1.Dynamic radiographic observation of 64 cases of primary bronchogenic carcinoma
Journal of Third Military Medical University 1983;0(04):-
The dynamic changes of 64 cases of primary bronchogenic carcinoma were reviewed with their serial chest films.Obstructive pneumonitis was found in 26 cases,of which 17 showed a dense and/or enlarged hilar shadow on the first chest film.Inflammation could be minimized after treatment but the hilar shadow remained dense and/or enlarged.Hilar masses could appear after total absorption of inflammation. Thus increased density and/or enlargement of the hilar shadow could be considered as an important sign of obstructive pneumonitis.Solitary pulmonary nodule(SPN)was found in 36 cases. The nodules with a diameter smaller than 2 cm on the first chest film usually showed a high incidence of vacuolization and could be taken as an early sign of lung carcinoma. The nodules with a diameter larger than 2 cm usually suggested the malignant nature of the lesion if they exhibited an increased tendency to have spicule sign.tabulation sign,or linear shadows in lung parenchyma which might be clearly or blurrily demarcated and connected the nodule with the pleura. No vacuolization was seen in those nodules larger than 3 cm in diameter. The appearance of pleural indentation showed no relation to the size of the nodule. The dynamic changes of the nodulessmaller than 2 cm in diameter were emphatically observed and they usually appeared as a small piece of thin cloudiness,a small nodule,or a reticu-lonodular shadow on the first chest film. The measures to increase the detection of the nodular lesion smaller than 2 cm in diagmeter on a chest film were discussed. In addition,a rare early feature of lung adenocarcinoma, which showed a large piece of interstitial changes, was presented.

Objective To investigate Hysterothylacium aduncum (Anisakidae) infection in marine fishes from Bohai Sea and Yellow Sea. Methods Nematodes were collected from the digestive tract of fishes, fixed with hot 4% formalin and preserved in 70% ethanol for study. The specimens were cleared in lactophenol for light microscopical examination, and properly treated for scanning electron microscopy (SEM). Results Among the fishes examined, 14 out of 93 species (15.1%) were found infected by H.aduncum, with a higher prevalence in the fish of Lophius litulon(66.7%), Scomber-omorus niphonius(47.5%), and Gadus macrocephalus(33.3%). H.aduncum infection was first recorded in elasmobranch-Raja smirnovi. Morphological differences of H.aduncum were observed, including the width of lateral alae and the length of intestinal caecum. Conclusion H.aduncum in fishes of Bohai Sea and Yellow Sea in China may be a complex species, and its high prevalence in some fishes reminds the risk of anisakiasis by eating raw fishes.

Objective To study the relationship between gastric mucosa associated lymphoid tissue (MALT) lymphoma and Helicobacter pylori (H.pyori) infection retrospectively. Methods The paraffin embedded samples of the tumors in gastric intestinal tract and its corresponding normal tissues from 35 patients with non Hondgkin's lymphomas in gastric intestinal tract were sectioned. The slides, except routine histopathologic study, were submitted to immuno histochemical staining (CD 3,CD 5,CD 10 ,CD 20 ,C 23 ,CD 45 RO, kappa, lamda, cyclin, Ki67, TUNEL) and to HE staining for the detection of H. pylori infection. Results ①Twenty one cases were identified to be MALT lymphomas according to the results of immunohistochemical staining, of them 16 cases were gastric MALT lymphomas, 1 case of intestinal MALT lymphoma, and 4 cases of colonic MALT lymphomas. ② H. pylori infection in gastric MALT lymphoma was 100% except in 3 cases who had only tumor tissues, which could not be evaluated. ③ Two cases were stage Ⅰ 1 gastric MALT lymphomas, 5 were stage Ⅱ 1,and 9 were stage Ⅱ E. ④ Only 6 cases had recognized as gastric MALT lymphomas by endoscopy and biopsy. Three patients were misdiagnosed as gastric cancer, 1 as M?n?trier disease, 1 as atrophic gastritis, and 5 as chronic gastritis endoscopiclly. ⑤ One patient got a complete regression after 3 courses of anti H. pylori therapy and was in good health after 3 years of the treatment. Conclusions ① A close relation was present between H.pyloi infection and gastric MALT lymphoma. ② Early gastric MALT lymphoma may regress after eradication of H.pylori.

BACKGROUND: How to solve the source of material substitute for repair of urethral dcfoct and improve the blood supply of new urethra has become a critical problem in the urethral repair and reconstruction.OBJECTIVE: To investigate the effects of endothelial progenitor cells (EPCs) on improving blood circulation in the new urethra following urethral defect repair.DESIGN,TIME AND SETTING: In vivo tissue engineering experiment, performed at the Laboratory of Deparanent of Surgery,First Affiliated Hospital of Zhengzhou University between January 2006 and February 2008.MATERIALS: Thirteen 3-5-month-old male Japanese rabbits were included for this study.Of them, one was used for preparation of bone marrow mononuclear cells, and the remaining twelve rabbits were divided into EPC repair group (n=8) and model group (n=4).METHODS: Under the aseptic condition, bone marrow was taken from the rabbit bilateral anterior superior lilac spine.Mononuclear cells isolated by Percoll method were induced in vitro using medium supplemented with vascular endothelial growth factors (VEGFs) and bovine basic fihroblast growth factors. When covering the whole bottom of culture flask, the mononuclear cells were digested with trypsin for passage.Animal models of urethral defect were developed in the two groups. One piece of aseptic fresh acellular human amnion (1 cm2) was sutured to each defected urethra using 0/6 DG suture for forming urethra. In the EPC repair group,1010/L passage 3 rabbit bone marrow mononuclear cell suspension was injected to two anastomotic stomas of the new urethra,0.1 mL for each stoma. The subcutaneous tissue was interruptedly sutured to the formed urethra using 0/6 DG suture. In addition,0.5 mL bone marrow mononuclear cell suspension was added to the region between each anastomotic stroma and newly repaired urethra. The same procedure was performed in the model group except that bone marrow mononuclear cell suspension was replaced by cell-free medium. At weeks 4 and 12 after surgery, paraffin sections of urethral tissue were made.MAIN OUTCOME MEASURES: Identification of cellular morphology; vascular regeneration of urethral tissue after urethral defect repair.RESULTS: After surgery, rabbit bone marrow mononuclear cells adhesively grew in vitro.Four days later, these cells exhibited rapid clone-like growth.Ten days later, they had typical slabstone-like change, presenting with strip-shaped and bundle-shaped growth. The phenotype of cultured cells gradually turned from CD34+/CD 133+/CD31+ to CD34+/CD 1337CD31+.At weeks 4 and 12 after surgery, the number of regenerate,d blood capillaries in the urethral tissue was significantly greater in the EPC repair group than in the model group (t=-9.034 to 5.985,P < 0.01).CONCLUSION: Rabbit bone marrow mononuclear cells-differentiated EPCs can apparently improve local blood circulation in the urethral defect repair.

Objective To investigate expression changes of Tau protein in retinal ganglion cells (RGCs) and oligodendrocytes (Ols) after stretch injury in rats and explore the relationship of Tau protein with pathological changes after axonal injury. Methods Morphological changes of optic nerves, RGCs and OLs after stretch injury were examined under light microscope in control group, stretch only group, heat stress only group and heat stress pretreatment plus stretch group. The expressions of Tau protein in RGCs and OLs after heat stress and/or stretch injury were observed by using immunohistechemical stai-ning. Results Pathological changes of axons, RGCs and OLs were identified morphologically or quan-titatively after stretch injury to the optic nerves, which was significantly ameliorated through pretreatment with heat stress plus stretch injury. The expressions of Tau protein in RGCs and OLs were increased in stretch only group. There was no significant expression change of Tau protein in heat stress only group. Expression of Tan protein was obviously decreased in heat stress pretreatment plus stretch group. Con-clusions Both neurons and glial cells are involved in pathological process after axonal injury. The ex-pression changes of Tau protein are probably related to delayed axotomy and neuron apoptosis. Heat stress can relieve the impairment of cystoskeleton through decreasing and delaying the expression of Tau protein.

Objective To discuss the application and effect of predictive care in stage divided treatment and salvage of children's persistent status of epilepsy (SE).Methods Sixty-four children with SE were admitted to the First Peoples Hospital of Wenling in Zhejiang Province from December 2014 to December 2016 as the research objects, a controlled study was adopted, in which the stage divided therapeutic method was compared with the conventional method for treatment of SE, and the children were divided into an observation group and a control group, 32 cases in each group. Foresight treatment method was used in the observation group: the first stage of treatment (early treatment or pre-hospital treatment) was intravenous injection of the first-line drugs (benzodiazepines); the second stage of treatment (progressive stage treatment or initial treatment) mainly used epileptic drugs to prevent recurrence; the third stage treatment (referred to after the first-line drug in combination with one second line antiepileptic drug treatment, the disease was still unable to be controlled) adopted the mild hypothermia therapy to reduce the SE attack amplitude and decrease recurrence; in terms of aspects of predictive nursing measures, the respiratory tract nursing, basic nursing, the prediction of complications and their nursing, etc. were implemented. The routine nursing without care about the stages was carried out in the control group. The SE control time, skin lesions, false aspiration, lung infection, tongue bite, cerebral edema, etc. the incidenceof complications were observed in two groups of children. Results The SE control time was significantly shorter in observation group than that in the control group (minutes: 18.13±3.15 vs. 25.19±2.69,P < 0.05), and the incidence of complications was obviously lower in observation group than that in the control group [6.25% (2/32) vs. 28.13% (9/32),P < 0.05]. Conclusion Using different therapy at different stages of children SE combined with predictive care can effectively shorten the SE control time, decrease the incidence of complications and elevate the therapeutic and nursing effects.

Objective To observe the effect of fenofibrate intervention on high-fructose-feeding-induced liver steatosis in rats and explore the possible mechanism. Methods Male Wistar rats were randomly divided into control group ,high fructose group and fenofibrate group[fenofibrate intervention started after 8 weeks of high fructose feeding ,30 mg/(kg · d)]. Rats were sacrificed after 12-week of high fructose feeding. Serum alanine aminotransferase(ALT),aspartate aminotransferase(AST),total cholesterol(TC),free triglyceride(TG)and liver TG content were determined;protein levels of fatty acid synthase(FAS),endoplasmic reticulum stress mark-er Bip and autophagy markers such as Atg7,Beclin1,LC3 and the related pathway mTOR in liver tissues were de-tected. Results Compared with those in control group and fenofibrate group, serum AST,serum total cholesterol, blood free TG and hepatic TG were significantly increased in high-fructose group(P < 0.01). The protein expres-sion of Fas,Bip and mTOR were significantly increased in high-fructose group compared with those in control group and fenofibrate group;the protein expression of Atg7,beclin1 and LC3 were significantly decreased in high-fructose group compared with those in control group and fenofibrate group. Conclusions Long-term high-fructose-feeding induces fatty liver and liver cell injury ,and may affect ERS and autophagy. High-fructose-feeding-in-duced fatty liver may be improved by fenofibrate and its underlying mechanism might be associated with Fas,ERS and autophagy in liver.
