1.Establishment of a method for detecting EGFR gene mutations and its preliminary application

Objective To establish a method for detecting the EGFR gene mutations by the real-time fluorescence quantification PCR combined with Sanger sequencing and to preliminarily explore its clinical application value .Methods With EGFR gene hotspot mutations region exon 19 and 21 as the research locus ,the specific amplification and the sequencing primer were designed ,the known wild-type and mutant samples were utilized to construct the corresponding plasmid as the standard substance by the TA clone technique .Then the EGFR gene mutation detection method by the real-time fluorescence quantification PCR combined with Sanger sequencing was established and the methodological and the application evaluation were performed .Results The wild-type and mutant standard plasmids of the EGFR gene exon 19 and 21 were constructed successfully .The EGFR gene mutations detection method of the real-time fluorescence quantification PCR combined with Sanger sequencing was established ,which had high sensitivi-ty(101copies/μL)andgoodrepeatability(intra-assayCVandinter-assayCVofthereal-timefluorescencequantificationPCRofex-on 19 and 21 were 1 .42% /3 .52% and 0 .97% /2 .44% ,respectively ) .20 clinical samples were simultaneously detected by this method and the traditional Sanger sequencing ,the results were completely consistent .Conclusion The EGFR gene mutations detec-tion method of the real-time fluorescence quantification PCR combined with Sanger sequencing is successfully established ,which can be used in the clinical sample detection .
2.Modeling and simulation of transmission dynamics of syphilis in Bayinguoleng Mongolia Autonomous Prefecture of Xinjiang

Objective By analyzing the data of new syphilis cases from 2008 to 2014 in Bayinguoleng Mongolia Autonomous Prefecture (Bazhou for short) of Xinjiang to further provide reference basis for setting up control strategies.Methods Using the new syphilis data reported in Bazhou of Xinjiang,we constructed a dynamic model of transmission dynamics of syphilis,and the model was simulated and quantitatively analyzed.Results The syphilis dynamical model was introduced,the methods of setting the relevant parameters were given.It was found that the established model fitted well (MA PE =1.59%,RMSPE =0.68%),and the basic reproduction number of outbreak epidemic was estimated to be R0 =1.06 (95% CI:1.01-1.15),it was predicted that the cumulative incidence of syphilis in Bazhou was 18 145 cases by 2024.In 2023,the cumulative number of cases was 16 465,and the number of new cases reached 1 680 in 2024.The infection rate,the number of core group partners and the treatment rate were main factors influencing the prevalence of syphilis after comparison of the sensitivity of the model parameters.Conclusion There is still an upward trend in the prevalence of syphilis infection in Bazhou of Xinjiang,and relevant departments should strengthen the prevention and control measures in high-risk groups,promote the use of condoms and other comprehensive intervention measures to control the prevalence of syphilis.
3.The effects of calcium dobesilate combined with irbesartan in treating early diabetic nephropathy

ObjectiveTo observe the therapeutic effects of calcium dobesilate combined with irbesartan in treating early diabetic nephropathy ( DN ).Methods65 patients with early DN were randomly divided into two groups,experiment group( irbesartan plus calcium dobesilate,n =34) and control group( irbesartan,n =31 ).After 24 week treatment,the urinary albumin excretion rate(UAER),fasting blood glucose ( FBG ),postprandial blood glucose (PBG) and glycosylated hemoglobin(HbA1c) were detected and compared between two groups before and after treat ment.ResultsUAER after treatment was significantly reduced in both 2 groups(t =8.074,12.89,all P<0.01),and the decrement in the experiment group was more obvious than that in the control group with statistical significance ( t =3.965,P < 0.05).ConclusionCalcium dobesilate combined with irbesartan has good effects in the treatment of early DN.
4.Motion signal extraction method based on sEMG energy Gauss distribution characteristics.

Motion segment and extraction from continuous signals is the premise of surface electromyography (sEMG) analysis. For the problem that sEMG energy threshold required repeated manual testing to set, this the paper established a this mathematical model of continuous actions based on Gaussian sEMG energy curve, in which the energy threshold was set according to the distribution of Gaussian signal section, and differentiated the action signals from no-action signals combined with energy comparison method. The experiment results showed the method can achieve continuous repetitive action segmentation, and compared with manual segmentation of the same signal, has a higher degree of coincidence.

5.Sentinel lymph node biopsy in patients with operable breast cancer after neoadjuvant systemic therapy

Objective: To investigate the feasibility and accuracy of sentinel lymph node biopsy(SLN) following neoadjuvant chemotherapy.Methods:Sentinel lymph node biopsy was evaluated following two or three cycles of neoadjuvant chemotherapy with pharmorubicin /cyclophosphamide/5-FU(CEF,48 patients) or pharmorubicin /cyclophosphamide / taxotere(CET,12 patients).Two ml 1% methylene blue was subcutaneously injected at 4 sites of the skin over the tumor before radical operation.Pathohistological results were assessed for all the specimen including blue lymph node and other axillary lymph node(ALND). Results:Among the 66 patients SLN was identified in 60 of them(90%).Residual metastatic disease was identified in 23 cases on SLN,including 15 positive and 8 negative node on ALND,37 cases were completely negative on SLN including 35 negative and 2 positive node on ALND,and SLN was falsely negative in 2 cases(8%).Conclusion:The success rate of sentinel node identification and the predict ability of the SLN for the pathologic status of the adjacent non-SLNs do not seem to be altered after neoadjuvant therapy.
6.Clinical Research of Moxibustion Therapy on Gastralgia Due to Deficiency and Cold

Objective: To observe the clinical effect of moxibustion on gastralgia of deficiency cold type. Methods: Sixty patients were randomly divided into 2 groups. The 30 patients in the treatment group were treated with Zhao's thunder-fire moxibustion while the other 30 patients in the control group were treated with oral medicine. Results: After one course of treatment, the total effective rate of treatment group was notably higher than that of the control group, showing statistical significance (P<0.05). Conclusion: The total effective rate of thunder-fire moxibustion on gastralgia of deficiency cold type was better than that of the traditional Chinese medicine in relieving epigastralgia.
7.Design, synthesis and cholinesterase inhibitory activity of quinoline-polyamine conjugates.

A series of quinoline-polyamine conjugates (8a-8n) were designed, synthesized and evaluated as inhibitors of cholinesterases (ChEs). Some of these compounds had potent ChEs inhibitory activity with IC50 values at micromolar range. Compound 8n exhibited the strongest inhibition on acetylcholinesterase (AChE) with an IC50 value of 8.78 micromol x L(-1), and compound 8i showed the most potent inhibition on butyrylcholinesterase (BChE) with IC50 value of 1.60 micromol x L(-1) which was slightly better than rivastigmine. The structure-activity relationship revealed that the chain length of polyamine and linker played important roles for inhibitory activity. Molecular modeling studies showed that 8i targeted both the catalytic active site (CAS) and the peripheral anionic site (PAS) of cholinesterases.
8.Evaluation of therapeutic effects with intra-operative iodine-125 seed implantation for advanced pancreatic carcinoma

Objective To analyze the clinical effects of internal radiation of iodine-125 seed implantation in the treatment of advanced pancreatic carcinoma.Methods Ten patients with unresectable and advanced pancreatic carcinoma admitted from Sept.2002 to June 2004 were treated by the internal and interstitial radiation using iodine-125 seed implantation. Sex:male 6, female 4. Age: median 60.8(45-81) years. Site by pre-operative CT: pancreatic head 4, body 2 and tail 4. Diameter: 6?cm 3. Pathology: all proved to be pancreatic adenocarcinoma. Staging(UICC 1997): stage Ⅲ 3, IV 7. Pain degree: Ⅲ 3,IV 7. Weight loss: median 5?kg(3-10?kg). Results All patients have been followed for 2-13 months, median 6 months. Survival of 12 months was observed in 1 patient,6 months in 3 patients, 3 months in 4 and 1 month in 2, with an average of 4.6 months. Among the 10 patients, complete response was obtained in 2, partial response in 3, no response in 4, and the other showing PD. The response rate (CR+PR) was 50%. The pain relieving rate was 60%. The seed number conformal rate was 100% by X-ray film, yet that of seed spatial distribution was only 30%. Conclusion Intra-operative internal radiation by iodine-125 seed implantation does show some therapeutic effects for advanced pancreatic carcinoma.Yet,the seed spatial distribution by seed computer treatment planning system does needs further consummate.
9.Efficacy evaluation of laparoscopy assisted ultrasound guided radiofrequency ablation in the treatment of hepatocellular carcinoma beneath the diaphragm

Objective To explore the feasibility,safety and efficacy of laparoscopy assisted ultrasound guided radiofrequency ablation (RFA) in the treatment of hepatocellular carcinoma (HCC) beneath the diaphragm.Methods Twentythree consecutive patients with solitary HCC beneath the diaphragm were treated by laparoscopy assisted ultrasound guided RFA in the Chinese PLA General Hospital from January 2013 to March 2016.We observed the p erioperative complications and followedup long-term effect.Results All the 23 patients successfully underwent laparoscopy assisted ultrasound guided radiofrequency ablation.No serious complications such as massive hemorrhage,biliary fistula and severe pleural effusion,hemopneumothorax occurred in the patients during perioperative period.CT examination 2-3 days after the operation revealed that the tumor was completely covered by the ablation area.Besides,the survival condition was satisfactory during follow-up period of 9-38 months.Conclusion Laparoscopy-assisted ultrasound-guided radiofrequency ablation is effective and safe for HCC beneath the diaphragm.
10.Clinical comparison of laparoscopic and open surgery for radical cystectomy

Objective To evaluated the clinical value of laparoscopic techniques in radical cystectomy surgery for the treatment of bladder cancer. Methods Clinical data of 49 patients underwent radical cystectomy with Bricker ileal conduit diversion were retrospectively analyzed from October 2009 to August 2014, which laparoscopic radical cystectomy with Bricker ileal conduit 20 cases (Group A), open radical cystectomy with Bricker ileal conduit 29 cas-es (Group B). The blood loss during operation, operating time, gastrointestinal function recovery after operation, hos-pital stay after operation and complications were observed between the two groups. Results The blood loss during operation was significantly lower in Group A (416.66 ± 232.73) ml than in Group B (964.16 ± 445.73) ml ( <0.05), and hospital stay after operation was significantly lower in Group A (14.93 ± 2.72) days than in Group B (19.50 ± 3.16) days ( < 0.05), complication after operation was significantly lower in Group A than in Group B ( < 0.05). The operating time and gastrointestinal function recovery has no significantly difference between the two groups. Conclusions Laparoscopic radical cystectomy have advantages of minimal invasion, less blood loss, rapid recovery and less postoperative complications. It is a safe and effective surgical method. Long term effect need evaluated by follow up.
