Objective To evaluate the relationship between recurrence of cryptogenic ischemic cerebrovascular disease (CICVD) and patent foramen ovale (PFO),as well as to access the clinical significance of PFO in ischemic cerebrovascular disease.Methods Consecutive patients with CICVD aged 15 to 70 years who were hospitalized in Department of Neurology,Xuanwu Hospital Capital Medical University from January 2008 to March 2011 were prospectively investigated.Identified by transesophageal echocardiography,patients were divided into two groups with respect to outcome:PFO group and non-PFO group.The recurrence of cerebral ischemic events was compared between the two groups after neurological follow-up.Results A total of 91 patients were recruited,including 57 patients with PFO and 34 patients without PFO.The follow-up period of two groups was 695 (506,1142) d.The recurrence rate at 15 months in patients with PFO (24.5％ (12/49)) was higher than those without PFO (6.9％ (2/29),x2 =4.391,P =0.036).Cum hazard curve indicated that recurrence risk of cerebral ischemic events in patients with CICVD in PFO group was higher than that of patients in non-PFO group during the follow-up period (P =0.044).Cox model used for multivariate survival analysis indicated that PFO was a risk factor for cerebral ischemic event recurrence among patients with CICVD (OR =4.159,95％ CI 1.178-14.689,P =0.027).Conclusions PFO is associated with increased recurrence risk of cerebral ischemia in CICVD patients.In addition,PFO may be a significant factor for ischemic cerebrovascular disease.

Objective To discuss the effect of operations for comminuted patella fractures with cable cerclage and revised tension band. Method Retrospective study of clinical effects of 120 patella fractures cases which used cable(cable group,55 cases) or NT-PC (NT-PC group,65 cases) to internal fix.Results At 10 to 38 ( 19.50 ± 1.15 ) months follow-up, all cured well in 12 weeks. Compared with the operative time, the amount of blood loss, length of hospitalization, time for cure, the incidence of complications,there were no significant differences between two groups (P ＞ 0.05 ). The B (o)stman score of cable group [(27.0 ±0.2) scores] was better than that of NT-PC group [(25.1 ±0.6) scores](P＜0.05). Conclusion Using cable cerclage and revised tension band to internal fix for comminuted patella fractures,the operations are facile and the results are satisfactory.

Objective To investigate the effects of trichostatin A (TSA) on Th1 and Th17 cells in the mice model of collagen induced arthritis (CIA).Methods Mice model of rheumatoid arthritis (RA) was induced in DBA/1 mice with type Ⅱ collagen.Paws were scored for histological severity of arthritis.The severity of inflammation of mice joints was evaluated by histological examination.Real time polymerase chain reaction (PCR) was used to determine mRNA of cytokines and transcriptional factors.Serum cytokine production was measured by enzyme linked immunosorbent assay (ELISA).T cell proliferation was examined by MTT method.One-way ANOVA and Student-Newman-Keuls were conducted in this study.Results The expressions of IFN-γand IL-17 mRNA of the CIA group were higher than that of the control group (8.27±0.64 vs 2.97±0.25,5.80±0.23 vs 0.70±0.26,all P＜0.01),but were inhibited significantly by TSA introduced at the onset of arthritis(6.60±0.52,2.50±0.41,all P＜0.01).Collagen specific T cell proliferation was significantly suppressed by the introduction of TSA.Increased level of IL-4 was observed in TSA treated group compared to that of CIA group(2.10±0.17 vs 1.01±0.08,P＜0.01).Conclusion Th1 and Th17 cells play crucial roles in the lesions of RA.TSA can suppress the progress of CIA by decreasing the percentage of Th1 and Th17.

Objective To observe the changes of cerebrovascular reactivity(CVR)in cerebral infarct, and ascertain the correlation between them. Methods Breath-holding index (BHI) was obtained with transcranial Doppler combined breath-holding test in 193 subjects with cerebral infarct and 113 controls. Results The BHI in cerebral infarct patients was significantly lower than that in controls(P=0.000),and CVR was a independent influential factor for cerebral infarct(P=0.000). Conclusion Impaired CVR is associated with cerebral infarct, which should be paid more attention to.

ObjectiveTo investigate the related factors of ischemic cerebrovascular diseases in patients with patent foramen ovale (PFO). Methods42 patients with PFO，aged ≤55 years old, with cerebral infarction or transient ischemic attack and treated with aspirin were involved. All patients were examined with transesophageal echocardiography. 6 cases relapsed and 36 cases did not. The size of PFO, serum fibrinogen level, and combination of atrial septal aneurysm (ASA) were compared. ResultsThey were followed up for (512.51±127.38) d. The serum fibrinogen level was (3.33±1.11) g/L in no relapse group, and was (4.40±1.49) g/L in relapse group （P=0.032）. There was not significant difference in the size of PFO (P=0.618) and combination ASA (P=0.554) between these groups. Logistic regression showed that patients with ischemic cerebrovascular diseases history involved in relapse (OR=46.913,95%CI:2.371～928.254), but the serum fibrinogen level did not （OR=2.656,95%CI:0.977～7.219）. ConclusionThe PFO patients with history of ischemic cerebrovascular diseases incline to relapse. It is not sure the influence of the serum fibrinogen level on it.

An acidic xylanase gene,named xyl3,was cloned from the genomic library of enviromental microbes constructed by using shotgun cloning strategy,and submitted to GeneBank with accession number of gb:AY300805.BLAST analysis indicated that the gene xyl3 has low similarity with other xylanase genes and the encoded xylanase,sorted as Glycosyl hydrolases family 10,has 77% similarity with the intra-cellular xylanase from Geobacillus stearothermophilus at amino acid level.Treated with T4 DNA polymerase,the gene xyl3 was ligated with the linearized Pichia pastoris expression vector pHBM905 produced by digestion of restriction endonuclease CpoI and NotI to generate the recombinant plasmid pHBM706.Then the plasmid pHBM706,digested by restriction endonuclease SalI,was transformed into P.pastoris GS115 to obtain the recombinant P.pastoris GS115(pHBM706),which was induced to produce the recombinant xylanase with 0.5% methanol at 28℃.At the 36th hours of induction,the porduced crude enzyme was detected to reach the higest enzyme activity of 0.177 IU/mL.The optimal pH and temperature of the enzyme activity is 5.5 and 50℃ respectively.

Objective To analyze the risk factors for patients with cryptogenic ischemic cerebrovascular disease (CICVD) and patent foramen ovale (PFO), as well as to evaluate the relationship between common risk factors and PFO in cerebral ischemia. Methods Consecutive patients with CICVD aged 15 to 70 years who referred to Department of Neurology, Xuanwu Hospital, Capital Medical University from January 2008 to July 2011 were investigated. Identified by transesophageal echocardiography, they were divided into PFO group and non-PFO group with respect to outcome. The common risk factors of cerebral ischemic between 2 groups were compared. The relationship between these risk factors and PFO was analyzed. Results A total of 102 patients were investigated, including 61 patients (59.80%) with PFO and 41 patients (40.20%) without PFO. Positive family history of ischemic cerebrovascular disease proportion in PFO group (31.1%)was higher than that in non-PFO group (9.8%) (P=0.011). There was no significant difference in other observed indicators (P>0.05). Positive family history of ischemic cerebrovascular disease correlated with PFO among CICVD patients (r=0.251, P=0.011). Conclusion PFO was not only more common in CICVD patients, but also correlated with positive family history of ischemic cerebrovascular disease.

A comprehensive analytical method based on ultra performance liquid chromatography tandem mass spectrometry has been developed for the simultaneous determination of 16 carcinogenic and allergenous dye stuffs (acid red 26, basic red 9, disperse blue 1, acid violet 49, disperse blue 3, solvent yellow 1, dispersed blue 106, disperse orange 3, disperse yellow 3, basic violet 1, basic violet 3, disperse red 1, solvent yellow 3, disperse blue 124, solvent yellow 2 and disperse orange 37).Various toy samples, including textile, leath er, paper, wood, balloon, modeling clay, limitation tattoo and aqueous liquid, were extracted under ultrason ication.Qualitative and quantitative analysis was carried out for the analyte under the MRM mode after the chromatographic separation on Waters ACQUITY UPLC BEH C_(18)(50 mm x 2.1 mm, 1.7 μm) column.The limits of quantitation(LOQ) for the 16 dyestuffs were in the range of 1.0-8.0 μg/kg.The mean recoveries at the three spiked levels of 5-100 μg/kg were 81.3%-98.6%, with the intra-day precision less than 11% and the inter-day precision less than 14%.The method is accurate, rapid, sensitive, and adapt to the inspec tion of the 16 dyestuffs in toys.

Objective To study the nosogenesis and the relationship of different levels of serum Eotaxin,IL-13 and total IgE(TIgE)in asthmatic children. Methods Thirty children with asthma aging from 5 to 12 years were selected as study group (attack period and remission period), and 22 healthy children were selected as control group. Serum Eotaxin and IL-13 were measured with enzyme linked immunosorbent assay, and serum TIgE was determined with enzyme-linked fluoroimmune assay in the study group and control group, and pulmonary function was measured at the same time in the study group. Results (1) There were significant differences in pulmonary function between the attack period and the remission period (P ＜ 0. 001). (2) There were significant differences in the level of serum Eotaxin, IL-13 and IgE between the study group and the control group,especially for the attack period and the control group(P ＜0. 001). (3) There were significant positive correlations between the level of serum Eotaxin and IL-13, Eotaxin and TIgE, IL-13 and TIgE respectively in the study group (P ＜0. 05). (4) There were negative correlcations between the level of serum Eotaxin and FEV1 ,PEF,and serum IL-13 and FEV1, PEF in asthmatic children (P ＜ 0. 05), there were no correlations between the level of serum TIgE and FEV1, PEF in the study group (P ＞ 0. 05). Conclusion Serum Eotaxin, IL-13 and TIgE were participated in the nosogenesis of asthma, and the level of serum Eotaxin and IL-13 can be a assessment of asthmatic patients' condition.

Objective To explore the impact of levels of serum IL-17,Leukotriene B4 and IgE on pathogenesis of childhood asthma.Methods Totally 60 children with asthma acute exacerbation ( 29 children with mild asthma,31 children with moderate-severe asthma) were selected as study group,24 healthy children were selected as control group.Serum IL-17 and LTB4 were measured with euryzemLinked immunosorbent assay,serum IgE was determined with enzyme-linked fluoroimmuneassay by pharmacia CAP Sytem,PMN was determined with automatic blood analyser,pulmonary function was measured in the study group.Results ( 1 ) The level of serum IL-17 ( 1.15 ± 0.10 μg/L,2.80 ± 2.30 μg/L,0.83 ± 0.10 μg/L),LTB4 (2.22 ± 1.01 μg/L,8.79 ± 9.36 μg/L,1.94 ± 1.13 μg/L) and IgE( 123.70 ±86.94 μg/L,322.27 ±332.28 μg/L,24.27 ±7.64 μg/L) were significantly different among mild asthma group,moderate-severe asthma group and control group( P ＜ 0.001 ).( 2 )The N％ of mild asthma group,moderate-severe asthma group and study group were( 55.06 ± 1 1.15 ) ％,( 64.44± 11.87)％,(47.96 ± 13.52)％,L％ were(42.20 ± 11.04)％,(33.93 ± 10.02)％,(49.65 ± 13.02)％,and there were significant differences in N％ and L％ between study group and control group( P ＜ 0.05 ).( 3 ) There were significant positive correlations between the serum IL-17 levels and IgE,LTB4 and IgE,IL-17 and LTB4 in asthmatic children( P ＜0.05).(4) There were significant negative correlcations between the level of serum IL-17,LTB4 and FEVI,PEF( P ＜0.001 ).There were significant positive correlations between serum IL-17,LTB4 and N％ (P ＜0.001 ).(5)There were not correlations between the level of serum IgE and FEV1,PEF and N％in asthmatic children( P ＞0.05 ).Conclusion The levels of serum IL-17,LTB4 and IgE participated in pathogenesis on asthmatic children patients.
