+1delAG are two new PHEX gene mutations. The type and site of PHEX gene mutation are not associated with the severity of XLH.

4.Correlation analysis between plasma D-dimer levels and orthopedic trauma severity.

BACKGROUNDThe correlation between the plasma D-dimer level and deep vein thrombosis has not been conclusive in various studies. The aim of this research was to study the relationship between plasma D-dimer levels and the severity of orthopedic trauma by retrospective examination of orthopedic trauma cases.
METHODSClinically acute trauma and non-acute trauma patients were selected and their plasma D-dimer levels were measured. Plasma D-dimer levels in patients of these two groups were compared. The relationship between the plasma D-dimer level and the severity of the trauma was also studied.
RESULTSThere were 548 cases in the acute trauma group and 501 cases in the non-acute trauma group. The levels of plasma D-dimer were significantly higher in the acute trauma group than in the non-acute trauma group (P < 0.01). In the acute trauma group, the correlation between the D-dimer level and the number of fractures was a positive linear correlation (r = 0.9532).
CONCLUSIONSElevated plasma D-dimer is common in trauma patients. The D-dimer level and the number of fractures in the trauma patients are closely correlated. D-dimer is not only an indicator for the diagnosis of deep vein thrombosis and pulmonary embolus, but also an indicator of the severity of trauma in acute trauma patients.

5.Study on the acupoints belonging to the three yin meridians of foot reflecting the variation pattern of uterine qi and blood in women with moderate constitution

Objective: To observe the blood perfusion volume variation pattern in the body surface microcirculation at the Yuan-Primary and the Xi-Cleft points during the menstrual cycle in female college students with moderate constitution under normal physiological state of the uterus; to explore the specific laws of the body surface microcirculation at the Yuan-Primary and Xi-Cleft points in response to the uterine qi and blood changes under normal physiological conditions, and to provide the experimental basis for the specificity of acupoints reflecting the uterine function. Methods: Forty-three healthy and moderate constitution female college students with regular menstrual cycles, without dysmenorrhea and not yet giving birth were recruited. Bilateral Yuan-Primary points [Taichong (LR 3), Taibai (SP 3) and Taixi (KI 3)] and Xi-Cleft points [Zhongdu (LR 6), Diji (SP 8) and Shuiquan (KI 5)], belonging to the three yin meridians of foot and adjacent to the spinal cord segment of the uterus, were selected as the detection acupoints; the crossing point of the three yin meridians of foot [Sanyinjiao (SP 6)], the uterus-related meridian acupoint [Xuehai (SP 10)], the uterus-non-related meridian acupoint [Xuanzhong (GB 39)], and the non-meridian non-acupoint point were selected as the control points. The laser speckle blood flow imaging technique was used to monitor the blood perfusion volume in skin microcirculation at the above points at the menstrual, follicular, ovulatory, and luteal phases of the subjects. Results: The blood perfusion volume in the body surface microcirculation at the right Zhongdu (LR 6) at the ovulatory phase was higher than that at the menstrual, follicular and luteal phases (all P<0.05); there was no significant difference in the microcirculation blood perfusion volume at the other points among different phases (all P>0.05). Conclusion: The blood perfusion volume in the body surface microcirculation at Zhongdu (LR 6), the Xi-Cleft point of the Liver Meridian, shows a specific response to qi and blood changes in the uterus of women with moderate constitution.
6.Efficacy of ibutilide for cardioversion of persistent atrial fibrillation during radiofrequency ablation.

OBJECTIVEThe aim of this study was to investigate the efficiency and safety of ibutilide for cardioversion of persistent atrial fibrillation (AF) during radiofrequency ablation.
METHODSEighteen patients (16 males) with persistent atrial fibrillation were enrolled in this study. All patients underwent circumferential pulmonary vein ablation guided by a Carto three-dimensional mapping system. In addition, linear ablation at the top of the left atrium and the isthmus of mitral valves and complex fractionated atrial electrogram (CAFE) ablation were performed. All patients were still in either atrial fibrillation or atrial flutter after ablation, the patients were treated with 1 mg intravenous ibutilide injection within 10 minutes after unsuccessful ablation. Intravenous injection was stopped in case of sinus rhythm (SR) restoration or occurrence of severe adverse reactions such as ventricular tachycardia. Cardioversion rate within 30 min and adverse reactions within 4 h were observed. Patients were divided into either conversion group or non-conversion group according to whether AF was converted to sinus rhythm within 30 minutes after injection.
RESULTSEleven patients (61.11%) converted to SR after ibutilide injection. There were no significant differences in gender, age, body mass index, left atrium and left ventricular function between conversion group and non-conversion groups. The average conversion time was (13.80 ± 7.64) min, left atrium scar area ratio was significantly larger in non-conversion group (12.40 ± 11.03)% than in conversion group (5.12 ± 3.83)%, P < 0.05. Ibutilide significantly prolonged the average wavelength of the AF wave (171.8 ± 29.5) ms vs. (242.0 ± 40.0) ms at baseline, P < 0.01. The QT interval at 30 min after ibutilide injection (0.39 ± 0.21) s was significantly longer than before injection (0.51 ± 0.08) s, P < 0.05. There was no serious arrhythmias or other adverse reactions post ibutilide injection.
CONCLUSIONSIbutilide is highly effective and safe agent for cardioversion in patients underwent unsuccessful ablation. Left atrium scar area ratio is an important determinant for the conversion rate in this cohort.

7.Clinical analysis and genetic diagnosis of short-limb inherited short stature diseases in children.

OBJECTIVETo analyze the clinical manifestations, bone X-ray findings and genetic analysis results of three short-limb inherited short stature diseases: achondroplasia (ACH), hypochondroplasia (HCH) and pseudoachondroplasia (PSACH).
METHODSThe clinical manifestations, bone X-ray findings, and genetic analysis results of 10 children with genetically confirmed short-limb inherited short stature diseases, including 4 cases of ACH 3 cases of HCH, and 3 cases of PSACH, were analyzed.
RESULTSThe 10 patients had a mean body height of -3.69±1.79 SD, a mean sitting height/standing height ratio of 0.65±0.03, and a mean finger spacing/body height ratio of 0.93±0.04. Four ACH cases and 3 PSACH cases showed typical bone X-ray findings; one HCH case showed a smaller sciatic notch, and another HCH case showed no widening of interpedicular distance. G380R mutation in FGFR3 gene was detected in 3 of 4 ACH cases, and Y278C mutation in the other ACH case, N540K mutation in FGFR3 gene was detected in 3 HCH cases, and heterozygous mutations in COMP gene were detected in 3 PSACH cases.
CONCLUSIONSChildren with ACH and PSACH have severer short stature and skeletal deformities than children with HCH, who have mild, atypical clinical manifestations. Bone X-ray and genetic analysis are helpful for the diagnosis and differential diagnosis of the three diseases. The mutational hotspots in two genes are involved in the three diseases, which is conducive to clinical genetic diagnosis.

8.The expressional alterations of CSF-1R after ischemic injury of cerebral cortex.

AIMTo observe the expressional alterations of colony stimulating factor-1 receptor (CSF-1R) after ischemic injury of cerebral cortex, and study the function of colony stimulating factor-1 (CSF-1)/CSF-1R signal during the process of ischemic injury and repair of central nervous system (CNS).
METHODSWe examined the distribution and expression of CSF-1R in normal brain tissues and ischemic brain tissues by immunohistology and Western blot analysis.
RESULTSThe expression of CSF-1R in neurons could be up-regulated by ischemic injury in CNS.
CONCLUSIONCSF-1/CSF-1R might take part in the process of ischemic injury and repair.

9.Serum hsCRP level and expression of CX3CR1 on peripheral blood mononuclear cells in patients with coronary heart disease and its clinical significance

Objective:To explore serum level of high sensitive C reactive protein(hsCRP)and expression of CX3C chemokine receptor 1(CX3CR1)on peripheral blood mononuclear cells(PBMC)in patients with coronary heart disease(CHD)and its clinical significance.Methods:A total of 153 CHD patients treated in our hospital from Mar 2016 to Mar 2017 were selected,including 50 cases with stable angina pectoris(SAP),52 cases with unstable angina pectoris(UAP)and 51 cases with acute myocardial infarction(AMI);according to coronary lesion degree assessed by Gensini score,153 CHD patients were divided into mild coronary disease group(n=43),moderate coronary dis-ease group(n=56)and severe coronary disease group(n=54).Another 45 healthy subjects were selected as healthy control group during the same period.Serum hsCRP level and CX3CR1 expression on PBMC were measured and compared among all groups,the correlation among serum hsCRP level,CX3CR1 expression and Gensini score were analyzed.Results:Compared with healthy control group,there were significant rise in serum hsCRP level[(2.36 ± 1.67)mg/L vs.(5.07 ± 2.16)mg/L vs.(13.59 ± 5.23)mg/L vs.(27.46 ± 8.24)mg/L]and CX3CR1 expression on PBMC[(0.48 ± 0.25)% vs.(11.13 ± 5.42)% vs.(20.56 ± 9.73)% vs.(37.49 ± 12.82)%]in SAP group,UAP group and AMI group,and AMI group> UAP group> SAP group,there was significant difference between any two groups,P=0.001 all.Compared with mild coronary disease group,there were significant rise in serum hsCRP level[(6.31 ± 1.83)mg/L vs.(17.42 ± 6.58)mg/L vs.(35.26 ± 9.74)mg/L]and CX3CR1 expression on PBMC [(6.59 ± 1.43)% vs.(25.86 ± 9.37)% vs.(42.57 ± 13.28)%]in moderate and severe coronary disease group,and those of severe coronary disease group were significantly higher than those of moderate coronary disease group,P=0.001 all.Linear correlation analysis indicated that serum hsCRP level and CX 3CR1 expression on PBMC were sig-nificantly correlated with Gensini score(r=0.768,0.897,P=0.001 both).Conclusion:Serum hsCRP level and CX3CR1 expression significantly rise in CHD patients along with the aggregation of coronary disease,which are ex-pected to be reference indexes predicting severity of CHD.
10.Influence of Anesthetic Brochure on Patient's Information Gain, Preoperative Anxiety, Trust, and Satisfaction.

Objective To assess the impacts of anesthetic brochure on anesthetic information gain, preoperative anxiety, trust to anesthesiologists, and satisfaction to the preanesthesia visit for patients undergoing general anesthesia. Methods Totally 134 patients scheduled for elective thyroid surgery under general anesthesia in Peking Union Medical College Hospital were assigned to two groups using the random number table method, among whom 68 patients received brochure before preoperative visit (brochure group) and 66 patients did not (control group). Questionnaires with items for evaluating patient's information gain, preoperative anxiety and trust were completed after preanesthetic visit. Patient's satisfaction with preanesthetic visit was evaluated on the second postoperative day. Results Compared with the control group, patients in the brochure group had significantly higher information gain scores (7.2±1.8 vs. 5.2±2.1, P<0.001) and satisfaction scores (25.0±3.4 vs. 22.7±3.1, P<0.001). There was no significant difference in anxiety scores and trust scores between these two groups. Conclusion Preoperative anesthetic brochure-assisted education can improve information gain and satisfaction among patients undergoing general anesthesia; however, it can not remarkably alter patient's preoperative anxiety and trust.
