1.Clinicopathologic characteristics and prognosis of Sj(o)gren's syndrome complicated with renal involvement

Objective To analyze the clinical manifestations,immunological characteristics,pathological changes and prognosis in primary Sj(o)gren's syndrome complicated with renal involvement.Methods Three hundred and eighty-seven patients with pSS were enrolled in this retrospective study.Among these patients,198 patients were complicated with renal involvement and 189 patients without renal involvement.The data of the two groups were analyzed for clinical manifestations,blood tests,urine tests and immunological characteristics by independent sample t test,x2 test,Fisher exact probability and Logistic regression model respectively.Seventy-two patients in the group with renal involvement had kidney biopsies.Results The average of these 387 patients (female/male 345/42 cases) were (55.3±13.2) years old.The average age of 198 patients with renal involvement (female/male 168/30 cases) was (55.3±13.2) years old.The serum RF and IgG of the group with renal volvement was significantly higher than the patients without renal involvement.In the 198 patients with renal involvement;102 patients (51.5%) developed type Ⅰ renal tubular acidosis (RTA Ⅰ).Among these RTA Ⅰ patients,54 patients presented hypokalemia;12 patients developed hypokalemic plegia,30 patients with urolithiasis,6 patients with osteoporosis;69 patients presented with overt renal glomerulus impairment,36 cases had nephrotic syndrome (NS),and 33 cases had chronic glomerulonephritis;In 81 patients with renal failure,including 27 were stage 1 CKD,18 were stage 2 CKD,9 were stage 3 CKD,15 were stage 4 CKD,12 were stage 5 CKD.Among these patients with renal biopsie,light microscopy showed chronic interstitial nephritis (IN,69/72),mesangial proliferative glomerulonephritis (36/72),hyperplastic and sclerosing glomerulonephritis (9/72),partly sclerosing glomerulonephritis (9/72) and membranous nephropathy (6/72).Immunofluorescent examination revealed:IgG deposition in 21 patients (29.1%),IgA in 21(29.1%),IgM in 33(45.8%),C3 in 12(16.6%),C4 in 18(25%) and C1q in 15(20.8%).Sixty-six cases with renal biopsies were followed for (8-30)months,except the patient died of cerebral bemorrhage,all the other patients survived,and renal functions were steady or improved.Conclusion ① The pSS patients with renal involvement mainly shows interstitial nephritis.However,the occurrence of glomerulonephritis is not rare.② Rheumatoid factor (RF) or IgG is significantly associated with renal involvement in patients with pSS.③ When renal involvement is presents,renal biopsies is a necessary for patients with pSS.
3.Effects of 1α, 25-dihydroxyvitamin D3 on memory CD4+ T cells of focal proliferative IgA nephropathy

Objective To explore the effects of 1α,25-dihydroxyvitamin D3 [1,25(OH)2D3] on memory CD4+T cells of focal proliferative IgA nephropathy (IgAN)patients.Methods (1) Total of twenty incipient focal proliferative IgAN patients (Lee classification:Ⅲ level) were chosen as IgAN group and 20 healthy volunteers were chosen as healthy control group.The level of serum 1,25(OH)2D3 was measured by radioimmunoassay (RIA).Peripheral blood mononuclear cells (PBMCs) were separated by the method of Ficoll density gradient centrifugation and were stimulated with anti-CD3/anti-CD28 in the absence or presence of various concentrations of 1,25(OH)2D3,Dexamethasone(DEX) and 1,25(OH)2D3and DEX combined.PBMCs were cultured for 72 hours and the levels of IFN-γ,IL-4,IL-17A,Foxp3 were measured by flow cytometry(FCM),standing for the levels of Th1,Th2,Th17,Treg.(2) IgAN group was divided into two subgroups (proteinuria < 1 g/24 h subgroup,proteinuria≥ 1 g/24 h subgroup),then the serum levels of 1,25(OH)2D3,IFN-γ,IL-4,IL-17A,Foxp3 were compared.Results Compared with healthy control group,serum 1,25(OH)2D3 level of IgAN group was significantly lower (P < 0.05).Serum 1,25(OH)2D3 level in proteinuria≥ 1 g/24 h subgroup was significantly lower than proteinuria < 1 g/24 h subgroup and healthy control group (P < 0.05).The level in proteinuria < 1 g/24 h subgroup was lower than healthy control group,but the difference was not statistically significant (P > 0.05).(2) The levels of IFN-γ and IL-17A and the ratios of IFN-γ/IL-4,IL-17A/Foxp3 in IgAN group increased significantly compared with healthy control group (all P < 0.05),and the level of Foxp3 decreased significantly (P < 0.05).The level of IL-4 also increased,but the difference was not statistically significant (P > 0.05).The levels of IFN-γand IL-17A and the ratio of IL-17A/Foxp3 in proteinuria≥ 1 g/24 h subgroup increased significantly,and the level of Foxp3 decreased significantly,compared with urinary protein < 1 g/24 h subgroup and healthy control group (P < 0.05).The ratio of IFN-γ/IL-4 in proteinuria≥1 g/24 h subgroup and proteinuria < 1 g/24 h subgroup all increased,compared with healthy control group,and the ratio in proteinuria≥ 1 g/24 h subgroup increased significantly (P < 0.05).There was no significant difference in the level of IL-4 among all groups.(3) After treatment with 1,25(OH)2D3,the levels of IFN-γ and IL-17A and the ratios of IFN-γ/IL-4 and IL-17A/Foxp3 decreased significantly,and the level of Foxp3 increased significantly (P < 0.05),and these effects were more obvious as the increase of the drug concentration.The level of IL-4 did not change significantly.The combination of 1,25(OH)2D3 and DEX had a synergistic inhibition on the production of IFN-γ,IL-4,IL-17A,and the ratios of IFN-γ/IL-4 and IL-17A/Foxp3,and had a synergistic promotion on the production of Foxp3.Conclusions There is a certain extent of vitamin D deficiency in focal proliferative IgAN patients,which may be associated with the severity of proteinuria.The disorder of immunomodulatory effects of memory CD4+ T cell might exist in the patients of focal proliferative IgAN.1α,25-dihydroxyvitamin D3 might have beneficial effects on the immunoregulation of memory CD4+T cells of focal proliferative IgAN patients.
4.The infection status of hrHPV and its effect and significance on expression of Caspase-1 and IL-1βin human cervical carcinoma

Objective To explore the infection status of high risk human papilloma virus (hrHPV) and its effects and clinical significance on expression of cysteinyl aspartate specific protease 1 (Caspase-1) and interleukin 1 β(IL-1β)in tissues of human cervical carcinoma. Methods A total of 102 patients with cervical carcinoma (cervical carcinoma group), 60 patients with cervical intraepithelial neoplasia (CIN group) and 30 patients with normal cervix (control group) were used as the research objects. PCR reverse dot hybridization method combined with DNA chip technique were used to detect hrHPV. The expressions of Caspase-1 and IL-1β were detected by immunohistochemical technique. Data were analyzed between hrHPV positive group and hrHPV negative group, between single type of hrHPV infection group and multiple type of hrHPV infection group. The relationship between caspase-1 and IL-1βexpression and clinicopathological parameters in cervical carcinoma patients were observed. Results HrHPV infection was detected in 75 cases(73.5%)in cervical carcinoma group and 11 types of hrHPV were detected. In these 11 cases, single type and multiple type of hrHPV infection were 61 cases(81.3%)and 14 cases(18.7%) separately. HrHPV infection rate was much higher in cervical carcinoma group than those in CIN group and control group(36.7%and 6.7%). Caspase-1 and IL-1βpositive rates were significantly higher in cervical carcinoma group(61.8%and 51.0%)than those in control group(26.7%and 23.3%). The positive rate of Caspase-1 was significantly higher in cervical carcinoma group than that in CIN group(40.0%, all P<0.01). The positive rates of Caspase-1 and IL-1β(77.8%and 74.1%)were higher in hrHPV DNA negative group than those in hrHPV DNA positive group(56.0%and 42.7%). There were no statistical differences in positive rates of Caspase-1 and IL-1βbetween single type of hrHPV infection group and multiple type of hrHPV infection group(P>0.05). The difference of positive expressions of Caspase-1 and IL-1βwere significantly related with cell differentiation, tumor size, lymphatic metastasis and clinical stage(P<0.05 or P<0.01). Conclusion There are single and multiple types of hrHPV infection in cervical carcinoma and the infection rate is high. HrHPVs may promote the progression of cervical carcinoma by restraining the expressions of Caspase-1 and IL-1β.
5.Development of Portable Multi-function Heart-Sound Auscultating System

Objective To overcome the disadvantages of the traditional auscultating method and design the electro-auscultating visual system integrating the monitoring and recording functions.Methods The portable system consisted of the module pre-gathering and electro-auscultating heart-sound signal,the circuit of A/D converting and MCU,as well as the device of signal LCD-displaying in real-time and the USB mass storage device.Results Portable electro-auscultating system was realized,which integrated the functions of watching,monitoring and recording.Conclusion The portable system is more convenient for clinical doctors to get correct information from patients,benefiting to the clinical pathology-statistics and profound development of the related data.
6.Association rule regarding chronic disease-relevant risk factors for the adults in Haidian District

Objective:To investigate the association patterns of chronic disease-relevant risk factors for the adults in Haidian District.Methods:Data for chronic disease-relevant risk factors for 3 219 adults in Haidian District in 2014 were collected and analyzed.SPSS 18.0 was used for statistical description and logistic regression.SPSS Modeler 14.1 was used to explore the association among the chronic disease-relevant risk factors.Results:Among men,5 patterns of chronic disease-relevant risk factors were identified,which suggested that heavy drinking,inadequate intake of fruit and vegetables,and physical inactivity were associated with smoking while inadequate intake of fruit and vegetables and smoking were associated with physical inactivity.Among women,one pattern of chronic disease-relevant risk factor was identified,which suggested that inadequate intake of fruit and vegetables was associated with physical inactivity.Conclusion:Chronic disease-relevant risk factors are intercorrelated among the adults in Haidian District.Information on patterns of chronic disease-relevant risk factors could assist interventions targeting multiple behaviors simultaneously.
7.In vivo MR tracking of transplanted bone marrow stromal cells labeled with Resovist in rat models with Parkinson disease

Objective To observe the migration ability of different quantity of BMSCs labled with Resovist after transplanted in rat models with Parkinson disease (PD) with 1.5T MR scanner and micro-47 coil, in order to determine the optimal transplanted dosage of BMSCs and observation time in vivo. Methods Forty PD rats were randomly assigned to 5 groups (each n=8) including 1×10~5 BMSCs group, 1.5×10~5 BMSCs group, 2×10~5 BMSCs group, 2.5×10~5 BMSCs group and control group. FFE-T2WI were obtained immediately and 1 week, 4 weeks, 8 weeks after transplantation to measure and compare the volume of hypointense areas of different groups in different time with 1.5T and 47 mm inner diameter micro-coil. At the meantime, rotational behavior was assessed in each group. After MR scanning, the rats were executed and prepared for immunohistochemistry staining at 12 weeks after transplantation. Results Only the extent of the dark region became wider 4 weeks after transplantation in 2×10~5 BMSCs group (P=0.005). The therapeutic efficacy in 2×10~5 BMSCs group was the best confirmed by behaviour studies (P=0.02). Conclusion For the treatment of PD and evaluation of the migratory ability of BMSCs, the optimal transplanted dosage of BMSCs is 2×10~5, while the best observation time is 4 weeks after transplantation.
8.Neurocognitive function among human immunodeficiency virus-infected patients evaluated by Montreal cognitive assessment and its relationship with the efficacy of highly active antiretroviral therapy

Objective To explore neurocognitive characteristics of human immunodeficiency virus (HIV)-infected patients ,and to compare the efficacy of highly active antiretroviral therapy (HAART ) among patients with different cognitive functions .Methods Cognitive function was evaluated using the Montreal cognitive assessment (MoCA) Chinese version in 118 HIV-positive patients and 62 HIV-negative controls .Among 59 patients on HAART ,CD4 + T cell count and viral load were assessed at enrollment and one-year follow-up .The mean of measurement data was compared using t test ,and enumeration data was analyzed using chi-squared or Fisher exact test when appropriate .Univariate and multivariate analysis were examined using bivariate Logistic regression models .Results Compared with control group ,HIV-infected group was characterized by higher rate of neurocognitive impairment (46 .6% vs 12 .9% , t =20 .30 ,P< 0 .05) ,and generally lower MoCA subscores for visuospatial abilities ,the clock drawing test , naming ,attention ,abstraction and delayed recall (t= - 3 .761 , - 2 .638 , - 4 .263 , - 3 .769 , - 3 .858 and- 3 .111 ,respectively ,all P< 0 .05) .Among patients on HAART ,subjects who scored < 26 showed no significant differences in viral load at three time points (pre-HAART ,post-HAART at enrollment and one-year follow-up) with those who scored ≥ 26 (t = 0 .557 ,0 .737 and - 0 .758 ,respectively ,all P >0 .05) .The former group had lower CD4 + T cell counts both at enrollment ([286 ± 127]/μL vs [363 ± 160]/μL) and one-year follow-up ([334 ± 122]/μL vs [411 ± 152]/μL) than the latter group (t= - 2 .027 and - 2 .067 ,respectively ,both P < 0 .05 ) ,while there were no obvious differences of pretreatment CD4 + T cell counts ([135 ± 77]/μL vs [155 ± 80]/μL) and HAART duration ([22 .29 ± 21 .20] months vs [18 .74 ± 16 .63] months) between these two groups (t= - 0 .968 and 0 .702 ,respectively ,both P>0 .05) .Multivariate analysis revealed that age (OR = 1 .044 ,95% CI :1 .008 - 1 .081 , P < 0 .05) and education time (OR = 0 .820 ,95% CI :0 .723 - 0 .930 , P < 0 .05 ) were independent predictors for neurocognitive impairment among HIV-infected patients . Conclusions Neurocognitive impairment is common among HIV-infected patients ,which is characterized by poor performance in multiple domains , and patients with neurocognitive impairment performed poorly in immune recovery .MoCA could be a useful screening tool of cognitive function in HIV-infected patients . Neurocognitive function has no relationship with pre- and post-treatment viral levels .
10.Screening and Optimization of the Extraction Technology of Total Flavonoids in Persimmon Leaves by Semi-bionic-enzyme Method

OBJECTIVE:To screen the method for extracting total flavonoids in persimmon leaves and optimize extraction tech-nology. METHODS:Using extract quality and flavonoids content as indexes,the effects of extracting total flavonoids in persim-mon leaves by ethanol refluxing method,enzyme method(cellulase,β-glucanase and complex enzyme mixed by equal amounts of both),semi-bionic method and semi-bionic-enzyme method (the same enzymes) were compared. Using flavonoids content as in-dex,solid-liquid radio,reflux temperature,reflux time as factors,orthogonal test was designed to optimize the extraction technolo-gy conditions of flavonoids in persimmon leaves by semi-bionic-enzyme method,and the verification test was conducted. RE-SULTS:The extract quantity and flavonoids content by semi-bionic-enzyme method was the highest among the 4 extraction meth-ods,and the complex enzyme was the most suitable;the optimized extracting condition of semi-bionic-enzyme method were as fol-lows as solid-liquid radio of 1:14,reflux temperature of 50 ℃,reflux time of 2.0 h;extraction rates of flavonoids in 3 verification tests were 5.9%,5.8%,5.9%(RSD=0.98%,n=3). CONCLUSIONS:The optimized semi-bionic-enzyme method is efficient and stable in extracting flavonoids in persimmon leaves.
