2.Clinical significance of the anticardiolipin antibodies in patients with hemorrhagic fever with renal syn-drome

Objective To investigate the clinical significance of anticardiolipin antibodies (ACA)in patients with hemorrhagic fever with renal syndrome(HFRS).Methods The serum ACA- IgM and ACA-IgG in 175 cases of renal diseases and 50 healthy cases were detected by enzyme-linked immunosorbent assay(ELISA). The positive rates of ACA were calculated and compared among different groups.Results The positive rates of ACA-IgM and ACA-IgG in control group were both 0 (0/50),while they were 85.2%(46/54)and 24.1%(13/54)respectively in patients with HERS. 25.0%(13/52)and 21.2%(11/52)respectively in patients with lupus nephritis and 13.0%(9/69) and 8.7%(6/69)respectively in patients with nephrotic syndrome.The poshive rate of ACA lgM in patients with HFRS was higher than that in control group,lupus nephritis group and nephritic syn- drome group(P

Objective To explore the vascular endothelial function in individuals with normal glucose tolerance (NGT),impaired glucose tolerance (IGT),and type 2 diabetes mellitus,and to study the role played by PPARγcoactivator 1 α (PGC-1 α) in diabetic macrovascular function.Methods A total of 94 subjects with NGT,87with IGT,and 306 with type 2 diabetes mellitus were recruited in the study.Carotid flow-mediated vasodilatation (FMD),glyceryl trinitrate dilatation (GTN),and carotid intima-media thickness (CIMT) were assessed by highresolution B-mode color vascular Doppler instrument.Finally,correlation analysis was conducted.NO,Akt-Ser473phosphorylation levels,IL-β,and PGC-1 α mRNA expression levels were measured by means of nitrate reductase,western blot,and reverse transcriptase real-time fluorescence quantitative PCR assay methods in peripheral blood mononuclear cells respectively.Results FMD in newly-diagnosed type 2 diabetic group was significantly lower than that in NGT and IGT groups(P<0.01).GTN and CIMT showed no difference.PGC-1 α mRNA transcription level in diabetic group was significantly lower than that in non-diabetic groups (P<0.01).Correlation analysis showed that PGC-1 α was positively correlated with FMD and GTN (P<0.05 or P<0.01).Logistic regression analysis revealed that PGC-1 α was independently associated with FMD (P < 0.01) after adjusting other influencing factors.Conclusion FMD is a more sensitive early warning indicator of endothelial function than CIMT and GTN.As indices of atherosclerosis,CIMT and GTN are more or less focused on the progression of macrovascular complications.PGC-1 α is a protective factor of macro-vasculature in diabetes mellitus,and its effect may be achieved through the protection of the vascular endothelium.
4.Clinical characteristics and SLC12A3 gene mutation profile of Gitelman syndrome: One case report

Objective To describe the clinical characteristics of a patient with Gitelman syndrome,and to identify the associated SLC12A3 gene mutations.Methods A suspected case of teenager-onset Gitelman syndrome was observed in our hospital.It was further confirmed by clinical manifestations and auxiliary examination.In addition,direct sequencing for the exons of SLC12A3 gene and CLCNKB gene region was conducted to identify the probable disease-associated mutations.Results The case showed characteristics of hypokalemia,hypomagnesemia,and low level of urinary calcium and onset by age of 18.By excluding the possibilities of long-term use of thiazide diuretics,laxatives,chronic vomiting and diarrhea,he was finally diagnosed as a case of Gitelman syndrome.Furthermore,by Sanger direct sequencing,2 coding variations were identified in SLC12A3 gene region,including T304M and L488P.L488P was a new heterozygous mutation.Conclusion Detection of SLC12A3 gene mutation could facilitate the diagnosis of Gitelman syndrome and improve prognosis.

Aim The effect of tetrandrine on TGF-?1 mRNA expression in glomerulosclerosis rat was observed. Methods The rats were randomly divided into four groups, such as the normal control group (sham operative rat), glomerulosclerosis model group,tetrandrine group and amlodipine group. The expression of TGF-?1 mRNA was analyzed by Northern blot hybridization. Results The expressions of TGF-?1 mRNA in two treating groups were much lower than untreated model group. There were no difference between these two treating groups. Conclusion Tetrandrine can decrease the expression of TGF-?1 mRNA in glomerulosclerosis rat induced by unilateral renctomy plus adriamycin.
6.Fluorous Solid-Phase Extraction Assisted New Methods for Analysis of Biomolecules with Mass Spectrometry

Fluorous solid-phase extraction ( FSPE ) is a solid-phase extraction technique based on fluorous affinity between perfluorous compounds. It requires derivatization on analytes with fluorous tags and further specific separation accomplished by perfluorinated solid phase. This technique has extended to various research fields with broad application including organic synthesize, catalysis, chemical and biological analysis. Recently, owing to its good compatibility with mass spectrometry, new analytic techniques relying on FSPE coupled to biological mass spectrometry have received wide attention. This review briefly introduced the principle of FSPE and emphasized on its application for the analysis of biomolecules with mass spectrometry, as well as its outlook of future development.

Chronic kidney disease (CKD) is becoming a global social problem. It is important to slow down the progression of CKD for economic and social concerns. In recent years, it has been found that colon is one of the vital organs which produce uremic toxins. And enterogenous uremic toxins are closely related to the prognosis of CKD. Theory of gut-kidney axis for the slowdown of CKD progression was raised by foreign scholars and became the research hot spot. Colon therapy with traditional Chinese medicine (TCM) has been widely used in clinical practice and is believed to slow down the progression of CKD by numerous clinical reports. However, low re-search quality and ambiguous results limited its further application. Under the guidance of senior TCM Professor Huang Chunlin, who emphasized the method of draining turbidity through bowels in the management of CKD, from the Nephrology Center, Guangdong Provincial Hospital of Chinese Medicine, as well as the modern theory of gut-kidney axis, we had carried out a series of exploratory researches which will provide data and methodology support for further confirmatory studies and improve its effectiveness.
8.Research on the aetiology of suspected enterovirus infected patients in Xuzhou district in 2009

Objective To understand and master the situation in which enteroviros caused handfoot-and-mouth disease(HFMD) in Xuzhou district in 2009 so as to provide scientific basis for the control and prevention of hand-foot-and-mouth disease. Methods The researchers adopted fluorescence RT-PCR method to detect EV and EV71 as well as the CAI6 specificity RNA from 222 samples of anal swabs and oropharyngeal swabs from the 240 cases who were diagnosed clinically as hand-foot-mouth disease infected by enterovirus. Also, the researchers conducted EV71-IgM antibody detection on 114 samples of acute phase serum with ELA method. Results Among the 240 enterovirus infected patients, the total EV infection rate is 72.50% ( 174/240), among which EV71 infection rate is 57. 92% ( 139/240), CoxA16 infection rate is 9. 17% (22/240), and other EV infection rate is 5.42% (13/240). The EV71-RNA positive rate of the samples of 222 anus swabs among the 240 suspected enterovirus infected patients is 45.94% ( 102/222 ),the samples of swallow swab EV71-RNA positive rate is 25.68% (57/222) and the EV71-IgM antibody positive rate of 114 samples of acute phase serum is 86. 84% (99/114). The EV71-RNA positive rate of oropharyngeal swabs of 254 healthy children is 1.57% (4/254), and no CoxA16-RNA was detected. In the oropharyngeal swabs of 54 close contacts (medical personnel), the EV-RNA detected is negative. The positive rate of EV71-lgM antibody of the 258 healthy children's serum samples is 2.71% (7/258).Conclusion The widespreading of hand-foot-mouth disease in Xuzhou district is caused mainly by type 71 enterovirus. Inapparent infection of type 71 enterovirus exists among children under the age of 3 during the time of widespreading period and IgM antibody develops in them. It is difficult for adults to be infected by EV71 even if they contact the contagion source closely. The positive rate of EV71-IgM antibody in the samples of acute phase serum of suspected cases is the highest ( 86.84% ), and the second highest is the positive rate of RNA of EV71 of anal swabs (45.94%) and of the EV71 of oropharyngeal swabs (25.68%). ELA reagent kit is used in the early diagnosis of EV71 infection for it is easy to operate, fast and economic, so, it is worth popularizing in the grass-root medical units.
9.Analysis on pathogen detection for 53 influenza outbreaks

Objective In order to provide a scientific basis for influenza prevention and control,analyzing the epidemic characteristics and laws of influenza outbreaks in Xuzhou area during 2005-2011.Method Using fluorescent-PCR method to detect influenza virus nucleic acid on Nasopharyngeal swab specimens collected from influenza outbreak cases during 2005-2011 and fast classifying influenza virus A1 (H1N1),A3 (H3N2),new H1N1 BV (Victoria) and BY (Yamagate) on subtypes.At the same time,isolating the influenza virus with MDCK cells,and sending them to the National Influenza Center for review,after the preliminary identification of the isolated influenza virus.Results During 2005-2011,there are 53 influenza outbreaks in Xuzhou area,which caused by influenza virus subtype BV accounting for 26.42% (14/53),A3 accounting for 49.1% (26/53),A3 and A 1 mixture accounting for 3.77% (2/53) and the new H1N1 accounting for 20.75% (11/53).The outbreaks in 2007 and 2009 mainly caused by A3,and show that the winter spring (January) and summer autumn (September) as two popular peaks during 2005-2011 ; BV mainly causes the outbreaks from Feb.to Jun.Conclusion In Xuzhou area,since the winter of 2005,influenza virus subtype BV,the A3,and new H1N1 has alternately as mainly predominant strain,caused local influenza outbreaks.In which BV has increased trend year by year during 2005-2011.The students in primary and secondary schools are the major crowd of influenza outbreaks.Fluorescent-PCR detection methods could be a preferred method for reliable and rapid diagnostic of epidemic influenza outbreaks.
10.2-Methyithio-adenosine-5'-triphosphate inhibits ventricular arrhythmogenesis in rabbits with chronic heart failure

Objective To investigate the effects and related mechanisms of 2-methylthio-adenosine-5'-triphosphate (2-MeSATP),an important extracellular agonist that activates receptors for purine nucleotides (P2XR),on ventricular arrhythmias in rabbits with chronic heart failure (CHF).Methods The male New Zealand rabbits were divide into control (n =12),CHF (n =12) and CHF + 2-MeSATP groups(2-MeSATP,n =12).CHF was induced by isoproterenol injection (0.3 mg· kg-1 · d-1 for 3 weeks) and rabbits were observed 6 months later.The main cardioelectrophysiological parameters and ventricular arrhythmias were tested by recording monophasic action potential (MAP) with burst-pacing (BCL) in rabbits in vivo.The transient outward potassium current (Ito) was recorded via whole-cell patch clamp technique and the fluorescence intensity of intracellular free Ca2+ was detected with Flup-3/AM loading by the laser scanning confocal microscope in enzymatically dissociated single rabbet ventricular myocytes.Results CHF rabbits developed severely clinical CHF signs and symptoms,reduced left ventricular ejection fraction and fractional shortening as well as enlarged end-diastolic dimension.Compared with CHF group,APA and MaxdV/dt were significantly increased,while APD20,APD50 and APD90 were significantly reduced in 2-MeSATP group (all P < 0.01).Moreover,2-MeSATP could obviously shorten BCL induced ventricular arrhythmias,and decrease deducibility and persistence time of ventricular arrhythmias with burst-pacing in 2-MeSATP group in vivo (all P < 0.05).With voltage clamp model,2-MeSATP could significantly increase the current density of Ito in different command potential in CHF ventricular myocytes (all P < 0.01).When holding potential was set at-50 mV and command potential was set at + 50 mV,the current densities of Ito increase was more significant in 2-MeSATP group than that in CHF group((11.79 ± 4.51) pA/pF vs.(7.94 ± 3.53) pA/pF,P < 0.01).2-MeSATP could completely change the Ⅰ-Ⅴ curve upward without changing the Ⅰ-Ⅴ curve direction in CHF ventricular myocytes.The fluorescence intensities of intracellular free Ca2+ increase was more significant in 2-MeSATP group compared to CHF group ((1 291.98 ± 123.31) μmol/L vs.(793.59 ± 114.65) μmol/L,P < 0.01).Conclusion 2-MeSATP as a potent agonist acting on P2XR could significantly shorten APD,increase heart rate and improve cardiac performance as well as decrease the susceptibility of ventricular arrhythmias in this rabbit CHF model.Our results suggest that Ito increase and sarcoplasmic reticulum uptake Ca2+ enhancement as well as dynamic balance of intracellular Ca2+ cycling sustenance might linked to the beneficial effects of 2-MeSATP in this CHF model.
