1.Bilateral pure gonadoblastoma in a 46 XY individual--a case report.

We report here a rare case of bilateral pure gonadoblastoma which accounts for only 0.2% of all ovarian tumours seen at Universiti Kebangsaan Malaysia from 1980 to 1987. This tumour occurred in an 18 year old Chinese "female" who presented with primary amenorrhoea. Examination showed a phenotypic female with poorly developed external gentalia. Exploratory laparotomy revealed a hypoplastic uterus, rudimentary fallopian tubes and streak gonads. Histological examination of the gonads showed a mixed tumour comprising large germ cells and smaller sex cord derivatives arranged in characteristic nests or islands containing hyaline material.

2.Pleomorphic Adenoma Originating from Submandibular Salivary Gland in an 8-year-old Girl: A Case Report

Pleomorphic adenoma (PA) typically presents as a benign slow growing, painless
neoplasm of the parotid gland. PA arising from the submandibular gland in the paediatric
age group is rare. Surgical excision is the treatment of choice. A case of PA in an 8-yearold
Malay girl is presented. Clinically she presented with a painless right submandibular
mass measuring 2x2cm. A diagnosis of Pleomorphic adenoma was made on Fine Needle
Aspiration Cytology (FNAC). She underwent submandibulectomy uneventfully.
Postoperatively after two years follow up, she is asymptomatic.
3.Anaplastic Large Cell Lymphoma Presenting as a Soft

Anaplastic large cell lymphoma (ALCL) is a rare tumour, accounting for approximately 3%
of adult non-Hodgkin lymphomas.1 Primary systemic ALCL frequently involves both lymph
nodes and extranodal sites. A 44-year-old woman presented with a firm, mobile mass in
the left iliac fossa region. Ultrasound findings showed a well defined inhomogenous soft
tissue mass, measuring 4x4x2.6cm in the deep subcutaneous region. Histopathological
examination revealed that the mass was infiltrated by large lymphoid cells with marked
nuclear atypia including kidney-shaped nuclei. These neoplastic cells expressed anaplastic lymphoma kinase (ALK) (both nuclear & cytoplasmic staining), CD30 and EMA but not for
T-cell (CD45RO and CD3), and B-cell (CD20 & CD79α) markers. Fluorescence in situ
hybridization (FISH) analysis showed a t(2;5)(p23;q35) chromosomal translocation.
Subsequently the patient developed shortness of the breath and a thoracic computed
tomography (CT) scan showed a mass encasing the right upper lobe bronchus. She also
had bilateral axillary lymph nodes, measuring 1 cm in diameter (biopsy was not done). The
mediastinum and endobronchial region did not show any abnormalities. She received 6
cycles of CHOP chemotherapy and remained disease free 2 years after diagnosis. ALCL,
rarely present as a soft tissue tumour and this disease should be included as a differential
diagnosis of any soft tissue mass.
4.Public trust in primary care doctors, the medical profession and the healthcare system among Redhill residents in Singapore.

INTRODUCTIONThere have been few studies on public trust in doctors and healthcare systems and this is the first in Singapore.
MATERIALS AND METHODSA cross-sectional survey was carried out in Redhill in January 2005. Citizens or Permanent Residents aged > or =18 years were randomly selected, one per household to avoid cluster bias, and 361 participated (response rate 68.7%). An interview administered questionnaire included 3 questionnaires measuring public trust: "Interpersonal Trust in Physicians Scale" for primary care doctors; "Trust in Physicians Generally Scale" for the medical profession; and "Trust in Healthcare System Scale" for the Healthcare System. Questions were answered on a Likert scale: 1. Strongly Disagree, 2. Disagree, 3. Neutral, 4. Agree, 5. Strongly Agree. Individual transformed scores of trust (range, 0 to 100) were equally divided into 5 categories with their average being the transformed mean.
RESULTSTrust in primary care doctors (mean 59.7) had proportions (prevalence rates) of: very low 0.3%, low 2.5%, neutral 40.4%, high 54.0%, and very high 2.8%. Trust in the medical profession (mean 61.8) had proportions of: very low 1.0%, low 7.7%, neutral 33.7%, high 47.0%, and very high 10.5%. Trust in the healthcare system (mean 61.5) had proportions of: very low 0.5%, low 4.1%, neutral 40.0%, high 48.7%, and very high 6.7%. For areas of the healthcare system, proportions of high/very high trust were: "Healthcare Providers' Expertise" (70.8%), "Quality of Care" (61.5%), "Patient Focus of Providers" (58.7%), "Information Supply and Communication by Care Providers" (52.3%), "Quality of Cooperation" (43.3%), and Policies of the Healthcare System" (24.6%).
CONCLUSIONSWhile low proportions had low/very low trust, the high proportions with neutral trust and the rather low level of trust in "Policies of the Healthcare System" are causes for concern.
