2.Early cornea healing response of PRK and free-flap Epi-LASIK under different cutting depth in rabbit

Background Free-flap Epi-LASIK surgery show a fast epithelial healing and mild trauma responses.Its mechanism is notable. Objective The aim of this study was to compare the corneal epithelial healing response following photorefractive keratectomy(PRK)and flap-free Epi-LASIK surgery. Methods Twelve experimental rabbits were randomly assigned to high cutting group(-10.00 D)and low cutting group(-3.00 D),and 2 normal matched rabbits were used as controls.PRK was performed in lateral eyes and flap-free epipolis laser in situkeratomileusis(Epi-LASIK)surgery was curried out in the fellow eyes.The corneal wound healing response in early stage of post-operation was examined under the slim lamp,and the inflammatory cells infiltration in cornea,basic fibroblast growth factor(bFGF)and nuclear transcription factor(NF-KB)expressions in corneal stroma were detected by hematine-eosin staining and immunohistochemstry in the seventh day after operation. Results The inflammatory reaction was milder in the eyes with Epi-LASIK than ones of PRK under the slim lamp.No significant difierences were found in numbers of inflammatory cells in cornea.expressions of bFGF and NF-κB in corneal stroma between two types of surgeries in low cutting group(P>0.05).In high cutting group,the numbers of inflammatory cells were(12.25±1.22)/400 field in PRK eyes,and those of free-flap Epi-LASIK eyes were(6.67±0.31)/400 field,showing obvious difference between them(t=-8.87,P<0.01).Expressions of NF-κB(A value)in cornea were 6.11±1.36 and 41.82±8.7 1 in PRK eyes of low cutting group and high cutting group.and those in free-flap Epi-LASIK were 3.01±0.81 and 11.59±4.55.indicating a drastically increase in PRK eyes(P<0.05).The expressions of bFGF(A value)in corneal stroma were 33.59±6.98 and 123.68±18.81 in PRK eyes and those in free-flap Epi-LASIK eyes were 69.20±8.85 and 123.68±1 8.81 in low cutting group and high cutting group,presenting a considerably decrease in PRK eyes (P<0.05).The corneal epithelial healing was faster in free-flap Epi-LASIK eyes than that in PRK eyes.and deeper cutting slowed the healing procedure. Conclusion In low cutting group,healing time of corneal epithelium is close between PRK and flap-free Epi-LASIK.But in high cutting group,epithelial healing is faster and inflammatory reaction is lighter in flap-free Epi-LASIK compared with PRK.

Biological process is an important approach to treat the dye wastewater. The azo dyes decolouration by special azoreductase of different aerobic bacteria and fungi was summarized. Under anaerobic condition, reductive decolourization of azo dyes was carried in the presence of redox mediators which act as electron shuttle. It was also pointed out that azo dye reduction occurred mainly under anaerobic condition. Different electron donor resulted in different decolourization rate. Problems of current biotechnology were analyzed and corresponding measures were discussed.
4.Clinical Analysis of Bone and Joint Tuberculosis

Prevalent is the concept that tuberculosis has completely been put under control and the actual number of the patients is decreasing. This study has been undertaken in part to give substantial evidence to this general idea about the tuberculosis. Total 921 patients with skeletal tuberculous lesions, selected out of all the patients consulted to the department of orthopedic surgery during the 13 consecutive years from Jaunary of 1963 to December of 1975, were subjected to this analysis. 1. Overall incidence of patients with skeletal tuberculosis was 4.8% of all the patients visited during the 13 years. 2. Of 921 patients, males were 591 and females 330, which showed the men were affected 1.8 times more than women. 3. More than half of them (51,5%) occurred under the age of 19 years. 4. Three anatomical sites were predisposed to this affection; 58.5% in spine, 10.5% in hip, and 11.6% in knee. 5. With the above study it can be safely be said that the actual number and also the incidence of skeletal tuberculosis has not shown any trend of decline and accordingly its management should be more regulated and thorough.

5.A Clinical Study on Fracture of Femoral Neck in Children

Attempt to treat degenerative arthritis of the knee by surgical means have been limited. Patellectomy or patelloplasty may be done in the presence of symptomatic patellofemoral arthritis; and joint debridement, including removal of loose bodies, large osteophytes, and damaged menisci, is occasionally used. Most patients with degenerative arthritis of the knee bear more of their weight on one tibial condyle than on the other. As the articular cartilage degenerates over the tibial condyle that bears the most weight, the natural varus or valgus deviation increases and a vicious cycle is set up in which increasing deformity creates increasing degenerative change. If weight-bearing and other stresses could be increased on this more normal area and decreased on the involved portion, it would been that pain might be relieved and the useful life-span of the knee joint considerably prolonged. The tibia in degenerative arthritis of the knee has become the preferred site for the osteotomy because of restriction of knee motion. The osteotomy is increasingly performed proximal, rather than distal, to the attachment of the patellar tendon at the tibial tuberosity because healing occurs faster in the cancellous bone of the metaphysis as compared with the cortical bone of the diaphysis, and particularly as the quadriceps extensor mechanism stabilizes the osteotomy. We have had 4 cases of modified high tibial osteotomy in the degenerative arthritis patients by division of proximal tibio-fibular ligament instead of removal of fibular head. And the follow-up resutts are “good” in all cases without any complication.

6.Medial Approach Open Reduction in Congenital Dislocation of the Hip

Medial approach open reduction (MAOR) for congenital dislocation of the hip, as is well known, is the most anatomically direct approach to the major obstacle to reduction. For the study on this method of open reduction, the authors reviewed 15 children who underwent MAOR and followed up to at least 1 year after operation with an average of 3 years and 2 months. The results were as follows: 1. The age distributions of the 15 children were; 1 child in below 12 months of age, 10 in 13 to 18 months, and 4 in 19 to 24 months. 2. The acetabular index was high before operation with an average of 32.7° (25-43). This improved makedly in the follow-up period to an average of 20.6° (13-29) . 3. The average of acetabulum-head index was 70.6° and the average of CE angle was 19.7° in the follow-up. 4. The complications were 2 cases of avascular necrosis and 1 case of redislocation. No contracture of joint was observed. 5. For the residual dysplasia of acetabulum,innominate osteotomy of Salter was performed in 2 children and was scheduled in the other 5 children. 6. It can be stated that MAOR is a very safe, simple and effective method of open reduction.

7.Quality Control of Plasma Lipids Measurement in CNHS 2002

Objective To describe the work of quality control for lipids measurement in CNHS 2002.Methods The quality control was applied over the whole project including staff training,making standard operating procedure(blood collection,separation,temporary storage,transportation,final storage),lab materials assignment,internal quality control in central lab and participating US Disease Control Center's Lipids Standard Program(LSP)as external quality control.Results Seven times of national training programs were held and 326 technicians from CDCs in 31 provinces,autonomous regions, municiplalities and 132 surveyed counties received related trainings.During the program,321 persons passed examination (98.5%)and among them 205 got the A score(62.9%);The field work was implemented strictly according to the procedure. Three automatic biochemical instruments were used in the measurement and there is no significant difference between means of serum TC,TG and HDL-C from several batches of quality control serum.Parallel measurements were conducted in five percent randomly selected samples,the ratio of bias less than 3% was 99.05%,96.40% and 98.30% for TC,TG and HLD-C, respectively.Compared with means of LSP control sera,the results showed that all bias was less than 5% except one bias of TG result(7.02%)in one batch of LSP controls.Conclusion The work of quality control in this survey guaranteed the accuracy of plasma lipids measurement,and provided the basic data for the epidemiological description of dyslipidemia status among Chinese and the further analysis.

9.Clinical Observation of Tibial Plateau Fracture

The tibial plateau fracture is a common fracture in the knee joint. Tibial plateau fracture involves most always the articulating cartilage and produce the disability of the knee joint oftenly. The 25 cases of tibial fractures have been classified by Hohls method and analysed their clinical courses more than three month period. The 13 cases out of 25 had treated in conservative method such as plaster of Paris with or without closed reduction. Remaining 12 cases were treated operatively due to the failure of closed reduction or associated injury of major ligaments. More than 8 mm of depression or 5 mm split of the plateau fragment is classified as failure of closed reduction. Excellent end result is obtained in 21 cases with free of pain on walking and full range of joint motion without instability and muscle weakness, Three cases were classified as Good result with occasional pain on walking, range of joint motion is available more than 0~90 degree and good grade of muscle power without instability. One case was poor in result with instability of joint and pain on walking. The radiological end result of accurate reduction of fragment is not closely correlated with functional result in the treatment of tibial plateau fracture. The most important factor in the treatment of tibial plateau fracture is the exercises of the joint as early as possible.
