1.Effect of AngⅡ on Insulin Gene Expression in RIN-m Cells and Its Molecular Mechanism

Objective:To discuss the effect of AngⅡ on insulin gene expression IN RIN-m cells and its molecular mechanism. Methods:RIN-m cells were cultured and divided into three groups, including the control group, 100 nmol·L-1 AngⅡ group and losartan pretreatment group. After 24-hour incubation, insulin gene expression in RIN-m cells was detected by RT-PCR, the mean flu-orescent intensity of 2', 7'-dichlorofluorescein ( DCF) was detected by flow cytometry, PDX-1 and MafA mRNA expression were detec-ted by RT-PCR and the protein expression was detected by Western-blot. Results: Insulin expression in RIN-m cells, cellular ROS level, PDX-1 expression and MafA expression in 100 nmol · L-1 AngⅡ group were significantly different from those in the control group and losartan pretreatment group (P<0. 05). The later two groups had no significant differentces in those indices (P>0. 05). Conclusion:AngⅡ can down-regulate PDX-1 and MafA expression inβ-cells through oxidative stress pathway, and then inhibit insu-lin gene expression. Pretreatment with losartan can antagonize the effect of AngⅡ, and has protective effect onβ-cells in the aspect of insulin gene expression.

Objective To investigate the pharmacologic action of the Magnolia biondii Pamp volatile oil nanometer bangosome. MethodsThe pharmacologic action of the Magnolia biondii Pamp volatile oil nanometer bangosome was evaluated by observing its effect on Jimpy mice with swelling ear,capillary permeability and rats with allergic symptoms. Results The Magnolia biondii Pamp volatile oil nanometer bangosome could significantly relieve the degree of swelling in Jimpy mice extended by p-xylene,inhibit the increased capillary permeability in Jimpy mice extended by HAC,and combat the symptoms of rhinocnesmus,sneezing,nasal discharge with a better effect than the Magnolia biondii Pamp volatile oil. Conclusion The Magnolia biondii Pamp Volatile oil nanometer bangosome has a good anti-inflammation and anti-hypersensitiveness effect,which upgrades the effect of the Magnolia biondii Pamp volatile oil.

6.Methods for Sediment Toxicity Evaluation

Three widely used methods of sediment toxicity evaluation were introduced in the present paper, including organism toxicity tests, toxicity identification evaluation(TIE) and sediment quality guidelines(SQGs). Compared with the chemical analysis, toxicity tests have an advantage of taking the bioavailability of POPs into account, however, it fails to identify the causative toxicants. TIE, integrating with physicochemical analysis, implicates the specific pollutants in a tiered approach, and by which effective remediation can be designed accordingly. The sediment toxicity can be identified more quickly and appropriately by SQGs than by the former two methods. The differences among the different SQGs constituted by different standards may affect their values for toxicity evaluation. Extensive and reliable SQGs had been acquired to improve their utility. Finally, the combination of chemical analysis, toxicity tests and in situ bioassays will be the trend of sediment toxicity evaluation in the future.
7.CLINICAL STUDY ON TRANSVAGINAL HYSTERECTOMY FOR NON-PROLAPSE AND LARGE UTERUS

Ninety eight transvaginal hysterectomies were performed for non prolapse and enlarged myomatous uteri or adenomyosis , with the size equivalent to 6~18 gestational weeks or 138~741g . Bisection , enucleation, morcellation , coring and combination thereof were used to facilitate the operation by reducing the uterine volume. The results showed that, out of 98, 93 cases of vaginal hysterectomy were performed successfully . The average weight of the uterus for the 98 cases were 376g (ranging from 138 to 741g), the mean time for the operation was 99 minutes (ranging from 40 to 135 minutes), the average quantity of blood loss was 245 ml (range 50 to 600ml). The morbidity rate was as low as 3.1% and the average postoperative hospital stay was 5.4 days (range 4 to 9 days). These data suggest that non prolapse and enlarged uterus can be safely removed transvaginally through reduction of the uteriune volume . Not only the size , but also the shape and the mobility of the uterus should be considered when indication for transvaginal hysterectomy existed . The successful operation was closely related to the operator’s skill and experiences .
8.Clinical analysis of ten cases of congenital middle ear cholesteatoma.

OBJECTIVE:
To investigate the clinical features and operation treatment of the congenital middle ear cholesteatoma.
METHOD:
A retrospective review of clinical and surgical records of 10 patients with congenital middle ear cholesteatoma were performed. All patients were treated by surgeries,5 of 10 cases deal with one-stage tympanoplasty after drum exploration by external auditory meatus, 3 cases dealed with closed mastoidotympanectomy and tympanomastoidectomy, 2 cases dealed with open mastoidotympanectomy and tympanomastoidectomy.
RESULT:
The cholesteatomas were located at or around the posterior tympanum or mesotympanum in 5 patients, confined to the tympanic cavity and attic in 3 patients, advanced cholesteatoma that extended from the tympanic cavity into the mastoid antrum was seen in 2 patients. The mean postoperative PTA was 30 dB HL, the mean ABG was within 20 dB, after six months. No residual or recurrence of cholesteatoma was found.
CONCLUSION
The congenital cholesteatoma often originates from the posterior or anterior of the middle ear, with hiding lesion, thus result in severe conductive hearing loss. Imaging examination plays an important role in diagnosing and treating of congenital cholesteatoma. Early stage surgical treatment can obtain a good hearing reconstruction effect.

9.Effects of polypharmacy assessment and intervention on the elderly inpatients

Objective To explore the effect of polypharmacy assessment and intervention on elderly inpatients mainly in both economy and safety. Methods One hundred and forty elderly inpatients were selected. Fifty patients took less than 5 drugs, 75 patients took 5- 9 drugs, and 15 patients took more than 10 drugs. According to medication appropriateness index (MAI) and Beers criteria, the polypharmacy of elderly inpatients was assessed and intervened. The results were compared before and after intervention. Results The discrepancy rate of Beers criteria, incidence of potential risk, total scores of prescription MAI and average daily drug cost after intervention were significantly lower than that before intervention: 7.14% (10/140) vs. 16.43% (23/140), 22.14% (31/140) vs. 90.71%(127/140), 0(0,0)scores vs. 6.00(2.25,10.00)scores and (30.60 ± 19.71) yuan vs. (35.34 ± 26.84) yuan, and there were statistical differences (P<0.01 or <0.05). Conclusions Irregular medication exists widely in elderly patients, and the polypharmacy assessment and intervention can lighten the financial burden of elderly patients and improve the safety of drug therapy.
10.The short-term efficacy and safety of methotrexate plus low dose prednisone in patients with rheumatoid arthritis

Objective To evaluate the clinical efficacy and safety of methotrexate(MTX) plus low dose glucocorticoid in the treatment of rheumatoid arthritis (RA) from thetarget control point of view.Methods Patients diagnosed as RA according to American College of Rheumatology(ACR)/European League against Rheumatism (EULAR) 2010 classification criteria were enrolled.All of the patients were prescribed with 15 mg/week MTX,5 mg/week folic acid and prednisone (not exceeding 10 mg/day) orally.At week 0,4,12,disease activity and clinical efficacy were recorded.Co-primary assessment criterion was disease activityscore (DAS28)-based on C-reactive protein (CRP).Secondary assessment criteria included EULAR response criteria,ACR response criteria,simplified disease activity index(SDAI),clinical disease activity index(CDAI).The tolerability and toxicity of MTX was recorded at week 4,12.All patients were evaluated for the occurrence of adverse drug reactions associated with prednisone at week 12.Results A total of 76 patients were enrolled in the study.At week 4 and 12,68 and 65 patients completed regular follow-up respectively.At week 12,there were 30(46.2%),9(13.8%),26(40.0%) patients who met DAS28-CRP remission,low disease activity,middle and high disease activity criterion respectively.Three of nine patients who grouped in low disease activity after therapy were early or intermediate patients and didn't reach the target.Thus 36 (55.4%) patients met the standard of target control.The percentage of patients who met the criteria of EULAR good response,the ACR criteria for 20% improvement (ACR20),the ACR criteria for 50% improvement (ACR50),the ACR criteria for 70% improvement (ACR70) were 29.2%,75.4%,69.2%,64.6%,respectively.The proportion of patients meeting the standard of treat to target using SDAI and CDAI were 76.9%,58.5% respectively.The rate of liver injury,abdominal pain,abdominal distention and acid reflux,nausea were 11.8%,4.4%,4.4%,2.9% respectively at week 4.At week 12,4.6% of patients reported abdominal distention.There was only one patient (1.5%) each who complained of abdominal pain,nausea,loss of hair,varicella zoster virus infection and pulmonary infection at week 12.No serious adverse event was observed during the study.Conclusions Based on the view of target control,drug efficacy and safety,MTX plus low dose prednisone is still a useful therapeutic regimen for RA at present.
