Background Measurement of ciliary sulcus distance is important for phakic intraocular lens implantation.Suowei panoramic 50-MHz digital ultrasound biomicroscopy makes it possible for the direct measurement of ciliary sulcus distance.Objective The aim of this study was to investigate the correlation between anterior chamber(AC)angle diameter and ciliary sulcus distance in different AC depth population and offer a clinical guide.Methods The anterior chamber angle diameter and ciliary sulcus distance were measured in 30 right eyes with shallow anterior chamber caused by suspect glaucoma and 30 right eyes with deep anterior chamber dut to the high myopia with Suowei panoramic 50-MHz digital ultrasound biomicroscopy.Three horizontal B-scans with the focal plane approximately at a depth corresponding to the iris plane were acquired for each eye.The differences of the anterior chamber angle diameter and ciliary sulcus distance were calculated and compared between the two groups.The correlation of the anterior chamber angle diameter and ciliary sulcus distance were evaluated in respective group.Results In shallow AC group,the mean angle diameter was(11.49+0.75)mm,the mean ciliary sulcus distance was (10.97±0.86)mm with the mean standard deviation of the difference of(-5.14± 0.55)mm,showing a statistically significant difference(t =-5.092,P =0.000).In shallow AC group,linear regression analysis showed a statistically significant correlation between ciliary sulcus distance and angle diameter(R2=0.593,P=0.000)and the Pearson's correlation coefficient was 0.767.In deep AC group,the angle diameter was(12.69±0.67)mm;while ciliary sulcus distance was(12.31 ± 0.61)mm with the mean slandard deviation(-0.38 ± 0.17)mm,which was statistically significantly different(t=-5.531,P=0.000).The ciliary sulcus distance was found to be linear correlation with the angle diameter(R2 =0.699,P=0.000)and Pearson' s correlation coefficient of 0.836.Conclusions Panoramic ultrasound biomicroscopy is an useful tool to the measurement of ciliary sulcus distance.This study exhibited that the ciliary sulcus distance is significant associated with the anterior chamber angle diameter,which may offer a guide for phakic intraocular lens implantation.

Objective: To observe the clinical efficacy of application in canicular days plus enteral nutrition in treating cough variant asthma (CVA) in kids, and to explore its action mechanism. Methods:Following a randomized controlled single-blind parallel-group design, 138 eligible kids with CVA were randomized into an observation group, a canicular-day application group, and an enteral nutrition group, 46 kids in each group. The canicular-day application group was intervened by application in canicular days, the enteral nutrition group was by enteral feeding, and the observation group was by both canicular-day application and enteral feeding. The therapeutic efficacies were evaluated after a treatment course. Results: The recovery rate and total effective rate were respectively 50.0% and 98.0% in the observation group, versus 23.9% and 91.3% in the canicular-day application group, and 13.0% and 78.6% in the enteral nutrition group. The observation group was significantly superior to the other two groups (bothP<0.05). In comparing the global symptom score, peak expiratory flow (PEF), forced expiratory volume in one second (FEV1), CD3+, CD4+, CD4+/CD8+, CD8+, hemoglobin (Hb), total protein (TP), albumin (ALB), eosinophil cationic protein (ECP), lipid peroxide (LPO), leukotriene (LT), body weight (BW), triceps skin-fold (TSF), and arm muscle circumference (AMC), the observation group was significantly better than the other two groups (bothP<0.05). Conclusion:Application in canicular days plus enteral nutrition can significantly improve the pulmonary function and symptoms in children’s CVA, and the effect is possibly produced by regulating cellular immune system, enhancing Hb, TP, ALB, BW, TSF, AMC, and inhibiting the production of ECP, LPO, and LT.

BACKGROUND:Posterior lumbar interbody fusion is a typical therapeutic method of lumbar degenerative disease. Present studies suggested that adjacent segment degeneration occurs after fusion. Recently, more and more scholars paid attention to the development of non-fusion of the spine. ＜br> OBJECTIVE:To compare clinical effects of Dynesys dynamic stabilization system fixation and posterior lumbar interbody fusion in treatment of lumbar degenerative disease. ＜br> METHODS:From July 2009 to July 2011, clinical data of 56 patients with lumbar degenerative disease were retrospectively analyzed. There were 28 cases of Dynesys dynamic stabilization system fixation, and 28 cases of posterior lumbar interbody fusion. Operation time, bleeding volume, and postoperative hospitalization time were compared in both groups. Visual analog scale was used to assess pain. Oswestry disability index was utilized to evaluate clinical effects. ＜br> RESULTS AND CONCLUSION:A total of 56 patients were fol owed up for 18-24 months. Visual analog scale and Oswestry disability index scores were significantly improved at 12 months after treatment in both groups (P＜0.01). Significant differences in operation time, bleeding volume, and postoperative hospitalization time were detected between both groups (P＜0.01). Dynesys dynamic stabilization system group was better than posterior lumbar interbody fusion group. Range of motion was better in the Dynesys dynamic stabilization system group than in the posterior lumbar interbody fusion group (P＜0.01). During fol ow-up, nail rope system and polyester sleeve loose were not detectable in the Dynesys dynamic stabilization system group. These results verified that compared with posterior lumbar interbody fusion, Dynesys dynamic stabilization system for lumbar degenerative diseases has a high safety and smal trauma, and can keep advantages of a fixed segment, and exert a certain effect on degeneration of intervertebral disc in the adjacent segment.

A growing body of evidence suggests that p300 histone acetyltransferase plays important roles in cancer cell differentiation and proliferation. Here, we employed structure-based hierarchical virtual screening method to identify novel lead compounds of p300 histone acetyltransferase. From a screening library containing approximate 100 000 diverse druglike compounds, 33 compounds were chosen for experimental testing and one compound, 4-acetyl-2-methyl-N-morpholino-3,4-dihydro-2H-benzo[b][1, 4]thiazine-7-sulfonamide (17), showed as micromolar inhibitor. Based on its predicted binding pose, we investigated its binding characteristics by designing two series of structural modifications. The obtained structure-activity relationship results are consistent with the predicted binding model. We expect that the identified novel p300 histone acetyltransferase inhibitors will serve as starting points for further development of more potent and specific histone acetyltransferase inhibitors.

Objective To investigate the relationship between the expression of MAGE-1,SSX-1,and CTp11 mRNA in the peripheral blood of patients with hepatocellular carcinoma (HCC) receiving orthotopic liver transplantation (OLT) and the prognosis of these patients. [WT5”HZ]Methods The expression of the three genes in the peripheral blood mononuclear cells (PBMCs) was detected by nested RT-PCR,and a follow-up was carried out in 26 patients. Results Before OLT,the positive rate of MAGE-1,SSX-1 and CTp11 transcripts was 40.0% (14/35),37.1% (13 /34),and 31.4% (11/35),respectively in the PBMC samples of HCC patients;Sixty percent (21/35) of the PBMC samples were positive for at least one of the three gene transcripts. The follow-up of 26 patients showed that,the tumor recurrence/metastasis rate after OLT (75.0%) in 16 patients with positive preoperative PBMC samples of at least one gene transcripts was significantly higher than that of the other 10 patients who was negative preoperatively for gene transcripts (30.0%)( P = 0.043).Postoperative tumor recurrence or metastasis developed in 15 out of 20(75%) patients with persistently perioperative positive MAGE-1,SSX-1 and /or CTp11 transcripts in PBMC samples or swithing from negative to positive perioperatively,in contrast to no recurrence nor metastasis in the other 6 patients with the gene transcripts switching from positive to negative postoperatively,or negative perioperatively ( P =0.002). [WT5”HZ]Conclusions MAGE-1,SSX-1 and CTp11 transcripts may act as combined tumor-specific makers to detect tumor cells in the peripheral circulation of HCC patients,and the detection is valuable for selecting OLT candidates and predicting the postoperative prognosis.

Objective To observe the effect of acupuncture and acupoint injection on dementia in elder.Methods 38 old dementia patients were divided into senile dementia(Alzheimer's disease,17 cases)and vascular dementia(21 cases).They were treated with acupuncture and acupoint injection with Piracetam.Results 7 cases improved in senile dementia patients,while 2 cases were excellent,7 cases were good,9 cases improved in vascular dementia patients.Conclusion Acupuncture and acupoint injection with Piracetam is more effective on senile dementia than vascular dementia.

Objective To investigate the causes of an outbreak of healthcare-associated infection with methicillin-resist-ant Staphylococcus aureus (MRSA)in a neurosurgical intensive care unit(NSICU).Methods Epidemiological investigation on 8 patients with lower respiratory tract infection (LRTI)in a NSICU between June 15 and June 28,2104 were performed by combination methods of prospective and retrospective survey.Results The attack rate of MRSA LRTI in NSICU patients was 22.86%,a total of 16 MRSA isolates were detected from patients’clinical specimens,nasal vestibule,as well as hospital surroundings during the period,pulsed-field gel electrophoresis (PFGE)result revealed that infection outbreak was caused by two subtypes of MRSA;risk factors analysis showed that long length of stay in ICU and aspiration of spu-tum through bronchoscopy were risk factors for MRSA LRTI.Conclusion Contamination of bronchoscope was the key factor for this epidemic spread of healthcare-associated MRSA infection.

Objective To investigate ACEI ( benazepril ) and tranilast exert renoprotective properties in diabetic nephropathy( DN) through the inhibition of thioredoxin( Trx) . Methods Forty male SD rats were randomly divided into normal control group,model control group,tranilast group and benazepril group (n=10 each).Normal control group was fed with normal diet. Other groups were fed with high-glucose high-fat diet to make DN models. Rats in model control, tranilast, and benazepril groups were fed with normal diet,400 mg??kg-1??d-1 tranilast plus normal diet,and 10 mg??kg-1??d-1 benazepril plus normal diet,respectively,via oral gavage for 12 weeks.The 24-hour proteinuria,blood glucose(BG),blood urea nitrogen (BUN),serum creatinine ( Scr) and renal pathology changes were detected. Expression of Trx was measured by Western-blot. Results The 24 h urine protein, BG, BUN, Scr, kidney/body weight, and glomerular sclerosis index were significantly decreased in tranilast group and benazepril group,as compaired with model control group ( P<0.05) ,but there was no statistical difference between the two drug groups (P>0.05).Both tranilast and benazepril can reduce renal pathological changes,and can increase the expression of Trx of DN rats, but benazepril had a more significant effect on increasing Trx expression. Conclusion Both tranilast and benazepril have renoprotective function in DN, and benazepril is more effective than tranilast in delaying the progression of diabetic nephropathy by increasing Trx expression and decreasomg oxidative stress.
