The corrected transposition of the great arteries is an usual congenital cardiac malformation, which consisitsof transposition of great arteries and ventricular inversion, and which is caused by abnormal developement ofconotruncus and ventricular looping. High frequency of associated cardiac malformations makes it difficult to getaccurate morphologic diagnosis. A total of 18 cases of corrected transposition of the great arteries is presented,in which cardiac catheterization and angiocardiography were done at the Department of Radiology, Seoul NationalUniversity Hospital between Sep. 1976 and June 1981. The clinical, radiogrpahic ,and operative findings with theemphasis on the angiocardiographic findings were analized. The resuls are as follows; 1. Among 18 cases, 13 caseshave normal cardiac position, 2 cases have dextrocardia with situs solitus, 2 cases have dextrocardia with situsinversus and 1 case has levocardia with situs inversus. 2. Segmental sets are {S.L.L.} in 15 cases, and {I.D.D} in3 cases and there is no exception to loop rule. 3. Side by side interrelationships of both ventricles and bothsemilunar valves are noticed in 10 and 12 cases respectively. 4. Subaortic type conus is noted in all 18 cases. 5.Associated cardiac malformations are VSD in 14 cases, PS in 11, PDA in 3, PFO in 3, ASD in 2, right aortic arch in2, tricuspid insufficiency, mitral prolapse, persistent left SVC and persistent right SVC in 1 case respectively.6. For accurate diagnosis of corrected TGA, selective biventriculography using biplane cineradiography is anessential procedure.

PURPOSE: The purpose.of this study was to characterize the enhancing patterns of hepatocellular carcinoma (HCC) on two-phase dynamic incremental liver scan with spiral CT. MATERIALS AND METHODS: Two-phase dynamic incremental liver scan using spiral CT was performed on 230 lesions in 107 patients with HCC. CT scanning was performed with a table speed of 13mm/sec and a section thickness of 10 mm;120 mL of contrast medium was injected intravenously with a automatic injector at the rate of 3mL/sec. CT scans were started 35 sec(early phase) and 3 min(delayed phase) after beginning injection of contrast medium. The tumors were divided into 2 groups according to size(< or = 3cm and > 3cm), the contrast enhancement patterns of HCCs and capsules in the early and delayed phases were analyzed in each group. RESULTS: Most of HCCs appeared as high-attenuating lesions in the early phase(75% in tumors smaller than 3cm and 61% in tumors larger than 3cm), and as low-attenuating lesions in the delayed phase(68% in tumors smaller than 3cm and 90% in tumors larger than 3cm). Forty-eight percent of HCCs smaller than 3cm and 58% of HCCs larger than 3cm were high-attenuating in the early phase and low-attenuating in the delayed phase. Thirty-two percent of capsules were low- or iso-attenuating in the early phase and high-attenuating in the delayed phase. Capsules were demonstrated in 22% in HCCs smaller than 3cm and 67% in HCCs larger than 3 cm(p <. 01). CONCLUSION: Two-phase dynamic scan with spiral CT is useful in the diagnosis of HCC because of a precise display of hemodynamic characteristics of HCCs.

Transcatheter embolization using absolute ethanol is a widely used technic in interventional radiology. Butits mechanism of action and embolization effect on various infusion speed are poorly understood. Authors performedan experimental study in rabbits to doccument the effect of absolute ethanol onvarious infusion rate. The resultsare as follows; 1. In high speed infusion group(>0.1ml/sec. n=13), 11 cases showed peripheral obstruction and 2 cases showed central obstruction. 2. In low speed infusion group(<0.1ml/sec. n=12), 4 cases showed peripheralobstruction and 8 cases showed central obstruction (p<0.02). 3. On follow-up angiography performed 4 weeks later(n=8), no demonstrable significant differences are found between two groups.

Toward the beginning of the twenty-first century, the world experiences the dramatic changes in politics, economics and culture, and it is evident that the Korean medical field will not be able to survive provided it doesn't prepare ourselves to adapt to those changes. The Korean Medical Society held a forum for the active operation of the medical society, inviting several leading affiliated societies, to meet the needs of the times. This review describes the summary of the presentation that the authors made on behalf of the Korean Radiological Society in the forum, including the organization, current status of academic activity, current status of international communication, and problems encountered in the internationalization of the Korean Radiological Society.

Eight patients with diffuse panbronchiolitis were evaluated with chest radiograph and high-resolution computed tomography(HRCT). Paients consisted of 5 med and 3 women, aged 27-75 years(average, 54 years). Chest radiographic findings were diffuse small nodular densities, linear shadows, and thickened bronchial wall predominantly in both lower lung fields. All 8 patients had pansinusitis. On HRCT, small nodules and branching linear structures, 1-3mm internal to the pleural surface, representing centrilobular bronchiolar lesion, were found along with thickening of medium and small sized bronchial wall. These nodules did not show coalescence. In conclusion, chest radiographs were usually suggestive and high-resolution CT was diagnostic of diffuse panbronchiolitis.

The authors experienced three cases of cholangiocarcinoma associated with profuse mucin production. We intended to review and summarize the clinical features of these patients to emphasize the clinical importance of mucin-producing cholangiocarcinoma. All patients were female. Symptoms were mainly right upper quadrant pain and jaundice. Diagnosis was made by characteristic endoscopic retrograde cholangiographic findings and computed tomography. Bile ducts were dilatated and obstructive jaundice had developed as a result of the accumulation of mucin realeased by the tumor. Treatments were hepatic lobectomies for two intrahepatic cholangiocarcinoma patients and extrahepatic bile duct resection and hepaticojejunostomy for one patient. There was no postoperative complication. Pathologies were well differentiated papillary adenocarcinoma in two cases. Two patients are still living without recurrence for over three and four years respectively. One patient who had T4 lesion died of recurrence 38 months after operation. Conclusively, aggressive surgical treatment may be justified in the treatment of mucin hypersecreting cholangiocarcinoma even in advanced stage in view of the favorable outcome after radical operation. Further study is needed to clarify its biological behavior.

PURPOSE: Algorithms to get cross-sectional bone density pattern(transverse histogram) to predict the loosening of hip prosthesis using DXA(Dual X-ray Absorptiometry) have been developed. We performed this study to analyze the correlation between radiologic findings and densitometric pattern of the THRA(Total Hip Replacement Arthroplasty) patients. MATERIALS AND METHODS: Thirty-six hips of 32 THRA patients were evaluated. The duration between THRA and DXA was from 1 year 7 months to 15 years. On transverse histogram, the periprosthetic bone density patterns were classified as 3 types;type I, rigid fixation in 17, type II, definite loosening in 8, and type III, partial loosening in 11 cases. Surgical findings, plain X-ray findnings and transverse histogram using DXA were correlated. RESULTS: Among 14 cases performing revision for acetabular prosthesis loosening, 5 cases revealed loosening of fernoral stems while 9 cases revealed rigid fixation of femoral stems. Sensitivity was 100% for either plain X-ray or DXA. Specificity was 88%, 77% for plain X-ray and DXA respectively. CONCLUSION: This preliminary study reveals that periprosthetic bone density pattern on transverse histogram on DXA may be useful in the evaluation of the loosening. However, further study will be needed for clinical application.

BACKGROUND: Almost all STD(sexually tranamitted diseases) have been increasing worldwide since the 1960s. In Korea, the occurrence of STD from recent trends of sexual openness and development in the pleasure-seeking industry is becoming a social issue. However, epidemiological studies and reports concerning the incidence of STD are lacking in Korea, especially in the sexually aetive young age group. OBJECTIVE: We evaluated the incidence of YTD and sexual behavior of recruits in a Korean arrny training center to contribute to the management of STD in Korea. METHODS: We studied 1304 recruits in a Korean army training center by questionnaire, physical examination and laboratory data. The subjects were divided into 3 groups: group 1(1072 persons), those who visited the hospital for evaluation of STD; group 2(641 persons), a control group for coitus rate; and group 3(1304 persons; group 1 plus 232 persons out of group 2), those who had coital experiences. RESULTS: 1. The incidence of STD in group 1 was 65.7%(704/1072) and that of the total number of recruits was 0.43%(704/164,134). 2. Among the 704 cases of STD, the most frequently observed STD was non-gonococcal urethritis, 292 cases(41.5%) and the second was gonorrhea, 236 cases(33.5%). The others were, 68 cases(9.7%) of pediculosis pubis, 41 cases(5.8%) of condyloma acuminatum, 29 cases(4.1%) of syphilis, 20 cases(2.8%) of combined STD, 14 cases(2%) of herpes progenitalis, 2 cases(0.28%) of molluscum cohtagiosum and 2 cases(0.28%) of chancroid. 3. The coitus rate in group 2 was 36.2%(232/641). 4. The incidence of previous STD in group 3 was 20.9%(273/1304). 5. The mn;t common form of STD which the patients experienced previously was urethritis(82.8%) and the others were pediculosis pubis(7.3%), syphilis(3.3%), condylcena acuminahun and herpes progenitalis. 6. The group with more sexual partners tended to have a lower educational level. 7. The experience rate of drug abuse was higher in the group with more sexual partners. 8. The experience rate of oral and anal sex was higher in the group with experiences of clrug abuse than in the ather one. CONCLUSION: In this study, urethritis was the most common form of STD(75% of all STD) and non-gonococcal urethritis showed a greater incidence than gonorrhea. The incidence of STD is thought to be closely related to the factors associated with social or behavioral science, such as the number of sexual partners, grade of education, drug abuse, experience of oral and anal sex. To accomplish more effective management of STD, therefore, the improvement of current social problems involving sexual openness, the pleasure-seeking industry and drug abuse should be achieved.
