1.Evaluation by MRI of Disc Injury in Fractures of the Thoracic and Lumbar Spine.

Injury to a lumbar disc is rare, and is seldom mentioned in the literature describing spinal fracture; even in patients with such fractures, disc herniation is infrequent and is often considered to be a sequella of trauma. Using magnetic resonance imaging(MRI) we investigated the incidence and pattern of disc injuries by an in 37 patients with fractures of the lumbar and thoracic spines. Discs adjacent to the fracture site were found to be normal in 19 patients(51.4%), but in ten(27.0%), the end plate was disrupted. The disc was ruptured in six patients(16.2%), and had degenerated in two patients(5.4%). Injured discs were found in the three patients with fracture-dislocation and three of the seven(42.9%) with burst fracture, but in none of the 27 with compression fracture. The three lower lumbar discs(L3-4, L4-5, and L5-S1) were found to be normal in 26 patients(70.3%), but had degenerated in seven(18.9%). Incidental asymptomatic disc herniation was found in four patients(10.8%), including one with degenerative spondylolisthesis. There was no traumatic disc herniation in the lower lumbar region. These results suggest that traumatic lumbar disc herniation is quite uncommon, even in the unstable thoracolumbar spinal injuries.

2.Extensive Multiple Morel-Lavallée Lesions: A Case Report.

Morel-Lavallée is a rare lesion caused by post-traumatic soft tissue injury. It usually occurs around the greater trochanter, and it occurs very rarely in the lumbar region. It is often difficult to be diagnosed in the emergency room. Delayed diagnosis may result in the need for open surgery. The authors report a patient with extensive multiple Morel-Lavallée lesions in the thoracolumbar, buttock, and thigh after trauma and provide a literature review.

3.Rupture of external iliac artery during microscopic lumbar disc surgery: A case report.

Iatrogenic vascular injury during lumbar disc surgery is a rare but serious complication. This paper reports a patient who sustained an injury to the iliac artery while undergoing intervertebral disc surgery at the lumbar region. He suffered from massive bleeding and shock. An urgent laparatomy was performed under cardiopulmonary bypass, and the vascular injuries were repaired successfully. This case shows that a rapid diagnosis and immediate intervention can result in a favorable outcome.

5.A Study about Changes in the Lumbosacral Region and Wrist of the Adolescent Weightilifters

The weightlifting is a sort of the sport, which has objects to promote the physical condition and to develop the fundamental strength in the adolescent period, but thete been no interests from the view points of sports injuries for weightlifting. So the authors have studied the changes in the lumbosacral region and wrist of the adolescent weightlifters and the results obtained are as follows; 1. The increase in the lumbosacral angle has a statistically significant relationship with the low back pain in the adolescent weightlifters. 2. The ulnar variance shows statistically significant increase than the control group and its increase seems to be a causative factor in developing wrist pain among the adolescent weightlifters.

6.Clinical observation on warming-promotion acupuncture for lumbar muscle strain.

OBJECTIVETo observe the theraputic effect of warming-promotion acupuncture for lumbar muscle strain.
METHODSSixty-four cases were randomly divided into a warming-promotion acupuncture group(group A) and a routine acupuncture group (group B), 32 cases in each group. The group A was treated with acupuncture at Shenshu (BL 23) and Guanyuanshu (BL 26) with warming-promotion acupuncture method, (Guanyuanshu (BL 26), Yaoyangguan (GV 3), Weizhong (BL 40) with twirling uniform reinforcing-reducing method. The group B was treated with acupuncture at same acupoints only with twirling uniform reinforcing-reducing method. After four-session treatment, the theraputic effect of two groups was compared.
RESULTSThe cured and markedly effective rate of 65.6% (21/32) in the group A, which was superior to that of 40.6% (13/32)in the group B (P < 0.05).
CONCLUSIONThe efficiency on lumbar muscle strain treated with warming-promotion acupuncture is superior to that with routine acupuncture, means a suitable manipulation can enhance the therapeutic effect.

9.Observation on short-term and long-term therapeutic effects of electroacupuncture at Houxi (SI 3) on acute lumbar sprain.

OBJECTIVETo compare therapeutic effects of electroacupuncture at Houxi (SI 3) and medicine on acute lumbar sprain.
METHODSThree hundred cases of acute lumbar sprain were randomly divided into two groups, a electroacupuncture (EA) group and a medication group, 150 cases in each group. The EA group were treated with EA at Houxi (SI 3), once each day, 3 sessions constituting one course, and the medication group with Mobike, once daily, 7. 5 mg each time. Their therapeutic effects were evaluated after treatment for 7 days and one month respectively.
RESULTSFor the short-term therapeutic effect, the effective rate was 97. 3% in the EA group and 89. 2% in the medication group with a very significant difference between the two groups (P<0. 01); for the long-term therapeutic effect, the effective rate was 99. 3% in the EA group and 93. 2% in the medication group with a very significant difference between the two groups (P<0. 01).
CONCLUSIONBoth the short-term and the long-term therapeutic effects of EA at Houxi (SI 3) on acute lumbar sprain are better than those in the medication group.

10.Spinal Subarachnoid Hemorrhage Migrated from Traumatic Intracranial Subarachnoid Hemorrhage.

Very rarely, spinal subarachnoid hemorrhage (SSAH) can occur without any direct spinal injury in patients with traumatic intracranial SAH. A-59-year-old male with traumatic intracranial subarachnoid hemorrhage (SAH) presented with pain and numbness in his buttock and thigh two days after trauma. Pain and numbness rapidly worsened and perianal numbness and voiding difficulty began on the next day. Magnetic resonance imaging showed intraspinal hemorrhage in the lumbosacral region. The cauda equina was displaced and compressed. Emergent laminectomy and drainage of hemorrhage were performed and SSAH was found intraoperatively. The symptoms were relieved immediately after the surgery. Patients with traumatic intracranial hemorrhage who present with delayed pain or neurological deficits should be evaluated for intraspinal hemorrhage promptly, even when the patients had no history of direct spinal injury and had no apparent symptoms related to the spinal injury in the initial period of trauma.
