1.Crystalline Retinopathy without Corneal Dystrophy.
Journal of the Korean Ophthalmological Society 2000;41(6):1445-1450
Bietti's crystalline retinopathy is a rare form of tapetoretinal degeneration characterized by yellow, polygonal, glistening intraretinal crystals in the posterior pole and in the superficial paralimbal cornea, which may be due to a systemic abnormality of lipid metabolism, and has been to have a autosomal recessive pattern. Also lots of the reports described similar cases without any corneal changes. 56 years-old female with complaint of progressive visual decrease had corrected visual acuity of 0.1 in her right eye and 1.0 in her left eye.Yellow intraretinal crystals with retinal pigment epithelial (RPE)dystrophy and choroidal sclerosis were noticed without any corneal changes.During follow-up for thirty months, visual acuity gradually decreased to counting fingers in her right eye and 0.7 in her left eye, and RPE degeneration and choroidal sclerosis worsened with no change in intraretinal crystals.

Journal of the Korean Ophthalmological Society 1997;38(1):116-120
Trabeculectomy with mitomycin C was performed on three patients with traumatic dislocate crystalline lens and refractory secondary glaucoma, combined with pars plana vitrectomy, intravitreal phacofragmentation and scleral fixation of the IOL. Over nine months after operations, intraocular pressures were well controlled not only without any antiglaucomatous medications and special complications but also with good visual recovery in all patients . Trebeculectomy combined with pars plana vitrectomy may be useful in some patients wit hvireoretinal disease and glaucoma.

Journal of the Korean Ophthalmological Society 2001;42(9):1277-1283
PURPOSE: To determine the difference among the results of three kinds of methods for the treatment of simple retinal detachment. METHODS: We analyzed the patients undergoing primary pars plana vitrectomy(Group I: January to June, 1997), scleral buckling(Group II: January to June, 1996) and pneumatic retinopexy(Group I: January to June, 1995) to treat simple retinal detachments with retinal breaks located in peripheral retina, smaller than one clock hour, breaks in the superior eight clock hours, multiple breaks extending less than two clock hours and no severe retinal traction. RESULTS: The final visual acuity was 5/200 or more(functional success) in 19 eyes(100%) of Group I, 19 eyes(100%) of Group cII and 20 eyes(90.9%) of Group I. Primary retinal reattachment(anatomical success) was in 17 eyes(89.5%) of Group I, 19 eyes(100%) of Group II and 18 eyes(81.8%) of Group I with no statistically significant difference among the three groups. The final retinal reattachment rate was 100% in all groups. CONCLUSIONS: This result suggests the scleral buckling might be a little bit better for the treatment of simple retinal detachment, though each of three surgical methods showed no statistical difference of functional and anatomical results.

Journal of the Korean Ophthalmological Society 1996;37(11):1865-1877
We performed pars plana vitrectomy combined with anterior capsule-sparing lensectomy on forty three eyes WJth coexisting cataract and vitreoretinal disease and followed up more than 6 months. Preoperative vitreoretinal diseases consisted of nondiabetic vitreous hemorrhage (thirteen eyes), retained metallic intraocular foreign bodies (nine eyes), posterior uveitis (nine eyes), proliferative diabetic retinopathy (seven eyes) and rhegmatogenous retinal detachment (five eyes) . Thirty six eyes (84%) revealed the visual acuity of 0.1 or less before operation. Thirty eight eyes (88%) could get a better visual acuity after operation than before operation. Posterior chamber intraocular lenses were implanted to thirty eight eyes. The severity of anterior capsular opacity showed a statistically significant difference between the eyes with injection of intraocular gas or silicone oil and them without injection of that (p=0.004). In this study, we can find that the technique of pars plana vitrectomy combined with anterior capsule-sparing lensectomy would be recommendable and the anterior capsular opacity may have relation with intraocular gas or silicone oil.

Journal of the Korean Ophthalmological Society 2000;41(3):784-787
Pigmented vitreal cyst is a benign and uncommon entity with uncertain etiology. A floating intravitreal material was incidentally noticed in a 44-year-old man. Visual acuity was 1.2 and intraocular pressure 16 millimeters of mercury in both eyes. All other ophthalmic examination were normal. On posterior segment examination, a free-floating, brownish, round cyst of about two disc diameter in size was demonstrated in the inferior vitreous cavity of the left eye without any other abnormalities. During the follow-up examination for thirty months, the shape, size, color, and location of the cyst was not changed with the same visual acuity and IOP. Pigmented vitreal cyst is benign with no change for a long time, and then generally need no special treatment unless it blocks visual axis

Journal of the Korean Ophthalmological Society 2000;41(4):831-837
Result and complications were evaluated in 28 eyes of 28 patients who had undergone pneumatic retinopexy for rhegmatogenous retinal detachment from December 1994 to May 1995 and were followed for at least 4 months. Twenty four eyes were phakic, one eye aphakic and three eyes pseudophakic. 0.3mlof perfluoropropane(4 eyes)or 0.45ml of sulfur hexafluoride(24 eyes)was injected. Mean follow-up time was 33.3 months. Perioperative complications included subconjunctival gas in 2 eyes, subretinal gas in 1 eye and delayed absorption of subretinal fluid in 5 eyes. Retinal reattachment was obtained in 22 eyes(79%) and final reattachment rate was 100% with an additional procedure of scleral buckle(4 eyes)or pars plana vitrectomy(2 eyes). Final visual acuity improved in 25 eyes(89%), and decreased in 2 eyes. Postoperatively following complications were noted;mild proliferative vitreoretinopathy(3 eyes), epiretinal membrane(1 eye), central retinal artery occlusion(1 eye), nucleosclerosis(4 eyes)and cataract operation(2 eyes). This result suggests that pneumatic retinopexy could be a good method for the treatment of rhegmatogenous retinal detachment.

Journal of the Korean Ophthalmological Society 1996;37(2):244-251
We reviewed the records of seven patients who underwent pars plana vitrectomy and intravitreal phacofragmentation for removal of posteriorly dislocated lenses from September 26, 1994. to January 12, 1995. More than half volumes of crystalline lenses were dislocated during cataract extraction in four eyes, and all three eyes of dislocated lenses with intact capsules had a history of blunt trauma. Five eyes had visual acuity of 15/20 or better at 3 months after removal of intravitreal crystalline lenses. But two eyes, undergone excessive manipulation for removing intravitreal nuclear fragments via the limbus during cataract operation, had 4/20 vision with complication of retinal or choroidal detachment. These results suggest that the overall prognosis for visual recovery after pars plana vitrectomy and phacofragmentation for removal of intravitreal crystalline lens, is quite good if there were no excessive attempts to remove intravitreal lens fragment during cataract extraction.

8.Aniridia Intraocular Lens.
Journal of the Korean Ophthalmological Society 1996;37(2):233-236
We implanted aniridia intraocular lenses (aniridia IOL) on three cases with complaint of glare caused by traumatic or congenital aniridias combined with cataract or aphakia. Aniridia IOLs were fixed by scleral fixation on one aphakia with traumatic aniridia, and at the ciliary sulcus on one aphakia with traumatic aniridia and one congenital aniridia with cataract. After follow-up of at least 2 months, visual improvement, absence of glare and cosmetic acceptance was observed without any complication in all cases.

9.Prognostic Factors in Idiopathic Macular Hole Surgery.
Journal of the Korean Ophthalmological Society 2000;41(8):1746-1752
We evaluated the relationship between the surgical success of idiopathic macular hole and sex, age, duration, epiretinal membrane, preoperative visual acuity, size of the hole, and surgical method. Of all 25 eyes in 23 patients, 14 eyes(56%)showed anatomical success(disappearance of the hole), and 17 eyes(68%)functional success(the final visual acuity of 0.15 or more with more than one line improvement). There were statistically significant differences of anatomical(p=0.001)and functional success(p=0.020)between the Group I where its macular hole was less than one-fourth of the optic disc in size and the other group, and anatomical success(p=0.039)between the group with the age of less than 70 years(Group A)and the other group. In Group I, anatomical and functional success were both 92%(11 eyes/12 eyes). In Group A, anatomical and functional success were 72%(13 eyes/18 eyes)and 77%(14 eyes/18 eyes), respectively, and there was statistically significant relationship between size of the hole and anatomical success(p=0.024), and difference of anatomical success between the group with duration of less than 6 months and the other group(p=0.023). Good surgical result may be expected in the idiopathic macular hole of the age of less than 70 years with about 350 micrometer or less in size and duration of less than 6 months.

Journal of the Korean Ophthalmological Society 2000;41(8):1740-1745
Adenosine deaminase(ADA)has been used for the diagnosis of localized tuberculosis. The titer of ADA was measured from the ocular fluid of the patients who had undergone vitreoretinal surgery due to non-tuberculous vitreoretinal disease. Forty five patients were included and twenty patients were male. Mean age was 51.9 years. Four patients had diabetes mellitus and two had essential hypertension. The most frequent cause of vitreoretinal surgery was proliferative vitreoretinopathy(12 eyes). For the analysis of ADA titer, vitreous fluid(group I)was aspirated in 36 patients undergoing pars plana vitrectomy and subretinal fluid(group II)in 9 patients undergoing scleral encircling. Mean titer of ADA was 9.01+/-15.69 IU/L, and there was no significant difference on the statistics between group I(6.06+/-13.75 IU/L)and group II(20.83+/-18.17 IU/L). All 45 eyes showed negative reaction to polymerase chain againt tuberculosis, and 25 of 29 patients had positive reaction to tuberculin skin test. There was no statistically significant relationship between ADA titer and tuberculin skin test. This result suggests that ADA may be used for the diagnosis of posterior ocular tuberculosis, since the normal concentration in the posterior ocular fluid was compatible with the one in other body fluids where it has been used for the diagnosis of localized tuberculosis.
