Objective Estimating the therapeutic effect of music therapy and speech language therapy on post-stroke patients with non-fluent aphasia.Methods Eighty-four post-stroke patients diagnosed with non-fluent aphasia who came from the First Affiliated Hospital of Medical College of Shihezi University were collected between June 2012 and May 2014,then they were randomly divided into music therapy group (n =42) and speech language therapy group (n =42;including chronic (n =46) and acute groups (n =38)) based on table of random numbers.On the basis of conventionally using neurological drugs,music therapy and speech language therapy were given to the patients for one month,respectively.And language function was assessed by partial items of Chinese Version-Western Aphasia Battery before and after therapy.Results No significant changes were found in spontaneous speech,comprehension,repetition,naming and aphasia quotient in chronic and acute group patients through the music therapy and speech language therapy.Significant improvements were revealed in repetition (32.00 (15.00,53.75) vs 48.50(24.50,72.00),Z =2.147,P =0.032;33.00(14.50,49.25) vs 48.50(18.50,63.75),Z =2.018,P=0.038),naming (20.00 (8.50,34.75) vs 37.5(12.50,64.75),Z =2.298,P =0.022;19.50 (7.00,31.25) vs 34.50 (15.00,52.75),Z =2.039,P =0.041) and aphasia quotient (24.50 (10.50,37.50) vs 43.00 (18.00,64.75),Z =2.432,P =0.015;22.50 (10.00,34.50) vs 36.00 (14.00,54.00),Z =2.027,P =0.043) through music therapy and speech language therapy in chronic group patients.Comprehension was significantly improved through music therapy in chronic group patients.Repetition (24.50 (11.00,38.75) vs 46.50 (24.50,67.75),Z =2.038,P =0.043;26.50 (9.50,36.25) vs 42.50(19.00,64.25),Z =1.972,P =0.048) was significantly improved through music therapy and speech language therapy in acute group patients.And spontaneous speech (2.00 (1.00,3.75) vs 8.00 (4.00,12.75),Z =2.012,P =0.036),comprehension (51.00 (17.50,73.75) vs 85.00 (48.00,101.00),Z =2.298,P =0.022),naming (17.50(6.00,29.00) vs 37.50(16.00,58.75),Z =2.161,P =0.031) and aphasia quotient (18.00 (7.50,31.25) vs 42.50 (20.50,63.75),Z =2.256,P =0.024) were significantly improved through music therapy in acute group patients.However,no significant improvements were found in the speech language therapy group of acute patients.Conclusion The two therapies are effective in the chronic patients with non-fluent aphasia,and music therapy is also effective in acute patients with non-fluent aphasia.

Objective To investigate the fluctuated pattern of endogenous bactericidal/permeability-increasing protein(BPI)levels,and to elucidate its significance in endotoxin response after major abdominal surgery.Methods Eleven patients undergoing major abdominal surgery were involved in present study.The levels of plasma BPI,lipopolysaccharide-binding protein(LBP)and interleukin-6(IL-6)were measured preoperatively and on postoperative days 1,3,5,7,and 14 by ELISA.Plasma endotoxin concentration was determined with the chromogenic limulus amebocyte lysate(LAL),which was modified by perchloric acid(PCA)pretreatment of samples.The polymorphonuclear neutrophil count was also done at each time point.Results In comparison with normal controls,plasma BPI/LBP ratios [(1.058?0.074)?10-3,P=0.004] were significantly increased on postoperative day 1,maintained at same level on day 3,and declined on day 5.No plasma IL-6 was detected from the blood of all the 11 patients prior to operation,but plasma IL-6 level rose rapidly on day 1 after operation(27.440?12.144pg/ml,P=0.04),maintaining a high level(11.530?5.312pg/ml)on postoperative day 3,and disappeared on day 5.Conclusions Plasma BPI/LBP ratios were markedly elevated during 3 days after a major abdominal surgery,and endogenous elevated BPI may play an important role in prevention from the development of endotoxemia in surgical patients.

The purpose of this study was to investigate the effect of high mobility group 1 protein (HMG 1) on TNF ? expression as well as release in the kidney, and their potential role in the pathogenesis of acute renal injury associated with sepsis. Using a sepsis model by cecal ligation puncture (CLP), 50 male Wistar rats were randomly divided into normal control group ( n =10), CLP group ( n =20), and sodium butyrate treatment group ( n =20). At serial time points animals in each group were sacrificed, and blood as well as kidney tissue samples were harvested to determine HMG 1/TNF ? mRNA expression, tissue TNF ? protein level, and serum creatinine content. Cancer metastasis is composed of a complex cascade that involves a variety of critical steps beginning with detachment from the primary tumor and ending with growth of tumor at a distant site, such as bone. The "seed-and-soil hypothesis" predicts that the bone microenvironment expresses factors through which attract a variety of cancer cells and promote the tumor development. The ending point of tumor development in bone is achieved through the bidirectional and dynamic interaction between tumor cells and the cells in their growth microenvironment. A variety of factors produced by the bone microenvironment, contribute to the pathogenesis of cancer skeletal metastasis. In this review, using prostate cancer (CaP) as an example, some of general mechanisms of cancer metastasis will be summarized. In addition, the current understanding of the interaction between tumor cells and the bone microenvironment will be addressed. Finally, the research directions in the near future will be suggested.

Objectives:To discuss clinical significance of enternal nutrition of Nutrison fibre in neurosurgical coma patients. Methods:The study group(20 cases,GCS≤5) was fed gastrically with Nutrison fibre by a nasal feeding tube, and the control group(20 cases,GCS≤5) was supported with common fluid diet.In all cases,triceps skin fold,blood sugar,albumin,total lymphocyte count,complications and nitrogen balance were observed. Results:There were not significant differences between the study group and the control group in nutritional status,but the rate of complication in study group was obviously lower than that in control group. Conclutions:Enteral nutrition with Nutrison fibre can decrease the complications in nurosurgical coma patients and improve the quality of life.

ObjectiveTo investigate the relationship between sepsis induced high mobility group 1 (HMG 1) gene expression and endotoxemia in rats. MethodsSepsis was induced by cecal ligation puncture (CLP). One hundred Wistar model rats were randomly divided into four groups: normal control group ( n =10), sham operation (SO) group ( n =10), CLP group (further subdivided into 2?h, 6?h, 12?h, 24?h, 48?h and 72?h post CLP, n =10 in each subgroup), and bactericidal/permeability increasing protein(rBPI 21 ) treatment group (further subdivided into 12?h, 24?h post CLP, n =10 in each subgroup). BACKGROUND:Coracoclavicular ligament reconstruction with transclavicular-transcoracoid driling is an effective surgical technique to treat acromioclavicular dislocation. A good driling in the clavicle leads to a perfect bony tunnel and a good surgery. OBJECTIVE: To observe the effects of different driling positions of the clavicle on the location of bony tunnels in coracoclavicular ligament reconstruction. METHODS:Sixty three-dimensional digital models of the clavicle and coracoid process were constructed by Mimics13.0. Virtual transclavicular-transcoracoid bony tunnels were established according to different surgical planes with different driling positions in the clavicle. Parameters of these bony tunnels were measured, and the safety was evaluated. Option 1: The driling was made 30 mm distal to the clavicle, located in the center of the front and rear edges of the clavicle surface. Option 2: The driling was made 40 mm distal to the clavicle, located in the center of the front and rear edges of the clavicle surface. Option 3: The driling was made at the straight line of tapered nodule tip and the midpoint of the base of the coracoid process, located at the rear edge of the clavicle upper surface. RESULTS AND CONCLUSION: Bony tunnels in option 1 were extremely on the inside of the coracoid. Bony tunnels in options 1 and 2 were not in the center of clavicle. Bony tunnels in option 3 were in the center of both clavicle and coracoid. The method of locating the driling position with a certain distance to the distal clavicle leads to different results in man’s and woman’s models. To ensure that the bony tunnel can pass through the center of clavicle and coracoid, it is suggested to dril at the straight line of tapered nodule tip and the midpoint of the base of the coracoid process and nearby the rear edge of the clavicle upper surface.
