1.Comparative study on conjoint fascial sheath suspension and levator muscle resection for moderate or severe congenital ptosis

?AIM:To compare the curative effect of conjoint fascial sheath( CSF) suspension and levator muscle resection for moderate or severe congenital ptosis.?METHODS: Forty - three patients ( 74 eyes ) with moderate or severe ptosis were treated by CSF suspension or levator muscle resection randomly, and followed up for 6mo. The normalization rates of the two operations were then compared by statistical method, and the complications of the two operations were analyzed.?RESULTS: The two operations appeared no significant difference on the normalization rate for moderate congenital ptosis (P>0. 05), while the normalization rate of CSF suspension on severe congenital ptosis was significantly higher than that of levator muscle resection (P<0. 05). Less complication was happened in the CSF suspension group than in the levator muscle resection group.?CONCLUSION:CSF suspension is more effective on the treatment of severe congenital ptosis than levator muscle resection, and has advantages such as less trauma, repeatable, and less complication.
2.The treatment of postoperative local recurrence of periampullary carcinoma

Objective To evaluate the treatment modalities for local recurrent periampullary carcinoma. Methods From January 1997 to May 2007, 23 patients with local recurrent periampullary carcinoma underwent different therapy. The recurrent tumor was resected through laparotomy in 3 cases and the unresectable tumors were treated with radiofrequency ablation(RFA) in 5 patients. In 10 patients and 5 patients respectively high intensity focused ultrasound ( HIFU ) and regional chemotherapy ( RC ) were performed. Results The resection rate for local recurrent periampullary carcinoma was 13% in this group, 2 patients have survived for 10 months and 13 months respectively after reoperation while one patient achieved tumor-free survival for six months so far. The symptoms were alleviated to some extent in 20 patients treated with RFA, HIFU or RC. Conclusions Resection of local recurrent periampullary carcinoma may benefit a longer survival time, and the improvement of the quality of life. RFA, HIFU and RC are the alternative modalities for patients with unresectable tumors.
3.Relation between Chinese Medical Constitutions of Female Patients with Late-onset Acne: an Epidemiological Investigation.

OBJECTIVETo explore the correlation between syndrome types of late-onset acne female patients and constitutions of Chinese medicine (CM).
METHODSA questionnaire was performed in 365 late-onset acne female patients and 135 healthy subjects (as the control) using Professor WANG Qi's. methods and Standards for Chinese Medical Constitutions Classification.
RESULTSTheir CM constitutions were sequenced as damp-heat constitution, yin-deficiency constitution, balanced constitution, yang-deficiency constitution, blood-stasis constitution, qi-stagnation constitution, qi-deficiency constitution, phlegm-damp constitution, inherited special constitution, with statistical difference when compared with those of the control group ( χ2 = 85.206, P < 0.01). In the 365 female late-onset acne patients, 114 (31.23%) were with Chongren imbalance syndrome, 108 (29.59%) were with blood stasis or coagulated phlegm syndrome, 83 (22.74%) were with dampness heat syndrome, and 60 (16.44%) were with wind heat syndrome. There was statistical difference in CM constitution distributions among different CM syndrome types (χ2 = 105.671, P < 0.01). The distribution of CM medical constitutions was different between the two groups. Biased constitutions were often seen in the patient group, while balanced constitution was often seen in the control group. Binary Logistic regression analysis indicated that influencing factors covered sweet food, light diet, roasted food, coffee, stress, work pressure, and family pressure. Of them light diet was one protective factor, while the rest were adverse factors.
CONCLUSIONThe etiology and syndrome types of female late-onset acne female patients were associated with CM constitution.

5.Expression of pituitary tumor transforming gene in human renal clear cell carcinoma and its significance.

OBJECTIVETo explore the expression of pituitary tumor transforming gene (PTTG) in human renal clear cell carcinoma (RCCC) and its significance.
METHODSPTTG protein expression was detected by immunohistochemistry in 87 RCCC and 45 paired normal renal tissues.
RESULTSPTTG was expressed differentially between the normal renal and RCCC tissues. Compared with normal tissues, the primary RCCC tissues showed significantly increased expression of PTTG protein (P<0.001). The positive rate of PTTG protein was positively correlated to the tumor size, clinical stage and Fuhrman grade of RCCC (P=0.009, 0.008 and 0.035, respectively).
CONCLUSIONIncreased PTTG protein expression may be involved in the carcinogenesis and progression of RCCC.

6.Effect of Shengji Huayu Recipe on the expression of MMP-3 and TIMP-1 in skin ulcer tissue of diabetic rats.

OBJECTIVETo study the effect of Shengji Huayu Recipe (SHR)on the expression of MMP-3 and TIMP-1 in the skin ulcer tissue of diabetic rats.
METHODSThe skin ulcer model was established in diabetic mice. Different compatibility proportions of SHR [the ratio of Shengji Recipe (SJR) to Huayu Recipe (HYR) = 2:1, 1:1, and 1:2, respectively] were used to intervene. The expression of MMP-3 protein in the skin ulcer of diabetic rats was detected by Western blot method,and TIMP-1 protein was detected by immunohistochemical assay.
RESULTSAt each time point, there was no statistical difference in the blood glucose level among groups (P > 0.05). But all of them increased significantly,when compared with those of the normal wound group (P < 0.01). As for the difference between after would area treatment and before would area treatment, better effect was obtained in the SHR No. 3 group and the normal ulcer group than in the diabetic ulcer model group (P < 0.05). Results of Western blot showed that the MMP-3 protein expression was higher in the SHR No. 2 group than in the SHR No.3 group (P < 0.05). Immunohistochemical results showed that TIMP-1 protein expression was lower in the SHR No. 2 group than in the SHR No. 3 group and the diabetic ulcer model group (P < 0.05). TIMP-1 protein expression was higherin the SHR No. 3 group than in the SHR No. 2 group (P < 0.01).
CONCLUSIONUsing SHR No.3 was conducive to the promotion of wound healing in early wound repair stage, and using SHR No. 2 might be conducive to inhibiting the formation of pathological scar.

7.Application of molecular adsorbents recirculating system in the treatment for liver failure of hepatitis B.

OBJECTIVETo evaluate the effect of treatments with the molecular adsorbents recirculating system (MARS) on liver failure patients of severe hepatitis B, in order to seek a safe and effective therapeutic method which contribute to the improved survival rate for severe hepatitis patients.
METHODS52 liver failure patients of severe hepatitis B were performed intermittent MARS therapy for 6 to 8 hours per time in addition to standard medical treatment. Parameters in blood chemical data were collected before and after every treatment and analyzed in comparison with those parameters from controlled groups by means of plasma exchange and standard medical therapy.
RESULTSMARS therapy achieved a remarkable improvement in clinical symptoms and physic signs, accompanied with a significant decrease in serum bilirubin, ammonia, urea nitrogen, fragrant amino acid, endotoxin, IL-6, and TNF-alpha levels (0.05); at the 72 hours bilirubin rebounding analysis. MARS treatments resulted in a significant decrease of bilirubin rebounding level in comparison with what PE did (0.01 ), though the bilirubin removal efficiency between two groups was not statistically significant. The overall survival rate of MARS therapy was 50% (26/52), which was better than that of standard medical therapy (40.5%, 17/42, P less than 0.05).
CONCLUSIONThe results indicated that MARS was a safe and promising technology in the field of liver support therapy. It might be associated with considerable improved survival rate for liver failure patients.

8.Effects of laminarin sulphate on the expressions of PTEN and P271kip1 in prostate cancer PC-3 cells.

OBJECTIVETo study the effect of laminarin sulphate (LAMS) on the expressions of PTEN and P27kip1 in androgen-independent prostate cancer PC-3 cells in vitro, and investigate the mechanism of its anti-tumor action.
METHODSThe inhibitory effects of different concentrations of LAMS (0, 50, 100, 200 microg/ml) on androgen-independent prostate cancer PC-3 cells were detected by WST-8 assay. The morphology of PC-3 cells was observed under the fluorescence microscope, and the cell cycle and apoptosis were analyzed by flow cytometry. The mRNA and protein levels of PTEN and P27kip1 were measured by RT-PCR and Western blot.
RESULTSLAMS inhibited the proliferation of androgen-independent prostate cancer PC-3 cells in a dose- and time-dependent manner, and the cell cycle analysis showed that PC-3 cells were arrested in the S phase after treated with different concentrations of LAMS. The rate of apoptosis was increased and many typical apoptotic morphological features were observed under the fluorescence microscope. The PTEN and P27kip1 expressions at mRNA and protein levels were increased in a dose-dependent manner.
CONCLUSIONLAMS can inhibit the proliferation, arrest the cell cycle in the S phase and induce apoptosis of prostate cancer PC-3 cells. The significantly increased expressions of PTEN and P27kip1 may be one of the mechanisms for LAMS inhibiting prostate cancer PC-3 cells.

9.Studies on chemical constituents from Davidia involucrata var. vilmoriniana.

OBJECTIVETo study the chemical constituents of Davidia involucrata var. vilmoriniana.
METHODThe chemical constituents were isolated by column chromatography on silical gel and sephadex LH -20. Their structures were elucidated on the basis of spectral analysis and physical constants.
RESULTSeven compounds were isolated from this plant and the structures of them were identified as 3, 4, 5-trihydroxybenzoic acid (1), ellagic acid (2), maleic acid (3), quercetin (4), quercetin-3-O-beta-D-galactoside (5), 2alpha, 3alpha, 19alpha, 23-tetrahydroxyurs-12-en-28-oic-acid-28-O-beta-D-glucopyranosyl ester (6), ethyl brevifolin carbosylate (7), respectively.
CONCLUSIONCompounds 1-7 were isolated from this plant for the first time.

10.Characteristics of surface electromyography and work load of the forearm extensors in repetitive wrist extending.

OBJECTIVETo evaluate the influence of the frequency, the weight and the motion angle on the stress and the fatigue of the forearm extensors in repetitive wrist extending at low force loading level with surface electromyography (SEMG).
METHODSSixteen male college student volunteers were recruited for the experiment. Eight tasks of wrist extending were performed for 20 minutes respectively in given weight (1.96, 4.90 N), frequency (8.0, 33.3 moves/minute) and motion angle (45 degrees, 90 degrees). The static wrist extending at the level of 20% maximum voluntary contraction (MVC) were performed before and after each task for 2 up to 3 seconds, and the SEMG signals of extensor carpi ulnaris muscle (ECU) and extensor digitorum (ED) were recorded and analyzed.
RESULTSThe weight loading level was approximately equal to 1.40% or 3.50% of the MVC force. The mean power frequency (MPF) and the median frequency (MF) were decreased with the increase of 3 kinds of loading levels. The decrease of MPF of the muscle ED was significant (P < 0.05 or P < 0.01). The MF was decreased with the increase of angle and weight loading levels (P < 0.05 or P < 0.01). The root mean square (RMS) value of SEMG could be divided into 3 or 4 groups with significant difference. All three kinds of loads had positive correlation with amplitude of SEMG according to the stepwise regression analysis.
CONCLUSIONThe fatigue level of ED is the highest. The primary load factor for the forearm extensors is the frequency followed by the weight and the angle. MF, MPF and RMS can be used as sensitive indexes for evaluating the stress and the fatigue of the forearm extensors during repetitive performance at lower force loading level.
