Circumcision has been popularly performed with mechanical device such as Gomco clamp, Plastibell device and other modified device. We experienced a case of denudation of entire penile epithelium and abnormal curvature during erection after circumcision using self-circumcision device (GM sense) at home. So we report with a brief review of literature.

3.A Case Report of Proliferative Myositis in Pectoralis Major Muscle.

Proliferative myositis is very rare and was first described as a specific entity in 1960 by Kern. Proliferative myositis is a benign reactive condition that appears as a rapidly growing mass in the proximal muscles of the extremities of adults. Proliferative myositis can be confused with sarcoma clinically as well as microscopically. Proliferative myositis is a bizarre, self-limiting fibroblastic proliferation, the presence of very large basophilic cells with vesicular nucleoli and very prominent nucleoli which resemble ganglion cells or rhabdomyoblast, the cause of which is unclear. The symptoms are non-specific and the diagnosis always rests on a histological examination of the tissue. Local excision is curative and recurrence or metastasis has not been reported even when the abnormal tissue has not been completely excised. The authors report a case of Proliferative myositis in a 59-year-old male.

4.Primary Hyperparathyroidism.

PURPOSE: Primary hyperparathyroidism is the most frequent cause of hypercalcemia and due to the routine examination of serum calcium levels, the number of patients with primary hyperparathyrodism has increased. METHODS: We reviewed sixty patients treated by surgery over the 20-year period at the Department of Surgery, Seoul National University Hospital and retrospectively analyzed both the clinicopathologic and bio-chemcal features of the presented cases and the effective methods of treatment and diagnosis retrospectively. RESULTS: The study group consists of 60 patients with primary hyperparathyroidism comprised of 18 males and 42 females. The most common presenting clinical manifestations were associated with bone pain in 38 (63%), and recurrent urinary stone in 27 (45%) patients; And 11 patients were asymptomatic. All patients showed hypercalcemia and the serum parathyroid hormone (PTH) level was elevated in all 49 patients after 1992. A preopeative localization study was performed with ultrasonography (USG), computed tomography (CT), Thallium-Technecium (Tl-Tc) substraction scan and Methoxylisobutyl isonitrile (MIBI) scan. Positive localization was made in 71%, 71%, 77% and 71% respectively. Histopathologic findings revealed solitary adenoma in 56 patients and carcinoma in 4 patients. Transient hypocalcemia following surgery occurred in 50 patients and they were treated with calcium. There were no recurrent cases with the exception of one who displayed carcinoma. CONCLUSION: In this study, most of the patients were discovered with an advanced clinical and biochemical status but the number of the asymptomatic patients is increasing. Through an accurate preoperative localization, they were successfully treated with parathyroidectomy without major complications.

5.Two Cases of Primary Intracranial Melanoma.

Primary malignant melanoma arising from the leptomeninges is a rare entity. We report two cases of primary intracranial melanoma developed in one aged 65 years female and the other 70 years male. One case died 2 years after the operation, and the other case is still living more than 1 year after operation. Both cases seems to have a relatively long survival. The literature on this subject is briefly reviewed.

6.A Case of Idiopathic Sclerosing Peritonitis.

Idiopathic sclerosing peritonitis is a rare disease characterized by fibrosis and adhesion of the peritoneum to loops of the small intestine. It may be the cause of an unusual surgical emergency such as small bowel ileus. It is diagnosed predominantly in female adolescents. We report the case of an idiopathic sclerosing peritonitis in Korea. A 38-year-old man visited emergency room for recurrent small bowel ileus and migrating mass like lesion. Computed tomography (CT) of abdomen showed acute peritonitis with a diffuse wall thickening of terminal ileum and extraluminal fluid collectionaround the terminal ileum. He underwent laparotomy. The ileocolectomy with adhesiolysis was performed and its pathological examination revealed the characteristic findings of idiopathic sclerosing peritonitis. Symptoms recurred 2 months after surgery, but improved with steroid treatment.

7.Trends in Anger and Aggression Across the Life Span in Korean Population.

OBJECTIVES: This study was to investigate the trends in anger and aggression with aging in Korean population aged from twenties to eighties. METHODS: Two hundreds ninety-five subjects included, ranged 22 to 92 years old. All subject participate in a clinical interview with psychiatrists who use the MINI (Mini international Neuropsychiatric Interview) to diagnosis psychiatric disorders, and were not any psychiatric disorders. RESULTS: In elderly group, the scores of Trait anger, Anger expression, and totol of STAXI were higher than adult group, where as the scores of State anger of STAXI, Hostility of AQ, and Disturbances, Embarrassing, Unpleasant circumstances, and total of RI were higher in adult group. Trait anger and Anger expression were shown trends to increase with age. On the other hands, State anger, Hostility, and Reaction to external stimuli were shown trends to decrease with age. CONCLUSION: The reaction to anger situation and aggression decrease with age, although anger increase with age. Various factor, such as defense mechanism may affect on aggressive expression and response. There will be needed to investigate associated factors between anger and aggression.

8.Sleep-Related Breathing Disorders in the Elderly: Clinical Feature, Diagnosis and Treatments.

Sleep-related breathing disorder in the elderly is common but not diagnosed and treated properly. Sleep apnea in the elderly differs from that in young adults and does not show overt sign of it such as snoring and excessive daytime sleepiness. Co morbidities of elderly patients make it difficult to diagnosis and treatment of sleep apnea. Untreated sleep-related breathing disorders can cause cardiovascular disease, cognitive impairment and dementia. Sleep-related breathing disorder is diagnosed with thorough history taking and polysomnography. Continuous positive airway pressure (CPAP) is treatment of choice for sleep apnea in elderly. When patient failed to adapt in CPAP, oral appliance or surgical approach can be considered.

9.Beckwith-Wiedemann Syndrome with Unusual Sialoadenomegaly.

Beckwith-Wiedemann syndrome is a rare clinical entity characterized by exomphalos, macroglossia, macrosomia, and renal hyperplasia/dysplasia. Although its entity is established, its etiology and obligatory features have not been settled. We report an autopsy case with the unusual involvement of the salivary gland. This infant was born to a 37-year-old mother as a normal full-term spontaneous delivery. At 11 days of age she developed with purulent eye discharge and weak sucking, and died suddenly. At autopsy the baby weighed 2,630 gm and the head circumference was 35 cm. She showed thick and prominent skin folds, bilateral aural fissures, macroglossia, hepatomegaly, cardiomegaly, dysmorphic kidneys, and nesidioblastosis. Both kidneys showed dysplastic tubules and hyperplastic cortical tissue enclosing the medulla. In this case there were characteristic findings in major and minor salivary glands with both acinar and ductal hyperplasia, and hypertrophy of mammary glands. Besides, she had generalized depletion of subcutaneous fat, immature buccal fat, patent ductus arteriosus, hyperlobation of the right lung, two accessory spleens, and hyperplasia of basophils and chromophobes in the pituitary gland. The lungs showed diffuse interstitial pneumonia and multiple fibrin thrombi. There were no adrenal cytomegaly, umbilical hernia and exophthalmos.

10.Two Cases of Prurigo Pigmentosa: A Histopathologic Anaylsis.

Prurigo pigmentosa - a rare dermatosis described first by Nagashima in 1971 - presents as intensely pruritic papules arranged in a reticular pattern and distributed symmetrically on the back of young women. Individual lesions involute within weeks, leaving net-like pigmentation, but recurrences are the rule. According to the recent report of Ackerman et al., prurigo pigmentosa shows discernable characteristic histopathologic stages successively in its natural course. We report two cases of prurigo pigmentosa with clinico-histopathologic analysis - one, occurred in 24-year old man who recovered spontaneously after 7 months of morbidity and the other, occurred in 23-year old woman who responded dramatically to dapsone.
