1.Effects of recombinant human bone morphogenetic protein 2 on integrin beta 1 expression during mandibular distraction

BACKGROUND:Integfin levels can affect cellular function,regulate bone formation and bone resorption.However,this process need activated by other factors,whether recombinant human bone morphogenetic protein 2 (rhBMP-2)has this effect is poorly understood.OBJECTIVE:To study the effects of the different concentration of rhBMP-2 on the expression of inergrin β_1 during the mandibular dfScraction in rabbits.DESIGN,TIME AND SETTING:The randomized,controlled.animal grouping experiment of histological observation was performed at the Animal ExperimentaI Center and Immunohistochemistry Center of Liaoning Medical University from May 2006 to May 2007.MATERIALS:A total of 45 Japanese white rabbits were used.rhBMP-2 was supplied by Beijing Academy of Military Medical Sciences.METHODS:The rabbits were prepared for mandibular distraction models and randomly assigned into control,1.5 mg rhBMP-2 and 3.0 mg rhBMP-2 groups.with 15 animals in each group.Collagen sponge and collagen binding proteins were implanted into mandibles of rabbits,respectively,and distraction 0.4 mm per time,twice per day for 5 days.The mandibles were lengthened 4 mm.MAIN OUTCOME MEASURES:The expression of integrin 131 was detected using immunohistochemistry at days 1,3,7,14,and 28 after distraction.RESULTS:All rabbits were included in the final analysis.The integdn β_1 mainly expressed at 7 days after distraction.At the same time point.when concenrtation rhBMP-2 was increased,the positive integrin β_1 rate and positive stained area percentage were increased(P<0.05).Compared to the same concentration,the positive integrin β_1 rate and positive stained area percentage were gradually increased as time prolonged (P<0.05).CONCLUSION:The integdn β_1 expression can be promoted by rhBMP-2 at the anaphase of mandibular distraction osteogenesis in a concentration-dependent manner.
2.Effect of concentrated growth factor fibrin membrane during distraction osteogenesis of the rabbit mandible

BACKGROUND: Concentrated growth factor (CGF) fibrin membrane is involved in metabolism regulation, which has been applied in bone remodeling. Previous Studies have found that the cytoplasts of osteoblasts, interstitial cells and osteocytes in distraction osteogenesis are positive for osteoprotegerin and receptor activator nuclear factor kappa B ligand (RANKL).OBJECTIVE: To explore the mechanism of bone orthopedic effect and the promotion effect of CGF fibrin membrane on the distraction osteogenesis of the rabbit mandible.METHODS: Twenty-four white rabbits were randomly selected. Distraction osteogenesis was performed unilaterally in the rabbit left mandible (control group); CGF fibrin membrane was fixed on the inner surface of distractor, and the distraction gap was then completely wrapped prior to distraction osteogenesis in the right mandible (experimental group).At 1, 7, 14 and 28 days of consolidation period, the rabbits were sacrificed and the bilateral mandibles were taken for hematoxylin-eosin staining, western blot assay and immunohistochemical staining to detect the expression of RANKL and osteoprotegerin in the newborn bones; the osteogenesis in the distraction gap was observed and compared between two groups.RESULTS AND CONCLUSION: The osteoprotegerin positive cells and positive area percentage in the experimental group were significantly higher than those in the control group at 1, 7 and 14 days after distraction osteogenesis (P < 0.01). The RANKL positive rate and positive area percentage in the experimental group were significantly higher than those in the control group at 1 and 14 days after distraction osteogenesis (P < 0.05 or P < 0.01). Western blot assay showed a higher ratio of RANKL/osteoprotegerin in the control group than the experimental group (P < 0.01). Our findings indicate that CGF fibrin membrane can promote osteogenesis and mineralization in the distraction gap of the rabbit mandible, which is beneficial for distraction osteogenesis in the mandible.
3.Comparative analysis on drug-resistant bacterial distribution and drug resistance characteristics of lower respiratory tract infection in different regions of respiratory department

Objective To investigate the distribution and constituent of drug‐resistant bacteria of lower respiratory tract in‐fection among different regions (outpatient department ,wards ,RICU) to provide the basis for the clinical reasonable application of antimicrobial agents .Methods The K‐B disc diffusion method and the instrument method (VITEK‐TWO) were adopted and the detection results were interpreted according to the standards of CLSI 2010 .The detection data of 480 drug‐resistant strains isolated from the sputum ,branchoalveolar lavage fluid samples submitted in 3 regions of respiratory outpatients department by bacterial cul‐ture identification and drug susceptibility test were analyzed by using the WHONET5 .6 statistical software .Results The distribu‐tion and constituent of drug‐resistant bacteria of lower respiratory tract infection had obvious difference among 3 different regions . The top 4 of drug resistant bacteria were dominated by Gram‐negative bacteria .The drug resistance rate of Klebsiella pneumoniae in RICU was higher than that in the respiratory outpatients department and wards(P<0 .05) ,the resistance rate in the respiratory outpatients department ,wards and RICU to commonly used antibacterial drugs was similar;the multiple drug resistance of ESBLs‐producing strains was obviously higher than that of non‐ESBLs‐producing strains (P<0 .05) .Pseudomonas aeruginosa maintained the higher antibacterial activity to quinolone ,aminoglucosides ,cefepime ,imipenem ,cefoperazone/sulbactam ,and piperacillin/tazobactam ,but the resistance rate in RICU was significantly higher that in the respiratory outpatient department and wards (P<0 .05);the drug resistance of Acinetobacter baumanii in the respiratory wards and RICU was higher than that in the respiratory out‐patient department ,the resistances to imipenem were 64 .6% and 70 .4% respectively .The resistance of MRSA to rifampin in RICU was higher than that in the respiratory outpatient department and wards(P<0 .05) .Conclusion The distribution constituent and drug‐resistance rates have obvious differences among the respiratory outpatient department ,wards and RICU .Except being familiar with the drug resitant bacterial distribution and drug resistance rate monitoring situation ,clinical doctors should grasp the drug re‐sistance situation of drug resistant bacteria among different areas in various departments of own unit in order to rationally and effec‐tively use antibacterial drugs .
4.Evaluation of Antimicrobial Efficacy of Local Drug Resistance Spectrum Antibiotics and Foreign Guideline on Community Acquired Pneumonia

Objective To evaluate the clinical efficacy of the local drug resistance spectrum antibiotics and foreign guideline in the treatment of patients with community acquired pneumonia(CAP). Methods A prospective,randomized,single blind,and positive drug parallel controlled design was used in the treatment. CAP patients with no underlying disease outpatients and inpatients<48 hours were selected as the research object. The patients in the trial group were given sensitive local drug resistance spectrum antibiotics: moxifloxacin,400 mg and 1 times a day. The patients in the control group were given azithromycin tablets(each 500 mg,once daily) promulgated by the 2007 version of the IDSA / ATS adult CAP guideline. Results There were 106 cases of CAP patients,of which 77 cases completed treatment,including 39 cases in the experimental group and 38 cases in the control group. There were significant differences in the clinical efficacy and bacterial clearance rate between the two groups,with the clinical efficacy of 89.7% and 68.4%(P < 0.01),the bacterial clearance rate of 87.9% and 54.5%(P < 0.05),respectively. Conclusion The clinical efficacy of drug resistant spectrum sensitive antibiotics in the treatment of CAP in Kunming was better than that of IDSA/ATS. Clinicians should pay attention to the characteristics and composition of resistance of common pathogenic bacteria in our country during the study and reference from foreign guideline,and adjust the therapeutic regimen according to the changes of the local drug resistance monitoring data rather than copy the recommended treatment plan by foreign countries.
5.Analysis of OCRL gene mutation in a male infant with Lowe syndrome.

OBJECTIVETo identify pathological mutation in a Chinese male infant featuring oculocerebrorenal syndrome (also called Lowe syndrome).
METHODSClinical data of the patient were collected. DNA was extracted from peripheral blood of the infant and his parents. All of the 24 exons and intron-exon splice sites of OCRL gene were amplified with PCR. Mutations were detected by direct sequencing the PCR products.
RESULTSThe infant was found to have carried a c.1499G>A (p.R500Q) mutation in exon 15 of the OCRL gene, which was transmitted from his mother, who was heterozygous for the same mutation. The c.1499G>A mutation, discovered in Chinese population for the first time, has been reported to cause severe Lowe syndrome in other ethnic populations.
CONCLUSIONThe c.1499G>A mutation of the OCRL gene is probably responsible for the disease in the patient. Further study of this mutation may facilitate delineation of the genotype-phenotype correlation of this disease.

6. The role of abnormal mitochondrial fusion and fission in PBDE-47-induced change in mitochondrial mass in PC12 cells

Objective:
To investigate the effect of 2, 2', 4, 4'-tetrabromodiphenyl ether (PBDE-47) on the mitochondrial mass in rat adrenal pheochromocytoma (PC12) cells and the potential mechanisms.
Methods:
Highly differentiated PC12 cells were divided into control, 1, 10 or 20 μmol/L PBDE-47-treated groups and cultured for 24 h. Transmission electron microscopy was employed to observe the changes in mitochondrial morphology and quantity in PC12 cells. Flow cytometry was used to measure the fluorescence intensity of Nonyl Acridine Orange (NAO) , a fluorescent indicator of mitochondrial membrane cardiolipin, to reflect mitochondria mass. Western blotting was used to determine the expression levels of Mitofusion 1 (Mfn1) and Fission 1 (Fis1) proteins. To further explore the role of abnormal mitochondrial fusion and fission in PBDE-47-induced mitochondrial mass changes, PC12 cells were divided into control group, 5 μmol/L M1 treatment group, 20 μmol/L PBDE-47 treatment group and 5 μmol/L M1+20 μmol/L PBDE-47 combined treatment group and cultured for 24 h, then the fluorescence intensity of NAO and expression levels of Mfn1 and Fis1 proteins were detected.
Results:
The control group showed numerous mitochondria with normal morphology, while the number of mitochondria decreased after PBDE-47 treatment. Especially, the disappeared cristae, swelling and vacuoles of mitochondria and decreased fluorescence intensity of NAO (
7.Test-retest Reliability of Isokinetic Test for Hip Concentric Flexion and Extension

Objective To investigate the reliability of isokinetic test for concentric flexion and extension of hip joint. Methods From September, 2014 to June, 2015, 30 healthy young people accepted isokinetic test for concentric flexion and extension of hip twice with the same procedure and method within a week. The intra-class correlation coefficient (ICC) of peak torque, peak torque to body weight, total work, total work to body weight and average power between the two tests was calculated. Results The ICCs were above 0.70 in all the parameters on both sides at 60°/s (P < 0.01). Meanwhile they were above 0.61 of right hip at 180°/s (P < 0.05), but below 0.55 on left (P > 0.05). Conclusion The isokinetic test for hip is reliable for clinical assessment.
