1.Pathogen Distribution and Antimicrobial Resistance after Liver Transplantation

OBJECTIVE To evaluate the distribution of pathogens causing infection and current status of antimicrobial resistance in order to offer experimental data for clinical administration.METHODS The resistance tests were performed using K-B method.WHONET5.2 and EXCEL were used to analyze bacteria distribution and resistance.RESULTS From 119 cases of patients were earned the routine bacteria culture results,the tested rate was 96.75%,the positive specimens were 379 and the postitive rate was 83.85%.The strains of Pseudomonas aeruginosa were the most frequent organisms isolated then were the strains of Streptococcus pyogenes,coagulase-negative Staphylococcus,Nesseria spp,and Stenotrophomonas maltophilia.CONCLUSIONS It is important to strengthen antibiotics administration,bacteria isolation and resistance surveillance.
2.Experimental study of the injury of unilateral testicular torsion on the contralateral testes with contrast enhanced ultrasound

Objective To evaluate the injury of unilateral testicular torsion on the contralateral testes with contrast-enhanced ultrasound.Methods Twenty-eight male rabbits were enrolled in this study.They were randomly divided into the control group,the detorsion after 3 hours group,the detorsion after 6 hours group,the detorsion after 24 hours group and the permanent torsion group.There were 8 rabbits in the control group,and 5 rabbits in each of the other groups.Just before torsion,immediately after torsion,just before detorsion,immediately after detorsion,and delay(6-12 hours after detorsion.the permanent torsion group was also at the corresponding time),contrast-enhanced ultrasound was used to observe the perfusion of contralateral testes.The control group was also performed contrast-enhanced ultrasound just before the shame operation and immediately after that.The time-intensity curves were draw according to the contrastenhanced ultrasound images.The parameters including arrival time(AT),time to peak intensity(TTP),peak intensity(PI),half time of descending peak intensity(HT)were yielded automatically,and the changes of them were analyzed.Results①There was no significance of enhancement AT,TTP,PI,and HT between pre-operation and post-operation in the control group(P>0.05).②When the parameters of immediately after torsion compared with those of before torsion in the groups of torsion,the perfusion was increased significantly displaying by PI increasing(P<0.05).③The longer time the duration of unilateral testicular torsion,the more obviously the perfusion of the contralateral testes increased.Conclusions The timeintensity curve of contrast-enhanced ultrasound is valuable in assessing of the injury of unilateral testicular torsion on the contralateral testis.
3.Fixation of proximal humeral fracture with proximal humeral locking intramedullary nail.

OBJECTIVESTo investigate the results of two-part surgical neck fractures of proximal humerus treated with locking intramedullary nail for proximal humerus, and to discuss the indication, surgical technique and the effectiveness.
METHODSThe data of the 22 patients with two-part surgical neck fractures fixed with proximal humeral nail was retrospectively reviewed. The mean age was 57 years. X-ray film and visual analogue scale (VAS), American shoulder and elbow surgeon (ASES) score, Constant-Murley score, University of California, Los Angeles (UCLA) scoring system and simple shoulder test (SST) questionnaire were used to evaluate the patients' shoulder function recovery at final follow-up.
RESULTAll the patients were followed up with an average time of 13 months. All fractures united within 8 weeks postoperatively. No infection, avascular necrosis or complications which concerning with hardware was found during follow-up. The average active forward flexion was 147.8 degrees , the average external rotation was 45.5 degrees and the average internal rotation was T10. The mean VAS score was 1.5. The mean ASES score, Constant-Murley score, UCLA score and SST score were 81.2, 85.4, 29.9 and 9.5 respectively. Eighteen patients were rated as "excellent" or "good", while 4 others were rated "poor". All patients were satisfied with their results.
CONCLUSIONWith good indication control, meticulous intraoperative management and strict postoperative rehabilitation, close reduction and internal fixation with proximal humeral nail can be an effective method for the treatment of two-part surgical neck fracture.

4.Clinical research on proximal humeral fractures treated by locking proximal humeral plate.

OBJECTIVETo evaluate the indications and clinical results of displaced proximal humeral fractures treated by locking proximal humeral plate (LPHP).
METHODSFrom September 2004 to March 2006, 75 patients which were treated by open reduction and internal fixation with LPHP were available to follow-up, with an average time of 17 months (12 - 30 months). There were 60 fresh fractures and 15 delayed fractures. The range of motion, muscle strength, visual analogue scale (VAS) for pain, American shoulder and elbow surgeon (ASES) score, Constant-Murley, University of California, Los Angeles (UCLA) scoring system and simple shoulder test (SST) for function evaluation were all recorded.
RESULTSThe average forward flexion was (147.3 +/- 17.7) degrees , external rotation was (30.5 +/- 16.2) degrees and internal rotation was to T9. The mean ASES was 86.7 +/- 12.7, Constant-Murley was 87.4 +/- 10.5, UCLA was 30.1 +/- 4.2. The total good or excellent rate was 89.3%. Compare with fresh fractures, the delayed group showed significant less forward flexion and ASES (P = 0.021 and 0.036 respectively). In 9 patients with late complications, there were significant differences regarding ASES, UCLA scoring system and Constant-Murley compared with patients without any complication.
CONCLUSIONWith strict indication control and appropriate surgical technique, satisfactory results can be expected for the displaced proximal humeral fractures treated with locking proximal humeral plates.

5.HBV transinfected enhances the PD-L expression of cytokines stimulating.

OBJECTIVETo investigate whether the PD-L expression in the liver cell lines transinfected with HBV (HepG2.2.15 cells) can be up-regulated after cytokines stimulating.
METHODSTo apply the liver cell lines (HepG2 cells and HepG2.2.15 cells) as a model, the cells were stimulated with IL-4, IFN-alpha and IFN-gamma (final concentration were 10 ng/ml, stimulated for 12 hours) and RT-PCR was carried out to determine the PD-L expression before and after cytokines stimulating.
RESULTSWhether or not transinfected with HBV, IFN-alpha and IFN-gamma both can induce the liver cell lines (HepG2 cells and HepG2.2.15 cells) PD-L1 expression while IL-4 can not; IL-4, IFN-alpha, IFN-gamma all can induce the PD-L2 expression in HepG2.2.15 cells which was transinfected with HBV, only IFN-gamma can induce the PD-L2 expression in HepG2 cells which was not transinfected with HBV.
CONCLUSIONIFN-alpha, IFN-gamma both can induce the PD-L1 expression in HepG2 cells and HepG2.2.15 cells, while it is easy for cytokines to induce the PD-L2 expression in HepG2.2.15 cells which was transinfected with HBV, this may provide a potential mechanism of the molecular basis for chronic HBV infection.

7.Complications of the locking plate for displaced proximal humeral fractures.

BACKGROUNDAlthough the locking plate has become popular for treating displaced proximal humeral fractures in recent years, the complications of this method are still underestimated. We tried to evaluate the factors that have an influence on the complications experienced after proximal humeral fractures fixed by locking plates and compare the results from patients having complications with those having no complications.
METHODSFrom September 2004 to September 2007, 92 out of 111 displaced proximal humeral fractures treated by open reduction and internal fixation with a locking plate were available for follow-up, with an average time of 15.2 months (12 - 36 months). The range of motion, Visual Analog Score (VAS) for pain, American Shoulder and Elbow Surgeons' Form (ASES), Constant-Murley, University of California-Los Angles scoring system (UCLA) score, and Simple Shoulder Test (SST) for function evaluation was all recorded at the latest follow-up. The results from patients with complications were evaluated according to the indices listed above and compared with those patients without any complications.
RESULTSThere were 17 patients with complications, an 18.5% complication rate. Among them, the forward flexion, external rotation and internal rotation were 139.1° ± 24.3°, 24.1° ± 19.6°, and up to T10 level on average. The mean VAS score was 1.0 ± 1.1, the ASES score was 82.9 ± 13.8, the Constant 82.1 ± 11.8, the UCLA 28.5 ± 4.1 and the mean SST 9.5 on average. There was no significant difference of complication rate among different age, sex, and injured side, fresh or delayed fracture, combined with other injury or not groups. Compared with the group without complications, patients with complications showed significantly less external rotation and lower Constant-Murley and UCLA functional scores (P < 0.05). A significant difference in results was seen between patients with complications and those without complications.
CONCLUSIONThe indication control and appropriate surgical technique were important while performing the locking plate fixation for proximal humeral fractures.

Objective:To investigate the influence of metastasis suppressor gene KAI1 on the proliferation,invasion and metastasis of endometrial carcinoma cell line AN3CA and HEC-1-B.Methods:The KAI1 cDNA was transfected into human endometrial carcinoma cells AN3CA and HEC-1-B via Lipofectamine 2000.The expression of KAI1 protein was ex- amined by Western blotting and flow cytometry before and after transfection.The proliferation ability of AN3CA and HEC- 1-B cells was observed by MTT assay and anchorage-independent growth assay.The changes of cell invasive ability were studied by transwell assays.Results:Stable expression of KAI1 protein was observed in AN3CA and HEC-1-B cells and on their surface after transfection with pcDNA3-KAI1 plasmid.Cells transfected with blank plasmid formed more colonies and had a larger size,with the colony forming rates being(54.2?3.1)% for AN3CA cells and(52.7?4.3)% for HEC- 1- B cells;the doubling time of AN3CA and HEC-1-B cells were 21.3 h and 20.1 h,respectively.Cells transfected with pcDNA3-KAI1 formed less colonies and had a smaller size,with the colony forming rates being(37.4?5.1)% for AN3CA cells and(32.1?3.7)% for HEC-1-B cells;the doubling time of AN3CA and HEC-1-B cells were 43.7h and 45.2 h,respectively.The cell proliferation abilities and colony-forming ability were significantly different between the two groups(P
9. Endoplasmic reticulum stressed hepatocellular carcinoma promotes macrophage M2 polarization through exosome-toll-like receptor 4 pathway

Aim To explore the effect of endoplasmic reticulum (ER) stress-related exosomes derived from hepatocellular carcinoma (HCC) cells on the M2 polarization of macrophages via Toll-like receptor 4 (TLR4) and the underlying mechanism. Methods Western blot experiments were conducted to evaluate the GRP78 levels in HCC tissues and HCC cells with or without pre-treatment of tunicamycin and the identification of the isolated exosomes. Immunohistochemical staining was also employed to detect the expression of GRP78 in HCC tissues. Immunofluorescence was used to confirm the effective absorbance of exosomes by macrophages. Flow cytometry ( FCM ) , Western blot and CBA kit were used to detect M2 polarization-asso ciated markers and the expression of TLR4 on macrophages. Affymetrix gene chip and bioinformatics analy-sis were used to screen the differential expression of mRNA in macrophages stimulated by ER stressed-relat- ed exosomes and TLR4 expression. Results In clinical samples, GRP78 expression in liver cancer tissues was higher than that in the adjacent tissues. In cell experiments, tunicamycin (2.5 jxmol • L"1 ) treatment for 24 hours in HCC cells could significantly increase the expression of ER stress marker protein GRP78 compared with control group. Western blot showed that HCC cell-derived exosomes positively expressed exosom- al marker proteins,such as CD63,TSG101 and HSP70, and negatively expressed Calnexin. Interestingly, HSP70-enriched exosomes released by ER-stressed HCC cells were observed. Laser confocal microscopy found that exosomes could be effectively taken up by RAW264.7 macrophages. Flow cytometry and Western blot further indicated that ER stress-related exosomes could significantly up-regulate the expression of CD206, transformed growth factor-beta (TGF-p) and arginase-1 (Arg-1) in macrophages. Meanwhile, these exosomes could promote the secretion of IL-6 and IL- 10,and up-regulate the expression of TLR4 in macrophages. Conclusions ER stressed-HCC cells could promote macrophage M2 polarization by releasing HSP70-enriched exosomes and activating TLR4 signaling.

Major and profound changes have taken place in China over the past 30 years. Rapid socioeconomic progress has exerted a great impact on lifestyle, ranging from food, clothing, working and living conditions, and means of transportation to leisure activities and entertainment. At the same time, new health problems have emerged, and health services are facing new challenges. Presently, cardiovascular diseases (CVD) are among the top health problems of the Chinese people, and pose a serious challenge to all engaged in the prevention and control of these diseases. An epidemic of CVD in China is emerging as a result of lifestyle changes, urbanization and longevity. Both national policy decision-making and medical practice urgently need an authoritative report which comprehensively reflects the trends in the epidemic of CVD and current preventive measures. Since 2005, guided by the Bureau of Disease Prevention of the Ministry of Health of the People's Republic of China and the National Center for Cardiovascular Diseases of China, nationwide experts in the fields of epidemiology, clinical medicine and health economics in the realms of CVD, cerebrovascular disease, diabetes and chronic kidney disease, completed the Report on Cardiovascular Diseases in China every year. The report aims to provide a timely review of the trend of the epidemic and to assess the progress of prevention and control of CVD. In addition, as the report is authoritative, representative and readable, it will become an information platform in the CVD field and an important reference book for government, academic institutes, medical organizations and clinical physicians. This publication is expected to play a positive role in the prevention and control of CVD in China. We present an abstract from the Report on Cardiovascular Diseases in China (2010), including trends in CVD, morbidity and mortality of major CVDs, up-to-date assessment of risk factors, as well as health resources for CVD, and a profile of medical expenditure, with the aim of providing evidence for decision-making in CVD prevention and control programs in China, and of delivering the most authoritative information on CVD prevention and control for all citizens.
