Diverticulosis of the vermiform appendix is an uncommon entity with incidence of 0.2~2.0% in surgical pathologic specimens. Inflammation, perforation, and hemorrhage will be complications of appendiceal diverticulum. Without these complications, appendiceal diverticulum is asymptomatic. Authors experienced intussusception combined with appendiceal diverticulum.

3.Simultaneous Laparoscopic Resection of Advanced Colorectal Cancer and Early Gastric Cancer.

Performing laparoscopic surgery for colorectal cancer and early gastric caner has recently been rapidally increasing. We report here on our experiences of 3 cases of laparoscopic combined resection for synchronous colorectal cancer with early gastric cancer. Case 1) An 87-year-old male had synchronous right colon cancer along with early gastric cancer, and he underwent laparoscopic right colectomy with laparoscopic assisted distal gastrectomy. Case 2) A 49-year-old male had synchronous rectal cancer with early gastric cancer, and he underwent laparoscopic abdominoperineal resection and totally laparoscopic distal gastrectomy with delta anastomosis. Case 3) A 76-year-old female had synchronous sigmoid colon cancer with early gastric cancer, and she underwent laparoscopic low anterior resection with laparoscopic assisted distal gastrectomy. The laparoscopic approach for synchronous cancer can be a treatment option for selected cases.

4.A Case of Malignant Mixed Mullerian Tumor after radiation therapy for Cervical cancer.

Malignant mixed mullerian tumors (MMMT) are uncommon neoplasms of the female genital tract that histologically consist of malignant epithelial components and stromal components. Most MMMTs are found in postmenopausal women. The clinical course is very poor due to frequent metastasis and recurrence. Among the carcinogenic epidemiologic predisposing factors, the relationship of previous pelvic irradiation to subsequent development of a malignant mixed mullerian tumor has been reported in recent years. We are reporting a case of MMMT of the uterus which occured in a woman who received radiation therapy for cervical cancer 11 years before the present date.

5.A New Technique of Angioplasty of the Left Pulmonary Artery Using an Autologous Main Pulmonary Artery Flap.

BACKGROUND: Stenosis of the left pulmonary artery (LPA) after repair of tetralogy of Fallot (TOF) is troublesome. A new technique of LPA angioplasty using an autologous MPA flap was performed in patients with TOF. MATERIALAND METHOD: From October 1998 to January 2001, 24 patients (median age; 10 months, range; 4 to 145 months)underwent total correction of TOF with LPA angioplasty using the autologous MPA flap. Five patients underwent pulmonary angioplasty without any patch over the MPA and LPA. The patches were required to enlarge only the MPA in 4 patients, and transannular RVOT widening was performed in 15. RESULT: There were no operative or late deaths. During follow-up (range: 6~42 months), reoperation for LPA stenosis was not required in any patients, but balloon angioplasty for branch pulmonary artery stenosis was performed in 3 patients. Echocardiography and CT angiography at the recent follow-up showed an obtuse angle between the MPA and LPA. CONCLUSION: Although further follow-up is needed, the angioplasty using the autologous MPA flap can be easily performed, avoiding patch-related complications, and allowing growth of the MPA flap. This angioplasty technique creates a more natural and obtuse angle between the MPA and LPA, which can minimize kinking of the LPA, especially in the patients who underwent transannular patch widening.

6.Human Umbilical Cord Blood Infusion in Paralyzed Rats: Histologic and Behavioral Alterations.

STUDY DESIGN: Experimental animal study OBJECTIVES: To examine the ability of human umbilical cord blood (hUCB) stem cells to target a zone of injury and to determine the efficacy of hUCB cells to ameliorate the behavioral deficits after a hUCB cell infusion in paralyzed rats. SUMMARY OF LITERTURE: Many groups have investigated the use of stem cells as potential treatments for a CNS injury. hUCB cells have recently been reported to alleviate the behavioral consequences of a stroke injury. MATERIALS AND METHODS: Thirty Sprague Dawley rats were divided into 6 groups (Gr) (Gr 1. SCI (spinal cord injury) + hUCB delivered at one day postinjury, Gr 2. SCI + hUCB delivered at 3 days postinjury, Gr 3. SCI + hUCB delivered at 5 days postinjury, Gr 4. laminectomy + hUCB, Gr 5. SCI only, Gr 6. Laminectomy only). SCI was produced by compressing the spinal cord to the level of the 8-9th thoracic spine for 1 minute with an aneurysm clip that was calibrated to a closing pressure of 50 gms. The hUCB cells (0.5 ml, 1.5x106) were administered intravenously to the rats. The rat was assessed behaviorally at one, two and three weeks using the BBB behavioral scale. Four weeks after the injury, the animals were sacrificed and the hUCB positiveresponse neural cells (mouse anti-human mitochondria monoclonal antibody=MAB 1273) at the injury level observed using optical and fluorescent microscopy. RESULTS: MAB 1273 positive cells were observed in groups 1, 2 and 3 but not in groups 4, 5 and 6. In particular, there were 870 cells distributed over an area of 1.2 mm(2) in group 3. Group 3 showed the most significant recovery over time in the open field exam, and the most improvement in another tests of incline, leg extension, and toe spread compared with group 1 (p<0.01). CONCLUSION: After infusing the hUCB stem cells to SCI rats, it was confirmed that hUCB cells migrate to an injured area and ameliorate the behavioral deficits. A hUCB infusion 5 days after the injury produced best results in terms of the number of cells and motor recovery.

7.A case of presacral teratoma in an adult female.

Presacral teratomas are rare tumors derived from embryonic germ layers. They present mostly in infancy and are extremely rare in adults. Patient with presacral teratoma in adults may be asymptomatic on initial presentation. The symptoms are due to their size, to the compression of pelvic viscera or to their complications. CT or MRI are the important investigations for characterization of the mass, evaluation of its intrapelvic extension and relationship to other structures. The treatment of presacral teratomas is mainly complete surgical resection. We report a case of presacral teratoma in 47-year-old woman with a brief review of the literatures.

8.Laparoscopic Splenic Flexure Mobilization Using a Medial Approach.

PURPOSE: Splenic flexure mobilization in an anterior resection is a subject of controversy, but a tension-free anastomosis is needed in case of a low anterior resection or a coloanal anastomosis. Classical splenic flexure mobilization by means of the lesser sac opening is technically difficult in the laparoscopic era. METHODS: This study retrospectively analyzed the medical records and operation videos of 16 patients who underwent laparoscopic splenic flexure mobilization in Our Lady of Mercy Hospital, The Catholic University of Korea by using a medial approach. The operation procedure is as follows: Under general anesthesia, the patient was placed in the Trendelenburg position, after making pneumoperitoneum, the inferior mesenteric artery (IMA) and the inferior mesenteric vein (IMV) were ligated and divided. A medial-to-lateral mesocolon dissection was done, and the pancreas was dissected from the mesocolon of the transverse colon; then, the greater omentum was dissected. RESULTS: The mean age was 60 years old, and the male- to-female sex ratio was 9:7. The mean operation time was 293 minutes. The mean distal margin was 5.3 cm, and the mean number of harvested lymph nodes was 15. An anterior resection was done in 2 cases, a low anterior resection is 10 cases, and a coloanal anastomosis in 4 cases. The mean time from ligation of the IMA to splenic flexure mobilization was 45 minutes. There was no mortality or morbidity. CONCLUSIONS: Laparoscopic medial splenic flexure mobilization is a technically feasible and safe method.

Primary mixed carcinoid with mucinous tumor of borderline malignancy of the ovary is very rare ovarian tumor. Most of them arose in dermoid cyst or in mature solid teratoma. 10.Stress Distribution According to Center-Edge Angle in Dysplastic Hip and the Biomechanical Effect of Rotational Acetabular Osteotomy.

PURPOSE: The purpose of this study was to estimate the biomechanical effect of rotational acetabular osteotomy by analyzing the stress on the surface of the femoral head according to center-edge (CE) angle by computer simulation. MATERIALS AND METHODS: We performed computer simulation by simplifying the pelvis and femur to 3-dimensional flat stress. We analyzed the change of distribution of stress and the amount of deformation according to the degree of acetabular dysplasia, represented by the change of CE angle, by using a Finite element model (FEM). Stress and deformation were calculated in two cases that underwent rotational acetabular osteotomy. RESULTS: In the FEM model, the stress was 3.74 MPa at a CE angle of 30.and the deformation was 1.37 mm. The stress was 14 MPa at 15., 34.2 MPa at 0., and 82.4 MPa at -15. In a case of rotational acetabular osteotomy, the pre-operative and post-operative stresses were 26.1 MPa and 5.16 MPa, and the maximal deformations were 16.1 mm and 1.6 mm, respectively. CONCLUSION: As the severity of acetabular dysplasia increases, the stress concentration also increases. When the CE angle is below 0., a high concentration of stress develops. After rotational acetabular osteotomy, maximal stress was reduced to normal levels and sufficient stress distribution was achieved.
