3.Metronidazole-induced Reversible Cerebellopathy

Metronidazole is a nitroimidazole antibiotic used for anaerobic infections, protozoan infections, and Helicobacter pylori infections. It may produce rare but well known neurological adverse effects including peripheral neuropathy, encephalopathy, cerebellar dysfunction, and seizures. We describe three patients with metronidazole-induced reversible cerebellar dysfunctions characterized by abnormal oculomotor findings, dysmetria, and ataxia. The diagnosis of metronidazole toxicity was made clinically and supported by the brain MRI findings. The neurological signs and abnormal signal intensity on MRI resolved after discontinuation of metronidazole.

4.The new approach to maxillary and mandibular anterior dental arch forms: The prediction to maxillary and mandibular anterior occlusal relationship by computer program.

When we deal with maxillary and mandibular anterior dental arches showing problems in occlusal relation and aesthetics caused by malformations of teeth and congenital missing, et al during the orthodontic treatment, we could not often decide the functional occlusion by only relying on the orthodontic treatment. If orthodontists can predict what kinds of treatments are needed for functional occlusion in maxillary and mandibular anterior dental arches, they can not only effectively treat patients but also facilitate the cooperation with other field during the treatment. Our previous research showed the correlation among intercanine width, segment depth and arch perimeter by using the Korean normal occlusion model. At this time, we produced the computer application program by taking advantage of this correlation. And then, we applied this program to setting up the treatment plans for 2 patients with the damaged maxillary and mandibular dentures. With the help of this program, we could not only easily acquire the information about the change of variables required by treatment plans but also understand the change of the anterior dental occlusion corresponding to the change of each variables such as the intercanine width, segment depth and arch perimeter. Later, if we can have the information about the relationship between the change of the angle of incisors depending on facial types and arch forms and, in addition, can acquire the appropriate intercanine width, we can have the ability to produce the 3 dimensional occlusogram for the anterior dental arch forms.

5.Horizontal Dorsal Septal Incision for High Deviation Correction During Septoplasty

When performing septoplasty in patients with a high deviation of the nasal septum, effective correction is difficult and postoperative complications such as a saddle nose may result if the bone or cartilage is removed inordinately. Although several surgical techniques have been introduced, some are difficult to apply easily. Furthermore, the deviation may persist despite the application of surgical techniques due to the rebound memory of the remaining cartilage. This study aimed to describe a simple and safe surgical technique for crooked nasal septa with a high deviation. This method using horizontal dorsal septal incision allows easy separation of the highly deviated portion from the upper lateral cartilage. Furthermore, it is less traumatic than other methods, and predictably preserves the keystone area.
6.c-erb B2 and p53 Overexpression in Presence of Human Papilloma Virus Type 16/18 Infection in Carcinomas of the Uterine Cervix: Genesis and Clinicopathologic Analysis.

The 43 cases of the primary uterine cervical carcinoma were analyzed for HPV type 16/18 infection and also analyzed for overexpression of p53 and c-erb B2 oncoprotein to evaluate theirs oncogenic and clinicopathologic relationships. HPV type 16/18 infection was examined by polymerase chain reaction(PCR) and the overexpression of p53 and c-erb B2 protein by using immunohistochemical method. The results were as follow: 1. The HPV infection rate in primary uterine cervical carcinomas was 83.7% respectively.The standard clinicopathologic characteristics(age, histologic type, koilocytosis, mitosis, clinical stage, tumor size, cervical invasion depth, lymph node metastasis, parametrial invasion) were nat significantly correlated with HPV type 16/18 infectivity. 2. The overexpression rate of p53 protein was 72.1% and there was no Significant correlation between expression of p53 protein and the Clinicopathologic characteristics. 3. The overexpression of c-erb B2 oncoprotein was 44.2% and there was no significant correlation between the overexpression of c-erb B2 oncoprotein and the clinicopathologic characteristics. 4. There was no significant correlation between HPV type 16/18 infection and overexpression of p53 and c-erb B2 oncoprotein.

The diffuse form of supravalvar aortic stenosis represents a surgical challenge when the ascending aorta, aortic arch, proximal descending thoracic aorta and arch arteries are involved. It can be treated by a variety of surgical approaches. We report a case of severe diffuse supravalvar aortic stenosis combined with an aortic valve anomaly and occlusion of the right coronary artery ostium in a 14-year-old boy with Williams syndrome. We enlarged the aortic root (Nick's procedure), ascending aorta, aortic arch, proximal descending thoracic aorta, and innominate artery with patches and replaced aortic valve with 19 mm St. Jude valve. Deep hypothermic circulatory arrest and retrograde cerebral perfusion were used during repair of the arch and arch artery.

8.A case of acute myocardial infarction caused by a septic coronary embolism and treated with embolectomy.

An acute myocardial infarction caused by a septic coronary embolism is a known complication of infective endocarditis and usually carries a fatal prognosis. We experienced a case of a 27-year-old man with an acute myocardial infarction caused by a coronary embolism due to bacterial endocarditis. An echocardiogram demonstrated mitral valve regurgitation with highly mobile multiple vegetations. Because of the high risk of systemic embolization and congestive heart failure, the patient underwent emergency surgery. Mitral valve replacement and an embolectomy were performed successfully without any surgical complications.

9.Development of Occupation Health Information System based on the Internet.

Recently the changes in medical informatics enabled us to use medical information whenever we want and wherever we are. However, there are few information supporting systems on the web in occupational health field. So we have tried to make an occupational health information system (OHIS) based on the Internet and to provide occupational information to occupational health personnel. There were two steps for developing OHIS. First, we analyzed user's needs and existing information systems and then developed contents of our OHIS. Second, we constructed web server under the environment of Windows user, using operating system of windows NT 4.0 and web browsers of Netscape Navigator. Many users in worksites wanted information through the Internet. The contents of OHS which they wanted were information related to health education of workers, health counselling, news, and so on. The contents of OHIS which we forwarded were occupational health service, information about organization of occupational health, book references and journals, link-sites related to occupational health. In the future, this Interned-based OHIS will be an useful site offering a lot of information to occupational health personnel.

10.A New Technique of Angioplasty of the Left Pulmonary Artery Using an Autologous Main Pulmonary Artery Flap.

BACKGROUND: Stenosis of the left pulmonary artery (LPA) after repair of tetralogy of Fallot (TOF) is troublesome. A new technique of LPA angioplasty using an autologous MPA flap was performed in patients with TOF. MATERIALAND METHOD: From October 1998 to January 2001, 24 patients (median age; 10 months, range; 4 to 145 months)underwent total correction of TOF with LPA angioplasty using the autologous MPA flap. Five patients underwent pulmonary angioplasty without any patch over the MPA and LPA. The patches were required to enlarge only the MPA in 4 patients, and transannular RVOT widening was performed in 15. RESULT: There were no operative or late deaths. During follow-up (range: 6~42 months), reoperation for LPA stenosis was not required in any patients, but balloon angioplasty for branch pulmonary artery stenosis was performed in 3 patients. Echocardiography and CT angiography at the recent follow-up showed an obtuse angle between the MPA and LPA. CONCLUSION: Although further follow-up is needed, the angioplasty using the autologous MPA flap can be easily performed, avoiding patch-related complications, and allowing growth of the MPA flap. This angioplasty technique creates a more natural and obtuse angle between the MPA and LPA, which can minimize kinking of the LPA, especially in the patients who underwent transannular patch widening.
