The primary influencing factors to NDS score 1, in decreasing order of impact were volume of infarction, position of infarction, hs-CRP and FBS. While the primary influencing factors to NDS score 3 at the end of 1 month treatment, in decreasing order of impact, were NDS score 1, size of infarction, FG, hs-CRP and TC. Conclusions Volume of infarction and hs-CRP are very important determining factors in both NDS score 1 at the beginning of the treatment and NDS score 3 at the end of 1 month treatment. However, in determining NDS score 3 at the end of 1 month treatment, the impact of FG is greater than hs-CRP. The other influencing factors include FBS, TC and so on. Therefore we conclude that it is helpful to recover from neurologic deficit through lowing these influencing factors directly by medication.

Objective: Medical waste management is not only the problem of hospital, but also is an important public health problem. Apply modern information technology to insure real-time tracking and monitoring medical waste recycling, storage, disposal, improve medical waste management. Methods: Construct medical waste management system with wireless network, bar code and RFID etc. Results:By this system, transportation, storage and disposal that process of medical waste within the hospital are monitoring and supervision effective. Conclusion:The collection and transportation sectors of medical waste have been standard, to ensure the security, enhanced transparency and improved hospital management.
3.Status of Traffic Pollution in Main Crossroads of Changsha

Objective To understand the traffic pollution in main crossroads of Changsha. Methods The levels of noise, NO 2, O 3 and TSP and traffic volumn were monitored in 6 main crossroads of Changsha in situ, 3 times per day, continuously for 5 days. Results The traffic volumns were 1 203~3 715/h, the noise levels were 69~88 dB(A), the levels of NO 2, O 3 and TSP in air of 6 main crossroads were 0.092~0.238 mg/m 3, 0.238~0.303 mg/m 3 and 0.71~1.47 mg/m 3 respectively. The over standard rates were 93%~100% for O 3 with a average rate of 98%. Conclusion The traffic pollution in main crossroads of Changsha mainly induced by noise and O 3, which might be associated with the traffic volumn, the environment around the crossroads and the types of the automobile running in the crossroads.

Inflammatory bowel disease (IBD) is a group of intestinal chronic non-specific inflammatory disease with unknown etiology, and includes ulcerative colitis (UC) and Crohn''s disease (CD).Studies have indicated that environmental factor, genetic factor, disorder of immunity are associated with the pathogenesis of IBD.More and more basic and clinical studies focus on treatment with helminths in IBD.The accumulated evidences have shown that helminths infection has beneficial effects on the alleviation of IBD and is relatively safe.This article reviewed the advances in study on helminths in the treatment of IBD.
6.Effects of inhaled nitric oxide on intrapulmonarynitric oxide production and expression of plasminogen activator inhibitor-1 in lipopolysaccharide-induced acute lung injury

Objective To explore the effects of inhaled nitric oxide(NO) on expression of plasminogen activator inhibitor-1 (PAI-1) mRNA and protein in the early-stage of experimental acute lung injury (ALI) in a rat model.And to investigate the relationship between endogenous NO system including inducible nitric oxide synthase (iNOS)and intrapulmonary NO production and expressions of PAI-1 in ALI.Methods In the study,endotoxemia followed by the second attack due to intratracheal injection of lipopolysaccharide (LPS) in rats caused ALI.Male SD rats aged 4-5 weeks (clean conventional rats,180-200 g) were randomly assigned to 2 groups:saline control (C) group,LPS-treated (LPS) group,and the 2 groups were randomly allocated to subgroups exposed to air (A) or 20 × 10-6 NO.They were sacrificed for 24 h.Expressions of PAI-1 mRNA of the lung tissue were evaluated by real-time polymerise chain reaction; PAI-1 proteins were determined by immunohistochemistry.NO production in the lung tissues and pulmonary iNOS activity were measured.Meanwhile,histopathological lung injury scores were evaluated and modified martius acid fuchsin brilliant blue(MSB) stains was performed to evaluate fibrin of the lung tissues.Results At 24 h time point with intervention of iNO,PAI-1 mRNA and protein levels in LPS-NO subgroup were decreased compared with those in LPS-A subgroup (4.94 ± 0.52 vs 5.56 ± 0.27 ; 1.31 ± 0.40 vs 1.69 ± 0.16,all P < 0.05).Meanwhile,iNOS activity and NO productions in LPS-NO subgroup were lower than those of LPS-A subgroup [(0.84 ± 0.36) U/mg prot vs (2.30 ± 0.25) U/mg prot ; (1.90 ± 0.84) μmol/g prot vs (3.38 ± 0.73) μmol/g prot,all P < 0.05].iNOS activity had significant correlation with expression of PAI-1 mRNA and protein in lung tissue (r =0.481,P =0.005 ; r =0.667,P =0.000) ; NO production had significant correlation with expression of PAI-1 mRNA and protein in lung tissue(r =0.532,P =0.002; r =0.784,P =0.000).At 24 h time point,the histopathologic lung injury scores in LPS-NO subgroup were decreased in contrast to LPS-A subgroup (4.28 ±0.94 vs 6.12 ± 1.51,P < 0.05).Fibrin deposition evaluated by modified MSB stains in LPS subgroups was found in alveolar space,lumen of blood vessel and mesenchymal ;LPS subgroup with NO appeared a decreasing trend in contrast to LPS subgroup with air.Conclusions Inhaled nitric oxide of 20 × 10 6 can suppress elevated expression of PAI-1 in ALI induced by endotoxin.This inhaled NO can improve the imba-lance of plasminogen activation system and alleviate lung injury.Meanwhile,inhaled NO down-regulates intrapulmonary iNOS activity as well as endogenous NO productions in rats with 2 hits of LPS induced ALI.These changes also have a close correlation with down-regulation of PAI-1 mRNA and protein.Thus,regulation of endogenous NO system on the expression of PAI-1 will be the future direction of new therapies for ALI.
7.Endoscopic retrograde cholangiopancreatography for biliary stricture with bile duct stone after liver transplantation

Objective To investigate the efficacy and safety of ERCP for biliary stricture with duct stone after liver transplantation.Methods Clinical data of 60 patients undergoing ERCP for biliary stricture with duct stone after liver transplantation between January 2013 and June 2014 were retrospectively analyzed.Results ERCP was successfully performed 78 times in 60 patients.Bile duct stenosis was cured in 24 cases (40%),improvement was observed in 27 cases (45%) and was not obvious in 9 cases (15%).Bile duct stones were successfully removed in 39 patients (65%).Incidence of post-ERCP complications was 13% (8/60),including mild pancreatitis in 1 case,hyperamylasemia in 5 cases,biliary tract infection in 2 cases,which were all resolved after conservative treatment.Conclusion ERCP is a safe and effective treatment for biliary stricture with duct stone following liver transplantation.
8.Effects of astragalus polysaccharides on lipid metabolism and PPAR-α gene expression in myocardium of diabetic hamsters

Objective This article was designed to observe the effects of astragalus polysaccharides (APS) on glucose and lipid metabolism, and on expressions of proxisome proliferator activated receptors-α (PPAR-α) and its downstream genes in diabetic hamsters cardiomyopathy. Methods Forty-five hamsters were divided into 3 groups randomly: normal control group (15 normal hamsters), diabetic control group [15 streptozotocin (STZ)-induced diabetic hamsters], and astragalus polysaccharides (APS)-therapy group (15 STZ-induced diabetic hamsters administered with APS 2 g/kg per day orally for 10 weeks). The levels of insulin, C-peptide, myocardial enzymes, glycosylated serum protein (GSP) and lipoprotein of all hamsters were measured. The ultrastructure of myocardium was studied, and the gene and protein expressions of PPAR-α, FATP and ACS were also detected by fluorescent quantitative RT-PCR and Western blot. Results It was shown that Compared with DM group, the levels of GSP, myocardial enzymes and lipoprotein of hamsters in APS-therapy group were lower, the myocardial ultrastructure of hamsters in APS-therapy group was well-protected, and the gene and protein expression of PPAR-α, FATP and ACS of hamsters in APS-therapy group were higher. Conclusions APS is partly effective in treating diabetic cardiomyopathy.
