Objective To explore the myocardial damage and the changes of MB isoenzyme of creatine kinase(CK-MB) and cardial troponin I(cTnI) of rat with endotoxemia at early stage.Methods Sixty SD rats were randomly divided into lipopolysaccharide(LPS) group(LPS group,n=48) and control group(n=12).Rats in LPS group were injected intraperitoneally with LPS(5 mg/kg) and then divided into 6 subgroups according sacrificed time after injection(0.5,1.0,2.0,3.0,12.0,24.0 h).CK-MB/ cTnI in blood was determined with chemiluminescent technique and myocardial pathological damage was observed under the light and transmission electron microscope.Control group was subjected to normal sodium and divided randomly into the same subgroups too.Results The rat diminished their action after beening injected endotoxin 15 minutes,and their reaction were slow.One hour later,they were short of action and reaction.Three hours later,they almost did not move at all,and the glossiness of their pelage were faint.Twenty-four hours later,their foodintake was diminished,their shout were weakened,and their response to stimulus were very thin,the glossiness of pelage were insufficient.Compared with control group,CK-MB increased significantly at 1.0,2.0,3.0,12.0,24.0 h(Pa

Objective To compare clinical outcomes of double-bundle and single-bundle in individualized arthroscopic anatomical reconstruction of anterior cruciate ligament (ACL) . Methods The clinical data of 117 patients were reviewed who had received double-bundle or single-bundle arthroscopic ACL reconstruction from March 2007 through September 2009 in our hospital and had undergone complete follow-up. Of them, 35 cases had single-bundle ACL reconstruction and 82 double-bundle reconstruction. In the single-bundle group(group A), there were 31 men and 4 women, aged 28. 6 ±5. 1 years. In the double-bundle group(group B), there were 73 men and 9 women, aged 27. 6 ±5. 4 years. The 2 groups were comparable in the preoperative demographic data ( P > 0. 05). To evaluate the outcomes, Lachman and Pivot Shift exams , KT-2000, Lysholm and IKDC (International Knee Documentation Committee) scores, were adopted. Results The 117 patients received a mean follow-up of 15 months (from 11 to 25 months). The Lachman test showed 88. 6% (31/35) were normal in group A and 95. 1% (78/82) were normal in group B.The pivot-shift test showed 88. 6%(31/35) were normal in group A and 96. 3% (79/82) were normal in group B. Group A had a mean Lysholm score of 93. 4 ± 8. 2 and group B a mean Lysholm score of 93. 7 ±7. 0. There were no significant differences between the 2 groups in the above indexes ( P > 0. 05). By IKDC score, 71. 4% (25/135) were normal in group A and 93. 9% (77/82) were normal in group B. The KT-2000 test showed a mean of 1. 4 ± 0. 6 mm in group A and a mean of 1. 1 ± 0. 5 mm in group B. These 2 values were significantly different between the 2 groups ( P < 0. 05). Conclusions The individualized arthroscopic double-bundle anatomical reconstruction of ACL can maximally restore the anteroposterior and rotational stability. Arrangement of the ACL insertion site on the femoral and tibial side, three-portal technique and ruler application are keys for individualized anatomical double-bundle ACL reconstruction.

Objective To discuss the strategies for diagnosis and treatment of severe pelvic fracture. Methods 38 patients with severe fracture of pelvis circle and different complications were studied in this paper from May, 1997 to June, 2005. 28 cases underwent emergency treatment for shock, 31 cases had internal fixation, and 14 cases received operative procedures for their complications. Results All the 38 cases survived. The integrity of the pelvic circle was restored in patients with unstable pelvic fracture. Most of the patients were back to their former work. Conclusions In treatment of severe pelvic fractures, the hemorrhagic shock and other complications endangering life should be promptly treated, and restoration of the integrity of the pelvic circle is the most important step.

Objective To study the influence of perdipine on the proliferation and proline-rich tyrosine kinase 2 (PYK_ 2 ) expression in cultured adult rat cardiac fibroblast stimulated by angiotensin Ⅱ, and to explore the mechanism of perdipine on cardiac fibrosis. Methods Adult rat cardiac fibroblasts (CFs) were treated with angiotensin Ⅱ(AngⅡ) to establish fibrosis model. The effects of perdipine on proliferation of CFs were analysed by MTT colorimetric assay, and fibronectin was tested by immunohistochemistry method. PYK_ 2 mRNA and protein level were examined by RT-PCR and Western blot analysis respectively. Results Perdipine inhibited CFs proliferation and fibronectin synthesis stimulated by angiotensin Ⅱ in a dose dependent, and the levels of PYK_2 mRNA and protein were decreased significantly. Conclusion Perdipine can inhibit cardiac fibrosis stimulated by angiotensin Ⅱ by suppressing CFs proliferation and fibronectin synthesis. PYK_2 is involved in the effect of perdipine.

Objective To explore the application value of coaxial semi automatic biopsy gun in the biopsy of small pulnonary nodules.Methods A retrospective analysis was performed on 40 patients with small pulmonary nodules who underwent CT guided percutaneous pulmonary nodules biopsy.Taking postoperative pathology diagnosis or imaging follow-up results as gold standard,the accuracy,specificity,sensitivity,intraoperative complications and puncture techniques of biopsy were analyzed.Results All operations were successfully completed.There were 29 malignant lesions and 11 benign lesions confirmed by postoperative pathology diagnosis or imaging follow-up results.The sensitivity,specificity,accuracy rate of puncture biopsy in diagnosis of malignant small pulmonary nodules was 89.65％ (26/29),100％ (11/11),92.50％ (37/40).The main complications were pneumothorax and local hemorrhage.Conclusion Percutaneous biopsy of small pulmonary nodules using coaxial semi-automatic biopsy gun has high accuracy,specificity and sensitivity,is a safe and effective method.

Objective To explore the efficacy of percutaneous nucleus pulposus atherectomy combined with ozone injection in treatment of lumbar disc herniation.Methods A total of 60 patients with lumbar disc herniation were randomly divided into 3 groups:20 patients were treated by ozone injection (group A),20 patients were treated by percutaneous atherectomy (group B),20 patients were treated by percutaneous atherectomy combined with ozone injection (group C).The changes of visual analogue scale (VAS) and clinical effects in three groups pre-and 1 week,1 month and 3 month postoperation were compared.Intraoperative and postoperative complications were analyzed.Results The VAS of postoperation in three groups were significantly lower than those of preoperation (all P＜0.017).The short term and long term VAS of group C were lower than those of group A and group B (all P＜0.017).But there was no statistically significant difference among three groups of recent and long-term excellent rates (all P＞0.017).related complications were mainly waist pain,lower limb soreness and pain.Conclusion Percutaneous nucleus pulposus atherectomy combined with ozone injection is a safe and effective method,which can improve the curative effect of operation.

Objective To observe the clinical therapeutic effect of danshenfenzhen and nutritional support in treating congestive heart failure accompanied with malnutrition on the basis of common treatment.Methods 42 cas- es of congestive heart failure accompanied with malnutrition were randomly divided into 2 groups:test group 21 cases (danshenfenzhen and nutritional support) and control group 21 cases(free diet),in the test group intravenous drip danshenfenzhen and vein nutrition were given if the internal nutrition can not provide enough energy on the basis of count of energy consumption,the serum albumin,total blood lymph cells and body weight were measured.Results After 14 days' treatment in the test group,the serum albumin,body weight and the total lymph cell increased signifi- cantly while in the control group,the serum albumin,body weight and the total lymph cells decreased significantly. Conclusion Intravenous drip danshenfenzhen and nutritional support can improve nutritional status and cardiac function of patients with congestive heart failure accompanied with malnutrition.

Objective To investigate effects of transcranial direct current stimulation on picture naming by those with Broca's aphasia recovering from a stroke.Methods Five patients with Broca's aphasia 6-12 months post-stroke served as their own controls.They all had lesions of the left frontal gyms or the basal ganglia.They received either anodic, cathodic or sham stimulation of the left Broca's area.They were then given a picture naming test.Results The average number of correct namings was 13.13 with anodic stimulation, 9.80 with cathodic and 10.15 with the sham stimulation.Compared with cathodic or sham stimulation, anodic stimulation of the left Broca's area can improve the picture-naming ability of aphasic stroke patients significantly more effectively.Conclusions Anodic stimulation of the left Broca's area can improve the picture-naming of cerebral apoplexy patients with Broca's aphasia.Such stimulation may increase the excitability of the left cortex, suggesting that the left Broca's area and its periphery play a very important role in language recovery after stroke.

OBJECTIVETo study the ischemic times of the main artery injury in the limbs and the influence on the limbs survival rate, and to analyze the reasons for the formation of the ischemia time.

METHODSFrom June 1996 to November 2012, 83 patients with completely severed limb main artery treated in our hospital were retrospectively studied. There were 77 males and 6 females, including 81 adults (ranging in age from 16 to 52 years old, with a median age of 35 years old) and 2 children (4 and 5 years old respectively). Seventy-five patients were treated with end to end anastomosis, 7 patients were treated with great saphenous vein transplantation, and 1 patient was treated with artificial blood vessel transplantation. The prior to admission ischemia time, after admission ischemia time and total ischemia time for successful and unsuccessful patients, as well as the causes of the formation of the ischemia time were studied. Limb survival rate of different ischemic time was counted. Sharpness injury or blunt injury ischemia time and amputation rate were researched statistically. All data were analyzed using SPSS statistical software.

RESULTSThere were 72 limbs survived, 11 amputated. The average ischemia time was (7.45±5.94) h for limb-salvage group and (13.73±14.00) h for amputation group. Prior to admission ischemia time between amputation and limb-salvage group had no significant difference. After admission the ischemia time in amputation group was longer than limb-salvage group. The amputation rate for ischemia time 21 to 44 h group was higher than other three groups (≤ 5 h, 6 to 10 h, 11 to 20 h) (P=0.023, 0.038, 0.044). Amputation reasons can be divided into anastomosis failure in the operation, vascular thrombosis 2 to 4 days after operation and late infection.

CONCLUSIONVascular injury limbs can tolerate longer ischemia time and the limb salvage succeed. Only too long ischemia time (> 20 h), limb amputation rate increases significantly. The main cause of long time ischemia is delayed diagnosis in the hospital. Whether limb-salvage success mainly depends on the degree of trauma and the quality of the anastomosis than ischemia time.
