1.Increase of Femoral Anteversion after Experimental Induction of Avascular Necrosis of Femoral Head

An experimental study on the effect of the induced avascular necrosis of the femoral head on the femoral anteversion was made in the hips of 74 three to four-week-old rabbits. The results were as follows; l. In 18 of the 74 operated rabbits, a relative increase of anteversion was observed on the operated side, as compared with the nonoperated femur, the differences ranging from 10 to 75°(average increase was 25.9°). 2. Anteversion increase was actually internal rotation of femoral shaft starting from the subtrochanteric level and progressively increasing downward with spiral fashion. 3. The cause of anteversion increase was not proved in this experiment, but anteversion increase was a result of avascular necrosis.

3.A Case of Livedoid Vasculitis.

A 47-year-old male patient presented us with erythematous, telangiectatic, purpuric or hyperpigmented patqhes, recurrent painful ulceration and ivory white atrophic scars on the lower portions of his legs, ankles and dorsal surfaces of both feet exacerbating on every summer for 5 years. The histopathalogical findings of an atrophic patch showed endothelial proliforetiion, partial occlusion of the dermal capillaries and fibrinoid material) on the superficial blood vessels, This patient was treated with aspirin and dipyriclamole for 4 months, and the skin lesions were improved significantly.

4.Clinical and Histopathologic Study of Tsutsugamushi disease.

The clinical and histopathologic study of 26 cases with tsutsugamushi disease seen at the Department of Derrnatology, Kosin Medical Center, Pusan, Korea from October 1986 to December 1988 was performed. The results were summarized as follows : 1. Age distribution was from 12 to 73 years and male to female ratio was 1: 1.2 (1P, males 14 females). 2. The disease occurred from September to December in a year. The peak incidence was in October (61.5%). 3. Major clinical manifestations were as follows : Chill, fever and headache (100 %), maculopapular eruption (90.2%), eschar (88.5%) and myalgia (84.6%). 4. Eschars were distributed on the abdomen (34.8%), chest (30.4%), leg (7.4%) and ot.her sites. 5. Histopathologic findings of the erythernatous macules in 24 patients showed epiderrnal changes including exocytosis of rnononuclear cells (66.7%), liquefaction degeneration of basal cells (54.2%), spongiosis (33.3%) and epidermal cell necrosis (29.2%). Dermal changes revealed vascular dilatation and perivascular infiltration of rnononuclear cells (100%), extravasated RBC (50%), and edema of papillary dermis (45.8%). Epidermal changes around the eschar were liquefaction degeneration of basal cells (52.2%), spongiosis (43.5%) and exocytosis of mononuclear cells (30.4%). Dermal changes included vascular dilatation and perivascular mononuclear cell infiltration (100%), vessel wall necrosis (73.9%), ederna of papillary dermis (65.2%) and endothelial cell swelling (65.2%).

5.Wells' Syndrome with Bullous and Vegetative Lesions.

We report a case of Wells' syndrome consisting chiefly of papular, vesicular, pustular and vegetative lesions on the trunk, lower extremities and palms in a 48-year-old female, though characteristic lesions of this syndrome are known to be cellulitis or urticaria like plaques. Only a few cases with such lesions as ours have been reported until now. Histologic finding showed spongiotic vesicles in the epidermis and diffuse dense infiltration of eosinophils with characteristic flame figures in the dermis. In addition, marked blood eosinophilia was noted. We tried oral prednisolone for 3 weeks, and the skin lesions improved almost completely.

6.Evaluation of ES-300 for the Detection of Anti-HCV Antibody.

BACKGROUND: A fully automated enzyme-immunoassay (EIA) analyzer, Enzymun System, ES-300 (Boehringer Mannheim, Germany) uses streptavidin technology and performs single test or panels of up to 12 tests per run. We evaluated the results of ES-300 for anti-HCV by comparing the results with microplate-EIA, radioimmunoassay (RIA), and confirmatory test. METHODS: Total 79 sera (51 positive, 24 negative, 4 indeterminate results confirmed by Lucky HCD Confirm) were analysed. ES-300 with Enzymun-Test(R) Anti-HCV (Boehringer Mannheim, Germany) and microplate-EIA (Green Cross Center Innotest HCV 3.0(R)) were used. Fifty one sera were examined additionally by 2nd-generation RIA method, NANBDINE 125C(General Biologicals Corp., R.O.C.). And all results were compared to the results of Lucky HCD Confirm. RESULTS: The overall concordance rate of ES-300 and Innotest(R) was 72/79 (91.1%). The results of Lucky HCD Confirm on seven discrepant samples were five negative and two indeterminate. The results of ES-300 and NANBDINE 125C showed concordance rate of 90.2%. The sensitivity and specificity of ES-300 with regard to Lucky HCD Confirm were 94.5%, and 87.5%, respectively, and that of Innotest(R) were 98.2% and 66.7%, respectively. Clear distinction of positive and negative results by signal/cut off ratio was available in both EIAs. The positive predictive values of ES-300 and Innotest(R) were 94.5%, and 87.1%, respectively. CONCLUSIONS: ES-300 showed relatively good results in sensitivity and positive predictive value with regard to confirmatory test. In EIA-positive persons, however, follow-up study would be necessary for reliable evaluation of HCV infection.

7.Factors affecting Compliance to Hormonal Replacement Therapy in Postmenopausal Women.

OBJECTIVES: The purpose of this study is to evaluate effects of patients' age, amenorrhea duration, motivation, purpose, regimen, side effects on compliance to hormone replacement therapy(HRT) Methods: Compliance to HRT was assessed in 258 postmenopausal women who began HRT in the Menopause Clinic of Hangang Sacred Heart Hospital, Hallym university. One hundred sixty nine women who had intact uterus received continuous combined therapy with 0.625 mg of conjugated equine estrogen (CEE) and 100 mg of micronized progesterone. Eighty nine women who were hysterectomized received only 0.625 mg of CEE. RESULTS: A total of 103 women (39.9%) reported that they continued HRT at least for one year. Continuation of HRT was significantly more common among women who started HRT becuase of recommendation by gynecologist after hysterectomy in our department (p< 0.05). A greater percentage of women who received only CEE without uterus continued HRT than women who received continuous combined therapy (p< 0.05). Duration of amenorrhea, reasons for taking HRT and side effects such as irregular bleeding or breast tenderness were not related to the continuation of HRT. CONCLUSION: Education of menopausal women about the benefit/risk and side effects of HRT may be critical in improving compliance.

8.The Lens Dislocation into the Anterior Chamber in a case of Marfan's syndrome.

The authors experienced a case of Marfan's syndrome in whom the right sided lens was dislocated into the anterior chamber and the left side inferio-temporally into vitreous. In addition, the lens of the right eye was complicated by cataract, and a favorable outcome was obtained by intracapsular lens extraction.

9.A Case of Kyrle's Disease.

We report a case of Kyrles disease occuring in a 19-year-old man with chronic renal failure. The patient had multiple hyperkeratotic papules with central plug on the upper extremities, lower extremities and buttocks. Histopathological findings showed epidermal invagination filled with parakeratotic column and basophilic debris. Many dyskeratotic and vacuolated cells were present below the keratotic plug, and penetration of the parakeratotic column into the dermis was seen.

10.Hemodynamic Responses and Oxygen Availability in Unanesthetized Dogs during Apnea.

BACKGROUND: This study examined hemodynamic variables, oxygen delivery, extraction, and consumption in response to acute progressive hypoxia and hypercarbia in the setting of apnea. METHODS: Apnea was induced in 9 healthy mongrel dogs by disconnecting animals from mechanical ventilation of 30 minutes with pure oxygen. Hemodynamic variables, oxygen transport, extraction, and consumption were rapidly and repeatedly measured using pulmonary arterial and arterial catheters until cardiac output was undetectable. RESULTS: The baseline PaO2, PaCO2, pH, base excess were 318 +/- 137 mm Hg, 36 +/- 3.5 mm Hg, 7.30 +/- 0.06, 6.81 +/- 2.65 mmol/l respectively. Hypercarbia and hypoxemia (76 +/- 33 mm Hg) was first noted at 1 and 4 minute respectively. Base excess was not changed. Indices of preload (PCWP and CVP) were increased early in the time course (p<0.05). In contrast, indices of afterload (SVR) increased later, just before cardiac decompensation began (p<0.05). No significant reduction of cardiac output, oxygen delivery, extraction, and consumption was detected just until abrupt cardiac decompensation started, 5 minute. CONCLUSIONS: These data suggest that the early increase in preload was primarily due to hypercarbia whereas the late increase in afterload was due to hypoxemia, but the main cause of acute cardiac decompensation was a critical decrease in arterial oxygen tension with some contribution of increased preload and afterload.
