1.Expression pattern of MAL in normal epithelial cells, benign tumor, and squamous cell carcinoma of larynx.

OBJECTIVE:
To compare the expression pattern of the MAL protein in normal and laryngeal carcinoma to derive possible implications of MAL in carcinoma development of larynx.
METHOD:
Use the immunohistochemical technique to analyze the distribution of MAL in normal laryngeal epithelial cells, polyp of vocal cords, laryngeal atypical hyperplasia and laryngeal squamous cell carcinoma.
RESULT:
MAL-like immunohistochemical reactions are strongly expressed in normal laryngeal epithelial cells and its expression is no significantly different in epithelial cells of the polyp of vocal cords. Comparatively, MAL expression is significantly down regulated in laryngeal atypical hyperplasia and laryngeal squamous cell carcinomas (P < 0.05).
CONCLUSION
MAL is normally expressed in laryngeal epithelial cells and its expression changes at early stages of carcinoma development. MAL, therefore, is a potential marker for early diagnosis of laryngeal squamous cell carcinoma.

Background:
and Purpose Visual hallucinations (VH) and subjective cognitive complaints (SCC) are associated with cognitive impairment (CI) in Parkinson’s disease. Our aims were to determine the association between VH and SCC and the risk of CI development in a cohort of patients with Parkinson’s disease and normal cognition (PD-NC).
Methods:
Patients with PD-NC (total score of >80 on the Parkinson’s Disease Cognitive Rating Scale [PD-CRS]) recruited from the Spanish COPPADIS cohort from January 2016 to November 2017 were followed up after 2 years. Subjects with a score of ≥1 on domain 5 and item 13 of the Non-Motor Symptoms Scale at baseline (V0) were considered as “with SCC” and “with VH,” respectively. CI at the 2-year follow-up (plus or minus 1 month) (V2) was defined as a PD-CRS total score of <81.
Results:
At V0 (n=376, 58.2% males, age 61.14±8.73 years [mean±SD]), the frequencies of VH and SCC were 13.6% and 62.2%, respectively. VH were more frequent in patients with SCC than in those without: 18.8% (44/234) vs 4.9% (7/142), p<0.0001. At V2, 15.2% (57/376) of the patients had developed CI. VH presenting at V0 was associated with a higher risk of CI at V2 (odds ratio [OR]=2.68, 95% confidence interval=1.05–6.83, p=0.039) after controlling for the effects of age, disease duration, education, medication, motor and nonmotor status, mood, and PD-CRS total score at V0. Although SCC were not associated with CI at V2, presenting both VH and SCC at V0 increased the probability of having CI at V2 (OR=3.71, 95% confidence interval=1.36–10.17, p=0.011).
Conclusions
VH were associated with the development of SCC and CI at the 2-year follow-up in patients with PD-NC.
3.Splenic myeloid metaplasia in warm autoimmune hemolytic anemia (wAIHA): a retrospective study.

BACKGROUND: Splenic myeloid metaplasia (SMM) is a kind of extramedullary hematopoiesis, whereas its clinical significance in wAIHA remains unclear. The aim of this study is evaluating the frequency and clinical characteristics of SMM, compared with splenic-congestion (SC). METHODS: We included patients with wAIHA treated in a Mexican tertiary hospital between January 1992 and December 2015. All patients received steroids as first-line treatment and splenectomy as second-line treatment. RESULTS: Among the thirty-six splenectomized patients, 15 (41.6%) and 21 (58.4%) were diagnosed as SMM and SC, respectively. No differences were found in clinical characteristics between two groups. SMM patients showed lower platelet count (147×109/L vs. 240×109/L, P=0.02) and higher presence of anti-dsDNA antibodies (40% vs. 4.7%, P=0.01) than SC patients. Although the complete response (CR) rate with first-line treatment was lower in SMM patients (13.3% vs. 47.6%; P=0.04), post-splenectomy median disease-free-survival (DFS) was longer (16.2 mo vs. 5.1 mo; P=0.19). Univariate/multivariate analysis showed that achieving CR during first-line treatment (OR 0.3, 95% CI: 0.03–0.94, P=0.03) and higher platelet count (OR 0.99, 95% CI: 0.98–0.99, P=0.03) were protective factors for SMM; and anti-dsDNA titer higher than 9.6 IU/dL was a risk factor for SMM (OR 2.76, 95% CI: 1.48–5.14, P < 0.001). CONCLUSION: The wAIHA patients with SMM have different biological profiles with those without SMM. This study is the first trial evaluating the significance of histopathological spleen findings and their association with rheumatologic profile.

INTRODUCTION:
Noise-induced hearing loss (NIHL) attributed to occupational noise exposure is one of the most common causes of permanent hearing impairment. In the Philippines, road traffic remains the biggest source of noise. The authors aimed to determine the prevalence of NIHL among traffic enforcers in Quezon City and quantify their occupational noise exposure levels.
METHODS:
Traffic enforcers were recruited via convenience sampling and screened using a questionnaire and otoscopic examination. Participants underwent pure tone audiometry and those found to have hearing loss were classified as “indicative” or “suspected” NIHL. Audiometric measurements of noise levels in areas where the traffic enforcers were assigned were taken using a calibrated smartphone application.
RESULTS:
“Indicative of NIHL” was highest in the 41 to 50-year age group and “suspected NIHL” was highest in the 31 to 40-year age group. “Indicative of NIHL” was highest among those working for 1 to 5 and 11 to 15 years in the right ear (25%) and 11 to 15 years in the left ear (30%). “Suspected NIHL” was highest among those working for 6 to 10 years. The average noise levels from the different areas measured at different times ranged from 79.0 to 82.5 dB.
CONCLUSION
“Indicative of NIHL” is more prevalent in the older age group while “suspected NIHL” is more prevalent in the middle age group. The prevalence of “indicative of NIHL” is highest among those in service for 1 to 5 and 11 to 15 years while “suspected NIHL” is highest among those in the service for 6 to 10 years. The average noise level measurements were within the safe values suggested by WHO.
