1.Effect Analysis of Different Courses of Saccharomyces Boulardii Powder on Helicobacter Pylori Eradication in Children

Objective To investigate the efficacy of different courses of saccharomyces boulardii powder combined with triple therapy in eradicating helicobacter pylori(Hp)in children.Methods A total of 135 children with Hp-related gastritis who received initial treatment in the Department of Pediatrics,Affiliated Hospital of Xuzhou Medical University from October 2021 to June 2022 were selected and divided into three groups according to random number table method:group A,group B and group C,with 45 cases in each group.Group A:triple therapy(omeprazole + clarithromycin + amoxicillin)for 14 days;group B and group C:on the basis of triple therapy,saccharomyces boulardii powder was added from the first day of treatment for 2 weeks and 4 weeks,respectively.The adverse reactions during treatment were recorded,the levels of serum pepsinogen Ⅰ(PGⅠ)and pepsinogen Ⅱ(PGⅡ),clinical efficacy and Hp eradica-tion rate were observed 4 weeks after the end of treatment.Results The levels of serum PGⅠ and PGⅡ in the three groups after treat-ment were significantly lower than those before treatment(P<0.05),the levels of serum PGⅠ and PGⅡ in group B and group C after treatment were lower than those in group A(P<0.05),and the levels of serum PGⅠ and PGⅡ in group B were lower than those in group C after treatment(P<0.05).The clinical effective rates of group B(93.0%)and group C(90.4%)were higher than those of group A(62.5%)(P<0.05).The clinical effective rate of group B was higher than that of group C(P>0.05).The Hp eradication rate of group B and group C was significantly higher than that of group A(P<0.05),and the Hp eradication rate of group B was higher than that of group C(P>0.05).The incidence of diarrhea and loss of appetite in group B and group C were lower than those in group A(P<0.05);the incidence of diarrhea and loss of appetite in group B was lower than that in group C(P<0.05).There was no signifi-cant difference in the incidence of abdominal pain,nausea and vomiting among the three groups(P>0.05).Conclusion Saccharomy-ces boulardii decoction combined with triple therapy for 2 or 4 weeks can effectively regulate PG level,improve the eradication rate of Hp,and reduce the incidence of adverse reactions.It is suggested that 2 weeks is the best course of treatment.
2.Change and clinical significance of microRNA-148b-3p level in the serum of patients with type 2 diabetes mellitus and diabetic nephropathy

Objective To detect the serum microRNA-148b-3p level in patients with diabetes mellitus and diabetic nephropathy,and to analyze its correlation with clinical and pathological indexes.Methods The research crowd was divided into three groups (1) diabetic nephropathy group:biopsy with diabetic nephropathy (n=25,14 males,11 females);(2) type 2 diabetes mellitus group:type 2 diabetes mellitus patients with normal urinary microalbumin/urinary creatinine value (n=10,4 males,6 females);(3) normal control group:healthy subjects (n=9,3 males and 6 females).Clinical indicators included gender,age,24-hour urine protein,systolic blood pressure (SBP),diastolic blood pressure (DBP),serum creatinine (Scr),urea (Urea),cystatin-C (Cys-C),blood albumin (ALB),urine microalbumin (UMA),triacylglycerol (TG),total cholesterol (TC),high-density lipoprotein cholesterol (HDL-C),low-density lipoprotein cholesterol (LDL-C),serum uric acid (UA),fasting blood glucose (FBG),glycated hemoglobin (HbA1c),urine microalbuminuria / urinary creatinine (UACR),and estimated glomerular filtration rate (eGFR) calculated by CKD-EPI formula.Real-time quantitative PCR was applied to verify the expression of microRNA-148b-3p in serum samples of the research crowds.The relationships between microRNA-148b-3p level and clinical features was also analyzed.Results The levels of serum microRNA-148b-3p in diabetic nephropathy group and in type 2 diabetes mellitus group were 1.82 times and 1.73 times of that in normal control group (P < 0.05,respectively).The level of serum microRNA-148b-3p was significantly correlated with HDL-C (r=-0.374,P=0.013),UMA (r=0.426,P=0.004),FBG (r=0.330,P=0.046) and TG (r=0.423,P=0.005).Multiple linear regression analysis showed that UMA level was independently associated with serum microRNA-148b-3p level (β=0.338,P=0.044).The area under the receiver operating characteristic curve (ROC) of serum microRNA-148b-3p in diagnosing type 2 diabetes mellitus and diabetic nephropathy was 0.835 and 0.665,respectively.Conclusions The level of serum microRNA-148b-3p of patients with type 2 diabetes mellitus or diabetic nephropathy significantly increases.The level of UMA is independently associated with serum microRNA-148b-3p level.Serum microRNA-148b-3p is expected to be a potential biomarker for the diagnosis of diabetic nephropathy.
3.Effectiveness of different specific exercise therapies in treatment of adolescent idiopathic scoliosis:a network meta-analysis

OBJECTIVE:At present,there are a variety of treatment methods for scoliosis using specific exercise therapy,but there is a lack of comparison of efficacy between different specific exercise therapy.This article compared the effectiveness of different specific exercise therapies to treat adolescent idiopathic scoliosis through a network meta-analysis. METHODS:Domestic and foreign electronic databases of relevant studies were searched for randomized controlled trials of specific exercise therapy for adolescent idiopathic scoliosis.Search time was from January 2000 to July 2023.The literature was screened by two reviewers using RevMan 5.4 and Stata 16.0 software to extract data and assess the bias risk of of inclusion studies. RESULTS:(1)This article includes 20 randomized controlled trials with 1 377 patients.Of them,12 studies involved Schroth therapy;2 studies involved BSPTS therapy,and 6 studies involved SEAS therapy.(2)The network meta-analysis indicated that in terms of improving Cobb angle and reducing trunk rotation angle in scoliosis patients,the BSPTS therapy group and Schroth therapy group were better than the conventional control group[WMD=-4.60,95%CI(-8.37,-0.82),P<0.05;WMD=-3.37,95%CI(-4.98,-1.75),P<0.05;WMD=-3.20,95%CI(-5.50,-0.90),P<0.05;WMD=-2.13,95%CI(-3.16,-1.09),P<0.05].The Schroth therapy group performed better than the conventional control group effective in improving the International Society for Scoliosis Research-22 Questionnaire quality of life score[WMD=1.41,95%CI(0.07,2.75),P<0.05]. CONCLUSION:Given the current evidence,BSPTS therapy group and Schroth therapy group were better than the conventional control group in improving Cobb angle and reducing trunk rotation angle.In the comparison of different specific exercise therapies,BSPTS therapy can be preferred to improve Cobb angle and reduce trunk rotation angle in adolescent idiopathic scoliosis patients.In addition,Schroth therapy may be the best treatment to improve the quality of life of adolescent idiopathic scoliosis patients.Limited by the quantity and quality of the included studies,the above conclusions should be interpreted with caution and need more high-quality studies to further validation.
4.Effect of neuromuscular exercise for knee osteoarthritis pain and function:a meta-analysis

OBJECTIVE:Neuromuscular exercise is a new comprehensive rehabilitation therapy in recent years,but its effect on knee osteoarthritis is still controversial.The purpose of this paper is to systematically evaluate the efficacy of neuromuscular exercise on knee osteoarthritis pain and function. METHODS:The randomized controlled trials addressing neuromuscular exercise in the treatment of knee osteoarthritis pain and function were retrieved from PubMed,Cochrane Library,Embase,EBSCO,CNKI,Web of Science,China Biomedical Database(CBM),VIP,and WanFang Database.The retrieval time ranged from database inception to October 2023.The neuromuscular training group(experimental group)was given neuromuscular training or neuromuscular training as the main intervention;the control group was a blank group or given conventional rehabilitation.Outcome indicators included the Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)score,walking time,knee stability,and the maximum number of knee flexion in 30 seconds.The risk of bias was evaluated by the Cochrane Collaboration tool and the Physiotherapy Evidence Database.Meta-analysis was performed using RevMan 5.4 software. RESULTS:A total of 11 randomized controlled trials were included,and 628 samples were extracted.The results of Meta-analysis showed that the experimental group was superior to the control group in terms of WOMAC pain score[standardized mean difference(SMD)=0.38,95%confidence interval(CI):0.08-0.69,P=0.01],knee stability(SMD=0.57,95%CI:0.23-0.92,P=0.001),the maximum number of knee joint flexion in 30 seconds(SMD=0.35,95%CI:0.05-0.65,P=0.02),and WOMAC physical function score(SMD=-0.79,95%CI:-1.30 to-0.28,P=0.002).In both groups,walking speed was increased and walking ability was improved in patients with knee osteoarthritis,but there was no significant difference(walking time:SMD=-0.22,95%CI:-0.48-0.03,P=0.09). CONCLUSION:Neuromuscular exercise can effectively improve knee joint pain,enhance the stability of the knee joint,and promote functional recovery in patients with knee osteoarthritis.However,more high-quality randomized controlled trials are still needed to further confirm the research.
5.Summary of the best evidence for diet management in patients with chronic kidney disease in stage 3-5 D

Objective:To summarize the evidence of diet management in patients with chronic kidney disease (CKD) in stage 3-5 D, so as to provide a reference for clinical nursing.Methods:After establishing evidence-based nursing questions, according to the "6S evidence model", the evidence on diet management of patients with CKD in stage 3-5 D was searched by computer, including computer decision support system evidence, guidelines, best evidence summary and systematic review. The search time limit was from the establishment of the database to September 2020. The guideline quality assessment was independently completed by two research nurses and a doctor of nephrology. The quality assessment of non-guideline article was independently completed by two research nurses, and an evidence-based instructor participated in decision-making. The evidence was extracted, evaluated and graded using the Joanna Briggs Institute (JBI) 2014 version of the intervention research evidence pre-grading system.Results:A total of 19 articles that met the requirements were included, including 5 guidelines, 8 systematic reviews, 4 randomized controlled trials, 1 clinical decision, and 1 national standard. A total of 22 pieces of the best evidence on diet management of patients with CKD in stage 3-5 D were summarized from three aspects, namely, diet/nutrition assessment, diet management, teamwork and education.Conclusions:The best evidence of diet management in patients with CKD in stage 3-5 D provides a certain reference for clinical practice, so as to improve the quality of diet management in patients with CKD and the clinical outcomes of patients.
6.Effects of long non-coding RNA H19 regulating miRNA-675 and PTEN on the cell proliferation of glioma

Objective:To investigate the effect of long non-coding RNA (lncRNA) H19 regulating miRNA-675 (miR-675) and phosphatase and tensin homologue-deleted chromosome ten gene (PTEN) on the cell proliferation of glioma.Methods:Glioma cell lines U87-MG and U251 were chosen. The siRNA online design tool wad used to design small interfering RNA (siRNA) targeting H19. U87-MG and U251 cell lines with the stable knockdown of H19 were constructed (the stable knockdown of H19 group), and the cells randomly transfected with siRNA plasmid were taken as the control group, and normal cultured cells were treated as the blank group. Additionally, miR-675 and control microRNA were transfected into U87-MG and U251 with the stable knockdown of H19 (the overexpressing miR-675 group and the corresponding control group). Quantitative real-time polymerase chain reaction (qRT-PCR) was used to detect the relative expression levels of miR-675 and H19 in each group; the methyl thiazolyl tetrazolium (MTT) assay was used to detect the cell proliferation ability; the dual luciferase reporter gene assay was used to verify the targeting relationship between miR-675 and PTEN; Western blot was used to detect the relative expression level of PTEN protein.Results:The MTT assay results showed that the proliferation ability of U87-MG and U251 cells in the stable knockdown of H19 group was lower than that of the corresponding control group; and the differences in cell proliferation ability of all the groups after 48 h of culture were statistically significant (all P < 0.05). qRT-PCR detection results showed that the relative expression level of miR-675 in U251 cells in the stable knockdown of H19 group and the corresponding control group was 0.329±0.009 and 1.043±0.087, respectively, and the difference was statistically significant ( t = 14.15, P < 0.001); the relative expression level of miR-675 in U87-MG cells in the stable knockdown of H19 group and the corresponding control group was 0.299±0.009 and 1.027±0.106, respectively, and the difference was statistically significant ( t = 11.85, P < 0.001); the relative expression level of miR-675 in U87-MG and U251 cells in the stable knockdown of H19 group was lower than that of the corresponding control group. The dual luciferase reporter gene assay verified that miR-675 could bind to the 3'-UTR of PTEN. Western blot detection results showed that the relative expression level of PTEN protein in U87-MG and U251 cells in the stable knockdown of H19 group was higher than that of the corresponding control group and the blank group; in the U87-MG and U251 cells in the stable knockdown of H19 group, the relative expression level of PTEN in the overexpressing miR-675 group was lower than that of the corresponding blank group and the control group. In the U87-MG and U251 cells in the stable knockdown of H19 group, the cell proliferation ability of the overexpressing miR-675 group was higher than that of the corresponding blank group and the control group; the differences in cell proliferation ability of all the groups after 48 h of culture were statistically significant (all P < 0.05). Conclusions:lncRNA H19 may regulate the cell proliferation of glioma cells through the miR-675-PTEN signaling pathway.
7.Meta-analysis of the Safety of Meropenem for Neonatal Infection

OBJECTIVE:To systematically evaluate the safety of meropenem for neonatal infection ,and to provide evidence-based reference for safe use of it in the neonatal population. METHODS :Retrieved from PubMed ,Embase,Cochrane Library,ISI Web of Science ,International Health Technology Assessment Network Website ,China Journal Full-text Database , Wanfang Database ,CBM,Chinese Sci-tech Periodical Full-text Database ,randomized controlled trials (RCTs)about meropenem or meropenem combined other drugs (trial group )versus the similar drugs that could replace meropenem (control group )for neonatal infection were collected during the inception to May 1st,2021. After literature screening and data extraction ,the quality of included literatures were evaluated with Cochrane systematically evaluator manual 5.1.0. Meta-analysis was conducted with RevMan 5.3 software. RESULTS :A total of 25 RCTs were included ,involving 2 090 children. Results of Meta-analysis showed that the incidence of overall ADR in trial group was significantly lower than control group [RR =0.53,95%CI(0.44,0.65),P<0.000 01]. Results of subgroup analysis showed that the incidence of overall ADR in trial group was significantly lower than control group receiving ceftazidime [RR =0.55,95%CI(0.41,0.74),P<0.000 1],tigecycline [RR =0.37,95%CI(0.23,0.59),P<0.000 1], ceftriaxone [RR =0.53,95%CI(0.35,0.80),P=0.003]. The incidence of overall ADR in trial group with neonatal purulent meningitis [RR =0.63,95%CI(0.44,0.92),P=0.02],severe neonatal multidrug-resistant bacterial infection [RR =0.37,95%CI(0.25, 0.55),P<0.000 01],neonatal severe bacterial infection [RR = 0.67,95%CI(0.48,0.94),P=0.02] were significantly lower than control group. The incidence of specific ADR such as mail: rash,gastrointestinal reaction ,hemoglobin reduction in trialgroup were significantly lower than control group (P<0.05). There was no statistical significance in the incidence of specific ADR between 2 groups,such as elevated transaminase ,secondary fungal infection and renal injury (P>0.05). Results of bias analysis showed that when the incidence of overall ADR was used as index ,there was a certain degree of publication bias in this study ,when the incidence of specific ADR was used as index ,there was less possibility of publication bias in this study. CONCLUSIONS:Meropenem is safe in the treatment of neonatal infection ,especially in the treatment of neonatal purulent meningitis,severe neonatal multidrug-resistant bacterial infection and neonatal severe bacterial infection ,it is superior to ceftazidime,tigecycline,ceftriaxone and other antibacterial drugs in safety.
8.Clinical comprehensive evaluation of desloratadine in the treatment of urticaria

OBJECTIVE To eva luate the clinical comprehensive value of desloratadine in the treatment of urticaria. METHODS The clinical comprehensive evaluation index system based on six dimensions such as safety ,effectiveness,economy,suitability, innovation and accessibility were preliminarily determined by using the methods of literature investigation and expert investigation ; the core contents of the evaluation index system were evaluated and screened by Delphi method and analytic hierarchy process ;the importance of the index was assigned by Likert 5-level scoring method ;the evidence from various sources were collected and qualitative and quantitative integration analysis were conducted according to the clinical comprehensive evaluation index system with the help of system evaluation ,drug instructions ,expert guidelines/consensus ,adverse drug reaction monitoring report ,etc; the experts scored its clinical comprehensive value according to the clinical comprehensive evaluation evidence of each dimension of desloratadine ,combined with the weight of the clinical comprehensive evaluation index system ,the clinical comprehensive evaluation score of the tertiary indexes of desloratadine were calculated. The total clinical comprehensive evaluation score was obtained by accumulating the scores of each index ,and compared with loratadine. RESULTS This study successfully constructed the clinical comprehensive evaluation index system of desloratadine in the treatment of urticaria ,including 6 primary indexes ,13 secondary indexes and 30 tertiary indexes. The total clinical comprehensive evaluation score of desloratadine was 93.63 and that of loratadine was 70.91. CONCLUSIONS The clinical comprehensive value of desloratadine is higher than that of loratadine ,which can provide a reference basis for clinical rational drug use in medical institutions ,selection of drug use catalogue and improvement of national drug policy.
9.The mechanism of effective fractions from Xiongma decoction in the treatment of migraine model rats with hyperactivity of liver-yang and blood stasis

OBJECTIVE To expl ore the mechanism of effe ctive fractions from Xiongma decoction in the treatment of migraine with hyperactivity of liver-yang and blood stasis. METHODS Totally 70 male SD rats were randomly divided into normal group , model group ,positive control group (Flunarizine hydrochloride capsules 0.9 mg/kg),low-dose and high-dose groups of Xiongma decoction effective fractions (ethyl acetate extract 0.87,3.46 g/kg,n-butanol extract 1.80,7.20 g/kg). Except for normal group , rats in other groups were given aconite decoction (2 g/kg),once a day ,for 4 consecutive weeks to establish the hyperactivity model of liver-yang. On the 15th day of modeling ,all administration groups were given corresponding drugs intragastrically at the same time ,once a day ,for 2 consecutive weeks. On the 29th day of modeling ,rats trigeminal ganglion was stimulated to establish the migraine model with hyperactivity of liver-yang and blood stasis ,then the medication was maintained for another time according to the above method. The macroscopic signs and behavior of the rats were observed ;positive expression ,mRNA and protein expression of transient receptor potential vanilloid 1 (TRPV1),calcitonin generelated peptide (CGRP),calcitonin receptor-like receptor (CRLR) and receptor-associated membrane proteins 1 (RAMP1) in trigeminal cervical spinal complex (TCC) were detected by immunohistochemistry ,RT-qPCR and Western blot assay. RESULTS Rats in model group showed macrophysical signs and behaviora l manifestations related to migraine with hyperactivity of liver-yang and blood stasis. Thirty minutes after last administration ,the above conditions of rats in Xiongma decoction effective fraction groups were improved significantly. Compared with normal group , positive expression,mRNA and protein expression of TRPV 1,CGRP, CRLR and RAMP 1 in TCC of rats in the model group were significantly increased (P<0.05). Compared with model zengguirong@hnse.org group, most of above indicators in Xiongma decoction effective fraction groups were significantly decreased (P<0.05). CONCLUSIONS The mechanism of Xiongma decoction in preventing and treating migraine with hyperactivity of liver-yang and blood stasis may be related to inhibit the activity of TRPV1-CGRP/CGRP receptor signaling pathway in TCC.
10.Correlation of Quality of Life, Disabilities Attitude and Quality of Care and Support for People with Disability in Urban Areas of Lanzhou, China

Objective To investigate the survival status of people with disabilities in urban areas of Lanzhou, China and discuss the correlation of their quality of life to disability attitude and quality of care and support of disabled people. Methods From August to November, 2016, 606 persons with disability registered in Chengguan District of Lanzhou were selected by multistage stratified cluster sampling. They were investigated in home-based visit with World Health Organization Quality of Life-Disability Scale for physical disability, World Health Organization-Disability Attitude Scale and World Health Organization Quality of Care and Support Scale-Disability Scale. Results The quality of life scored in average of (40.76±14.79), and different with the demographic characteristics (P<0.05). Pearson's correlation analysis showed that the score in all fields of quality of life and the total score positively correlated with most dimensions of disability attitude score (r>0.080, P<0.05) and most dimensions score of quality of care and support (r>0.083, P<0.05).Conclusion It is important to strengthen the precise security system for people with disabilities, and strive to establish a full-cycle, all-round social support system for them.
