1.Research on the interactions of job embeddedness with the psychological capital,organizational commitment, and intention to stay of nurses

Objective To explore the interactions mechanism of job embeddedness of nurses with psychological capital,organizational commitment,intention to stay and their influencing factors.Methods By convenient sampling method,722 nurses were investigated with Job Embeddedness Questionnaire, Psychological Capital Questionnaire,Organizational Commitment Questionnaire and Nurses′Intention to Stay Questionnaire.Path analysis was made on relations between the variants.Results Night shift intervals pose positive influence on nurses′psychological capital,organizational commitment and intention to stay.Psychological capital can influence job embeddedness by both directly and indirectly,and can influence their intention to stay indirectly.Organizational commitment can influence job embeddedness strongly and directly (r = 0.494 ),and their intention to stay both directly and indirectly.Job embeddedness can influence their intention to stay directly (r=0.453 ).The model fitting is found desirable.Conclusions Nursing managers can enhance nurses′organizational commitment by developing the psychological capital to enhance nurses′organizational commitment effectively.Nurses with higher level of psychological capital and organization commitment will embed into their workplace more deeply, then increasing their intention to stay ultimately.

Annual salary system reform at public hospitals in Sanming was cited as an example,to touch upon the five components of equity in such a reform,namely external equity,internal equity,individual equity,procedural equity and interactive equality.The significance,roadblocks and essential conditions were then analyzed,to come up with such recommendations as development of a mechanism for remuneration survey and dynamic adjustment,and a system for better posts grading and evaluation,as well as better performance appraisal tools,and a competent performance appraisal system.
4.Appraisal of efficiency and performance of public hospitals in Sanming

Objective To evaluate the performance and efficiency of public hospitals in Sanming city,for suggestions on bettering their medical resources deployment efficiency and operational efficiency.Methods Descriptive analysis was used to longitudinally describe the general performances of such hospitals,leveraging the operating data of 21 public hospitals of the city.Meanwhile,appropriate indexes were selected to construct data envelopment analysis models for analysis of hospital operational efficiency and variations.Results Longitudinal analysis showed the operation process as stable,the efficiency as improved,and the structure as optimized thanks to the reform.Horizontal analysis found three hospitals (14% of all) as efficient in general,and seven (33%) as efficient technically.Those hospitals found as inefficient technically suffer from overinvestment or output deficiency to varying extents.Three hospitals (14%) were found with scale efficiency,while those found without scale efficiency were regarded as diminishing returns on scale.Conclusions Reforms measures must be adhered to consolidate outcomes of the ongoing reform;Regional healthcare resources planning must be consolidated to improve the efficiency of resource allocation;Delicacy management must be made to enhance service delivery capabilities.
5.Application of Liechtenstein hernioplasty in the therapy of oblique or direct hernia

Objective To inquire into the value of Liechtenstein Hernioplasty in the therapy of oblique or direct hernia.Method Liechtenstein was used in the therapy of 163 cases of oblique hernia (154 cases) and direct hernia (9 cases) from Feb.1998 to Jun.2001 and the clinical data was retrospectively studied.Most of them are of onset hernia except 8 cases of recurrent hernia.In 148 cases,the diameter of hernia ring is less than 2.5 cm and 30.1% of patients are elder than 65.The emergent operation was adopted in 12 cases.Results Off-bed exercises can be admitted at early stage of postoperation with slight pain and no local tension.Total recovery was obtained in 5-10 days after operation.After 6-36 months' follow-up,no mortality and less recurrence was detected.Conclusions There are so many advantages to treat oblique or direct hernia with Liechtenstein, such as minimized trauma, accelerated recovery,low recurrence rate and low cost. The indication of this operation involved all kinds of hernia (onset or recurrence of oblique or direct hernia,emergent operation of incarcerated oblique hernia, etc).
6.Value of pre- and postoperative red cell distribution width-to-lymphocyte count ratio in judging the prognosis of patients with non-metastatic colorectal cancer

Objective:To explore the value of pre- and postoperative red cell distribution width-to-lymphocyte count ratio (RLR) in predicting the disease-free survival (DFS) for non-metastatic colorectal cancer patients.Methods:The data of 108 patients pathologically diagnosed with non-metastatic colorectal adenocarcinoma after surgery from December 2012 to January 2020 in Hainan Hospital of PLA General Hospital were retrospectively analyzed. RLR was calculated and its differences in patients with varied clinicopathological characteristics were analyzed. The receiver operating characteristics (ROC) curve was applied to analyze the value of pre- and postoperative RLR in predicting DFS of patients, and the cut-off value was determined by Youden index. DFS of patients with different stratification stratified by a cut-off value of RLR was analyzed by using Kaplan-Meier method followed by log-rank test. Cox proportional hazards model was used to make univariate and multivariate analysis.Results:Youden index of pre- and postoperative RLR in predicting DFS was 8.86%, 9.15%; based on the above index, the patients were divided into the preoperative RLR<8.86% group (73 cases) and preoperative RLR≥8.86% group (35 cases), postoperative RLR<9.15% group (48 cases) and postoperative RLR≥9.15% group (60 cases). According to ROC curve, the area under the curve (AUC) of preoperative RLR in predicating DFS was 0.66 (95% CI 0.55-0.77, P = 0.01), and AUC of postoperative RLR in predicating DFS was 0.62 (95% CI 0.51-0.74, P = 0.04). Pre- and postoperative RLR cut-off values were used to predict 3-year DFS rate of patients with the sensitivity of 51.60% and 71.00%, respectively, and the specificity of 76.60% and 50.60%, respectively. Kaplan-Meier analysis showed that patients in preoperative RLR<8.86% group had a better DFS compared with those in preoperative RLR≥8.86% group ( χ2 = 7.35, P < 0.01); there was no statistical difference in DFS between postoperative RLR<9.15% group and postoperative RLR≥9.15% group ( χ2 = 3.69, P = 0.06). Preoperative RLR was an independent affecting factor for DFS of patients ( HR = 1.13, 95% CI 1.04-1.22, P <0.01). Conclusions:Pre- and postoperative RLR could be useful in predicating 3-year DFS for patients with non-metastatic colorectal cancer. Preoperative RLR is an independent affecting factor for DFS, and the patients with low RLR have a better prognosis.
7.The effect of curcumin on the hyperoxia-induced acute lung injury in neonatal rat

Objective To investigate the effect of curcumin on the hyperoxia-induced acute lung injury in neonatal rat.Methods A model of hyperoxia acute lung injury in neonatal rat was established by exposure to 95% O2 for 3 days.After the model was established,40 rats were randomly divided into the treated group ( n =20) and the control group ( n =20).The 20 injured rats which were treated by curcumin ( 100 mg/( kg· d)were treated group,and the other 20 injured rats were control group.The 20 normal rats were blank group ( n =20).Lung tissues were collected for doing pathological section and measuring the levels of tumor necrosis factor(TNF)-a,interleukin(IL)-1β and myeloperoxidase(MPO) after 4 d and 7 d.Results The gross and micrographic injury of lung was milder in treated group than that in control group.The levels of TNF-α,IL-1β and MPO in treated group were significantly lower than those in control group at 4 d and 7 d ( P <0.05 ).Compared with the blank group,the levels of TNF-α,IL-1β and MPO in control group increased significantly at 4 d and 7 d ( P < 0.01 ).Conclusion Curcumin can offer effective protection against acute lung injury induced by hyperoxia in neonatal rat.
8.Preliminary study on pharmacodynamic evaluation method of Houpo formula particles.

To discuss the feasibility of the pharmacodynamic evaluation method for traditional Chinese medicine (TCM) formula particles, with traditional decoction for reference and the intervention of Magnoliae Officinalis Cortex in rats with ulcerative colitis (UC). First of all, the similarity of traditional Magnoliae Officinalis Cortex decoction and formula particles of different manufacturers was defined by using the IR fingerprint. The UC rat model was established and given Houpo formula particles of different doses and manufacturers, with the decoction for reference, in order to observe disease activity index (DAI), colon mucosa damage index (CMDI), pathologic changes, nitric oxide (NO), endothdin (ET), substance P, vasoactive intestinal peptide (VIP). Their intervention effects on UC rats were compared to study the difference between Sanjiu and Tianjiang Houpo formula particles, in order to demonstrate the feasibility of the pharmacodynamic evaluation method for Houpo formula particles. According to the results, Houpo formula particles showed similar pharmacodynamic actions with the traditional decoction. The pharmacodynamic comparison of Houpo formula particles of different manufacturers showed no statistical significance. The experiment showed that on the basis of the TCM compounds, a prescription dismantlement study was conducted to define target points of various drugs. The traditional decoction was selected for reference in the comparison of corresponding formula particles for their pharmacodynamic equivalence. This method could avoid controversies about single or combined boiling of formula particles, and give objective comments on the pharmacodynamic effect of the formula particles. The method is proved to be feasible.

9.Practice and thinking of LBL combined with PBL in teaching cancer pain standardization training for residents

Resident doctor standardization training program is the necessary and important part of medical education after graduation.The Hainan branch of PLA General Hospital combines scientific and practical medical conditions and discipline characteristics of Hainan to scientifically plan the standardized treatment of cancer pain in training.This model combines lecture-based learning (LBL) teaching method and virtual demonstration for theoretical training.In practical training,we conduct problem-based learning (PBL) teaching method,situational teaching and standardized examination to systemically improve resident doctors' skill for cancer pain management.This combined teaching strategy can help resident doctors to establish correct and standard concept in future clinical practice,and enable them to establish a correct concept of treatment,and adopt two-way evaluation to continuously improve teaching quality before the end of training.
