1.A Case of Spontaneous Rupture of the Kidney.

Spontaneous renal rupture is relatively uncommon entity. In Korean literature, 8 cases have been reported. We experienced 1 case of spontaneous renal rupture secondary to hydronephrosis caused by metastatic adenocarcinoma of undetermined origin invading upper ureter and presented it with a brief review of literature.

2.A Case of Renal Cell Carcinoma which was Metastasized to Ureter, Bladder and Genital Organ.

The metastasis of renal cell carcinoma to ureter, urinary bladder and uterus are not common, only a few cases were reported. A 63 years old female was admitted to department of urology, Korea university hospital with intractable hematuria, vaginal bleeding and severe frank pain on left is side with abdominal distension. Herein we are presenting a case of metastatic renal cell carcinoma with review and literature.

3.Association of Body Mass Index with Suicide Behaviors, Perceived Stress, and Life Dissatisfaction in the Korean General Population.

OBJECTIVE: The purpose of the present study was to investigate the association between Body Mass Index, suicide, perceived stress, and life dissatisfaction in a general population sample of Korean adults. METHODS: A total of 6,022 nationally representative adults aged 18 to 74 were selected using a multistage cross-sectional cluster sampling method. Questionnaires regarding suicide behaviors, perceived stress, and life satisfaction were completed by the participants. They also reported their heights and weights, which were used to calculate BMI. Psychiatric disorders were diagnosed according to the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, using the Korean version of the Composite International Diagnostic Interview. RESULTS: The results showed that being underweight was associated with higher risk for suicide ideation [odds ratio (OR), 1.6; 95% confidence interval (Cl), 1.18–2.05] and suicide attempt (OR, 2.0, 95% Cl, 1.23–3.31). Likewise, obesity also increased the risk of suicide ideation (OR, 1.3; 95% Cl, 1.11–1.56) although not suicide attempt. Furthermore, underweight individuals were more likely to report severe level of perceived stress (OR, 1.7; 95% Cl, 1.26–2.17) and life dissatisfaction (OR, 1.3; 95% Cl, 1.07–1.68). All of the results remained significant after adjusting for age, gender, education, and psychiatric illnesses. CONCLUSION: This study found that being underweight is a significant risk factor for suicide and poor subjective wellbeing in Korea. It suggests that BMI status may be an important modifiable factor for improving mental health in Korea.

The Author's are presenting a case of hypoplastic ectopic kidney in pelvic cavity with short ureteral length and ectopic ureteral orifice in vaginal vestibulum. The size of ectopic kidney was unusually small, chest nut size, as dislocated kidney congenitally which made put more effort then usual diagnostic approach to locate it.

5.Prevalence of Cognitive Impairment and Depressive Symptoms among the Elderly in an Urban Community.

OBJECTIVES: This study was to investigate the prevalence of cognitive impairment and depressive symptoms among the elderly in an urban community. METHODS: Korean version of Mini-Mental State Examination(MMSE-K), Short version of Geriatric Depression Scale(SGDS), and the Center for Epidemiologic Studies Depression(CES-D) were administered to 447 elderly persons(37.7% of all residents aged 65 and older) who resided in "Sanggye 10 Dong" by lay interviewers. Then a Psychiatrist and two Psychiatric residents interviewed the elderly scoring 23 or less on MMSE-K and diagnosed with dementia according to the criteria of DSM-III-R. RESULTS: The prevalence of cognitive impairment was estimated as 37.1%(male 18.8%, female 47.0%) by MMSE-K

8.Comparing Various Short-form Geriatric Depression Scales in Elderly Psychiatric Patients in Korea.

OBJECTIVES:The purpose of this study is to evaluate the diagnostic validity of the three published short Geriatric Depression Scales and to identify a valid and reliable alternative to the Korean version of the Geriatric Depression Scale (GDS-K) and short form Geriatric Depression Scales (SGDS-K). METHODS:All subjects were registered elderly psychiatric patients who first visited the Geriatric Psychiatry Clinic of Seoul National University Hospital in Seoul between April 1996 and May 1999. The GDS-K, SGDS-K, Hoyl GDS-5, D'Ath GDS-4, van Marwijk GDS-4 were administered. Receiver operating characteristics (ROC) curve analysis performed to investigate the diagnostic validity of the Hoyl GDS-5, D'Ath GDS-4, and van Marwijk GDS-4. RESULTS:The best trade-off between sensitivity and specificity was achieved with a Hoyl GDS-5 score of 3 (sensitivity:0.7258, specificity:0.7717), a D'Ath GDS-4 score of 3 (sensitivity:0.5968, specificity:0.7391), and a van Marwijk GDS-4 score of 3 (sensitivity:0.6613, specificity:0.7391). The area under the ROC curves of each diagnostic tool were calculated into the Hoyl GDS-5 of 0.8198, the D'Ath GDS-4 of 0.7525, and the van Marwijk GDS-4 of 0.7528. CONCLUSION:The Hoyl GDS-5 may be utilized in screening for major depression among the elderly psychiatric patients in Korea.

9.A Case of Trichilemmal Cyst Invading Submandibular Gland.

A trichilemmal cyst is a skin adnexal tumor that usually occurs on the scalps of elderly women. We report a trichilemmal cyst in the submandibular area of a 16-year-old male, masquerading as a second branchial cleft cyst. During surgery, the mass was found to have invaded the submandibular gland and needed to be excised totally in continuity with the submandibular gland. To our knowledge, this is the first reported case of a trichilemmal cyst involving the submandibular gland. Although rare, a trichilemmal cyst may be considered in the differential diagnosis of cystic lesions in the submandibular area. It should also be differentiated from other cystic malignant tumors for its tendency to invade the surrounding structure.

10.A Clinical Observation on Craniocerebral Injuries in Infants and Children.

A Clinical analysis was carried out with 340 cases of craniocerebral injuries under 15 years of age, admitted at the Department of Neurosurgery, Seoul National University Hospital in 12 years between Jan., 1987 and Dec., 1969. The matrial was classified four groups according to main lesions, i.e. (1) mild cerebral contusion without skull fractures. (2) moderate or severe cerebral contusion without skull fractures. (3) various types of skull fractures. (4) intracranial hemorrhagic lesions, representing such lesions as follows: a. epidural hematoma, b. subdural hematoma, c. subdural hygroma, d. intracerebral hematoma, e. traumatic subarachnoid hemorrhage. The results were as follows: 1) The age incidence was greatest in 6 years of age, and 120 cases (32.6% of the total) were included in the age group between four and six. 2) Males outnumbered females by almost 2 to 1. 3) The accident occurred mostly between 1 p.m. and 4 p.m. in a day. 4) The craniocerebral injuries were caused by traffic accident(173 cases; 50.8%), falls from heights(108 cases; 31.8%), hit(55 cases; 10.9%), firearms(2 cases; 0.6%) and birth injury (2 cases; 0.6%). 5) Early convulsion was the frequent symptom in the age group between 2 and 7. It was felt that convulsions frequently occurred in epidural and subdural hematoma patients. 6) Forty eight per cent of patients had no deterioration of consciousness on admission examination. 7) Epidural hematoma was the common finding in most of the depressed skull fracture cases. 8) The location of skull fractures in the order of frequency were parietal, temporal, frontal and occipital bone. 9) The epidural hematoma was most common lesion in the 13 to 15 year-old group. 10) The over-all mortality rate was 7.1%. In the surgical group(79 cases) the operative mortality was 11.4%(9 cases): the mortality rate of the subdural hematoma, 25.0%(3 cases); the epidural hematoma, 11.1%(2 cases); the compound depressed fracture, 7.8%(2 cases). In the non-surgical group(261 cases) it was 5.8%. The mortality rate of those who and been unconscious after injuries was significantly high(75.0%). 11) The period of hospitalization was two weeks on an average.
