3.Results of Lung Cancer Screening with a Spiral Computed Tomography Van - A Study of Findings

In recent years, spiral computed tomography has played an increasingly significant role in mass health screenings for lung cancer. Given this development, the Specialists’Committee for Lung Cancer Mass Screening in Nagano-Ken Koseiren (Nagano Prefectural Federation of Agricultural Cooperatives for Health and Welfare) came out with a van of its own suitable for mass screening by use of a spiral CT scanner and organized a caravan team of lung cancer screenings in 2001. With the collaboration of 11 general hospitals affiliated with Nagano-Ken Koseiren and its Health Care Center, our program includes films reading, guidance based on findings, workups, treatment and prognostic management. We examined 6,633 people in the first year and another 6,639 in the second year. In the two years, 55 examinees were found to have lung cancer. In the two years, we had 11,140 initial examinees, 54 of whom were found to suffer from lung cancer (detection rate : 0.48%). As many as 1,862 persons participated in the annual lung cancer screening two years running. Of them, one examinee was found to have lung cancer in the second year (detection rate : 0.05%). The sex-specific rate of detection in the initial screening was 0.25% for males and 0.80% for females, suggesting that the ratio for women was overwhelmingly higher. A histological check of 55 cases found to have lung cancer reveals that 51 cases (93%) had adenocarcinoma. After the removal of detected carcinomas, the diameters of tumors were found for 54 cases. Of them, there were 24 cases (44%) with a micro-cancer less than 10 mm in diameter and 43 cases (80%) with small-sized lung cancer less than 20 mm. In terms of stages, therefore, as many as 48 cases (87%) were in the IA stage, suggesting that this lung cancer screening is significantly useful in detecting lung cancer in an early phase.

4.A Case of Patient with Lung Cancer in the Terminal Stage whose Appetite and General Condition had been Maintained Following the Oral Administration of \it{Shoyogan}

Shoyogan is a Kampo prescription described in the Jinkuiyaolue text, but there have been no case reports regarding this medicine. We report a patient with terminal stage lung cancer, whose appetite and general well-being was maintained following oral administration of this medicine.A 62-year old woman had her main tumor in the left pulmonary lobe, which invaded to the bilateral peritoneum and the mediastinum. Metastasis was recognized in the left femoral bone which had been broken, the ilium, chest and lumbar vertebral bones, and the liver. Her general condition was very poor in spite of the administration of corticosteroids and narcotic medicines, when she began to take shoyogan 5 months after the onset of her symptoms. Cytological examination of her pleural effusion revealed adenocarcinoma. Following the administration of shoyogan, her appetite recovered, nutrition improved, and she was able to survive while maintaining her quality of life for another 2 months.In this paper we discuss the efficacy of shoyogan in improving the general well-being of patients having the above-mentioned conditions, or jueyinbing in Kampo parlance. Although preparing shoyogan is laborious, we believe this medicine should be used more frequently.

5.Current Update on the Management of Locally Advanced Non-small Cell Lung Cancer.

Locally advanced NSCLC is a heterogenous group of bronchogenic malignancies that are traditionally thought to be unresectable without overt distant metastasis or malignant pleural effusion. The mainstay of treatment for this class of diseases until the early 1990s was radiation alone, which resulted in a dismal outcome. The new technologies in radiation therapy (e.g. 3D-CRT) and the shift in paradigm (e.g. omission of ENI) have enabled the dose-escalation, which translated to improved outcome compared to the conventional radiotherapy using 2-D planning. The trials combining chemotherapy with radiotherapy, first sequentially, then concurrently, have changed the standard of care for patients with good functional status to concurrent chemoradiation. Some studies have shown survival benefits to adding consolidative systemic therapy with concurrent chemoradiation. We will outline the development of the current treatment standard of locally advanced NSCLC and present selected topics undergoing active research to forecast the next generation of NSCLC therapy

Malignant fibrous histiocytoma (MFH) is the most common soft tissue sarcoma. It is regarded as an undifferentiated pleomorphic sarcoma with an unproven histogenesis. It most commonly occurs in the skeletal muscles of the extremities or the retroperitoneum of adults. It often metastasizes to the lungs, lymph nodes, bones and liver, but rarely to the skin. We report here a case of malignant fibrous histiocytoma that metastasized to the skin.

Most chemical carcinogens are metabolized and activated in vivo by phase I enzymes including the microsomal cytochromes P450 and epoxide hydroxylases. The carcinogens and their metabolites are detoxified by phase II enzymes that include various transferases such as glutathion-S-transferases (GST). Increasing numbers of studies have demonstrated the association of the polymorphisms in GSTM1 (a member of GST) and CYP1A1 genes with the susceptibility to lung cancer. Subsequently, the polymorphisms appear to be important biomarkers that provide information for assessment of exposure and total burden of environmental carcinogens. Therefore, the investigation of the polymorphisms in these genes will provide information not only for the prediction of individual cancer risk but also for the prevention of cancer. In this review, we will summarize the polymorphisms in the GSTM1 and CYP1A1 genes and their relation to lung cancer susceptibility.

8.Peritoneal Dissemination from Malignant Fibrous Histiocytoma of the Buttock: Unusual Metastases from a Rare Tumor.

We report a case of malignant fibrous histiocytoma (MFH) of the right buttock with multiple metastases to the lung, bone, and small intestine. He received resection and end-to-end anastomosis of the jejunum for the jejunal metastatic tumor, and mass excision of the metastatic tumor of the left femur followed by closed reduction and internal fixation and palliative radiotherapy. In addition, he received palliative radiotherapy to the metastatic pulmonary tumor with suspicious invasion into the thoracic aorta. However, one month after the completion of the aggressive local treatments, metastatic tumors recurred in the abdominal cavity, an extremely unusual site, resulting in peritoneal dissemination. He died of progressive disease 5 months after the initial diagnosis.

9.Metastatic Malignant Fibrous Histiocytoma Mimicking Keratoacanthoma.

Malignant fibrous histiocytoma (MFH) has been considered to be the most common malignant soft tissue tumor of middle and late adulthood. This tumor is usually a skin-colored subcutaneous nodule. It metastasizes to the lung, lymph node, bone and liver. Metastasis to the skin is very rare. We present a case of a 69-year-old man who presented with an asymptomatic dome-shaped large nodule with central umbilical ulceration on his scalp that had grown abruptly over 3 months. The lesion clinically mimicked keratoacanthoma. Biopsy of the nodule was done to differentiate keratoacanthoma from MFH confirmed the latter. The result of biopsy was MFH. Diagnosis was MFH of unknown primary tumor origin. We report a case of metastatic MFH mimicking keratoacanthoma.

10.A Case of Pleomorphic Adenocarcinoma (Malignant Mixed Tumor) of the Lacrimal Gland.

Pleomorphic adenocarcinoma of the lacrimal gland is a rare malignant epithelial tumor. It is commonly metastasize to regional lymph node at early stage and sometimes to the lungs and bone. A 64-year-old man visited with complaints of progressive orbital protrusion and visual disturbance on his left eye for five months. CT scans demonstrated a huge mass on the superotemporal portion of the left orbit. Histopathological diagnosis of pleomorphic adenocarcinoma of the lacrimal gland was made. Bone scintigram and chest CT scans demonstrated distant metastases to the bone and lungs. We report a case of malignant mixed tumor of the lacrimal gland with mltiple distance metastases.
