1. Rat chronic gastroesophageal reflux disease model: A study of 3 preparation methods

Objective: To introduce 3 methods for preparation of stomach-conserving gastroesophageal reflux disease (GERD)model in rats, so as to improve the quality and successful rate of model preparation. Methods: Male Sprague-Dawley rats were divided into sham-operated(SO) group (n=20) and 3 stomach-conserving reflex model groups: duodenogastroesophageal reflux (DGER) group (n=47, esophagogastroduodenal anastomosis), gastroesophageal reflux(GER) group(n=20, partial pyloric ligation + cardiomyotomy followed by gastric fundus-esophagus anastomosis) and duodenoesophageal reflux (DER) group (n=21, jejuno-esophageal anastomosis followed by duodeno-esophageal anastomosis). Rats were observed closely after operation and their body weights were monitored. The rats that died unexpectedly were autopsied immediately and the surviving rats were sacrificed to observe the esophageal pathological changes at 1, 2, 3, and 4 months after operation. Results: Three rats died due to anesthetic accidents; 26 (24 .1%) died unexpectedly after operation, 18(38.3%)in DGER group, 1 (5%) in GER group, and 7(33.3%)in DER group; no rat died in SO group. Necropsy showed that the death cause of rats included obstruction, infection, asphyxia, hemorrhage, anastomosis leakage and perforation, etc. Esophageal pathological changes were noticed in all rats of 3 model groups; the life quality of surviving rats was good. Conclusion: The 3 methods for establishing rat stomach-conserving GERD model can produce high quality model and high rate of pathological changes, but with high post-operation mortality. In addition to rational operative methods, avoiding post-operation complications and unexpected death are also very important for successful GERD model preparation.
2.Application of magnetic nanoparticles in repair and regeneration of peripheral nerve

Peripheral nerve injuries, the worldwide difficult and complicated diseases, cause serious dysfunction but without ideal strategies to assist the successful treatment and recovery. The primary strategy to repair the peripheral nerve injuries is to bridge the lesions by promoting axon regeneration. The importance of mechanical factors in the nervous system has been appreciated only recently. It has been demonstrated that the application of external mechanical force is able to direct axonal outgrowth and to stimulate axonal elongation. For this purpose, to direct axonal growth cones in vivo with accurate force appears to be a challenge that must be resolved. Bionanotechnology is promising a new tool that offers the possibility to influence the process of axonal regeneration. The magnetic force generated by the interaction between Magnetic nanoparticles (MNPs) and magnetic fields provides remote, non-invasive, accurate, and controllable lead and control of neuronal axon regeneration.
3.Distribution and drug resistance of pathogens in 132 cases of neonatal sepsis

Objective To investigate the distribution and antibiotic resistance of isolated pathogens in neonatal sepsis. Methods The results of blood culture and drug susceptibility test in neonates sepsis from January 2012 to June 2013 were retro-spectively analyzed. Results One hundred and thirty-two strains were detected in the blood samples, with 100(75.76%)Gram-positive bacteria, 30 (22.73%) Gram-negative bacteria and 2 (1.52%) fungus. Staphylococcus epidermidis, Escherichia coli and Staphylococcus aureus were the three most common pathogens. Gram-positive cocci was strongly resistant to penicillin (100.00%), erythromycin, selectrin and ampicillin/sulbactam (62.50%-100.00%), but still sensitive to vancomycin and teico-planin. The resistance rate of Gram-negative bacilli to ampicillin was 100.00%, and the resistance rate to cefatriaxone, selectrin and cefuroxime was 61.54%-100.00%. The resistance rate to imipenem and piperacillin/tazobactam was lower. Conclusions The selection of sensitive antibiotics should be based on the pathogens and drug resistance testing for the treatment of neonatal sepsis.
4.Impact of Ticagrelor on Inflammatory Factors in Patients With Unstable Angina Pectoris After Percutaneous Coronary Intervention

Objective: To study the impact of ticagrelor on inlfammatory factors in patients with unstable angina pectoris (UAP) after percutaneous coronary intervention (PCI).
Methods: Based on conventional medication, a total of 100 UAP patients after PCI were randomly divided into 2 groups for anti-platelet therapy:Treatment group, the patients received aspirin+ticagrelor and Control group, the patients received aspirin+clopidogrel. n=50 in each group, all patients were treated for 12 months. Plasma levels of inlfammatory factors were examined before treatment and 24h, 7d, 28d after the operation. The patients were followed-up for 12 months for ischemic events, bleeding events and adverse drug reactions.
Results: Before treatment, plasma levels of IL-6, IL-18, TNF-α, PDGF were similar between 2 groups (t=0.1356, 0.1668, 0.2473, 0.5780, all P>0.05). At 24h post-operation, the above inlfammatory factors were signiifcantly increased by peaks in both groups and decreased thereafter. At 24h, 7d and 28d post-operation, the levels of IL-6, IL-18, TNF-a, PDGF in Treatment group were all lower than Control group at each corresponding time point (at 24 post-operation, t=2.0856, 2.4399, 2.2217, 2.2053, all P<0.05), (at 7d post-operation, t=6.0978, 5.9705, 4.4631, 4.3963, all P<0.01) and (at 28d post-operation, t=9.3779, 9.7724, 5.5855, 6.1700, all P<0.01). The follow-up study presented that at 12 months post-operation, the ischemic events in Treatment group was lower than Control group (χ2=4.3956, P<0.05);there were no small hemorrhage or major bleeding events occurred;the slight adverse drug reaction was similar between 2 groups (χ2=0.1773, P>0.05).
Conclusion: Compared to clopidogrel, ticagrelor could better decrease plasma levels of inlfammatory factors, it may reduce ischemic events without elevate bleeding events.
5.Granulocyte Macrophage Colony Stimulating Factor as an Adjunctive Therapy for Acute Myeloid Leukemia:A Systematic Evaluation

OBJECTIVE:To evaluate the curative efficacy of granulocyte macrophage colony stimulating factor (GM-CSF) as an adjunctive therapy for the treatment of acute myeloid leukemia (AML). METHODS: Randomized controlled trials (RCTs) of GM-CSF in the treatment of AML were retrieved from PubMed,EMBase,Cochrane Library,CBMdisc and CNKI database by computer as well as from the leukemia-related journals manually for a quality evaluation of the methodology and Meta analysis using RevMan5.0 software. RESULTS: A total of 10 RCTs were included,of which,3 RCTs were graded as A,5 as B and 2 as C. The results demonstrated that there were no significant differences of indexes among groups as complete remission (CR) rate,relapse rate,infection rate and length of hospital stay,etc. CONCLUSION: The available clinical research results showed that GM-CSF adjunctive therapy could neither increase CR rate,nor decrease the relapse rate,infection rate or length of hospital stay.
6.Research Advances in Pharmacological Activities of Paris Polyphylla

Paris polyphylla, the dried rhizome of Paris polyphylla Smith Var.yunnanensis (Franch.) Hand-Mazz.or Paris polyphylla Smith Var.chinensis (Franch.) Hara, has been commonly used as a traditional Chinese folk medicine for thousands of years.The extensive pharmacological studies indicated that steroidal saponins had multiple pharmacological activities, such as potential anticancer activity, antibacterial, antivirus, hemostasis, sedative activity, protecting cardiovascular and liver tissues.Obviously, the studies on steroidal saponins activity are very important in the development and clinical application of the potential new drug.The recent research advances in the pharmacological activities of Paris polyphylla reported in the paper could provide reference for the further utilization of Paris polyphylla.
7.Value of dynamic contrast-enhanced MRI parameters of Extended Tofts and Exchange model in the differential diagnosis of hepatocellular carcinoma and hepatic metastases

Objective To analyze the value of dynamic contrast-enhanced MRI(DCE-MRI) parameters of Extended Tofts and Exchange model in the differential diagnosis of hepatocellular carcinoma (HCC) and hepatic metastases of colorectal cancer. Methods A retrospective analysis of 40 pathologically confirmed cases was conducted , including 25 cases of HCC and 15 cases of hepatic metastases of colorectal
cancer,all patients underwent DCE-MRI. Applying liver double blood supplement model , and respectively using Extended Tofts two-compartment model and Exchange model liver microvascular permeability parameters [volume transfer constant of the contrast agent(Ktrans), efflux rate from extracellular extravascular space to plasma(Kep), extravascular extracellular volume fraction(Ve)and blood plasma space volume fraction (Vp)] and the perfusion parameter hepatic arterial perfusion index(HPI) in the lesion parenchyma of HCC and metastases were calculated. We used t test to compare the differences of the parameters measured from the two types of tumors , statistically significant parameters between HCC and metastases were screened which compared with the gold standard of pathological findings in order to draw the ROC curves to evaluate the diagnostic efficacy of different model parameters, using χ2 test compared the diagnostic accuracy of optimal parameters between the two models. Results By using Extended Tofts and Exchange model , Ktrans value of HCC were (0.661 ± 0.402)/min and (0.604 ± 0.316)/min respectively, Ktrans value of hepatic metastases were (0.196±0.175)/min and (0.179±0.135)/min respectively;Vp value of HCC were (0.334±0.217) and (0.294± 0.098), Vp value of hepatic metastases were (0.089 ± 0.015) and (0.089 ± 0.022),respectively; HPI value of HCC were (0.680±0.281) and (0.769±0.245) , HPI value of hepatic metastases were (0.326±0.216), (0.373± 0.298), respectively. There were significant differences between HCC and hepatic metastases in Ktrans, Vp and HPI values in both models(P<0.05). There was not statistically significant difference between HCC and hepatic metastases in Kep, Ve values(P>0.05).Drawn ROC curves of Ktrans, Vp, HPI , the Ktrans value (area under the curve of 0.869) among Extended Tofts model parameters was selected as optimal parameter to identify HCC and hepatic metastases; the HPI value (area under the curve of 0.845) among Exchange model parameters was selected as optimal parameter to differentiate these two types of tumors.The diagnostic accordance rate of Extended Tofts model and Exchange model were 80.0%(32/40), 82.5%(33/40), respectively ; There was no significant difference between them(χ2=0.082,P=0.775). Conclusions Parameters including Ktrans, Vp, HPI of the Extended Tofts model and Exchange model in DCE-MRI perfusion can be used to identify HCC and hepatic metastases of colorectal cancer. Among these parameters, Ktrans of Extended Tofts model and HPI of Exchange model have higher diagnostic value in differentiating HCC and hepatic metastases of colorectal cancer.
8.Clinical outcomes of four-point fixation for posterior chamber intraocular lens dislocation

Objective:To observe the clinical effect of four-point fixation in patients with posterior chamber intraocular lens dislocation.Methods:A retrospective case series study was adopted.Sixteen patients (16 eyes) with posterior chamber intraocular lens dislocation who underwent suture suspension techniques with four-point fixation in Changzhou No.2 people's Hospital from January 2015 to January 2018 were enrolled.Postoperative effects were observed during follow-up, ranging from 6 to 13 months.The preoperative and 6-month postoperative uncorrected visual acuity (UCVA), best corrected visual acuity (BCVA), corneal endothelium cell count and astigmatism were measured and the differences were compared, and the relationships between total astigmatism and corneal astigmatism or intraocular lens induced astigmatism were analyzed, and the postoperative position of intraocular lens and complications were observed.This study adhered to the Declaration of Helsinki.The study protocol was approved by an Ethics Committee of Changzhou No.2 People's Hospital (No.2015-C-012-01).Written informed consent was obtained from each patient before surgery.Results:The mean preoperative UCVA (LogMAR) and BCVA (LogMAR) were 1.09±0.24 and 0.48±0.20, respectively, which were significantly improved to 0.30±0.12 and 0.26±0.13 at 6 months after operation, respectively.And the differences were statistically significant ( t=11.782, 3.795; both at P<0.01).The preoperative and 6-month postoperative corneal endothelium cell count were (2 270±360)/mm 2 and (2 032±327)/mm 2, respectively, and the difference was not significant ( t=1.921, P=0.074).The 6-month postoperative mean total astigmatism, corneal astigmatism and intraocular lens induced astigmatism were (-1.47±0.82)D, (-1.34±0.61)D and (-0.22±0.35)D, respectively.There was a highly positive correlation between total astigmatism and corneal astigmatism ( r=0.885, P<0.05), but there was no significant correlation between total astigmatism and intraocular lens induced astigmatism ( r=-0.432, P=0.095).No dislocation, deviation or torsion of intraocular lens were observed during the follow-up.Varying degree of symptoms of iridocyclitis were observed during early stage after operation, which disappeared after treatment.There were two cases of high intraocular pressure, which were normal after treatment.No retinal detachment, choroidal detachment, expulsive suprachoroidal hemorrhage, endophthalmitis, corneal endothelial decompensation or other complications occurred during and after operation. Conclusions:There is a stable position of intraocular lens, good visual acuity and few complications after four-point fixation with suture and suspension, which is a feasible method to treat dislocated intraocular lens.
9.Effects of Celastrus orbiculatus Thunb.Extract on the Overexpression of mTOR in Human HepG2 Cells

This study aimed at investigating the effects of the extract of Chinese herb,Nansheteng (C.orbiculatus Thunb.),on human HepG2 cells through the overexpression of mTOR.The GV238-mTOR recombinant plasmids were transfected into HepG2 cells using molecular biological technology.The expression level of mTOR was evaluated by means of relative activity of luciferase and western blot.Human hepatic carcinoma HepG2/mTOR++ cells were treated with C.orbiculatus extract in different concentrations (20,40,80,160 and 320 tg·mL-1) for 24 h.The mTOR protein expression was detected by western blot.It was found that the protein expression of mTOR in transfected HepG2 cells was significantly enhanced.C.orbiculatus extract significantly inhibited the proliferation of HepG2/mTOR++ cells.Simultaneously,C.orbiculatus extract inhibited mTOR at its protein level in a dose-dependent manner.In conclusion,we successfully constructed recombinant mTOR cloning vectors,and established the stable HepG2 cell line with the overexpression of mTOR.Besides,C.orbiculatus extract significantly inhibited mTOR protein expression in human hepatic carcinoma HepG2 cells.

Objective To explore the possible mechanisms of Arsenic Trioxide in treating rheumatoid arthritis(RA)by observing the changes of HE staining and NF-KB expression as well as the apoptosis of syn- oviocytes in adjuvant-induced arthritis rats.Methods After the animal model was set up on Wistar rats sue- cessfully,they were randomly divided into AA model group and arsenic trioxide treatment group.The treat- ment group were injected with 4 mg'kg~(-1)9?d~(-1)arsenic trioxid fluid for 7 days.All of the rats were killed 3 days after the complete of injections.The joint specimens were exposed,fixed,decalcified,wrapped and cut into slices.All slices were examined by HE stain and immunohistological evaluation.Results HE staining showed that when compared with the normal control group,the layers of synoviocytes of the AA group were increased to 6-8,and the arrangement of synoviocytes was disordered and heavy inflammatory cell infiltration were found in the AA group.In the arsenic trioxide treatment group,the layers of synoviocytes increased to 3~4,and medi- um amount of inflammatory cell infiltration were found.The intensity of synovial NF-kB(p65)positive stain in AA model group was significantly higher than that in the normal control group.The synovial expression and ac- tivation of NF-kB in the treatment group were decreased markedly,and did not return to normal level.The average gray scale calculation showed that there were significant differences between the three groups(P
