1.Effect of respirator resistance on tolerant capacity during graded load exercise.

Respirator breathing resistance impacts performance of wearers during constant work load. However, it is less clear as to how breathing resistance affects the tolerant capacity of users during graded work load. The present study investigated the tolerant capacity of 8 individuals during incremental work load. The 8 subjects were required to wear two matched respirators (respirators I and II which were designed to have different breathing resistances and the same dead space) respectively on separate days and then work to end points. Minute ventilation (V(E)), breathing frequency (BF), oxygen consumption (VO(2)) and heart rate (HR) were recorded during exercise, while tolerant time, response time and breathing discomfort were measured at the end of each test trial. The test variables were compared between the two respirators by using matched-pairs t-test. The results showed that the tolerant time was significantly reduced for the respirator I with higher level of breathing resistance when compared with its counterpart with lower breathing resistance (respirator II) (P<0.05). The same changes occurred for response time. Results also showed a significant increase in V(E) and BF for respirator I wearers when the work load was above 125 W. The O(2) consumption was similar under the two breathing resistance conditions. These findings suggested that the respiratory resistance caused by self-contained breathing apparatus (SCBA) has an impact on the tolerant capacity of users.
2.Expressions of TLR-2 and TRL-4 in gingival tissues of patients with periodontitis

Objective To detect the distributions and expressions of Toll-like receptor-2(TLR-2)and TLR-4 in different kinds of periodontitis and different extent of the inflammation of gingival tissues and to discuss the roles of TLR-2 and TLR-4 in the progress of periodontal inflammation.Methods Gingival biopsies were divided into 5 groups:control group(n=10),chronic periodontitis group(n=10),chronic periodontitis clinically healthy group(n=10),aggressive periodontitis group(n=10),and aggressive periodontitis clinically healthy group(n=10).The distributions and expressions of TLR-2 and TLR-4 were detected by immunohistochemistry.Results TLR-2 and TLR-4 expressed in all layers of gingival connective tissues.TLR-4 was also observed in gingival epithelium.Compared to control group,expressions of TLR-2 and TLR-4 were significantly higher than those in the other 4 groups(P
3.Cox regression analysis of prognostic factors of operative patients with colorectal cancer with time covariates

Objective To study the prognostic factors of operative patients with colorectal cancer.Methods Four hundreds and ninety-four patients with colorectal cancer treated from January 2003 to December 2009 in Shanxi Cancer Hospital were involved in this study.The demographic data,clinical and pathological features,serum levels of tumor markers were analyzed retrospectively.The prognostic factors were analyzed by univariate Kaplan-Meier survival analysis and multivariate Cox proportional hazards models.Results The 1-,2-,3-,4-,5-year survival rates of 494 patients were 92.31%,69.43 %,50.00 %,31.17 % and 12.96 % respectively.Non-Cox proportional hazards model with time-according to coefficient of multivariate analysis showed that Duke stage,tumor metastasis,pathological diagnosis and serum carcinocmbryonic antigen (CEA) levels were the prognostic factors of colorectal cancer.Conclusion The Duke stage,tumor metastasis,pathological diagnosis and CEA levels are the prognostic factors of colorectal cancer,which should be considered in the choice of clinical treatment and prognosis judgement.
4.Advance in studies on long-circulating tumor targeted compound preparations.

Tumor has long been a hard-nut problem in the world medical field. The effect of the conventional drugs is very limited because of the intervention of multiple micro-environmental factors during the occurrence and progression of tumors. With the characteristics of high efficiency, low toxicity and multi-targets synergistic effect, the long-circulating tumor targeted compound preparations show its unique advantages in improving tumor microenvironment and enhancing the therapeutic effect of treatment, thus it has gradually become a hotspot of studies both at home and abroad. Through consulting a great number of professional literatures at home and abroad in recent years, the authors summarized the current studies in vitro and in vive on long-circulating tumor targeted compound preparations in different carriers, in the expectation of providing new ideas and methods for the development of long-circulating tumor targeted compound preparations.

Objective To investigate the developmental profiles on surface expression and colocalization of NMDA receptor and AMPA receptor on dendritic structures in cultured hippocampal neurons of rats. Methods Two vectors expressing green fluorescent protein tagged GluR1 subunit(GFP-GluR1) and FLAG tagged NR2B subunit(FLAG-NR2B) were co-transfected into cultured hippocampal neurons at 5 days in vitro(DIV5).Surface expressed GFP-GluR1 containing AMPA receptor clusters and FLAG-NR2B containing NMDA receptor clusters were then labeled in living neurons by using anti-GFP primary antibody followed by Alex488-conjugated secondary antibody,and anti-FLAG primary antibody followed by Cy3-conjugated secondary antibody.Furthermore,distribution of the surface clusters was observed on the detailed dendritic structures visualized with co-expression of cyan fluorescent protein tagged actin(CFP-Actin). 7.Point prevalence of healthcare-associated infection and antimicrobial use in Buyi autonomous prefecture hospitals in Guizhou Province

Objective To explore the status of healthcare-associated infection(HAI)in hospitals in Buyi autono-mous prefecture of Guizhou Povince,and provide basis for formulating HAI control measures.Methods A survey was conducted by combined methods of bed-side survey and medical record reviewing,prevalence rates of secondary and above hospitals in Buyi autonomous prefecture in Guizhou Province between September 10 and October 5,2014 were surveyed.Results 6 577 hospitalized patients should be investigated,6 541(99.45%)were actually investiga-ted.The prevalence rate and case prevalence rate of HAI were 1 .83% (n=120)and 1 .94%(n=127)respectively. The top three departments of HAI distribution were intensive care unit (26.32%),neurosurgery (6.10%),and neonatal intensive care unit(5.13%);the main infection site was lower respiratory tract(n=39,30.71 %),followed by skin-soft tissue (n=24,18.90%)and superficial incision (n=23,18.11 %).58 pathogenic isolates were detec-ted,gram-negative bacteria were the major pathogens (n=44),gram-positive bacteria and fungi were 10 and 3 iso-lates respectively.Antimicrobial usage rate at survey day was 42.12%,64.75% of which were for therapeutic, 26.83% for prophylactic,and 8.42% for therapeutic+prophylactic use;the percentage of mono-drug,two drugs combination,and three or more drugs combination use were 79.53%,19.89%,and 0.58% respectively;bacterial detection rate in patients receiving therapeutic as well as therapeutic+prophylactic antimicrobial use was 13.76%. Conclusion Survey on prevalence of HAI is helpful for understanding the current status of HAI,monitoring on HAI in key departments of hospital and key sites of patients should be strengthened to reduce the occurrence HAI effectively.

Objective To investigate the current situation of healthcare-associated infection(HAI)in hospitals of Miao and Dong Autonomous Prefecture of Guizhou Province,and provide basis for formulating prevention and con-trol measures of HAI.Methods According to the unified plan of the National HAI Surveillance Network,26 hospi-tals in Miao and Dong Autonomous Prefecture of Guizhou Province were performed cross-sectional survey on HAI prevalence rate,antimicrobial use,and specimen bacterial culture rate.Results A total of 3 tertiary and 23 seconda-ry hospitals were investigated,7 799 inpatients were included,the prevalence rate of HAI was 2.54%(n =198), and case prevalence rate was 2.65% (n=205).HAI mainly distributed in intensive care unit (29.63%);the main infection site was lower respiratory tract (44.44%);HAI mainly caused by gram-negative bacteria,the major pathogens were Escherichia coli ,Pseudomonas aeruginosa ,and Klebsiella pneumoniae .The usage rate of antimi-crobial agents was 45.66%,secondary hospitals was higher than tertiary hospitals (53.65% vs 31 .14%,χ2 =148.53,P <0.001 ).74.02% of antimicrobial agents were for therapeutic purpose,19.77% for prophylaxis,and 6.21 % for both prophylactic and therapeutic application;81 .02% of patients received one agent,17.21 % received two,and 1 .77% received three and more agents;among patients who received antimicrobials for therapeutic as well as for both therapeutic and prophylactic purpose,only 29.37% were sent specimens for pathogenic detection.Conclusion The prevalence rate in this region is lower than national average level,antimicrobial usage rate is lower than national standard,management of key departments and key sites should be strengthened,antimicrobial agents,especially used in secondary hospitals should be used rationally.
9.Genetic diagnosis of a patient with non-syndromic variants of congenital neutropenia

ObjectiveTo explore the procedures and methods for genetic diagnosis in one nonsyndromic variants of congenital neutropenia (NSVCN) patient and its pathogenic mutation.Methods Genomic DNA was prepared from one NSVCN patient who had progressed to chronic myelomonocytic leukemia and ELA2,HAX1,WASp and GFI1 genes were amplified and sequenced.Results A novel compound heterogeneous mutation consisting of two frame-shift mutations (c.430-1insG and c.655- 9del5bp) was found in HAX1 gene.ConclusionA practically genetic diagnosis procedure for NSVCN has been established,and the novel HAX1 gene mutation may contribute to the etiology of NSVCN.
10.Thermal Physiological Consideration of Precooling Procedures in Manned Space Craft

Objective To explore a space craft precooling temperature at which excessive thermal stress on the crew member could be prevented or reduced in an overheated launch or reentry module. Method Five young male volunteers wearing a space suit participated in 25 tests at sea level.The space suit was either ventilated in a volume air flow rate of 100 L/min (STPD) with ambient air at temperatures (Ta) of 15℃,10℃,and 5℃,respectively,or not ventilated. Rectal (Tr),mean skin (Tsk) and mean body (Tb) temperatures were measured. Result At Ta 15℃,Tr decreased without significance (from 37.0±0.2℃to 36.7±0.3℃) in 120-min tests,whereas Tsk and Tb decreased significantly,and subjects had local cold strain whether the space suit was ventilated or not; while at Ta 10℃,Tr decreased from 37.0±0.3℃ to 36.3±0.3℃(P<0.05),subjects had a whole body cold strain,and both Tsk and Tb dropped continuously and significantly. Conclusion Ambient temperature 15℃,at which the thermal comfort states of crew was not significantly degraded,was acceptable after precooling in a space craft.
