1.Curative effect analysis of super-early keyhole surgery for hypertensive cerebral hemorrhage

Objective To analyze the curative effect of super-early keyhole surgery in treatment of hypertensive cerebral hemorrhage. Methods Clinical data of patients with hypertensive cerebral hemorrhage who received treatment in our hospital from April 2010 to October 2012 were retrospectively analyzed. Patients were divided into the observation group ( keyhole surgery group) and the control group ( routine group) by different therapeutic methods. The operation time, rate of hematoma clearance, postoperative complications, and recovery of neu-ral function of the two groups were compared. Results The observation group had a shorter operation time than the control group, and the difference was statistically significant(P<0. 001). There was no singnificant difference between the two groups in rate of hematoma clear-ance at the time of tube drawing(P>0. 05). The observation group had less postoperative complications than the control group, and the difference was statistically significant(P<0. 05). The observation group had a lower rate of metabolic disorders than the control group with a statistically significant difference(P<0. 05). Patients of the observation group had a higher scores of SSS than patients of the control group 28 days after operation, and the difference was statistically significant(P<0. 05). 6 months after operation, the observation group had a bet-ter ADL than the control group, and the difference was statistically significant(P<0. 05). Conclusion Super-early keyhole surgery which has less intraoperative injury, postoperative complication, and metabolic disorders, can achieve a better effect than routine operation. And it is of better neural functional recovery.
2.Endoscopic ultrasound guided fine needle aspiration biopsy of the pelvic lesions

Objective Endoscopic ultrasound guided fine needle aspiration biopsy (EUS-FNA) was performed for diagnosis and treatment of the pelvic Lesions and safety of this method were evaluated.Methods EUS-FNA was performed in 52 patients with pelvic Lesions selected by EUS,CT or MRI between March 2009 and June 2012.Both 19 and 22 gauge needles were used for EUS-FNA.The tissue specimens were analyzed by cytologic or histologic examination.The pelvic cystic lesions were drained and in which purulent lesions were lavaged with Metronidazole repeatedly.Results All patients recieved fine needle biopsy of the pelvic lesions.Among the 52 lesions,there were 42 solid lesions,10 cystic lesions.In solid lesions,cytology and pathology demonstrated malignant tumors in 28 patients,3 cases of malignant stromal tumors,3 cases of Inflammatory mass cases,2 cases of lymphoma,1 case of dermoid cyst,5 cases of other.In cystic lesions,2 cases of serous cystadenoma,perirectal abscess in 8 cases.6 purulent lesions were lavaged with Metronidazole repeatedly.Diagnosis rates of samples for immunohistology remained similar between 22 gauge and 19 gauge needles (P >0.05).During the operation,8 cases of perirectal abscess patients have different degree of pain.There were no other complications after the procedures except one patient suffering from fever.Conclusion EUS-guided FNA is minimaly invasive,a safe and accurate method for diagnosis of pelvic lesions.
3.Achilles tendon extension and tibialis transfer combined with tendon transposition for cerebral palsy-induced talipes equinovarus:ankle recovery and foot function evaluation

BACKGROUND:It has a variety of treatments for equinovarus in spastic cerebral palsy, including physical therapy, orthotic therapy, systemic anti-spasticity drug therapy, local intramuscular injection of botulinum toxin type A and a variety of surgical operations. These treatments aim at different severity of deformities and different age in children patients, and each has its own merits. OBJECTIVE:To investigate the therapeutic effect of Achiles tendon “Z”-type extension, tibialis anterior and posterior transfer combined with tendon transposition to treat equinovarus in spastic cerebral palsy. METHODS: Twenty-two children with spastic cerebral palsy combined with equinovarus were colected from 2012 to 2014, and then these children were subject to Achiles tendon “Z”-type extension, tibialis anterior and posterior transfer combined with tendon transposition. Postoperative plaster external fixation was done for 6 weeks, and then rehabilitation training was given. RESULTS AND CONCLUSION:Al the 22 patients were folowed for an average of 1-2 years, and then were evaluated according to the degree of patient’s satisfaction and the degree of foot deformity. Satisfactory results were obtained from al the patients. Achiles tendon “Z”-type extension, tibialis anterior and posterior transfer combined with tendon transposition to treat spastic cerebral palsy combined with equinovarus is a good method, and it is characterized as good appearance of the ankle and favorable foot function.
4.Progress of ELMO family on malignant tumor invasion and metastasis

It is one of the main characters of malignant tumors that malignant tumor cells invade surrounding tissues and metastasize to distant tissues .Multiple factors are involved in this complicated dynamic process .Metastasis is the major factor influencing recurrence and prognosis .Therefore, it is important to explore the mechanism of invasion and metastasis for reducing recurrence rate and mortality of malignant tumors .Engulfment and cell mobility ( ELMO) family is one kind of conserved protein in evolutional process .It includes 3 members, ELMO1, ELMO2 and ELMO3.The members of ELMO family play an important role in cell phagocytosis and cell migration , and they also have close correlation with ma-lignant tumor cell invasion and metastasis .In this paper , we review the progress of the relationship between ELMO family and malignant tumor invasion and metastasis .
5.Study on the changes of regional left ventricular function with aging in normal subjects by Doppler tissue imaging

Objective To evaluate the changes of left ventricular regional function in normal subjects with aging and the influencing factors by Doppler tissue imaging. Methods One hundred and twelve normal subjects were divided into four groups according to age: I group with age≤18 years old ; II group with 18 60 years old. Using a quantitative tissue velocity analysis system and tissue tracking, the peak velocities of different ventricular segments at systole, early and late diastole (Vs, Ve,Va) , the Ve/Va ratios were obtained and the effects of age, gender, heart rate and STd were analyzed. Results (1) Peak velocities of different segments of each wall were decreased one by one from mitral annulus, the basal, middle segment to the apical portion. (2) Peak velocities of different ventricular walls were significantly different, with the antero- and posterio-inter-ventricular septum being the lowest CVe (6. 89?2. 60) ,(7. 94?2. 64)cm/s]and with the latero-,posterio-,antero, and inferior walls of the left ventricle being higherCVe (9. 85?3. 02) , (9. 80?3. 09), (8. 86?3. 16), (8. 23?2. 48) cm/s] . (3) Peak velocities of different segments were associated with age, STd, BMI, heart rate and not associated with gender. (4) The Ve/Va ratios were increased one by one from mitral annulus, basal, middle segment to apical portion, and were significantly lower in III group and IV group than in I group and II group. Conclusions Peak velocities of different segments at different walls change regularly and age is the most important influencing factor to the diastolic velocities.
6.Ancient clinical application of herb-participated moxibustion on umbilicus.

In order to further improve the curative effect of the herb-participated moxibustion on umbilicus, we collected a wide range of literature on herb-participated moxibustion on umbilicus and then systematically arranged them to analyze and summarize the technology and operating methods of herb-participated moxibustion on umbilicus. We also briefly discussed issues on application of medicine, locust tree skin and ginger, the method of kneading dough for medical uses, and the appropriate size of moxa cone and its using frequency in order to form clear concepts and standardized operations to provide theories and operational basis for the clinical application of modern herb-participated moxibustion on umbilicus.

7.The relationship between plasma triglycerides, cholesterol levels and breast cancer

Objective To investigate the relationship between plasma triglycerides ( TG ) , cholesterol (TC)levels and breast cancer.Methods 105 cases of breast cancer(age from 22 to 69 years)and 261 cases of healthy control(age from 16 to 52 years)admitted from May.2014 to Oct.2014 were studied.TG and TC were tested.Subgroup analysis was performed by age (<35, 35-45, ≥45 years) and BMI( <18.5, 18.5-24, ≥24).Results TG level in breast cancer group was significantly higher than that of the healthy control group (t=-3.97,P=0.00), and TC level was lower than that in the healthy control group (t=2.49, P=0.01).TG lev-el of breast cancer patients≥45 years old was higher than that in the healthy control group ( t=-4.31, P=0.00).TC level of breast cancer patients <35 years and from 35 to 45 years was lower than that of the healthy control group(t=3.12, P=0.00及t=2.41, P=0.02); TG levels in breast cancer group were significantly lower than those of the healthy control group for all BMI subgroup ( t=-3.67, P=0.01 for BMI<18.5; t=-3.87, P=0.00 for BMI from 18.5 to 24.0;t=-2.01, P=0.04 for BMI≥24).TC level of breast cancer patients was higher than that of the healthy control group for patients whose BMI <18.5(t=-3.41, P=0.01) and it was lower than that of the healthy control group for patients whose BMI≥24.Conclusions The relation-ship between TG , TC and breast cancer is associated with age .Compared with healthy group , breast cancer pa-tients≥45 years have a higher TG level and breast cancer patients <45 years have a lower TC level .
8.Prenatal evaluation and management of 25 cases of twin reversed arterial perfusion sequence

Objective To analyze the clinical characteristics of twin reversed arterial perfusion sequence (TRAP),and investigate its prenatal evaluation and clinical management.Methods Karyotype results and ultrasound data of 25 TRAP cases were retrospectively reviewed,including estimated weight and umbilical blood flow of acardiac twin,cardiac function and middle cerebral artery peak systolic velocity of pump twin.Various managements and the outcomes were analyzed.Results (1) Karyotype of amniotie fluid were tested in 16 pump twins.Mosaicism was found in 1 case (46,XX[36]/46,XY [14]).(2) According to the ultrasound evaluation,large acardia accounted for 87.0% (20/23) cases.Abundant blood perfusion (inter-twin difference of umbilical resistance index ≤0.20) was indicated in 86.4% (19/22) cases.Decompensation of cardiac function was suggested in 66.7% (10/15) pump twins.Fetal anemia of pump twin indicated by middle cerebral artery peak systolic velocity>1.5 multiples of the median was diagnosed in 75.0% (12/16) cases.(3) The acardiac twin with abundant blood perfusion was more likely to be a large acardia than those without [94.7%(18/19) vs 1/3,Fisher exact test,P=0.04]; More pump twin with large acardia tended to have cardiac decompensation than non-large acardia pump twins [83.3 % (10/12) vs 0/3,Fisher exact test,P=0.02].(4) Eleven patients chose to terminate their pregnancies after being diagnosed.In 14 cases who continue the pregnancies,the survival rate of pump twin was 64.3% (9/14).In 3 cases of non-large acardia without cardiac decompensation of pump twin,the patients selected conservative observation resulting in 2 term deliveries and 1 termination of pregnancy due to for exacerbation.Among 11 cases with large acardia,which the pump twins were complicated by cardiac decompensation or anemia,five cases selected conservative observation.One ended in spontaneous abortion; three exacerbated (one termination and two cesarean section before term with living births) ; one was stable until delivery.Another 6 cases received bipolar cord coagulation,and successful interruptions of acardiac blood flow were achieved in 5 cases among which 4 pump twins survived.Conclusions Prenatal diagnosis,cardiac function and fetal anemia of pump twin,together with the growth and blood supply of acardia are important indexes for prenatal evaluation of TRAP,on which our prompt management should be based.
9.Expression of DLK1 protein and its correlation with renal cell carcinoma pathological characteristics

Objective To identify the expression of DLK1 protein in different types of renal cell carcinomas and its correlations with pathological characteristics and metastasis. Methods Immunohistochemistry analysis was performed to evaluate the expression of DLK1 protein in 94 cases of primary clear cell renal cell carcinoma, 76 cases of papillary renal cell carcinoma, 45 cases of chromophobe renal cell carcinoma, 71 cases of distal metastatic and 24 cases of lymph node metastatic clear cell renal cell carcinoma, as well as 18 cases of normal renal tissue. The correlations of DLK1 protein expression with pathological characteristics were analyzed. Results DLK1 protein was expressed in proximal and distal renal tubular epithelial cells in all the normal renal cases. In contrast, DLK1 protein expression was lower in different types of renal cell carcinoma. The low or negative expression of DLK1 protein in clear cell renal cell carcinoma, papillary renal cell carcinoma and chromophobe renal cell carcinoma was 33.0% (31/94), 27.6% (21/76) and 33.3% (15/45), respectively. Compared to normal renal tissue, DLK1 protein expression was significantly down-regulated in renal cell carcinomas (P>0.05), whereas there was no significant difference on DLK1 protein expressions among the different types (P>0.05) of renal cell carcinomas. DLK1 protein expression was not correlated with sex (60 male and 34 female cases), age (≥55, 50 cases and 55, 44 cases), grade (41 cases in grade I, 9 cases in grade II, 21 cases in grade III and 23 cases in grade Ⅳ respectively) and lymph node metastasis (76 cases with and 18 cases without lymph node metastasis) in clear cell renal cell carcinoma (P>0.05). There was also no significant difference among primary, lymph node and distal metastatic lesions of clear cell carcinoma (P>0.05). Conclusions DLK1 protein expression is commonly down-regulated in different types of renal cell carcinomas. Down-regulation of DLK1 protein expression is not associated with pathological characteristics and metastasis in clear cell renal cell carcinoma.
10.Observation of Preventive Effect of Lonicera japonica Alcohol Extract on Mice with Liver Injury by Metab-olomics Method

OBJECTIVE:To study the preventive effect of Lonicera japonica alcohol extract on mice with liver injury based on metabolomics method. METHODS:30 mice were equally randomized into a normal control (isometric normal saline) group,a model(isometric normal saline)group and a group of L. japonica alcohol extract(2 g/kg). The mice were given drugs by ig once a day for 14 consecutive days. On the 8th day of administration,the models were established by giving 0.2% dimethyl sulfoxide (DMN,10 ml/kg)ip once a day for 7 consecutive days. Gas chromatography-mass spectrum(GC-MS)was used to analyze 24 h total ions chromatogram of the urine sample on the 1st,3rd,5th and 7th day of administration of drugs and DMN. The change in endogenic small molecule metabolites in urine was observed. Principal component analysis was employed to explore the change in the metabolite chromatogram and underlying biomarkers in urine. RESULTS:The contour of the chromatogram changed to a largest extent 1 to 5 d after given DMN,but showed an obvious trend towards regression 7 days thereafter. DMN resulted in increase in the contents of 8-phenyl-8-azbicyclo-[4,3,0]non-3-ene-7,9-dione,2-(6-heptynyl)-1,3 dioxolane,bis-(O-methyloxime)-4-ketoglu-cose,and decrease in the contents of malonic acid,2-(4- chlorophenylthiomethoxyl)ethyl,tetrahydro-2-furanacetaldehyde,D-ga-lactose,erythro-pentonic acid and galacturonic acid,in endogenic small molecule metabolites in mouse urine,for which Lonicera japonica alcohol extract can improve that. CONCLUSIONS:Previous administration of L. japonica alcohol extract ig has preven-tive effect to some extent on the physiological and metabolic conditions of mice with liver injury induced by DMN.
