1.KL-6 as a useful serum marker for interstitial lung disease in patients with idiopathic inflammatory myopathies

Objective To determine the serum level of KL-6 in patients with polymyositis(PM)and dermatomyositis(DM),and to investigate the possible diagnostic value for interstitial lung diseases(ILD)in pa- tients with PM/DM.Methods Serum KL-6 concentrations were measured by ELISA in 53 adult PM/DM pa- tients,the control groups of 22 patients with infectious lung disease,and 50 healthy subjects.The association with clinical features and serum KL-6 levels was analyzed.Results The serum levels of KL-6 were(1543?761)、(429?106)、(336?196)and(289?105)U/ml in PM/DM patients with ILD and without ILD,patients with infectious lung disease and healthy controls,respectively.Serum KL-6 levels in the PM/DM patients with ILD were significantly higher compared with PM/DM patients without ILD,patients with infectious lung disease and healthy controls(all P<0.01 ).However,no significant differences of serum KL-6 levels was found among PM/ DM patients without ILD,patients with infectious lung disease and healthy controls(P>0.05).Significant cor- relations were found between the elevated levels of serum KL-6 and the presence of ILD in patients with PM/ DM(P<0.01).The sensitivity and specificity of serum abnormal KL-6 levels for ILD in patients with PM/DM were 90.9% and 80.6%.Additionally,follow-up study showed the mean serum levels of KL-6 in six patients who died were significantly higher than that in other PM/DM patients with ILD(P<0.01).Conclusion Serum KL-6 level is a reliable serum marker for ILD in idiopathic inflammatory myopathies,which may contribute to early differentiate ILD from lung infectious disease.Increased serum level of KL-6 may predict a poor out- come.
2.The clinical significance of myositis specific autoantibodies in polymyositis and dermatomyositis

Objective To detect the prevalence of myositis-specific autoantibodies (MSAs) and myositis-associated autoantibodies (MAAs) in patients with polymyositis (PM) and dermatomyositis (DM), and analyse the correlation between MSAs and the clinical features and prognosis of PM/DM. Methods Serum samples of 31 PM and DM patients were screened for MSAs (including anti Jo-1, anti Mi-2, anti PL-7, anti PL-12 antibodies) and MAAs (including anti Ku, anti PM-Scl antibodies) by immunoblotting test. Results Serum MSAs/MAAs were detected in 18 out of 31 PM/DM patients (58%). MSAs were present in 12 patients (39%). The most frequently encountered MSAs was anti-Jo-1 autoantibody (29%), followed by anti-Mi-2 (7%), anti-PL-7 (3%), and anti-PL-12 (3%). MAAs were present in 10 patients (32%), including anti-Ku-72 (16%), anti-Ku-86 (23%) and anti-PM/Scl (7%). 3.En bloc resection of tumor involving partial sacrum and ipsilateral sacroiliac joint with reservation of contralateral sacral foramina

Objective To evaluate effect of en bloc resection of malignant sacral tumor involving partial sacrum and ipsilateral sacroiliac joint with reservation of contralateral sacral foramina.Methods Between February 2005 and July 2010,16 patients with malignant sacral tumor involving partial sacrum and ipsilateral sacroiliac joint underwent hemi-sacrectomy and functional reconstruction.The operation achieved en bloc resection of tumor with reservation of contralateral sacral foramina.There were 10 males and 6 females,aged from 17 to 70 years (average,37.9 years).There were 5 cases of chondrosarcoma,3 cases of Ewing's sarcoma,2 cases of malignant schwannoma,2 cases of osteosarcoma,1 case of malignant giant cell tumor of bone,1 case of lymphoma and 2 cases of metastatic tumor.Results The surgical time ranged from 4 to 11.5 hours (average,6.5 hours).The average blood loss was 3600 ml (range,1500 to 5500 ml).Five patients underwent type Ⅰ resection,while 11 patients underwent type Ⅱ resection.All patients were followed up for 21 to 59 months (average,34.4 months).Three months after operation,13 patients had difficulty with ankle plantar flexion,while sphincter function was more or less reserved in 16 patients.Four patients (25%)had wound healing complications,which was cured after debridement and drainage.Local recurrence occurred in 7 patients.There were 5 cases of local recurrence among 6 patients who had intralesional margin;4 recurrence lesions located in the sacrum and 1 in soft tissue.Eight patients (50.0%) survived without tumor,2 patients (12.5%)survived with tumor,and 6 patients (37.5%) died of tumor.Conclusion The en bloc resection of tumor involving partial sacrum and ipsilateral sacroiliac joint with reservation of contralateral sacral foramina was feasible and had better outcome compared with total sacrectomy.
4.Serum anti-Jo-1 antibody level correlates with disease activity in polymyositis and dermatomyositis

ObjectiveTo evaluate the correlation between serum anti-Jo-1 antibody level and myositis disease activity in polymyositis/dermatomyositis.MethodsAnti-Jo-1 antibody levels in serum from 148 polymyositis/dermatomyositis patients and 130 healthy controls were measured by both ELISA and immunoblot assay.Disease activity of the anti-Jo-1 antibody positive patients was assessed by the global myositis disease activity score (visual analogue score,VAS) established by the International Myositis Assessment and Clinical Studies(IMACS) Group.The correlation between disease activity and level of serum antiJo-1 antibody was assessed.Comparisons between groups were performed by x2 test or t test.ResultsThe positive rate of ELISA and EUROLINE was 24.3%(36/148) and 27.0%(40/148) respectively.Fever,ILD,arthritis/arthralgia were found to be more predominant in anti-Jo-1 antibody positive patients than those who were anti-Jo-1 antibody negative patients.There was significant positive correlation between the global myositis disease activity score (VAS) and serum level of anti-Jo-1 antibody (r=0.874,P=9.000).Serum antiJo-1 antibody levels,together with global disease activity,were significantly decreased in the 7 patients after treatment.ConclusionAnti-Jo-1 antibody level measured by ELISA is associated with disease activity of PM/DM,and could be a marker of disease activity.
5.The expression of major histocompatibility complex in idiopathic inflammatory myopathies and its clinical application

Objective To study the expression of major histocompatibility complex(MHC)on muscle biopsy specimens of idiopathic inflammatory myopathies(IIM),and assess diagnostic value of MHC in IIM. Methods Forty-five patients with IIM(19 polymyositis and 26 dermatomyositis)were selected for this study.Thirty healthy subjects were included as controls.Immunohistochemical staining was applied to identify the expression of HLA-A/B/C and HLA-DR on muscle biopsy-specimens in polymyositis (PM)/deFmatomyosiris (DM) patients and healthy controls.Results HIA-A/B/C antigens were expressed in muscle fibers in 18 out of 19 PM patients(95%),24 out of 26 DM patients(92%) and 3 out 0f 30 healthy controls(10%) respectively.The positive expression rate of HLA-DR in PM,DM patients and healthy controls were 84%, 81%,13%,respectively.The expressions of HLA-A/B/C and HLA-DR were significantly increased in PM and DM patients than those in healthy controls(both P<0.05),but no significant differences were found between PM and DM groups(both P>0.05).No significant correlations were demonstrated between the over-expression of HLA-A/B/C or HLA-DR and the extent of inflammatory infiltrations,muscle damage or clinical features in PM/DM groups (all P values>0.05).Conclusion MHC-Ⅰ and MHC-Ⅱover-expression in muscle fibers are the early events in PM and DM,and may occur in the absence of lymphocyte infihration and muscle damage.Immunostaining for MHC-Ⅰ and MHC-Ⅱcan be used as a routine test in the diagnosis of PM and DM.
6.Effect of shikonin on proliferation of keratinocytes induced by interleukin-17 and expression of chemokines.

OBJECTIVETo observe the effect of shikonin on the proliferation of human keratinocytes induced by IL-17 and secretion of chemokines, in order to discuss the mechanism of Shikonin in the treatment of psoriasis.
METHODIn vitro cultured HaCaT cells were stimulated by IL-17A (200 μg x L(-1)) and mixed with different concentrations (2, 1 mg x L(-1)) of shikonin for 24 hours. The cell proliferation was detected by CCK-8 assay. Cell secretion inflammatory factor interleukin-23 (IL-23) was detected by ELISA. The expressions of intracellular chemokines CXCL1, CXCL2, CCL20 and 6-defensin 4 (DEFB4) were detected by Real-time PCR.
RESULTShikonin (2,1 mg x L(-1)) could distinctly inhibit HaCaT cell proliferation induced by IL-17A, with statistical difference (P < 0.01). Each shikonin group showed decreases in the secretion of IL-23 and inhibition in expressions of intracellular CXCL1, CXCL2, CCL20 and DEFB4.
CONCLUSIONShikonin could inhibit HaCaT cells proliferation induced by IL-17 and secretion of relevant cytokines and recruit leukocytes by inhibiting chemokines, so as to show the effect in treating psoriasis.

7.Chemosensitivity Test of Human Head and Neck Tumors Cell Line with MTT Assay

In vitro cell culture of head and neck tumors and chemosensitivity test to select sensitivity antitumor bogs for individual chemotherapy. Methods:The chemosensitivity of 32 fresh specimens of human head and neck tumors were tested with MTT assay and eight atitumor drugs. Results:The sensitivity was 84.4% for 5-Fu, 2l.9% for VCR, 62.5% for MTX, 8l.3% for BLM, 8l.3% for DDP, 40.6% for ADM, 65.6% for PYM and 3.l% for CTX. The results of chemotherapy with sensitivity drugs of 32 patients were encouraged. There were no serious side-effects in the chemotherapy and no recurrence and metastasis after the treatment. 32 patients were still alive. Conclusion: Chemosensitivity test of tumor cell lines with MTT assay was accuracy, fast,simple,ecoromical,effective method and was valuable for chemotherapy.
8.Study on effect of oligochitosan in promoting intestinal absorption of protoberberine alkaloids in extracts from Corydalis saxicola total alkaloids.

OBJECTIVETo investigate the effect of oligochitosan in promoting intestinal absorption of protoberberine alkaloids in extracts from Corydalis saxicola total alkaloids.
METHODThe in vitro single-pass intestinal perfusion model in rats was established to study the changes in absorption kinetic parameters of dehydrocavidine, berberine hydrochloride and palmatine chloride in C. saxicola total alkaloids after the addition of different concentrations oligochitosan and evaluate the effect of oligochitosan in promoting intestinal absorption of the drugs.
RESULTThe concentration of oligochitosan had different effects on the absorption rate constant (Ka) and apparent permeability coefficient (Peff) of the three active component in rat intestines. Ka and Peff in 0.5% oligochitosan group significantly increased, indicating a stronger effect in promoting the absorption.
CONCLUSIONOligochitosan has a certain effect in promoting the intestinal absorptions of protoberberine alkaloids in C. saxicola total alkaloids.

9.The relationship of the folate metabolism related gene polymorphisms of MTHFR and MTRR with unexplained recurrent spontaneous abortion

Objectives To investigate the relationship of methylenetetrahydrofolate reductase (MTHFR) and methioninesynthase reductase (MTRR) with unexplained recurrent spontaneous abortion (URSA). Methods Case control study was used to select 244 patients with URSA (miscarriage group) and 116 normal women (control group) who were admitted to Tianjin Medical University General Hospital and Tianjin Women’s and Children’s Health Center from January 2013 to March 2015. The oral mucosal epithelial cells were extracted using fluorescence quantitative PCR to detect MTHFR gene C677T, A1298C and MTRR gene loci of A66G single nucleotide polymorphisms (SNP). The relationship between folate metabolism related gene polymorphisms of MTHFR and MTRR and URSA was analysed. Results The frequency of C677T genotype MTHFR was significantly higher in URSA group than that in the control group, and the frequency of CT genotype was significantly lower than that of the control group (P<0.05). There was no significant difference in the frequencies of A1298C MTRR and A66G MTHFR between the two groups. The activity of MTHFR, red cell folate and plasma folate levels were significantly lower in URSA group than those of control group. Homocysteine levels were significantly higher in URSA group than those of control group (P<0.05). There were no significant differences in serum folic acid, red cell folate, homocysteine cysteine levels between patients <35 years old and ≥ 35 years old in URSA group. Conclusion C677TMTHFR gene polymorphism is associated with unexplained recurrent spontaneous abortion.
