China Pharmacy 2005;0(14):-
OBJECTIVE:To analyze the blood drug monitoring results of vancomycin and clinical administration behavior and then to provide a clinical reference for its rational utilization. METHODS:We retrospectively collected the blood drug monitoring results of vancomycin and drugrelated information from July 2007 to October 2008 in our hospital. These data were from the electronic clinical pharmacy workstation and initial written case history. RESULTS:Among the 76 cases,non-clinical significant results due to doctor’s irregular manipulation in collecting blood sample accounted for 32.89%. In renal failure group, the rate of peak concentration above the effective blood concentration range was 44.46% . Every patient was averagely administered 21.85 kinds of drugs mainly by intravenous injection during vancomycin application. Clinical response rate of vancomycin was 65.00%. CONCLUSION:During the blood concentration monitoring of vancomycin,we should take clinical need into consideration, collect blood samples rightly, consider the factors such as special pathophysiologic status of patients, drug combination, and adjust the dosage timely.

Chinese Journal of Laboratory Medicine 2016;39(4):315-318
Coronary atherosclerotic heart disease is the leading cause of death in the world of non-infectious diseases.High density lipoprotein ( HDL ) plays an important role in the progression of anti-atherosclerosis.Recent studies have found that HDL can carry microRNA, and the anti-inflammation function of endothelial cell is achieved by the transport of microRNA.The specific functions of different HDL subtypes in the anti-atherosclerosis have been gradually recognized. At the same time, a series of traditional HDL anti-atherosclerotic functions also have been newly discovered, such as reverse cholesterol transport, antioxidant, anti-inflammatory and the protection of vascular endothelium. All of these advances provide a new basis for the clinical application of HDL.

Chinese Journal of General Practitioners 2015;14(3):181-184
Objective To investigate the factors related to choice of community health service centers (CHSCs) in standardized training general practitioners (GPs) in Shanghai. Methods Totally 152 GPs,(including those of Traditional Chinese Medicine) who completed the residency training in 2013 and signed employment contract with community in Shanghai were surveyed with a self-designed questionnaire. Results Among all the participants,113 (74.3%) attached the greatest importance to the salary level when choosing CHSC,58 (38.2%) expected annual salary more than 120 000 Yuan during the first 3 years.GPs tended to work in the city center(40.1%) or CHSCs near to their home(38.2%),mainly because of the job commuting (59.9%).Considering career development, the CHSCs with high level comprehensive service (51.3%) and with large population (5.0%) were more attractive.GPs paid more attention to the national demonstration community (57.2%) and CHSCs with member ofShanghai outstanding young physician training program(51.7%).In this survey, household register was the main influencing factor for choosing CHSCs,Shanghai natives paid more attention to the location of the community than those not Shanghai natives (61.8% vs.34.2%),more inclined to CHSCs that close to their house (55.3% vs.21.1%),non-Shanghai natives preferred authorized strength (51.3% vs.28.9%).GPs of Western medicine expected higher salary than GPs of TCM (x2 =13.648,P < 0.05),Males expected higher salary than females (x2 =7.799,P < 0.05),Conclusions GPs pay the most attention on salaries and tend to work in the city central area, prefer CHSC with high level comprehensive service and large served population. The main factor influencing GPs to chose CHSCs is the household register, the category of Gps and gender.

This paper is to report the study of the metabolism of forscolin in plasma and liver microsomes for guiding clinical therapy. Forscolin was quantified by HPLC-MS/MS. The metabolic stability of forscolin in rat, Beagle dog, monkey and human plasma and liver microsomes, mediated enzymes of forscolin and its inhibition on cytochrome P450 isoforms in human liver microsomes were studied. Results showed that forscolin was not metabolized in plasma of the four species but metabolized in liver microsomes of the four species. The t1/2 of forscolin in rat, Beagle dog, monkey and human liver microsomes were (52.0 +/- 15.0), (51.2 +/- 5.9), (6.0 +/- 0.2) and (11.9 +/- 1.8) min; CL(int) were (75.6 +/- 18.7), (60.9 +/- 6.8), (513.8 +/- 14.3) and (176.2 +/- 25.6) mL x min(-1) x kg(-1); CL were (34.8 +/- 4.5), (23.3 +/- 1.0), (40.3 +/- 0.5) and (17.9 +/- 0.3) mL x min(-1) x kg(-1), respectively. Forscolin was metabolized by CYP3A4 in human liver microsomes. There was definite inhibition on CYP3A4 at the concentrations of forscolin between 0.1 ng x mL(-1) and 5 microg x mL(-1). Therefore, forscolin is rapidly excreted from liver microsomes. Attention should be paid to the drug interaction when forscolin was used along with other drugs metabolized by CYP3A4 in clinics.

AIM: To study the changes of bacterial ultrastructure, membrance potential and membrance patency of Escherichia coli during the postantibiotic effect after exposure to gatifloxacin and ciprofloxacin in order to investigate the mechanisms of PAE. METHODS: During the Postantibiotic effect after exposure to gatifloxacin and ciprofloxacin, the aliquots were taken from the bacterial culture at regular intervals. Then the fluorescence microscope was used to observe changes in bacterial ultrastructure and at the same time we studied the changes of membrance potential and membrance patency in Escherichia coli during the postantibiotic effect by flow cytometry in conjunction with fluorescent probes. RESULTS: The PAE phase characterized by filament formation after exposure to gatifloxacin by fluorescence microscopy, yet no significant changes in membrance potential and membrance patency of Escherichia coli were observed. CONCLUSION: Gatifloxacin and ciprofloxacin can induce filamentation, and this change is indifferent with membrance potential and membrance patency of Escherichia coli.
