1.Construction and Implementation of Risk Prevention and Control System on Essential Medicine in Our Hospital

OBJECTIVE:To provide reference for promoting rational use of essential medicines. METHODS:Combined with the actual situation of our hospital,a set of risk prevention and control system on essential medicine was constructed on the basis of relevant laws and regulations and support of information,and then implemented. RESULTS:The implementation of the system greatly promoted cognition of all medical staffs on reasonable,economical and safe use of essential drugs,standardized medication behavior and improved essential medicine use more reasonable. After the implementation of risk prevention and control system on essential medicines,medicine ratio of whole hospital decreased from 34.62% to 32.64%,inpatient medicine ratio decreased from 33.02% to 27.89%;essential medicine ratio of whole hospital rose from 31.22% to 43.34%,inpatient essential medicine ratio in-creased from 23.46%to 39.17%;the proportion of outpatient antibiotics prescriptions decreased from 15.04%to 13.46%,the utili-zation of inpatient antibiotics decreased from 62.11% to 48.47%. Various early warning were sent out 104 times in total,and 23 medical staff were punished due to irrational drug use. The prescription form and rationality of drug use were improved gradually. CONCLUSIONS:The construction of risk prevention and control system on essential medicine is one of the keys to the success of health care reform. It is suggested to make full use of thesystem+technologymeans,establish long-term mechanism for risk pre-vention and control,and constantly improve the system.
2.Pharmacy administration in pilot hospitals with the separation policy between medicine and pharmacy

Objective To provide the supporting evidence for the separation policy between medicine and pharmacy by investigating the effect of the policy on pharmacy administration in four three grade hospitals.Methods The drug supply chain management was researched in 4 pilot hospitals.Specifically, the difference between Shanghai Sixth People′s Hospital and the Sixth People′s Hospital East Campus was compared.Changes of rational drug use index before and after the separation policy were documented for the Sixth People′s Hospital East Campus.Results ① 4 pilot hospitals with professional management of hospital drug supply chain achieved savings in staff, equipment and drug inventory.② The Sixth People′s Hospital East Campus has higher out of stock rate than Shanghai Sixth People′s Hospital.③ The rational drug use indicators were ameliorated after implementation of the separation policy in the Sixth People′s hospital East Campus.Conclusions The separation policy between medicine and pharmacy not only reduces hospital cost, but also improves hospital pharmacy management.
3.Quantitative evaluation on internal control of drug management in a tertiary public hospital in Shanghai

Objective To establish the quantitative evaluation system on the internal control of drug management, which could reflect the improvement and optimization of the internal control of drug management and be reference for the drug management in the hospital. Methods Brainstorm method was used to design a quantitative evaluation table. The methods of walking test, field inspection and comparative analysis were used to carry out the internal control design and execution effectiveness evaluation. Results Evaluation score increased from 36.50 points in 2015 to 43.75 points in 2021 when the full score was 50 points. The proportion of drug intotal income decreased from 37.97% in 2015 to 21.62% in 2021.The drug markup rate decreased from 14.38% to 0.96%, and the intensity of antibiotic DDDs decreased from 55.48 in 2018 to 39.30 in 2021. The proportion of essential drug prescriptions was higher than the national examination data. Conclusion The optimizing quantitative evaluation system on the internal control of drug management could improve the management specialization, the structure of income and payout and the level of rational drug use, promote the high-quality development of hospitals and safeguard the implementation of the Healthy China strategy.
