2.A Case of Cutaneous T Cell Lymphoma Presenting as Papuloerythroderma of Ofuji.

Papuloerythroderma of Ofuji (PEO) is an uncommon entity of unknown etiology, characterized by coalescing erythematous papules sparing skin folds. A number of cases have described the association of this disorder with malignant with pathology, mainly with cutaneous T cell lymphoma. Such reports give rise to the suggestion that PEO may be a precursor of lymphoma or a form of cutaneous T cell lymphoma. We report a case of PEO, which was diagnosed as cutaneous T-cell lymphoma only 2 months after the development of skin lesions, and rapidly progressed. This case suggests the presence of a variant of cutaneous T cell lymphoma with clinical feature of PEO, which is different from mycosis fungoides or S zary syndrome.

4.Eosinophilic fasciitis preceding relapse of peripheral T-cell lymphoma.

Although eosinophilic fasciitis (EF) may precede hematologic malignancy or Hodgkin's disease, association with peripheral T-cell lymphoma (PTCL) is extremely rare. Only four cases of EF preceding or concomitant PTCL have been reported in the world literature. We experienced the first Korean case of EF complicated by the later relapse of peripheral T-cell lymphoma. A 63-year-old Korean male has been followed at our outpatient clinic periodically after treatment for stage IV PTCL. He had been in complete remission for seven and a half years when he developed edema of both lower extremities followed by sclerodermatous skin change in both hands with peripheral eosinophilia. Biopsy from the left hand showed fibrous thickening of the fascia with lymphoplasmacytic and eosinophilic infiltrate, consistent with EF. Twenty-five months later, a newly developed lymph node from the left neck showed recurrence of PTCL. EF may occur as a paraneoplastic syndrome associated with the relapse of PTCL. Therefore, in a patient with EF, the possibility of coexisting and/or future occurrence of hematologic neoplasm should be considered.

5.Peripheral T-cell lymphoma-unspecified with intestinal and liver involvement besides the lymph nodes: a case report.

The majority of intestinal lymphoma is derived from B cells, and peripheral T-cell lymphoma-unspecified (PTCL-U) rarely invades the intestines and liver. We report a case of PTCL-U that invaded also the intestines and liver besides the lymph nodes, characterized by multiple irregular ulcers of the intestine and ileocecal junction, and multiple intrahepatic low density foci. The diagnosis was established by mucosal biopsy and pathological examination. This case highlights the possibility of PTCL-U involving uncommon organs, for which a definite diagnosis has to be established pathologically. PTCL-U is highly malignant with a poor prognosis, and so far no standard treatment strategy has been available.

9.Analysis of mature T-cell and NK-cell lymphoma with CD30 expression based on latest WHO classification.

OBJECTIVETo investigate the frequency of different types of mature T- and NK-cell lymphomas diagnosed in a 4-year period at Sun Yat-sen University Cancer Center, and to study baseline CD30 for potential anti-CD30 targeted therapy in mature T- and NK-cell lymphoma.
METHODSAll cases of mature T- and NK-cell lymphoma diagnosed at Sun Yat-sen University Cancer Center from September 1, 2009 to August 31, 2013, were reviewed. Paraffin-blocks of available 164 consecutive cases were stained for CD30 immunohistochemistry using EnVision protocol.
RESULTSA total of 625 cases of mature T- and NK-cell lymphomas were diagnosed and the most common type was extranodal NK/T cell lymphoma (ENKTL), nasal type 319 (51.0%) cases, followed by angioimmunoblastic T-cell lymphoma (AITL) (119 cases, 19.0%), peripheral T-cell lymphoma, not otherwise specified (PTCL-NOS) (81 cases, 13.0%), and anaplastic large-cell lymphoma (ALCL), including 48 cases (7.7%) of systematic ALCL and 11 cases (1.8%) of primary cutaneous ALCL. Besides ALCL, ENKTL had the highest expression rate of CD30 among the 164 cases, with positivity observed in 41 cases (62.1%, 41/66). Only 1 case of PTCL-NOS was CD30 positive. CD30 was not expressed in all 28 cases of AITL and other rare types of mature T- and NK-cell lymphoma.
CONCLUSIONSThe frequency of different types of mature T- and NK-cell lymphoma encountered at Sun Yat-sen University Cancer Center was similar to that seen in other areas of China and other Asia countries. CD30 expression is different among several types of mature T- and NK-cell lymphoma. In addition to ALCL, ENKTL has the highest expression rate of CD30, which may be a candidate disease for anti-CD30 targeted therapy.

10.Expression of survivin and NF-kappaB in peripheral T-cell lymphoma and its significance.

This study was aimed to explore the expression of survivin and NF-kappaB in the peripheral T-cell lymphoma and its significance. Immunohistochemistry method was used to detect the expressions of survivin and NF-kappaB in 26 cases of lymphosarcoma. 30 cases of benign reactive lymphohistiocytosis were selected as control tor analysis. The results showed that the expression of survivin in 21 patients with lymphoma was positive, the positive rate reached to 80.8%; the expression of NF-kappaB in 17 cases was positive, the positive rate reached to 65.4%. Compared with the control group, the difference was statistically significant (p < 0.01). In the experimental group, the expression level of survivin was positively correlated with the positive rate of NF-kappaB. It is concluded that survivin and NF-kappaB widely expressed in lymphoma cells and they play an important role in enhancing proliferation and inhibiting apoptosis of tumor cells.
