This study selected the top five general hospitals in the United States of America, collected the practice information of acupuncturists through the official website, and analyzed the professional department, expertise, professional title, education background, residency, fellowship, board certification, etc. of these acupuncturists to understand the practice situation of acupuncturists in the hospitals. The results of the study showed the practice of acupuncturists in the United States of America is improving. With further localization, locally trained acupuncturists have gradually become the main body of acupuncturists; acupuncture treatment is still mainly for analgesia, but the scope of treatment continues to expand, and departments that accept acupuncturists are gradually increasing. However, the group of licensed acupuncturists is still a minority in the group of acupuncturists in the United States of America. Native American physicians still use acupuncture as an alternative option and neglect to obtain a license. The entry barrier for acupuncturists still needs to be raised.

Objective To construct and preliminarily apply a graded exercise rehabilitation program for patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD),and to provide a theoretical basis for the scientific implementation of exercise rehabilitation by medical staff. Methods Based on Triangle model and evidence-based method, the first draft of the graded exercise rehabilitation program was constructed, and the items were revised through 2 rounds of expert consultations from March to October,2023.The graded exercise rehabilitation program was preliminarily applied in 10 patients with AECOPD.Results The effective recovery rates of the 2 rounds of expert consultation questionnaires were 100%;the expert authority coefficients were 0.857 and 0.863;the coefficients of variation were 0-0.285 and 0.052-0.244;the Kendall's harmony coefficients were 0.167 and 0.145,respectively(all P<0.001).The final plan includes 4 first-level indicators,13 second-level indicators, and 25 third-level indicators. The scores of 6 min walking test,mMRC and CAT after exercise were improved compared with those before exercise, and the differences were statistically significant(all P<0.05).Conclusion The graded exercise rehabilitation program for patients with AECOPD constructed in this study has good scientificity and practicability, which can provide references for clinical implementation of exercise rehabilitation.

OBJECTIVE:To e xplore the health economic burden of hypertension patients at county-level areas and its influential factors in China. METHODS :A questionnaire survey was conducted on hypertension patients in 7 county-level public hospitals from 6 provinces as Hebei ,Shandong,Shanxi provinces by using a convenient sampling method. Catastrophic health expenditure was defined by the standard of “medical and health expenditure exceeding 10% of household income ”. The incidence , average gap and relative gap of catastrophic health expenditure were analyzed. A multi-factor Logistic regression model analysis was used to analyze the influential factors that lead to catastrophi c health expenditure. RESULTS :A total of 1 378 questionnaires were sent out ,and 925 valid questionnaires were collected with effective rate of 67.13% . The incidence of catastrophic health expenditure, average gap and relative gap among hypertension patients were 23.03%,19.37% and 84.12%,respectively. At different income levels ,the incidence of catastrophic health expenditure, average gap and relative gap were 72.67% , 96.79% and 133.18% in the poorest household group ,and were 1.94% ,0.47% and 24.23% in the richest household group. Among different types of medical insurance ,the incidence of catastrophic health expenditure in patients covered by “New Rural Cooperative Medical Scheme (NRCMS)”the highest (31.30%). The household income ,complications and the type of health insurance had significant impacts on the incidence of catastrophic health expenditure in hypertension patients (P<0.05). CONCLUSIONS:The incidence of catastrophic health expenditure in hypertension patients with different income levels is different. As the income level raised ,the incidence of catastrophic health expenditures continued to decrease. But the protection of household health expenditure by NRCMS is weak. It is suggested that a certain policy preference should be given to families with low income and patients with chronic diseases ,so as to ensure the rights and interests of patients with hypertension .
