3.Studies on the molluscicidal and cercariacidal effects of various pesticides in liver fluke.

Clonorchiasis for which no specific therapy is available has been recognized as one of the serious public health problems in terms of waste of human productivity in Korea today and an urgent consideration must be given to its prevention and control. For past several years, the author has been finding that the infection rate of liver fluke in fish from the Kum-Ho river which has long been recognized as the world's notorious prevalent area of the disease, has been decreasing gradually. Here the author formulated the hypothesis that the decreasing of infection rate might be influenced by the common use of pesticide in plants along the river. Basing this hypothesis, the effective use of pesticide may contribute to the control of the disease by cutting the life cycle of liver fluke. To test above hypothesis, molluscicidal effect on Parafossarulus manchouricus and cercariacidal effect of various pesticides in liver fluke have been determined and the following results were obtained. In molluscicidal effect, the most potent agent in low concentration was Bayer 73 which is 3 to 70 times stronger than other pesticides. When compared with NaPCP which has been widely used for the eradication of Onchomelania, Bayer 73 shows 5 to 10 times stronger effect. The order of pesticide in molluscicidal effect was: Bayer 73, Nicotin, NaPCP, Sevin, Parathion, Copper sulfate and Lindane. In cercariacidal effect, the most potent agent was Parathion which had 2 to 5 times stronger effect than Bayer 73 or NaPCP. The order of pesticide in the cercariacidal effect was: Parathion, Bayer73, NaPCP, Lindane, Nicotin,Sevin, and Copper sulfate. By applying the common used concentration of pesticides considering the toxicity to plants, Bayer 73 is most potent in molluscicidal effect and then Nicotin, NaPCP, Copper sulfate, Sevin, Parathion and Lindane in order. In cercariacidal effect, Parathion is the most potent and NaPCP, Bayer 73, Lindane, and Copper sulfate are followed. Considering with the toxicity to human body the most safe and effective pesticides were Bayer 73, Seven, NaPCP, and Copper sulfate but among those agents, Bayer 73 and NaPCP show relatively stronger effect in both Parafossarulus manchouricus and cercaria of liver fluke.

5.Comparison of between Efficacy of Intravaginal Misoprostol and Intravenous Sulprostone in Termination of Second Trimester Pregnancy.

To compare the efficiency, success rate and abortion time of applications of intravaginal misoprostol versus intravenous Sulprostone(Nalador) for mid-trimester pregnancy termination. Eighty three patients between 17-29 weeks of gestation with medical, obstetric, or genetic reasons for termination of pregnancy were randomized to receive either 50 ug tablets of misoprostol placed in the posterior vaginal fomix or 1,000ug sulprostone intravenously diluted I L of isotonic saline solution given as a 12-h infusion. Among eighty three patients recruited, fourty five patients received misoprostol and thirty eight patients received sulprostone intravenously. The average interval from start of induction to vaginal delivery was 13.35+/-3.34 hours in misoprostol group and 21.14+/-6.64 hours in the sulprostone group. The success rate of complete termination within 12 and 24 hours in misoprostol group were 57.7%, 93.3%, respectively, while in sulprostone group were 15.8%, 92.1% respectively. Oxytocin augumentation was 6.7% in misoprostol group and 7.9% in the sulprostone group. No serious complication occurred. Intravaginal misoprostol appears to be acceptably safe and effective agents for second trimester pregnancy termination. Misoprostol has the advantage of being inexpensive, easily stored and readily available. The regimen of 100 ug misoprostol inserted intracervicovaginally every 8 hours is the optimal method for pregnancy termination.

7.Fractures of Forearm Bones

Forearm bone fractures are difficult to manage, because of their special functions (pronation and supination) and peculiar anatomy. Authors have experienced 68 cases of various types of forearm bone fractures from January, 1968 to December, 1971. They were analized as following. 1. Out of 68 patients, males were 54 cases (79.4%) and females were 14 cases (30.6%). Among them, 91.2% of the cases were occured in people over 15 years old. About 47% of the cases were caused by automobile accidents. 2. Both forearm bone fractures were 51 cases (75%), and most common site of the fractures were mid 1/3(66.2%). Stable (transverse or incomplete) fractures were 24%, and unstable(comminuted, segmental, oblique or spiral) fractures were 76%. Open fractures were 26.5% 3. Conservative treatment was given to 41 cases (60.3%), and open reduction & internal fixation was given to 27 cases (39%). 4. A comparison has been made between the groups treated by conservative method and open reduction & internal fixation. 1) Average healing time of open reduction group was 19.5 weeks and conservative group was 16.4 weeks. 2) Non-union was occured in 7.4% of conservative treatment group, and 22.7% of open reduction group. 3) Satisfactory functional results were shown in 23.5% of conservative group and 54.5% of open reduction & internal fixation group. 4) Rotational deformities were noticed in 53.3% of the conservative treatment group and 54.5% of the open reduction group (No difference was noticed). 5) Angulatory deformities were shown in 70.4% of the conservative treatment group and 44.4% of the open reduction group. 5. Three cases were complicated by cross unoin, and no postoperative wound infection was noted. But two cases of open fracture developed to chronic osteomyelitis.
