Objective To leam how the doctors and patients perceive whether the medical examinations made are rational and to use this finding as a reference for medical examinations provision. Methods By way of convenience sampling and individual interviews, 58 medical staff and 329 patients were surveyed to find their perception of the contents and meaning of rational medical examinations, the factors affecting choices of medical examinations, and the causes of unreasonable medical examinations. Results Differences of perception by medical staff from different departments and academic titles are not statically significant as P>0. 05; a huge gap exists between the perception of outpatients and inpatients, as well as between patients of different genders. The most obvious gap is found between doctors and patients, as P<0. 05; Conclusions Patients ought to be the one to decide whether the medical examinations provided to them are rational, and their rights and interests ought to be the key criterion for the rationality evaluation of medical examinations. Objective evaluation and correct coping style to the different perceptions of medical examinations by doctors and patients will guarantee reasonable choices of medical examinations.

To study the effect of different exercise intensities on cardiac endocrine function, with radioimmunoassay, immunohistochemistry and the technique of computer image analysis, the level of plasma ANP and the expression of ANP in myocardial cells of rats with different training intensities were investigated. The result showed that with the increasing of exercise intensity,the level of ANP in plasma increased progressively, the expression of ANP in myocardial cells increased remarkably after moderate and heavy exercise training while it decreased remarkably after exhaustive exercise training. Results suggested that different exercise intensities revealed different effects on adaptation of cardiac endocrine function.

Ischemic stroke is the major cause of disability and death.Although autophagy is widely involved in the process of acute brain ischemia,the exact role of autophagy is still unclear.As autophagy is a double-edged sword,its protect or damage impact mainly depends on the degree of phagocytosis.In general,moderate phagocytosis can effectively remove excess metabolites,aging organelles,etc;whereas the excessive autophagy is vulnerable to autophagy-like cell death by virture of the normal intracellular organelles and materials degraded by lysosome.In conclusion,this review focuses on the following two points:1.What's the role of autophagy in the ischemic brain injury——protective or damage 2.How does autophagy interact with apoptosis and necrosis.

Objective To research the content changes of Aristolochic Acid-A from Caulis Aristolochiae Manshuriensis and its processed products.Method Chromatographic assay was performed on Lichrospher-C18 column (4.6 mm?200 mm,5 ?m) with methanol-0.1% glacial acetic acid solution (70∶30) as mobile phase,the flow rate was 1.0 mL/min and the detection wavelength was set at 310 nm,the column temperature was 30 ℃.Result The content of Aristolochic Acid-A was lower in three processed products than in crude drugs,and the reduction rate of the one which was boiled by NaHCO3 was the highest.Conclusion The three processed method can reduce the content of Aristolochic Acid-A,and achieve the aim of reducing the toxicity.

Rare diseases, also known as "orphan diseases", refer to diseases with very low incidence. Countries and regions define rare diseases according to epidemiological standards, economic standards of rare drugs and disease severity. The World Health Organization (WHO) has suggested the prevalence rate of less than 6.5 to 10 per 10 000 people to define rare diseases. In May 2018, "China's First List of Rare Diseases" was released, including 121 rare diseases. Most rare diseases are hereditary diseases with early onset, severe disease, and poor prognosis. About 75% of rare genetic diseases occur in the neonatal period or childhood, which are important part of human birth defects and brings a huge burden to society and families. The effective prevention and treatment of rare diseases is one of the important goals of building a "Healthy China". With the development of molecular biology technology and the continuous research and development of advanced medical products in the field of gene therapy, the level of clinical diagnosis and treatment of rare diseases has risen to a new level, which provides a possibility for the cure of some rare diseases. In China, most rare diseases rely on imported drugs, which cost a lot and bring heavy economic burden to patients. Improving the medical insurance system for rare diseases has become a difficult point in the current medical reform. This paper mainly discusses the definition of rare diseases, the research status, efforts and future development direction of rare diseases in China, in order to deepen the understanding and response of medical workers and the whole society to rare diseases.

Based on Chinese clinical guidance for COVID-19 pneumonia diagnosis and treatment (7^th edition), the metabolism and pharmacokinetics of drugs used in clinical treatment of COVID-19 were reviewed. The antiviral drugs include remdesivir, chloroquine/hydroxychloroquine, lopinavir/ritonavir, favipiravir, arbidol, baicalin, baicalein and forsythin. Among them, the metabolism and pharmacokinetics of arbidol, baicalin and forsythin are the research results of the author's laboratory. This article aims to provide reference for the efficacy evaluation and rational drug use of COVID-19.

Objective To investigate the effect of continuous renal replacement therapy ( CRRT) on the reg-ulation of Treg/Th17 in patients with severe sepsis,and related inflammatory factors IL-6,IL-17,IL-10,and TNF-α.Methods 60 patients with severe sepsis were randomly divided into two groups,30 cases in each group.The control group received conventional treatment,and the observation group was treated with CRRT on the basis of the control group.Flow cytometry was used to detect the levels of Tregs and Th17 cells,and IL-6,IL-10,IL-17 and TNF-αwere detected by ELISA method.At the same time,the APACHEII score,ICU length of hospital stay were observed and recorded.Results After treatment,APACHE Ⅱscore,ICU length of hospital of the observation group were lower than the control group,there were statistically significant differences(t=4.258,t=4.518,all P<0.05). The expression levels of Th17,Treg and Tregs/Th17 in the observation group were significantly lower than the control group,the differences were statistically significant (t=5.872,4.267,4.285,all P<0.05).The expression levels of IL-6,IL-17,TNF-αin the observation group decreased more significantly than the control group,the differences were statistically significant (t=5.829,5.257,5.983,all P<0.05).But the expression level of IL-10 in the two groups had no obvious change[(35.26 ±18.63) pg/mL vs (35.52 ±20.09) pg/mL,t=2.164,P>0.05].Conclusion CRRT can not only remove the inflammatory mediators of abnormal expression,improve the function of T cells,but also can maintain the balance between Th17 and Treg,improve the immune disorders,and improve the prognosis of sepsis.
