1.Fungous Sinusitis and CT of Diagnosis

Objective To analyze the clinical symptoms and CT findings of fungus sinusitis in order to evaluate CT diagnosis. Methods To analyze retrospectively the collected data of 12 patients who underwent operations for fungos sinusitis which was proved pathologically. Results The main clinical feature of fungos sinusitis was bloody supperative rhinorrhea in all cases. In the CT scans, we found the density of the involved sinus cavities rised inhomogeneities in all patients. CT scans also showed spot-like calcification in 7 cases, bony proliferation in 5 cases and bony absorption in 2 cases. The operation revealed a lot of filthy or inflammatory polypoid substances in the involved sinus cavities in all patients. Pathological examination demonstrated clusters of fungus in the filthy substances. Conclusion The diagnosis of fungus sinusitis mainly derived from clinical symptoms and radiological examinations. The final diagnosis depended on pathology. It is of great value for reference that CT scans can show the anatomical structures of the sinuses and help to diagnose fungus sinusitis.
2.Research Advancement of TOF-MS for Analysising Protein Components in Patients with Diabetic Nephropathy Urine

TOF-MS has been more and more widely used in clinical and scientific research due to its advantages such as fast and easily operated.Diabetic nephropathy is the most common complications of diabetes.But clinical existing detection meth-ods to some extent have some hysteresis.The applications and developments of TOF-MS coupled with other proteomics technologies provided new ideas and methods for early diagnosis and prognosis of diabetic nephropathy.In this paper,the lat-est research results in nearest years about the application of TOF-MS to diabetic nephropathy will be reviewed.
3.The effects of Ligustrazine on the expression of bFGF and bFGFR in bone marrow in radiation injured mice.

To study the expression of the bFGF and its receptor in the mouse bone marrow by treatment with acute radioactive injury and Ligustrazine, 56 mice were divided into 3 groups: normal group, radiation-injured group and Ligustrazine group. After irradiation by 6.0 Gy 60Co gamma-ray, each mouse was orally given 0.1 ml Ligustrazine twice a day for 13 days in Ligustrazine group, and each mouse in radiation injured group was orally given equal amount of saline. On the 3rd, 7th, 14th day after irradiation, bone marrow mono-nuclear cells (BMMNC) were counted, and the expression levels of bPGF and bFGFR in bone marrow were evaluated by immunohistochemistry and flow cytometry analysis respectively. On the 3rd, 7th, 14th day after irradiation, expression of bFGF in bone marrow were significantly lower than in normal group (P<0.05 or P<0.01). Expressions of bFGF and bFGFR were much higher in Ligustrazine treated group than that in the control group (P<0.05 or P<0.01). Ligustrazine potentiate the expression of bFGF and bFGFR in bone marrow MNC to recover the bone marrow hematopoiesis inductive microenvironment, which is one of the mechanisms by which Ligustrazine rebuild the bone marrow hematopoiesis after acute radioactive injury.

4.Effect of Xiexin Decoction on early diabetic nephropathy in diabetic rats

Objective To investigate the effect of Xiexin Decoction(XXD) on the early diabetic nephropathy(DN) of diabetic rats induced by high-fat diet with injection of streptozotocin(STZ).MethodsMale Sprague-Dawley rats were divided into two groups and fed with normal pellet diet(NPD) or high-fat diet(HFD),respectively,for a period of four weeks and then HFD-fed rats were ip injected with STZ.The diabetic rats were divided into three groups: model group,XXD group,and metformin group.After 13-weeks ig administration,fasting blood glucose(FBG),glycated hemoglobin(HbAlc),blood lipids(TG and TC),serum insulin(INS),renal function,kidney index,albumin in urine,and the renal histology and ultrastructure were observed.Results Compared with the model group,XXD reduced the levels of water intake,food consumption,urine volume,HbAlc,insulin resistance index(IRI),creatinine clearance rate(CCr),albumin in urine and blood lipids(P
5.The combination of trastuzumab and lapatinib added to neoadjuvant chemotherapy for breast cancer:a meta-analysis of randomized evidence

Objective To compare the efficacy and safety of trastuzumab versus the combination of tras-tuzumab and lapatinib added to neoadjuvant chemotherapy in HER2-positive breast cancer. Methods We searched PubMed,MEDLINE,The Cochrane Library,Web of Science,CNKI,Wanfang datebase and the abstracts of major international conferences in recent 5 years to identify randomized controlled trials which met the inclusion and exclusion criteria. Study selection and analyses were undertaken according to the Cochrane Hand-book. Meta-analysis was performed using RevMan 5. 0 software. Results Four trials were identified with 779 eli-gible patients. The results of meta-analyses showed that the rate of pathological complete response was significant-ly higher in the group receiving rastuzumab and tlapatinib than that in the group with trastuzumab alone(53. 3%vs 38. 8% ,RR =1. 39,95 % CI:1. 20-1. 63;P < 0. 001). No statistical differences were observed in regarding adverse events among patients receiving trastuzumab or the combination of trastuzumab and lapatinib,except the grade Ⅲ-Ⅳ diarrhea(2. 2% vs 25. 6% ,RR =11. 54,95% CI:5. 69-23. 41;P <0. 001). Conclusion The com-bination of trastuzumab and lapatinib added to neoadjuvant chemotherapy in HER2-positive breast cancer is more effective,without more adverse reactions except diarrhea;it ia an effective and safe treatment.
6.Application of one-stage hybrid technique in treatment of pernicious placenta previa

Objective To assess the value of one-stage hybrid technique for treatment of pernicious placenta previa. Methods 12 patients with pernicious placenta previa who had received simultaneously Cesarean section and temporary balloon occlusion of abdominal aorta or internal iliac artery in the hybrid operation room were included in this study. 12 patients with pernicious placenta previa who had preserved balloon ducts in internal iliac artery before Cesarean section were chosen as a control group. Balloons were filled to control hemorrhage during the procedure. The hemodynamic parameters including blood pressure and heart rate were monitored during the opera-tion. Surgical duration ,amount of blood loss ,amount of blood transfusion ,volume of infusion ,urine volume during the procedure,postoperative volume of blood loss within 24 hours,uterine hysterectomy rate and neonatal conditions were compared between the two groups. Results During the operation,the blood pressure and heart rate in hybrid group were more stable. The mean surgical duration was(72 ± 8)min,the intraoperative mean amount of blood loss was(620 ± 95)mL,the mean amount of blood transfusion was(550 ± 40)mL,the mean volume of infusion was(1850 ± 160)mL,the mean amount of blood loss in 24 h after the operation was(75 ± 9) mL in the hybrid group,which were significantly lower than those in the control group(P<0.05 for all comparisons). No serious complications occurred in any of the two groups after the operation. There were no significant differences in neonatal conditions between the two groups. Conclusions One-stage hybrid technique has an obvious effect in the control of intraoperative bleeding in patients with pernicious placenta previa. It is worth popularizing in the hospital whose conditions are permitted.
7.Interference of OX40 gene expression induced by small interfering RNA

Objective:To investigate the specific interference of OX40 gene expression induced by RNAi technique in 293T cell lines transfected with rat OX40 gene.Methods:293T cells were transfected with recombined plasmid pEGFP-N1-GFP/OX40,and the positive cell clones were selected by fluorescence protein observation and RT-PCR.One specific dicer siRNA targeted to OX40 mRNA was designed and synthesized,which shared no homology with exons of known human gene.Quantitative real-time PCR was performed to measure the inhibitory rate of target gene expression by comparing OX40 mRNA concentrations before and after siRNA transfection.Results:10 nmol/L siRNA-OX40 elicited the highest level of gene silence in 293T cells which was transfected with siRNA after 48 h (68.3?8.7)%);The time of maximal inhibitory effect was at 48-72 h [(61.7?8.4)%,(39.6?5.6)%].Conclusion:The exogenous OX40 expression can be significantly inhibited by treatment with specific siRNA in a dose and time -dependent manner in 293T cells,which may provide a useful profile for further investigation of inhibition of OX40 protein,and a promising control approach for preventing immune reaction.
8.Orthotopic liver transplantation in the treatment of hepatic cancer:a report of 70 cases

Objective To summarize the clinical experience of liver transplantation in the treatment of hepatocellular carcinoma(HCC).Methods From June 2004 to March 2007,70 consecutive HCC patients underwent liver transplantation in our hospital,including classic orthotopic liver transplantation in 41 cases,and piggyback liver transplantation in 29 cases.All data of patients were retrospectively analyzed.Results All liver transplantation were successfully conducted.The average warm ischemia duration was 4.5 minutes,and cold ischemia duration was 8 hours.There were 3 cases of postoperative deaths.Surgical complications were intra-abdominal hemorrhage in 2 cases,and biliary anastomotic stricture in 4 cases.Sixty-seven cases were followed up for 12-33(average 21) months,10 cases had recurrence of liver cancer after transplatation and 1 dead.Conclusions Liver transplatation can used on selected patients with HCC.Successful liver transplantation relies on good quality of liver graft,and idealized technique of vascular and bile duct reconstruction are key factors of liver transplantation.Proper postoperative management can effectively reduce the complications of operation.
9.CT features of giant lymph node hyperplasia in abdomen

Objective To describe the CT features of giant lymph node hyperplasia in abdomen.Methods A retrospective study of CT features of giant lymph node hyperplasia in abdomen confirmed by surgery or biopsy pathology was performed.CT findings of all lesions were assessed by two radiologists including size,location,pattern of enhancement and density characteristics.Results 1 2 patients with giant lymph node hyperplasia in abdomen were confirmed including 7 women and 5 men with age range from 11 to 75 years (median age 3 9 years).All patients underwent pre-and post-contrast enhanced CT.CT showed a single mass in 9 patients lo-cated at retroperitoneum in 5,porta hepatic in 2,and mesentery in 2.Multiple masses were located at chest,abdomen and neck in 3.The lesions ranged in size from 1.2 to 11.9 cm in maximum diameter with an average size of 3.7cm.CT showed all lesions with well-defined margin and regular shape.The lesions less than 5 cm in diameter usually showed homogeneous enhancement,and how-ever those more than 5 cm showed heterogeneous enhancement.The calcification was seen in two patients.Conclusion CT features of giant lymph node hyperplasia in abdomen are characteristic.
10.Contribution of IL-17 to mouse hepatitis virus strain 3-induced acute liver failure.

Recently, the Th17 cells and IL-17 have been shown to play a critical role in the immune-mediated liver injury in hepatitis B, while their functions in acute liver failure have not been well elucidated yet. In this study, we primarily investigated the role of IL-17 in the development of mouse hepatitis virus strain 3 (MHV-3)-induced acute liver failure. IL-17 mRNA levels in liver tissue were quantified by using quantitative real-time polymerase chain reaction, and cytokine IL-17 levels in liver tissue and serum were determined by using ELISA in MHV-3-induced murine fulminant hepatitis model. The IL-17 expression levels on CD4(+)T and CD8(+)T cells were determined by using flow cytometry. The correlation between IL-17 level and liver injury was studied. Th17 associated cytokines were also investigated by intracellular staining. Our results showed that the IL-17 expression was significantly elevated in the liver and serum of BALB/cJ mice infected with MHV-3. Moreover, a time course study showed that the percentage of both IL-17-producing CD4(+)T cells and IL-17-producing CD8(+)T cells was increased remarkably in the liver starting from 48 h and peaked at 72 h post-infection. There was a close correlation between hepatic or serum IL-17 concentration and the severity of liver injury defined by ALT level, respectively. Th17 associated cytokines, IL-6, IL-21 and IL-22, were also increased significantly at 72 h post-infection. It was concluded that IL-17 may contribute to the pathogenesis of MHV-3-induced acute liver failure.
