2.Surgical treatment of 516 cases with acute traumatic brain injury

Objective To summarize the experience and lesson of surgical treatment of acute traumatic brain injury.Methods A total of 516 cases with acute traumatic brain injury treated surgi- cally from January 2001 to December 2004 in our hospital were analyzed retrospectively.Of all,there were 56 cases with simple comminuted depressed fractures,138 with brain contusion and laceration and/ or intracerebral hematoma,122 with epidural hematoma,126 with suhdural hematoma,48 with diffuse brain swelling,18 with open brain injuries and eight with other kind of injury,all of whom were treated with standard large eraniectomy under general anaesthesia.The treatment results were evaluated by Glas- cow Outcome Scale(GOS).Results Of all,standard large craniectomy was performed in 194 cases, of which 304 cases(58.9%)were with good recovery,66(12.8%)with moderate deficit,72(13.9%) with severe deficit,12(2.3%)under persistent vegetative status and 62(12.0%)died 3-6 months after surgery.Conclusions More attention should be paid to surgery for traumatic brain injury.Individual surgical treatment should be performed under surgical principles.
3.Therapeutic efficacy of extended therapy with interferon and a nucleoside analogue for treating patients with HBeAg-positive chronic hepatitis B

Objective To observe the therapeutic efficacy of extended therapy with interferon and a nucleoside analogue for treating patients with HBeAg?positive chronic hepatitis B( CHB)?Methods Sixty cases patients diagnosed with CHB in You′an Hospital of Beijing Affiliated to Capital Medical University from March 2012 to May 2015 were retrospectively analyzed?They were divided into group A with 29 cases and group B with 31 cases according to different treatment methods?Group A were treated with pegylated interferonα?2a( Peg?IFNα?2a) combined with adefovir dipivoxil for 96 weeks,group B were treated with Peg?IFN α?2a combined with adefovir and lamivudine for 96 weeks?The patients'recovery rate of ALT,hepatitis B virus( HBV) DNA response rate and HBeAg and HBsAg seroconversion rate and conversion rate of 12,24,48,96 weeks in the treatment were observed?Results There were no significant differences in recovery rate of ALT,hepatitis B virus( HBV) DNA response rate,HBeAg seroconversion rate and HBsAg seroconversion rate between the two groups at differenct time points( P>0?05)?HBeAg seroconversion rates in group A were 34?5% at 48 weeks,62?1% at 96 weeks, and the difference was significant( P=0?036)?HBeAg seroconversion rates in group B were 35?5% at 48 weeks, 61?3% at 96 weeks,and the difference was significant ( P=0?042)?Conclusion The treatment of peginterferonα?2a in combination with adefovir dipivoxil and lamivudine is not more efficacious than peginterferon α?2a in combination with adefovir dipivoxil?The extended treatment of Peg? IFNα?2a plus a nucleoside analogue can achieve high rates of HBeAg seroconversion in patients with HBeAg?positive CHB.
4.Analysis of Clinical Drug Use and Adverse Drug Reactions in Children's Hospital

OBJECTIVE:To evaluate the characteristics and regularity of adverse drug reactions (ADR) in children's hospital. METHODS:The clinical drug use and ADR of 3 653 children during 2006~2008 in our hospital were analyzed statistically by designing a child medication registration form. RESULTS:The incidence of 498 ADR cases was about 13.63%; ADR were mostly occurred in 1~3 year-old children(36.14%); 71.29% were caused by intravenously; 66.87% were induced by antibiotics; ADR were more common in spring and winter. CONCLUSION:The drugs have a dual nature. Moreover,children belong to a special group. The measures such as strictly mastering clinical indications,reducing irrational drug use,sufficient therapy and avoiding or reducing the occurrence of ADR can guarantee children grow healthily.
5.Coumarins from Leonurus japonicus and their anti-platelet aggregative activity.

Chemical constituents of Leonurus japonicus were isolated and purified by a combination of various chromatographic techniques including column chromatography over silica gel, Sephadex LH-20, MCI, and Rp C18. Structures of the isolates were determined by spectroscopic analysis as 10 coumarins: bergapten (1), xanthotoxin (2), isopimpinellin (3), isogosferal (4), imperatorin (5), meransin hydrate(6), isomeranzin(7), murrayone(8) , auraptenol(9), and osthol(10). In addition to compound 9, the others were isolated from the genus Leonurus for the first time. In the in vitro assay, compounds 4 and 8 significantly inhibited the abnormal increase of platelet aggregation induced by ADP.

6.Differentiation of Human Amniotic Mesenchymal Cells into Cardiomyocyte-like Cells in Vitro

To explore the plasticity of human amniotic mesenchymal cells(hAMCs) into cardiomyocyte-like cells,hAMCs were isolated from human amnion with collagenase digestion.Phenotype of the isolated cells was analyzed by flow cytometry(FCM).hAMCs were treated with 5-azacytidine and basic fibroblast growth factor(bFGF) to investigate their ability of differentiation into cardiomyocytes.The induced differentiated cells were evaluated by immunofluorescence for desmin and ?-actin expression and by RT-PCR for Nkx2.5,GATA-4 and alpha-myosin heavy chain(?-MHC) mRNA expression.The results showed that,after primary culture,hAMCs could reach a confluence of 80% with swirl like growth at 6 days.The cells proliferated rapidly after passages with a 100% confluence at 3~4 d.hAMCs were positive expression of CD44 and vimentin,but negative for CK19.After induced differentiation at 8~10d,the differentiated cells have close-up arranged with long spindle-shape.At 2 weeks and 4 weeks,induced cells expressed ?-actinin and cardiac-specific transcription factor Nkx2.5.Expressions of GATA-4 and desmin can be detected but ?-MHC can not in the hAMCs both before and after the induction.In conclusion,hAMCs have the ability of differentiation into cardiomyocyte-like cells,which means that hAMCs can be regarded as candidate cells for cellular cardiomyoplasty(CCM).
7.Plasma level of myeloperoxidase is not elevated in patients with stable angina pectoris

Objective To investigate the plasma level change of myeloperoxidase(MPO)in patients suffering from stable angina pectoris.Methods Five hundred and seventy three patients underwent elective coronary angiography in a bi-racial cohort study,which included 295 patients with stable angina peetoris(SAP)and 278 subjects served as control.Plasma level of MPO and traditional risk factors of coronary artery disease(CAD)were measured.Results MPO levels did not differ significantly between control group and SAP group[126.3(95.8-160.2)mg/L vs 123.6(97.4-150.0) mg/L P>0.05].MPO levels were similar across ethnicity and gender[black male 119.6(94.8-146.9) mg/L,white male 124.6(99.9~154.6)mg/L,black female 124.0(93.3~152.3)mg/L and white female 127.5(95.3~159.8)mg/L],and were correlated positively with the levels ofⅦfactor(r= 0.251,P<0.01),fasting plasma glucose(r=0.095,P<0.05),triglyceride(r=0.186,P<0.01), total cholesterol(r=0.081,P<0.05),high-sensitivity C-reactive protein(r=0.123,P<0.01) and fibrinogen levels(r=0.077,P<0.01),negatively correlated with adiponectin level(r=-0.115, P<0.01).Conclusions Plasma MPO level is not elevated in patients with SAP.This suggests that MPO is not a characteristic feature of SAP.There are also no significant relationships between different genders and between different ethnicities.
8.Comparison of the Effect of Different Establishment Methods on the Rat Models of Polycystic Ovary Syndrome

Objective To establish rat models of polycystic ovary syndrome(PCOS) induced by different methods,to assess the serum levels of several related hormones,to examine the morphological changes in the ovaries,and to discuss their significance.Methods Letrozol,sodium prasterone sulfate,or sodium prasterone sulfate combined with human chorionic gonadotropin were used to establish rat models of PCOS.Radioimmunoassay was used to measure the serum levels of hteinizing hormone(LH),follicle-stimulating hormone(FSH),estrogen(E_2),progesterone(P),testosterone(T),prolactin(PRL),and insulin(INS).HE staining was used to examine the morphological changes of the ovaries.Results Comparing with the normal group A,the serum FSH was increased and the serum progesterone was reduced in the group B,with a statistically significant difference(P<0.05).The serum testosterone was significantly higher in the group B than in the group A(P<0.01).The levels of serum sex hormones and insulin were not significantly different in the group D and C(P>0.05).In comparison with the group C,the levels of serum testosterone and LH/FSH ratio was significantly increased in the group E.(P<0.05).Comparing with the group D,the serum levels of progesterone and testosterone were significantly increased in the group E(P<0.05).The ovaries in the rats of groups A and C showed almost a normal histyology,with mature follicles and dominant follicles.Polycystic changes were observed only in the ovaries of groups B,D and E.Conclusion At the aspect of affecting the level of sex hormones in serum and changing the ovarian morphology.adopting letrozol tablets or sodium prasterone sulfate combined with HCG to induce rat PCO model is more close to clinic manifestations and meets the criteria of PCO animals.In the rat PCOS models induced with letrozol or with sodium prasterone sulfate combined with HCG,either the serum levels of sex hormones and ovarian histology are quite similar to those of human clinical appearance,and may well meet the modeling requirements for future experimental studies of polycystic ovary syndrome.
9.Predisposing factors for postoperative epilepsy in patients with gliomas.

OBJECTIVETo explore the predisposing factors for postoperative epilepsy in patients with gliomas.
METHODSA total of 258 glioma patients with complete clinical data receiving cranial surgeries were analyzed retrospectively. With gender, age, predominant symptoms, positive signs, history of preoperative epilepsy, time of epilepsy onset, tumor location, surgical approaches, cortical injury, arterial and venous injury, scope of tumor resection, postoperative edema, tumor pathology, tumor recurrence, number of operation, radiation therapy as the independent variables, the occurrence of postoperative epilepsy was analyzed as the dependent variable using logistic regression to identify the risk factors for postoperative epilepsy.
RESULTSHistory of preoperative epilepsy, surgical approaches, postoperative edema, tumor pathology and tumor recurrence were identified as the risk factors for postoperative epilepsy in glioma patients.
CONCLUSIONSPostoperative epilepsy severely affected the quality of life of glioma patients, and rigorous treatment targeting the risk factors may decrease the occurrence of postoperative epilepsy.

10.Clinical pathology of pineal region tumors

Objective To investigate the pathological distribution and clinical characteristics of pineal region tumors (PRTs) with variant pathology.Methods We retrospectively analyzed the clinical and pathological data of 133 patients with PRTs that were performed surgical removal from January 2000 to January 2009 in our hospital.Results Sixty-one patients (45.9%) were diagnosed as having germ cell tumors,17 (12.8%) pineal parenchymal tumors,28 (21.1%) gliomas and 27 (20.2%)other resource tumors.Sex ratio of patients with germ cell tumors,pineal parenchymal tumors and gliomas were 14.25:1 (male:female),2.4:1 and 1.15:1,respectively; their median ages were 15.3,24.7 and 28.1,respectively.The serum immunologic test showed abnormal results in 33 patients; except for 1 with metastatic tumors,the others were diagnosed as having germ cell tumors.Conclusion PRTs have many pathological types and patients with PRTs are mainly diagnosed as having germ cell tumors.Correct diagnosis can not be made by imaging,serum immunologic test or biopsy, thus,obtaining complete clinical specimen during the total tumor removal is very necessary in studying the pathology of PRTs.
