1.Comparison of Effects on Intraoperative Cerebral Metabolism between Total Intravenous Anesthesia and Intravenous Inhalation Anesthesia in Patients in Neurosurgery

Objective To compare the effect on intraoperative cerebral metabolism between the propofol combined with remifentanil infusion in total intravenous anesthesia and the desflurane combined with remifentanil intravenous inhalation anesthesia in patients in neurosurgery.Methods Thirty-four patients were randomly divided into the propofol combined with remifentanil group (group A,n=16) and the desflurane combined with remifentanil group (group B,n =18).The B-ultrasound under the guide of retrograde catheterization through right internal jugular vein and artery was prepared after the patients entered the operation room.Atropine,propofol,fentanyl,rocuronium were used in the induction of anesthesia.The mechanical ventilation was applied after conventional trachea cannula.Once patients were anesthetized steadily,jugular bulb venous oxygen saturation (SjvO2),jugular bulb venous oxygen partial pressure (PjvO2),Jugular Bulb venous hemoglogin (Hbv),jugular bulb venous lactate (Ljv),jugular bulb venous gluxose (Gv),arterial oxygen saturation (SaO2),arterial oxygen pressure (PaO2),arterin (Hba),arterial blood lactate (La),arterial blood gluxose (Ga),arterial blood oxygen content (CaO2),jugular bulb venous oxygen content (CjvO2),arteriovenous oxygen difference (AVDO2),cerebral extraction of oxygen (CEO2),cerebral lactate production rate (CLP) and cerebral glucose uptake rate (CGU) at different time [before anesthesia induction (T1),1 hours after the start of the operation (T2),2 hours after the start of the operation (T3),half an hour after the operation]were collected.Results (1) The value of SjvO2,PjvO2,CjvO2 and CaO2 in group B was significantly higher than that in group A (P<0.001);The value of AVDO2 and CEO2 in group B was lower than that in group A (P<0.05);(2) The value of Gv and Ga in group B was higher than that in group A (P<0.05);There were no significant differences about CGU in two groups (P>0.05);(3) The value of CLP in group B was lower than that in group A (P<0.05);there were no significant differences about Ljv and La in two groups (P>0.05);(4) Compared with the value at moment of T1 between group A and group B,the value ofPaO2,SaO2,PjvO2 and SjvO2 were increased with time (P<0.05),the value of CaO2,CjvO2,AVDO2 and CEO2 showed a downward trend (P <0.05).Conclusion (1) Both total intravenous anesthesia and intravenous inhalation anesthesia can reduce the cerebral oxygen metabolism;(2) For the cerebral protection of neurosurgery operation,it seems that the effect of intravenous inhalation anesthesia is more stronger than total intravenous anesthesia.
2.Experiment studies on growth and proliferation of Hep-2 cells by silence of TRF2 gene.

OBJECTIVE:
To investigate the effect of RNA interference silencing telomere repeat factor 2 by observing Hep-2 cells' proliferation and apoptosis in larynx carcinoma cell line.
METHOD:
A recombinant plasmid containing a single shRNA (shTRF2) was constructed. The expression of TRF2 gene was detected by PCR and cell proliferation was examined using CCK-8. Hep-2 cells' apoptosis was detected by flow cytometer (FCM).
RESULT:
After treatment with shTRF2, the expression of TRF2 was distinctively depressed, and Hep-2 cells proliferation was obviously inhibited. Compared with control and negative group, cells with treatment of RNAi exhibited significantly more apoptosis.
CONCLUSION
Using RNA interference technique to silence TRF2 gene is effective on inhibiting cancer cells' proliferation and help to induce cancer cells' apoptosis.

3.Effects of moxibustion at "Shenque" (CV 8) on superoxide dismutase (SOD) in rabbits with kidney-yang deficiency.

OBJECTIVETo explore the influence of moxibustion at Shenque (CV 8) point on aging and its mechanism.
METHODSForty-eight health rabbits were randomly divided into 8 groups, ie., group I (normal control group), group II (model group), group III (prevention with moxibustion at Shenque (CV 8) group), group lY (prevention with moxibustion both at Shenque (CV 8) and Zusanli (ST 36) group), group V (prevention with moxibustion both at Shenque (CV 8) and Mingmen (GV 4) group), group VI (treatment with moxibustion at Shenque (CV 8) group), group XII (treatment with moxibustion both at Shenque (CV 8) and Zusanli (ST 36) group), and group VIII (treatment with moxibustion both at Shenque (CV 8) and Mingmen (GV 4) group). The rabbit models of kidney-yang deficiency were created by pouring Hydroxyurea. The serum superoxide dismutase (SOD) activity was detected and the effect of the moxibustion from both prevention and treatment aspects were compared. And then the effect of different compatibility of acupoints was observed.
RESULTSAfter modeling, obvious symptoms of kidney-yang deficiency and decrease of SOD activity (all P < 0.01) were found in groups II, VI, VII and VIII. After the moxibustion treatment, the syndromes of kidney-yang deficiency were significantly improved (all P < 0.01) and SOD activities were increased obviously in groups VI, VII and VIII (all P < 0.01); a few syndromes of kidney-yang deficiency were found in groups III, IV and V after modeling, but with no significant changes of SOD activity (all P > 0.05). There was no significant difference in three moxibustion treatment groups (all P > 0.05).
CONCLUSIONThree acupoints compatibilities of moxibustion treatment can all regulate serum SOD activity in rabbits with kidney-yang deficiency effectively and have the significant effect both at prevention and treatment aspects and there is close relationship between their mechanisms on aging and regulating serum SOD activity.

4.New progress of airway management in children with cystic fibrosis

Cystic fibrosis (CF) is a multisystem disease mainly caused by pathogenic mutation of the cystic fibrosis transmembrane conduction regulator gene.In recent years, with the deepened understanding of the disease and the popularization of gene detection technology, an increasing number of children are diagnosed with CF in China.Lung involvement is reported to affect the prognosis of the disease.Lung involvement is closely related to the airway, and good airway management can prolong the life of children.In this paper, the selection of airway clearance techniques and inhaled drugs were expounded, so as to improve the long-term airway management of children with CF in China.
