Nowadays the agricultural production system in the farm area has remarkably been changed and this has influenced the farmers in many aspects of life.
We tried by epidemiologic cross-sectional survey to ascertain the health level of 6, 597 farmers, who are self-supporting and middle class in the sixty-seven different districts of Kumamoto Prefecture. It is noticeable that judging from the difference of districts, the health level of the farmers who have a big gricultural production is low, and judging from the difference of the class, that of the farmers who have other jobs besides agriculture is low, too In general, the health level of males is relatively higher than that of females, but the health level of males is higher than that of females, depending on the class difference.
Generally speaking, we noticed that the health level of female is very low, and at the same time the variation of the health level of females is more remarkable than that of males, depending on the quality of the district in which they live.
These resutls of these investigations should be utilized as the basic materials source for a prospective survey of changing farm village from now.
2.Deep Vein Thrombosis in the Lower Extremities in Comatose Elderly Patients with Acute Neurological Diseases.

PURPOSE: Comatose elderly patients with acute neurological illness have a great risk of deep vein thrombosis (DVT). In this study, the incidence of DVT and the effectiveness of early initiation of treatment were evaluated in those patients. MATERIALS AND METHODS: Total 323 patients were admitted to our ward due to neurological diseases in one year, and 43 patients, whose Glasgow Coma Scale was < or =11 and who was older than > or =60 years, were included in this study. D-dimer was measured on admission and day 7, and lower-extremity ultrasonography was performed on day 7. When DVT was positive, heparin treatment was initiated, and further evaluation of pulmonary embolism (PE) was conducted. Vena cava filter protection was inserted in PE-positive patients. Incidence of DVT and PE, alteration of D-dimer value, and effect of heparin treatment were analyzed. RESULTS: DVT was positive in 19 (44.2%) patients, and PE was in 4 (9.3%). D-dimer was significantly higher in DVT-positive group on day 7 (p<0.01). No DVT were identified in patients with ischemic disease, while 66.7% of intracerebral hemorrhage and 53.3% of brain contusion patients were DVT positive. Surgery was a definite risk factor for DVT, with an odds ratio of 5.25. DVT and PE disappeared by treatment in all cases, and no patients were succumbed to the thrombosis. CONCLUSION: Patients with hemorrhagic diseases or who undergo operation possess high risk of DVT, and initiation of heparin treatment in 7 days after admission is an effective prophylaxis for DVT in comatose elderly patients without causing bleeding.

3.Ross Operation for Prosthetic Valve Endocarditis in a Patient with Aortitis Syndrome

A 60-year-old man was admitted to our hospital due to cerebellum infarction. He had undergone replacement of the aortic valve and ascending aorta because of aortitis syndrome 2 years ago. Electrocardiogram showed complete atrioventricular block. Echocardiography showed aortic annular abscess and vegetation on the prosthetic aortic valve. A pulmonary autograft was transplanted of the aortic root (Ross operation) after complete resection of the infected sites. The postoperative course was uneventful. The ross operation was considered to be a treatment of choice for prosthetic aortic valve endocarditis.
6.Relationship between questionnaire survey results of vibration complaints of wheelchair users and vibration transmissibility of manual wheelchair.

OBJECTIVEClarify the relation between the complaints of wheelchair users and the vibration characteristics of the wheelchair, to improve wheelchair comfort and design.
METHODSThe question naires were distributed to 33 wheelchair users directly by the experimenter in order to identify the causes of complaints from wheelchair vibrations that they experienced. The vibration transmissibility of wheelchairs of ten subjects was measured to clarify the causes of complaints of wheelchair vibration according to the ISO 10326-1 standard in the laboratory using a broadband random vibration spectrum with a frequency-weighted vibration magnitude of 0.1 ms(-2) r.m.s. over the frequency range from 0.2 to 100 Hz. Each vibration exposure lasted 60 seconds.
RESULTSThe following findings were clear from the questionnaire results; (i) the vibration rom the wheelchair affected psychological comfort; (ii) the effects of different riding surfaces were important engineering issues affecting wheelchair ride comfort; (iii) the wheelchair users felt the vibration during wheelchair usage at locations on the neck, lower back and buttocks; (iv) vertical vibration was the most noticeable vibration from the wheelchair to each participant's body. The following findings were clear from the results of the transmissibility measurement of the wheelchair; (i) the resonance frequency-ranges of the transmissibility of the wheelchair showed significant differences between the subjects; (ii) intra-subject variability from three repeated transmissibility measurements was small; (iii) the first resonant frequency occurred approximately 5 to 7 Hz and the second resonant frequency occurred at around 8 Hz and the third resonant frequency occurred approximately 13 to 15 Hz; (iv) the magnitude of the peak transmissibility varied from 1.3 to 2.6.
CONCLUSIONFrom the comparison of the results of questionnaires and the transmissibility measurement of the wheelchair, the resonance frequency-ranges of the maximum vibration transmissibility of the manual wheelchairs were consistent with the frequency-ranges of the body parts of the causes of the complaints of wheelchair users. In addition, from these experimental results, it was suggested that the main point for improving a wheelchair user's comfort was to reduce the wheelchair seat vibration transmissibility at around 8 Hz and also to design wheelchair stiffness and damping characteristics to minimize vibration transmission at specific frequencies at body locations that caused the discomfort reported by wheelchair users.
7.Role of LPS-stimulated human monocyte-derived dendritic cells in the modulation of autologous CD4+ CD25+ T Cell activation.

Dendritic cells (DC) are now recognized as the most potent professional antigen presenting cells (APC). Several studies on cancer immunotherapy using different approaches to induce cytotoxic T lymphocytes (CTL) in vivo recognizing tumor-associated antigens have been reported. However, the efficacy of immunotherapy in vivo may be limited by the local or systemic suppression of CTL generation or function. To explore the ability of lipopolysaccharide (LPS) stimulated human monocyte-derived DC involved in activity of autologous CD4(+)CD25(+) T cells, HLA-A2 restricted p53(264 - 272) peptide was used as tumor antigen, DC generated with LPS (DC-LPS(+)) or without LPS (DC-LPS(-)) were co-cultured with autologous T cells respectively. The results showed that CD4(+)CD25(+) T cell population in the DC-LPS(+) activated T cells was lower than that in the DC-LPS(-) activated T cells. This finding suggest that the relationship between DC-LPS(+) and population of CD4(+)CD25(+) T cells exists and this property may contribute to regulation of T cell responses to tumor-associated antigens.

8.Differences in the effects of BMI on bone microstructure between loaded and unloaded bones assessed by HR-pQCT in Japanese postmenopausal women

Objectives:
The relationship between weight-related load and bone mineral density (BMD)/bone microstructure under normal load conditions using high-resolution peripheral quantitative computed tomography (HR-pQCT) remains unconfirmed. The study aims to investigate the differences in effect of body mass index (BMI) on BMD/bone microstructure of loaded and unloaded bones, respectively, in Japanese postmenopausal women.
Methods:
Fifty-seven postmenopausal women underwent HR-pQCT on the tibia and radius. Correlation analysis, principal component (PC) analysis, and hierarchical multiple regression were performed to examine the relationship between BMI and HR-pQCT parameters.
Results:
Several microstructural parameters of the tibia and radius correlated with BMI through a simple correlation analysis, and these relationships remained unchanged even with an age-adjusted partial correlation analysis. PC analysis was conducted using seven bone microstructure parameters. The first PC (PC1) reflected all parameters of trabecular and cortical bone microstructures, except for cortical porosity, whereas the second PC (PC2) reflected only cortical bone microstructure. Hierarchical multiple regression analysis indicated that BMI was more strongly related to BMD/bone microstructure in the tibia than in the radius. Furthermore, BMI was associated with trabecular/cortical BMD, and PC1 (not PC2) of the tibia and radius. Thus, BMI was strongly related to the trabecular bone microstructure rather than the cortical bone microstructure.
Conclusions
Our data confirmed that BMI is associated with volumetric BMD and trabecular bone microstructure parameters in the tibia and radius. However, although BMI may be more related to HRpQCT parameters in the tibia than in the radius, the magnitude of association is modest.
9.Immunohistochemical differentiation between chronic enteropathy associated with SLCO2A1 gene and other inflammatory bowel diseases.

BACKGROUND/AIMS: We recently identified recessive mutations in the solute carrier organic anion transporter family member 2A1 gene (SLCO2A1) as causative variants of chronic enteropathy associated with SLCO2A1 (CEAS). The aim of this study was to evaluate SLCO2A1 protein expression in the intestinal tissues of patients with CEAS, intestinal Behçet's disease (BD), simple ulcer (SU), and Crohn's disease (CD). METHODS: Immunohistochemical staining using a polyclonal anti-SLCO2A1 antibody was performed on the resected intestinal specimens from 13 cases of CD, 9 cases of intestinal BD/SU, and 3 cases of CEAS. The extent of SLCO2A1 expression was determined by counting positively-staining vascular endothelial cells and scored as 0 (no cells), 1 (1%–30% cells), 2 (31%–60%), or 3 (>60%). The intensity of SLCO2A1 expression was scored either as 0 (negative), 1 (intermediate), or 2 (strong). The extent score and intensity score were summed for the final score of 0, 2, 3, 4, or 5. RESULTS: SLCO2A1 protein expression was observed in 1 of 3 cases of CEAS (33%), all 13 cases of CD (100%), and all 9 cases of BD/SU (100%). The mean final expression scores of CEAS, CD, and BD/SU were 1.6 (range, 0–5), 4.8 (range, 4–5), and 4.3 (range, 4–5), respectively. The final expression score in CEAS was significantly lower than in CD (P=0.03). CONCLUSIONS: Immunohistochemical staining of the SLCO2A1 protein is considered useful to distinguish CEAS from other inflammatory bowel diseases.
